Validation And Implementation Of Automated Planning Optimization In Utah Valley Hospital, Oluwatobi Adeniji

UNLV Theses, Dissertations, Professional Papers, and Capstones

The increasing complexity of modern radiotherapy demands planning workflows that are efficient, standardized, and dosimetrically robust across diverse disease sites. Knowledge-based planning (KBP) systems such as RapidPlan offer a data-driven approach to automate and improve treatment planning by learning geometric–dosimetric relationships from high-quality clinical plans. In this work, I am evaluating the performance, generalizability, and clinical applicability of vendor-provided RapidPlan models across seven anatomical sites: intracranial SRS, prostate SBRT, right and left lung SBRT, liver SBRT, head and neck, and glioblastoma. Subsequently, a complementary institution-specific SRS model tailored to single-isocenter multitarget workflows was created. Seventy retrospectively selected patients were replanned …

Computational Study For Tissue-Specific Dosimetric Quantities Of Secondary Radiation For Mars Exploration, Sungmin Pak

UNLV Theses, Dissertations, Professional Papers, and Capstones

The two main sources of primary space radiation, Galactic Cosmic Rays (GCRs) and Solar Particle Events (SPEs), encompass a spectrum of ions, ranging from protons (Z=1) to nickel ions (Z=28), with energies spanning from less than 1 keV to exceeding a few TeV. While SPEs represent sporadic events primarily comprised of a high flux of low- to intermediate-energy protons, GCRs are characterized by continuous low flux of diverse ions with higher energies, peaking in intensity near the solar minimum. During a Mars mission, astronauts will encounter primary particles of high energy from GCR and SPE spectra, capable of penetrating spacecraft …

Modeling Radiation Exposure On Flight Missions To Analyze Aircrew Risk, Camila V. Quintero Hilsaca Mar 2023

Modeling Radiation Exposure On Flight Missions To Analyze Aircrew Risk, Camila V. Quintero Hilsaca

Theses and Dissertations

GCR and SPE comprise the majority of the ionizing radiation experienced in the upper atmosphere within flight-altitude environments. Although previous studies have analyzed radiation doses from single sources on civilian flight operations, there is a lack of research focused on dose received by military personnel during flight from both sources simultaneously. In-flight radiation environments are modeled through the MCNP6 for two separate aircraft, an Air Force A-10 and a Boeing 737. Particle fluence values for galactic cosmic rays and solar particle events for four separate flight paths are determined using the CARI-7A software and the SIRE2 toolkit, respectively. MCNP6 code …

Absolute Quantification Of Tc-99m Activity Distributions Using A Planar Molecular Breast Imaging Commercial System, Benjamin P. Lopez

Dissertations and Theses (Open Access)

Molecular breast imaging (MBI) uses two dedicated-breast semiconductor detectors to visualize the preferential uptake of technetium-99m-sestamibi (99mTc-sestamibi) by breast cancer cells relative to surrounding benign breast tissues. Clinically, MBI is used primarily as a supplementary tool to standard-of-care mammography because of its improved detection of breast cancers, especially in women with mammographically-dense breasts. Because of a lack of image corrections, MBI applications are currently limited to qualitative evaluations of relative pixel intensities between image regions with suspected lesions and normal tissue.

The objective of this dissertation was to use Monte Carlo simulations to better characterize the MBI imaging …

Hepatocellular Carcinoma Image-Guided Intervention: Quantitative Characterization Of Reagents For Thermochemical Ablation, Emily A. Thompson May 2022

Hepatocellular Carcinoma Image-Guided Intervention: Quantitative Characterization Of Reagents For Thermochemical Ablation, Emily A. Thompson

Dissertations and Theses (Open Access)

Thermochemical ablation (TCA) is a minimally invasive therapy under development for hepatocellular carcinoma, a leading cause of cancer death worldwide. TCA utilizes acid-base chemistry delivered simultaneously to induce local ablation when administered. When delivered via a mixing catheter placed directly into the tumor, acid (e.g., AcOH) and base (e.g., NaOH) react to completion at the catheter tip, producing the acetate salt, water, and releasing heat (Δ>50°C) in sufficient quantities to induce lethal osmotic and thermal stress in tumor cells. However, these two reagents are not distinguishable from tissues with noninvasive imaging modalities, which makes monitoring the delivery of TCA …

Reinvigorating A Technical Countering Weapons Of Mass Destruction Distance Learning Graduate Certificate Program, James C. Petrosky, Gaiven Varshney, Jeremy Slagley, Sara Shaghaghi

Faculty Publications

Current Countering Weapons of Mass Destruction (CWMD) demands can be divided broadly into policy and science. The science of chemical, biological, and radiological/nuclear weapons informs the limits of development, production, employment, operation, detection, risk characterization, human and material protection, and medical intervention. In short, the science of weapons of mass destruction (WMD) should precede and inform the development of policy. It is to this end that the Air Force Institute of Technology (AFIT) CWMD program was re-established, providing a technical educational option for practitioners to understand the science behind a very technically challenging subject.

Structural Shielding Considerations For Vmat, Ana Lucia Dieguez Jul 2021

Structural Shielding Considerations For Vmat, Ana Lucia Dieguez

LSU Master's Theses

Introduction: As noted in National Council on Radiation Protection and Measurements (NCRP) Report 151, the medical physicist or other qualified expert has the responsibility to keep abreast of any new technology or treatment method that could potentially impact structural shielding design. Volumetric arc therapy (VMAT) became prevalent after the publication of Report 151 and thus was not explicitly addressed in Report 151. If the shielding-related characteristics of VMAT differ enough from the expectations of Report 151, especially in the circumstance of a vault utilized exclusively for VMAT, a shielding design based on Report 151 could potentially be inadequate. The …

Skin cancer is the most common type of malignancy affecting both men and women in the United States. A majority of these cases are nonmelanoma skin cancers (NMSC) such as basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). The goal of any skin cancer treatment is to cure the lesion itself while also preserving function and optimizing superficial cosmesis. High dose rate (HDR) brachytherapy provides an alternative to superficial x-ray or electron beam for skin cancer treatment. Utilizing an Iridium 192 source soldered to the end of a wire, surface dose can be administered at predetermined positions with sharper …

The Associations Between Self-Reported Exposure To The Chernobyl Nuclear Disaster Zone And Mental Health Disorders In Ukraine, Matthew A. Bolt, Luralyn M. Helming, Nathan L. Tintle

Faculty Work Comprehensive List

In 1986, Reactor 4 of the Chernobyl nuclear power plant near Pripyat, Ukraine exploded, releasing highly-radioactive materials into the surrounding environment. Although the physical effects of the disaster have been well-documented, a limited amount of research has been conducted on association of the disaster with long-term, clinically-diagnosable mental health disorders. According to the diathesis–stress model, the stress of potential and unknown exposure to radioactive materials and the ensuing changes to ones life or environment due to the disaster might lead those with previous vulnerabilities to fall into a poor state of mental health. Previous studies of this disaster have found …

Planning for the future of nuclear security is a vital and complex task, requiring cooperation and contribution from many disciplines and industries. This diversity of expertise should include the medical sector, which faces many of the same challenges as the nuclear industry: controlling access to dangerous material, creating a strong security culture, cooperating with the wider world and engaging the public.

Medical physicists, of which the author is one, oversee all aspects of small-scale radiation use. This paper discusses three key areas increasingly important to both medical and nuclear uses of radioactive materials: public engagement, prevention of nuclear and radiological …

Radiation Therapy Medical Physics Review – Delivery, Interactions, Safety, Feasibility, And Head To Head Comparisons Of The Leading Radiation Therapy Techniques, Cielle Collins Jan 2017

Radiation Therapy Medical Physics Review – Delivery, Interactions, Safety, Feasibility, And Head To Head Comparisons Of The Leading Radiation Therapy Techniques, Cielle Collins

Honors Theses and Capstones

Radiation therapy uses high energy radiation to kill cancer cells. Radiation therapy for cancer treatment can take the form of photon therapy (using x-rays and gamma rays), or charged particle therapy including proton therapy and electron therapy. Within these categories, numerous methods of delivery have been developed. For example, a certain type of radiation can be administered by a machine outside of the body, called external-beam radiation therapy, or by a “seed” placed inside of the body near cancer cells, called internal radiation therapy or brachytherapy. Approximately half of all cancer patients receive radiation therapy, and the form of radiation …

Structuring A Risk-Based Bioassay Program For Uranium Usage In University Laboratories, Johnne' Talia Dawson Dec 2016

Structuring A Risk-Based Bioassay Program For Uranium Usage In University Laboratories, Johnne' Talia Dawson

UNLV Theses, Dissertations, Professional Papers, and Capstones

Bioassay programs are integral in a radiation safety program. They are used as a method of determining whether individuals working with radioactive material have been exposed and have received a resulting dose. For radionuclides that are not found in nature, determining an exposure is straightforward. However, for a naturally occurring radionuclide like uranium, it is not as straightforward to determine whether a dose is the result of an occupational exposure. The purpose of this project is to address this issue within the University of Nevada, Las Vegas’s (UNLV) bioassay program. This project consisted of two components that studied the effectiveness …

Evaluating The Risk To Ostomy Patients Working With Uranium Using The Icrp Biokinetic Model For The Gastrointestinal Tract, Sara Taylor

UNLV Theses, Dissertations, Professional Papers, and Capstones

The ICRP has published two biokinetic models providing a basis for radiation dose assessment due toradionuclides incorporated inside the gastrointestinal tract. These models are a sufficient tool to assess the committed dose equivalent for occupational exposures to workers assuming normal anatomy. The colostomy is becoming a more prevalent procedure. A colostomy permanently or temporarily bypasses a portion of colon to allow rest and healing. There are four different colostomies; ascending, transverse, descending and sigmoid and an ileostomy. As a patient’s strength returns, they can return to normal daily activities, including returning to work. Therefore, as an ostomy patient returns to …

Measurement Of Neutron Activation From A High Energy Varian Linear Accelerator, Timothy Thatcher Aug 2016

Measurement Of Neutron Activation From A High Energy Varian Linear Accelerator, Timothy Thatcher

UNLV Theses, Dissertations, Professional Papers, and Capstones

Linear accelerators producing photons above 10 MeV may induce photonuclear reactions in high Z components of the accelerator. These liberated neutrons can then activate the structural components of the accelerator and other materials in the beam path through neutron capture reactions. The induced activity within the accelerator may contribute to additional dose to both patients and personnel. This project seeks to determine the total activity and activity per activated isotope following irradiation from a Varian medical linear accelerator at energies above 10 MeV. A Varian 21iX accelerator was used to irradiate a 30 cm x 30 cm x 20 cm …

Voxel-Level Absorbed Dose Calculations With A Deterministic Grid-Based Boltzmann Solver For Nuclear Medicine And The Clinical Value Of Voxel-Level Calculations, Justin Mikell Dec 2015

Voxel-Level Absorbed Dose Calculations With A Deterministic Grid-Based Boltzmann Solver For Nuclear Medicine And The Clinical Value Of Voxel-Level Calculations, Justin Mikell

Dissertations and Theses (Open Access)

Voxel-level absorbed dose (VLAD) is rarely calculated for nuclear medicine (NM) procedures involving unsealed sources or 90Y microspheres (YM). The current standard of practice for absorbed dose calculations in NM utilizes MIRD S-values, which 1) assume a uniform distribution in organs, 2) do not use patient specific geometry, and 3) lack a tumor model. VLADs overcome these limitations. One reason VLADs are not routinely performed is the difficulty in obtaining accurate absorbed doses in a clinically acceptable time. The deterministic grid-based Boltzmann solver (GBBS) was recently applied to radiation oncology where it was reported as fast and accurate for both …

By imaging tissue activation following proton treatment, attempts have been made to verify proton dose and range. In this dissertation, two novel approaches were developed and tested for the purpose of help improve the proton dose and range estimation as well as verification.

Although there are still some challenges, attempts for proton dose verification using PET has been made by comparing Monte Carlo dose and PET simulations with treatment planned dose and measured PET. In this approach, …

Radiogenic Second Cancer Risk Differences In Female Hodgkin Lymphoma Patients Treated With Proton Versus Photon Radiotherapies, Kenneth L. Homann Dec 2013

Radiogenic Second Cancer Risk Differences In Female Hodgkin Lymphoma Patients Treated With Proton Versus Photon Radiotherapies, Kenneth L. Homann

Dissertations and Theses (Open Access)

Hodgkin Lymphoma (HL) is the most common cancer diagnosis of young adults in the United States. Advances in curative treatments for HL, including the use of photon radiation therapy (RT) techniques, have increased 10 year survival rates to approximately 90% among young patients. These RT treatments, however, contribute to an increased incidence of radiogenic second cancer (RSC) formation to the healthy tissue surrounding the tumor volume relative to the general population. These RSCs are the leading cause of death among long-term HL survivors. Proton therapy has been shown to reduce the therapeutic dose, and therefore, the risk of developing a …

The purpose of this research was to characterize the neutron energy distribution and flux emitted from the UNLV plutonium-beryllium source, serial number MRC-N-W PuBe 453. This was accomplished through the use of the MCNPX/5 Monte-Carlo particle transport code to simulate radiation interactions within the physical environment of the source and its surroundings. The moderating drum currently containing the source as well as all of the sampling ports were accurately modeled in MCNPX/5. This geometry was then used to simulate the neutron interactions taking place in these geometries. The results of the simulations were then verified by the use of specifically …

Application Of X-Ray Diffraction To Material Analysis And Medical Imaging, Wei Zhou Jan 2010

Application Of X-Ray Diffraction To Material Analysis And Medical Imaging, Wei Zhou

Legacy Theses & Dissertations (2009 - 2024)

Powder diffraction is commonly used to determine the structures of both inorganic and organic materials. The angle and intensity of the diffraction (also called coherent scatter) peak depends on the nanostructure of the material. When no x-ray optic is used, the peak width broadens, and hence the resolution worsens, as the sample area is increased. However, a small sample area gives low diffracted signal intensity, particularly for thin films and for organic materials, which have low diffraction cross sections. X-ray optics can be used in x-ray powder diffraction to increase the diffraction intensity, thus decreasing exposure times. For a small …

Session 1 - Rationalizing The Home Front: The Cold War, The Nevada Test Site, And Radiation Exposure, Leisl A. Carr

International Symposium on Technology and Society

Beginning in 1953, radiation exposure and its effects became a hotly contested issue between the government, members of communities surrounding the Nevada Test Site (NTS), and researchers within the national scientific community. The Atomic Energy Commission (AEC), concerned about maintaining a continental testing facility and aware that atomic testing would impact communities surrounding the NTS, sent representatives to areas potentially affected by fallout to instruct and reassure the affected public. How government officials perceived neighboring communities and how these communities perceived these representatives of the government often determined public responses to the atomic testing program. The story of radiation monitors …

Development Of Dose Coefficients For Radionuclides Produced In Spallation Targets, Phillip W. Patton, Mark Rudin

Transmutation Sciences Physics (TRP)

Dose coefficients permit simple determination of radiation dose associated with various exposure scenarios, and ultimately permit radiation safety personnel to assess the health risks to workers in a nuclear facility. Specifically, radiation safety personnel use dose coefficients to determine the radiation dose incurred to a tissue or organ system from a given exposure. These parameters are often expressed in terms of Annual Limits on Intake (ALIs) and Derived Air Concentrations (DACs).

The research consortium comprised of representatives from several universities and national laboratories has successfully generated internal and external dose conversion coefficients for twenty radionuclides produced in spallation neutron sources. …
