Off-Label Psychopharmacologic Prescribing For Children: History Supports Close Clinical Monitoring., Julie M Zito, Albert T Derivan, Christopher J Kratochvil, Daniel J Safer, Joerg M Fegert, Laurence L Greenhill

Department of Psychiatry and Human Behavior Faculty Papers

The review presents pediatric adverse drug events from a historical perspective and focuses on selected safety issues associated with off-label use of medications for the psychiatric treatment of youth. Clinical monitoring procedures for major psychotropic drug classes are reviewed. Prior studies suggest that systematic treatment monitoring is warranted so as to both minimize risk of unexpected adverse events and exposures to ineffective treatments. Clinical trials to establish the efficacy and safety of drugs currently being used off-label in the pediatric population are needed. In the meantime, clinicians should consider the existing evidence-base for these drugs and institute close clinical monitoring.

Love-Variant: The Wakin-Vo I. D. R. Model Of Limerence, Albert H. Wakin, Duyen B. Vo

The purpose of the current paper is to 1) propose the Wakin-Vo I.D.R model of limerence and 2) establish grounds for the scientific query of limerence. Limerence is an involuntary interpersonal state that involves intrusive, obsessive, and compulsive thoughts, feelings, and behaviors that are contingent on perceived emotional reciprocation from the object of interest. The model that the authors propose holds that although limerence resembles normative love, it is a state that is necessarily negative, problematic, and impairing, with clinical implications. The model frames limerence as consisting of three functional components: initiating force, driving forces, and resultant forces. Parallels between …

Priming is the benefit that an event receives when its processing has been preceded by the processing of a related or identical event. Context effects on priming are evident when priming changes as a function of some feature of experimental trials. The most commonly explored context effect is that of relatedness proportion (RP), where it has often been shown that the magnitude of priming (semantic or repetition) is directly related to the proportion of related trials: Increasing the related trials results in greater priming. More treatment options exist today for persons diagnosed with terminal cancerextending lives longer than expected though there is little known about the psychosocial needs or resources for these individuals. This study describes the experience of living past the expiration date and still living with Stage IV cancer. A transcendental phenomenological approach was used to elucidate vivid expressions of this experience in a sample population of five Caucasian women. The women survived beyond their prognoses of an earlier expiration are not close to imminent death and are still living with incurable breast cancer metastases. Based on interviews with six women, this study describes each participant’s personal experience of childhood sexual victimization, and revictimization while serving in the military. These traumatic experiences in childhood may have increased their risk of developing Post Traumatic Stress Disorder when exposed to sexual trauma in adulthood. Using a grounded theory approach, the interviewer identified common themes among the stories: early sexual abuse, and subsequent revictimization, poor family support, and poor choice of intimate partners as adults. Moreover, they experienced lessened ability to protect self and low self-esteem and denial. The subjects described a personal culture that included abuse as …

This study explores the meaning of place and the role of the physical setting, relative to sociocultural elements, for long-term, year-round residents of northern New Hampshire for two dimensions of place: attachment and identity. Resident-employed photography was used to prompt research participants to think about how they are attached to place and how it shapes their identity, and their photos served as a way to enter quickly and deeply into a conversation about place meaning. There is a growing body of evidence showing that periodontal disease is more prevalent among diabetic patients than among non-diabetic patients. This same evidence also shows that the more poorly controlled the diabetes, the more severe the periodontal disease. However, it is unclear if the increased risk of periodontal disease is known by the diabetes community. Two hundred diabetic patients voluntarily participated in an intervention to increase the diabetic patient’s knowledge of and attitudes toward periodontal disease. This report, prepared for the Maine Hospice Council and funded by the Carpenter Foundation, presents findings of a qualitative study of barriers to hospice utilization. This study includes the perspectives of all 26 Medicare certified hospice providers in Maine. Significant findings of the qualitative study include: A continuing need to educate the general public about hospice and the Medicare benefit. Maine health care providers have low referral rates to the hospice programs and often misunderstand the regulations and guidelines of the hospice benefit. Industry practices such as advertising, production of unsafe products, and efforts to defeat health legislation play a major role in current patterns of U.S. ill health. Changing these practices may be a promising strategy to promote health. The authors analyze 12 campaigns designed to modify the health-related practices of U.S. corporations in the alcohol, automobile, food and beverage, firearms, pharmaceutical, and tobacco industries. The objectives are to examine the interactions between advocacy campaigns and industry opponents; explore the roles of government, researchers, and media; and identify characteristics of campaigns that are effective in changing health-damaging practices. During the past few years, the AIDS campaign in Haiti has been targeting Vodou officiants and organizations. These awareness and training programmes in- form officiants about the transmission and prevention of AIDS, tests for HIV and anti- retroviral drugs, or even try to encourage them to become involved in a medical referral system. These culturalist interventions are grounded in an essentialist concept of culture that can have harmful effects on the targeted groups. Neuroanatomy Of The Subadult And Fetal Brain Of The Atlantic White-Sided Dolphin (Lagenorhynchus Acutus) From In Situ Magnetic Resonance Images, Eric W. Montie, Gerald E. Schneider, Darlene R. Ketten, Lori Marino, Katie E. Touhey, Mark E. Hahn

This article provides the first anatomically labeled, magnetic resonance imaging (MRI) -based atlas of the subadult and fetal Atlantic white-sided dolphin (Lagenorhynchus acutus) brain. It differs from previous MRI-based atlases of cetaceans in that it was created from images of fresh, postmortem brains in situ rather than extracted, formalin-fixed brains. The in situ images displayed the classic hallmarks of odontocete brains: fore-shortened orbital lobes and pronounced temporal width. Olfactory structures were absent and auditory regions (e.g., temporal lobes and inferior colliculi) were enlarged. In the subadult and fetal postmortem MRI scans, the hippocampus was identifiable, despite the relatively small size …

Many believe community-based ecotourism (CBE) can assist in conservation efforts and community development; however, little research has been done to assess the potential of CBE in a specific region. As part of a large, long-term, international research project, I define three problematic areas that may impede successful CBE efforts in the Kasigau location of southeast Kenya: bushmeat utilization, community attitudes and wildlife abundance. Although Muscle Dsymorphia (MD) has received more attention in recent years, it still lacks an official classification. The current study incorporates trait anxiety and obsessive-compulsive features into an etiological model of MD in the hopes of establishing them as reliable predictors of such MD symptomatology as drive for muscularity, social physique anxiety, and negative body attitude (self). Premenstrual Dysphoric Disorder (PMDD) is proposed as a cluster of severe emotional, physical and behavioral symptoms that affect a small percentage or women during the late luteal phase of their menstrual cycle and cease shortly after the onset or menses. Although some researchers hypothesize there is a difference in the amount or pain actually experienced during the premenstrual phase, others argue that there muy be differences in how women interpret the 'normal' fluctuations and bodily changes of the premenstrual phase. Identifying what stressors affect an officer and how he or she copes is essential in maintaining psychologically healthy officers. Studies have found that police officers consider the organizationally based aspects of the job to greatly influence their level of stress. The present research used the Police Stress Survey and COPE Inventory to identify common stressors and coping strategies among forty-five officers in the state of Indiana. The study attempted to determine whether or not a chronic overall stress -produced by frequent organizational stressors existed, and if this stress led to an increased use of maladaptive coping strategies. Exercise has been shown to be positively related to many different variables including sexual satisfaction, mental health, and physical health. These variables, in turn, have been linked to increased life satisfaction and couple satisfaction. The primary purpose of this study was to examine the relationships between exercise and couple satisfaction as well as exercise and life satisfaction. A secondary purpose was to replicate previous findings on the relationships between communication, love, sexual satisfaction, self-esteem, and physical health and couple satisfaction.

The purpose of this study was twofold: (1) to replicate Victor and Cullen's (1987; 1988) studies to confirm whether ethical climates (ECs) arise in skilled nursing facilities (SNFs), and whether organizations and groups within organizations have identifiable ECs; and (2) to test a model that examines the effects of ECs and perceived organizational support (POS) on licensed nurses' turnover intention (TOI) through job satisfaction (JS) and organizational commitment (OC).

Efficacy Of Cognitive Therapy And Pharmacotherapy In Depression: A Meta-Analysis, Daniel B. Michel

A meta-analysis comparing the long-term effectiveness of cognitive behavioral therapy (CBT) and pharmacotherapy in preventing relapse following treatment discontinuation was performed using published studies of depressed participants. Twenty-three articles met inclusionary criteria and were included in the analyses. Weighted effect sizes and moderators, treatment type, were calculated using statistical analysis software. Initially, fixed effects models were applied to the data, but due to significant between-group heterogeneity that could not be fully explained by treatment type, mixed effect models were used to account for the residual heterogeneity. Results indicated that overall, depressed individuals treated to remission with CBT evidence decreased relapse …
