In the past ten years, there has been a significant rise in the number of asylum seekers and refugee populations worldwide (UNHCR, 2022). Displaced individuals are extremely vulnerable to mental illness because of the compounded trauma experienced in their countries of origin with the stress of immigration, asylum seeking, culture shock, and language barriers. PTSD is affecting 500 million individuals globally, and refugees are especially susceptible to having these symptoms (Farrell, 2020). This qualitative and arts-based capstone thesis discusses working with individuals with multiple disabilities, songwriting in music therapy, intersubjective communication and theory, and countertransference in music therapy, by sharing the literature and presenting a method of how a music therapist’s own clinical improvised songwriting explores countertransference in the therapeutic relationship for an individual with multiple disabilities. Music therapy is used to treat individuals with multiple disabilities. The literature demonstrates that music therapy and improvisation can support communication development in clients with multiple disabilities. Phenomenologists identify the subjective body and its felt-senses as the basis for human development and consciousness, including mental health. Individuals with reactive attachment disorder present as guarded towards therapeutic care and respond passively to treatment or become combative and aggressive. Previous body-based interventions for individuals with reactive attachment disorder included dangerous and unethical approaches that led to traumatization, distrust, and even death. Through the development and implementation of a method, intermodal exchange between visual art and music was integrated into both the school’s art program and its group counseling offerings. Attachment theory examines the infant/caregiver’s relationship by the attachment style they develop in the first years of life. Over time, these same attachment styles affect adult romantic relationships. Bowlby defines four infant/children attachment styles as securely attached, anxious-ambivalent-insecurely attached, avoidant-insecurely attached, and disorganized-disoriented-insecurely attached. These four styles transferred into three main types for adults: secure, anxious-ambivalent, and avoidant. The purpose of this qualitative study was to investigate how a serious medical diagnosis, such as cancer, can act as an acute or prolonged trauma. This was explored through the lived experiences of psychological distress of young cancer survivors. Participants were 12 men and women who had been diagnosed with cancer within the past seven years when they were between the ages of 18 and 39. From these interviews several themes emerged that describe the phenomenon of what I have termed medical traumatic stress.
