Abstract

The therapeutic alliance is central to occupational rehabilitation, particularly for immigrant workers who face unique challenges of migration and of social and occupational integration. This study explores the development and maintenance of this alliance between immigrant workers with compensated work injuries and their care providers during work rehabilitation. Using ethnography, semi-structured interviews, and focus groups, the qualitative case study involved 7 injured immigrant workers and their interdisciplinary clinical team. The study identified several factors that weakened the alliance, including administrative complexity, conflicting views on pain and disability, cultural stereotypes, and interorganizational communication issues. Environ une personne sur trois dans le monde vivrait avec un trouble musculosquelettique (TMS) persistant. Plusieurs auteurs soulignent l’influence significative des facteurs psychosociaux, dont l’anxiété, sur le développement de l’incapacité prolongée. Associées à l’anxiété, des inquiétudes excessives ont été documentées chez des travailleurs ayant une douleur persistante d’origine musculosquelettique. Les inquiétudes ne favorisent pas l’adaptation en contexte d’une situation problématique. Cependant, aucune étude n’a jusqu’à présent tenté de comprendre ces inquiétudes, en lien avec l’environnement de travail. Limited research has been conducted regarding the characteristics, services, and outcomes of consumers served by Vocational Rehabilitation (VR) agencies in the United States who are deaf-blind. This article provides descriptive information about this population based on Rehabilitation Services Administration Case Service Report (RSA-911) data from fiscal years 2013, 2014, and 2015. The article also includes a discussion of inconsistencies in use of disability category codes to classify VR consumers who are deaf-blind, emphasizing the need for agencies to utilize a consistent classification system that reflects the diverse characteristics of this population.

Interviews with 16 employment consultants-triangulated with job seekers, family members, and supervisors-revealed a model of employment supports aligned with the elements described in the literature, although with an added emphasis on (a) building trust as a key element starting from day one; (b) a circular process converging on the job match; (c) and flexible intensity of supports. The model can be used for improving clarity in communication with employment consultants about effective employment support practices for assisting job seekers with intellectual and developmental disabilities.

This study found that about four percent (4.2%) of VHA users with psychiatric diagnoses accessed employment services in a given year. VHA patients with a psychiatric diagnosis of schizophrenia had higher odds of accessing employment services relative to VHA users with other psychiatric diagnoses, including PTSD, depression, and other anxiety disorders. In practice, veterans should be aware that receiving VHA employment services should not affect VA benefits and should discuss their medical and financial benefits with benefits advisors and other appropriate advisors. In policy, Policymakers could consider allocating funds to further study the efficacy and effectiveness of various employment services …

This study was the first of its kind to study the effectiveness of any type of vocational rehabilitation (VR) intervention after a spinal cord injury, and it found that veterans who were provided supported employment were 2.5 times more likely than veterans in the treatment as usual-interventional site (TAU-IS) group and 11.4 times more likely than the treatment as usual-observational site (TAU-OS) group to obtain competitive employment. In practice, veterans seeking employment should enroll in a supported employment program at their local VA. This study examines the effects of Cognitive Behavior Therapy (CBT) on the vocational identity, self-efficacy, and vocational satisfaction of persons with disabilities receiving SSI/SSDI. This study was carefully planned to help persons with disabilities obtain employment. A review of the relevant literature was used to determine the need for the study and to support the study. Forty participants, all were enrolled into a vocational rehabilitation program were selected for the study. Threats to internal and external validity were taken into consideration and controlled for. Women with disabilities have different vocational rehabilitation (VR) experiences than men with disabilities. When they enter the VR system, they tend to be older, divorced, primary caregivers, more dependent on public assistance, and have less education and less work experience than their male counterparts. Given these differences, women may need to receive different treatment than men, yet the Civil Rights Act of 1964 (Pub.L. 88-352, 78 Stat. 241, July 2, 1964) and the Equal Protection clause of the Fourteenth Amendment state that men and women must be treated equally within the state-federal vocational rehabilitation system. Job retention is an important psychosocial rehabilitation goal, but one that is not often achieved. We investigate facilitators of and barriers to employment retention among homeless individuals with psychiatric and substance abuse diagnoses who were re-interviewed eight or more years after participating in a traditional vocational rehabilitation program. Most program graduates who maintained employment had secured social support from a variety of sources; personal motivation was also a critical element in job retention and compensated in some cases for an absence of social support. This monograph presents the results of secondary analysis of the RSA-911 database from the Rehabilitation Services Administration. All successful VR closures for individuals with mental retardation, cerebral palsy, and epilepsy for six data points between 1985 and 1998 were investigated. Trends in competitive labor market and extended employment (sheltered workshops) closures were examined. The use of supported employment in the VR system and its outcomes were also discussed. Findings include increased incidence of competitive labor market closures and supported employment services, with a decrease in extended employment closures.
