The expansion of managed care, which is premised on containing costs while maintaining market share, represents a radical transformation of the U.S. health care system. A central question for medical sociologists is to determine how the changes brought by managed care influence health insurance and health care options. In evaluating the impacts of managed care, obstetrics is a particularly important site of study. Part of this agenda involves gathering baseline data on a specific transformation in obstetrics practice new to the managed care scene: the emergence of health insurance coverage for the option of home birth. This policy brief1 is the second in a series of reports focusing on the design of state SCHIP programs as they near full implementation. It examines the extent to which state agencies adopt conventional insurance norms or adhere to special principles of Medicaid coverage design for children in designing separately administered (or freestanding) SCHIP programs. The issue of coverage design is particularly relevant for children with low prevalence conditions and special health care needs. Increasingly, conventional insurance uses standardized coverage norms to limit coverage and treatment. The purpose of this study was to explore the association between drug usage, mental well-being and felony convictions among a sample of adult recreational drug users. The primary purpose of this analysis was to develop a set of predictor variables from the DRUGNET data set (i.e., lifestyle, drug usage, GWBS) that were able to account for the criterion variables (i.e., drug-related felony vs. no drug- related felony and non-drug felony vs. no felony ). The interrelationships of participation in counseling, level of hope, and care service utilization in a sample of individuals with HIV disease was the focus of this study. It was hypothesized that individual counseling would improve an HIV-positive person's level of hope, and would increase participation in additional care services. It was also hypothesized that individuals with more hope would seek a greater number of care services. The Health Status Questionnaire, which contains the Hope Scale, and the Social Support Questionnaire were used to collect data. Diabetes mellitus is a serious problem that affects 15.7 million individuals in the United States. The complications of this disease are catastrophic and can lead to blindness, kidney disease, lower limb amputations, nerve damage, increased risk of heart disease, stroke, and death. Among Native Americans, diabetes has reached epidemic proportions. A variety of psychosocial variables has demonstrated relationships to diabetic outcome. Past research has shown a relationship between psychological variables and glucose control. The current study of Native Americans shows a similar pattern using the psychosocial instruments to measure the constructs of depression, anger, hostility, social support, and perceived stress. …

Chronic back pain is a serious problem in the U.S. for which about 10% of back pain sufferers will undergo elective surgery. Unfortunately, back surgery is not successful in alleviating back pain in a substantial number of surgery patients. Various psychological and psychosocial variables have a demonstrated relationship to back surgery outcome. The most widely used personality test used to predict back surgery outcome is the Minnesota Multiphasic Personality Inventory (MMPI). The present study was designed to assess the impact of two dispositional variables, psychological reactance and desire for control, on individual perceptions of common elements of psychological change. These common elements represent cognitive, affective, and behavioral aspects of psychological change. The study tested whether individuals with different levels of psychological reactance and desire for control systematically differed in their perception of the importance of elements relevant to psychological change. Participants (N = 420) completed three self-report assessment instruments: (a) the Common Elements of Change Questionnaire, (b) Hong's Psychological Reactance Scale, and (c) the Desirability of Control Scale. Reengineering was a management tactic designed to reduce costs and increase efficiencies in the work place. Although many hospitals reported reengineering efforts were a priority during the latter part of the 1990s, little research has been done to determine the effects of reengineering on cost and quality performance indicators.

This research studied a large regional medical center located in southwest Michigan, which implemented patient-focused care redesign. The purpose of the present investigation was to examine determinants of teachers' intentions to teach physically active physical education classes (i.e., spend at least 50% of class time with the students engaged in moderate to vigorous physical activity). Based on the theories of reasoned action, planned behavior, and self-efficacy, a model was examined hypothesizing that teachers' intentions were determined by subjective norm, attitude, perceived behavioral control, and self-efficacy. Using hierarchical regression analyses, the theories of reasoned action and planned behavior were supported by accounting for 65% of the variance in intention due to the main effects of attitude and subjective …

The purpose of this study was to determine what factors predict whether female military retirees or the female beneficiary of a military retiree, ages 40 to 64, will obtain preventive health services, specifically, Pap smears, mammograms, and clinical breast examinations. Based on the findings of the study, it is suggested that it may be important for the Department of Defense to broaden their scope of interest to include those areas that are most prominent in affecting female military retirees or the female beneficiary of a military retiree, particularly those 40 to 64, in obtaining preventive health services.

Skin cancer is the most commonly diagnosed cancer in the United States with 20% of people developing some form of skin cancer in their lifetime (American Cancer Society, 1999). In spite of the high incidence of skin cancer, it is highly preventable. Approximately 90% of the cases are caused by exposure to ultraviolet radiation from the sun (Skin Cancer Foundation, 1992). The effect of an intervention aimed at reducing skin cancer risk was compared to a survey only control group in 99 Chicago beach-goers. Tissue microarrays allow high-throughput molecular profiling of cancer specimens by immunohistochemistry. Phenotype information of sections from arrayed biopsies on a multitissue block needs to be representative of full sections, as protein expression varies throughout the entire tumor specimen. To validate the use of tissue microarrays for immunophenotyping, we studied a group of 59 fibroblastic tumors with variable protein expression patterns by immunohistochemistry for Ki-67, p53, and the retinoblastoma protein (pRB). Offenders with mental illness who are serving correctional sentences are released to the community.Without support systems linking their transition to community-based programs following release from prison, the services necessary for their community reintegration are often fragmented and attenuated. Nearly two thirds of all inmates return to prison, and offenders with mental illness face major challenges during reintegration and have an even more difficult time living in the community without specialized, informed services.
