This policy brief describes the APHIA II Operations Research Project through which the Population Council and Kenya’s Ministry of Health (MOH) developed and tested an approach to systematically link HIV-positive family planning (FP) clients to treatment and care services. Key activities included provider training and testing of a referral package which showed how, where, when, and who is to make and receive referrals. This referral package consisted of a directory listing all facilities in the vicinity available for services including HIV counseling and testing, FP, STIs, tuberculosis, and well-child checks; and referral forms directing clients to appropriate receiving clinics. Most-at-risk populations (MARPS), including men who have sex with men (MSM) and injecting drug users (IDUs), represent 1 percent of Nigeria’s population yet account for 38 percent of new HIV infections. Despite their elevated risk, MSM and IDUs are less likely than the general population to access HIV prevention and sexual health services because of stigmatization. There is a dearth of data on prevalence of HIV and sexually transmitted infections (STIs) among MSM and IDUs because their behaviors make them difficult to be reached programmatically and engaged in research. It is widely acknowledged that Palestinian youths between the ages of 15–29 have a low participation rate in the labor force. Population Council research has shown that this age group faces many barriers and challenges that make it difficult to pursue work. The prevailing political, economic, social, cultural, and educational conditions are major contributors to the low work participation, in addition to a decreased sense of self-confidence and low self-esteem. Education on sexuality and HIV can help adolescents develop the capacity for healthy, respectful relationships and protect themselves from unwanted and unsafe sex, unintended pregnancy, and sexually transmitted infections. A 2015 Population Council study found that sexuality and HIV education programs that address gender and power in intimate relationships are five times more likely to be effective than programs that do not. However, most curricula still do not address these issues. Young people growing up in South Africa face many challenges. HIV, orphanhood, early pregnancy, and limited employment prospects are some of the major obstacles to a healthy and productive adulthood. In KwaZulu-Natal (KZN), the province with the highest HIV prevalence rates, the impact of HIV and AIDS is felt especially strongly, and already-disadvantaged adolescents face the prospect of falling even further behind socially and economically because of the disruption of key relationships in their lives. This study explores the extremely biased division of labor within Egyptian households. The effects of marriage on women’s market and domestic labor supplies are important aspects of this study conducted by the Population Council for its working paper series on gender and work in the Mideast-North Africa region. New explanations for married women’s low participation rates are proposed. A matching model is estimated to determine how selection into marriage alters the time allocation of women. The empirical results show that marriage significantly affects both types of work with married women spending about eight hours less on market work weekly relative …

In Bangladesh, maternal health programs are not yet reaching the desired level of facility-based obstetric care service. Most deliveries are conducted by untrained persons at home, demonstrating inequity in access to recommended maternal health care services and underutilization of existing obstetric and newborn care services. Suboptimal performance by providers is a key barrier to improving availability and quality of maternal, newborn, and child health (MNCH) services. To increase use of facility-based services, an operations research study of “pay-for-performance” (P4P) incentives for providers and subsidization of consumer costs was initiated in 2010. In Egypt, as in many developing countries, the convergence of poverty and lack of social security places the working poor in a vulnerable situation that mandates immediate action in research and programmatic interventions. Before the January 25th revolution, a focus on social security was relevant in view of the insecurities instigated by increasing market liberalization and the shrinking role of the state in services and employment. Married adolescent girls form a large segment of Kenyan youth, yet they are largely overlooked by researchers and program managers concerned with the lives of adolescents. In settings such as Nyanza Province, Kenya, rates of HIV infection are extremely high, and evidence is increasing in some settings that girls who are married are much more likely to be infected, compared with their unmarried sexually active counterparts. In 2001, the Population Council and partners designed and implement an intervention program to address the needs of out-of-school adolescent girls in rural Upper Egypt. As noted in Promoting Healthy, Safe, and Productive Transitions to Adulthood Brief No. 12, the pilot, referred to as Ishraq (“Sunrise”), was launched in four rural villages of el-Minya governorate in Upper Egypt, the country’s least developed and most disadvantaged region. Under the USAID funded FRONTIERS program, the Population Council conducted a study in six health facilities in Western Kenya that demonstrated that screening for TB within an antenatal care (ANC) setting is feasible and acceptable among the service providers. However, a major challenge remains: although TB detection is encouraged among ANC clients within the maternal and child health clinics in the country, providers in these settings fail to appreciate the need for a continuum of care from pregnancy through to the postnatal period. Health workers in Kenya often miss opportunities to discuss pediatric HIV with clients and to track exposed children in order to initiate treatment. To address this problem, the Population Council’s APHIA II Operations Research Project collaborated with the Christian Health Association of Kenya, Catholic Relief Services, and the Kendu Adventist Hospital to improve HIV testing and treatment services for children at the hospital. The Population Council’s APHIA II Operations Research Project in Kenya collaborated with Kendu Adventist Hospital and the Christian Health Association to launch a study to examine the effect of improved services and awareness-creation on the uptake of HIV testing and treatment services for children and infants exposed to HIV. The study showed that community education and improvements in individual case tracking can increase the proportion of infants exposed to HIV who are tested for HIV and put on appropriate treatment. Guatemala’s population is poorer, less educated, faster growing, and more ethnically diverse than most others in Latin America. The country’s fertility and infant mortality rates are among the highest in the region. Nearly half of the Guatemalan population is indigenous, and more than 20 Mayan ethnic groups live in isolated rural areas with limited access to basic services such as water, sanitation, schooling, and health care. In 2001, UNICEF initiated a pilot intervention to test whether livelihoods opportunities could ameliorate early marriage and other adverse outcomes for girls in rural Bangladesh. Kishori Abhijan (“Adolescent Girls’ Adventure”) aimed to lower school dropout rates, increase girls’ independent economic activity, and raise the age at which girls marry. Life-skills training consisted of enhancing self-esteem and leadership skills and providing education related to gender roles and discrimination, health and nutrition, and legislation and legal rights, particularly early marriage and girls’ and women’s rights. In Ethiopia, the HIV epidemic is increasingly urban and female. An estimated 7.7 percent of the urban population in Ethiopia is HIV-positive, compared with less than 1 percent of the rural population. Nationally, the female-to-male ratio of HIV infection is 3 to 2. Girls who migrate from rural to urban areas may be less protected, more vulnerable to violence and sexual abuse, and less equipped to avoid abusive situations—all of which may increase HIV risk. The Biruh Tesfa (“Bright Future”) program addresses the social isolation of out-of-school adolescent girls aged 10–19, including rural-to-urban migrants, domestic workers, and orphans. Using a …

Almost 1 billion people around the globe live in urban informal settlements, or “slums.” In Kenya, 60 percent of Nairobi’s nearly 3 million inhabitants live in slum areas characterized by high levels of poverty and HIV. Kibera is the largest of these slums. Life is particularly perilous for adolescent girls in slums, as they make their transition to adulthood. Girls are several times more likely than their male age-mates to become infected with HIV, and sexual initiation for girls often occurs in the context of violence and coercion.
