The fact that national health care reform in the United States has been stalled is not reason for resign. More than ever, one has to design and implement creative options to achieve satisfactory health service at low costs. The political turnover in Congress shifts more responsibility to local governments. This means less funding and less willingness by the national government to be held accountable for health and social services. On the other hand, this situation may carry opportunity to impact social policies on a local level.

In 1978, members of the Southeastern Organ Procurement Foundation approached us concerning the disparity between the large number of African American patients, 50% to 70% of all patients on dialysis (artificial kidney machines), and the small number of African American donors (3%), and asked us why and what could be done about it? From my perspective as an African American transplant surgeon at Howard University, these observations piqued my curiosity and I agreed to investigate them. For African Americans, the rate was more than 5 times as high as that for whites. Further, the disease has equally affected women and children in the African American community; 84% of the AIDS cases involving children age 12 and under can be found in the African American community. Even a cursory review of data on the health status of women reveals striking differences by race. According to data from the National Center for Health Statistics, death rates among Black women from the three leading causes of death (cardiac disease, cancer and cerebrovascular disease) exceed those of white, Asian, Native American and Latina women for each age category from 45-84. With the exception of Black women, the death rates among white women from these diseases exceed those of other ethnic groups of women. African Americans, Asian Americans, Hispanic/Latino Americans, and Native Americans have had relatively less access to the resources of society compared to white Americans. These resources include such things as educational and employment opportunities, political and economic power, and the goods and services that a prosperous society can produce. Health care is an important resource to which access is not equal for all groups. African Americans and other ethnic minority groups are, by most indices of health care access and utilization, underserved. Traumatic Brain Injury (TBI) produces cognitive, behavioral, and affective deficits with resulting problems such as improper social behavior, increased aggression, emotional, personality and characterological changes. The impact upon the survivor, the sibling, as well as the parental subsystem has been well documented in the literature. Post-Traumatic Stress Disorder (PTSD) has been diagnosed in several different types of trauma survivors, although rarely have individual psychological symptoms been studied in parents.

Child Protective Services ( CPS) have been under growing pressure to implement risk assessment systems over the past ten years (Dueck, H. J., English, D.J., Depanfilis, and Moote, G.T. (1993). This emphasis has come largely from the increasing number of reported allegations of child abuse and neglect (Doueck, et. al.) However, increasingly individuals involved in custody battles use the CPS reporting system to retaliate against one another. This study investigated the variables influencing women's breast cancer detection behavior. Using social learning theory as an organizing framework. factors influencing women's decisions to seek breast cancer detection and the respondent's personal demographics were examined for their contribution to explaining women's decisions to seek breast cancer detection.

A [convience] sample from a university medical center provided data for the study. Data were gathered with a voluntary questionnaire. A total of 25 employees from the medical center participated in the study during the winter quarter of 1994.

Although males still constitute a substantial number of persons with AIDS, it is becoming clear that this is a disease affecting women and minority populations more adversely. Today women, while representing approximately 16 percent of all AIDS cases nationwide that are reported to the Centers for Disease Control, make up the fastest-growing segment of the population with AIDS. This article contends that AIDS is increasingly afflicting women who have little economic, political, or social power. Furthermore, misdirected public policy has been partly responsible for the greater incidence of the disease in certain regions and populations.

Intervention with children with attention deficit disorders is complex and requires change at multiple system levels using a social work perspective. Case management, macro social work practice, time allocation issues, the structure of a professional self, constraints in expanding a narrow definition of the social worker, and specific macro level intervention areas for social workers are examined in this context.

Case management services are an expanding program in the Texas Department of Mental Health and Mental Retardation (TXMHMR) system's treatment for chronically mentally ill individuals (CMI). CMI’s are individuals who are unable to maintain a stable adjustment to community life because of persistent and serious emotional disabilities. Previous research findings as to the effectiveness of this type of service in other programs have been mixed, and because of differences in program guidelines are not directly applicable to the TXMHMR program. Twenty infertile couples participated in the present study to investigate whether infertile couples receiving medical treatment experience changes in their emotional, marital and sexual adjustment as a result of the length of time they have been in treatment. The couples were divided into three groups based on how long they had been seeking medical attention for infertility. The instruments administered were: SCL-90-R (Symptom Checklist), Dyadic Adjustment Scale (DAS) and Index of Sexual Satisfaction (ISS). Due to the presumed associations of dopamine (DA) receptor supersensitivity phenomena in both long-term neuroleptic-treated tardive dyskinetic rats and neonatal 6-hydroxydopamine (n6-OHDA)-lesioned rats, we studied the influence of haloperidol on n6-OHDA-lesioned rats. At 3 days after birth rats received 6-OHDA-HBr (200 $\mu$g, bilateral intracerebroventricularly; desipramine pretreatment, 20 mg/kg, 1h) or vehicle. Two months later haloperidol (1.5/kg/day $\times$ 2 days/week for 4 weeks, then 1.5 mg/kg/day, every day for 10 months) was added to the drinking water. Spontaneous oral activity of intact and n6-OHDA-lesioned rats receiving haloperidol was reached and maintained at significantly higher levels after 15 weeks of haloperidol treatment. …

In 1992, the Minnesota Multiphasic Personality Inventory - Adolescent (MMPI-A) was developed to meet the unique experiences and needs of adolescents. Despite evidence that adolescents often demonstrate response biases in taking the MMPI-A, currently there is no method to systematically "correct" for the effect of test-taking attitude on profile configuration with this age group. The K-correction factor has been widely used to correct for defensiveness or underreporting of symptomatology on the MMPI among adult respondents, although results of cross-validation research on the effectiveness of the K-correction factor have been inconclusive. This study assessed the relationship of cognitive and noncognitive variables in relationship to academic success of students from selected allied health majors: respiratory therapist (RTT), radiologic technology (RT), surgical technology (ST) and medical records technician (MRT) from hospital based and community college programs in Virginia. Student academic success was defined as academic (classroom) grade point average, clinical grade point average, cumulative grade point average and passing status. The cognitive variables were preclinical (high school) grade point average, academic, clinical and cumulative grade point averages. Athletic Identity And Sport Orientation Of Adolescent Swimmers With Disabilities, Jeffrey J. Martin, Carol Adams Mushett, Kari L. Smith

Kinesiology, Health and Sport Studies

Measures of athletic identity and sport orientation, developed from self-schema theory, social role theory, and achievement motivation theory, were used to examine international adolescent swimmers with disabilities. The multidimensional Athletic Identity Measurement Scale (Brewer, Van Raalte, & Linder, 1993) was used to assess self-identity, social identity, exclusivity, and negative affectivity. The Sport Orientation Questionnaire (Gill & Deeter, 1988) measured competitiveness, win orientation, and goal orientation. Swimmers reported (a) a strong self-identity, (b) a moderate to strong social identity, (c) negative affectivity with lower levels of exclusivity, (d) strong competitiveness and goal orientation, and (e) moderate win orientation. This is an external validation study of the Alcohol and Drug dependence scales of the Millon Clinical Multiaxial Inventory II ([MCMI-II], Millon, 1987). The Structured Clinical Interview for the DSM-III-R ([SCID], Spitzer, Williams, Gibbon, & First, 1990) was administered to a sample of 73 adults who presented for treatment at a substance abuse facility to establish all possible substance abuse diagnoses based on the nosology of the revised Diagnostic Statistical Manual for Mental Disorders (3rd ed. [DSM-III], American Psychiatric Association, 1980). Subsequently, scores on the Alcohol and Drug Dependence scales of the MCMI-II for the present sample were compared to …
