Objective: This study aimed to determine the usefulness of the strength of the muscles in hip abduction with flexion while seated (SMHAF-S) in older adults who need nursing care. To achieve this, we examined the reproducibility and validity of a method to measure the SMHAF-S in older adults who need nursing care using a hand-held dynamometer. Methods: This cross-sectional study enrolled 40 older women who needed nursing care in September 2021 and completed the SMHAF-S and underwent quadriceps muscle strength, grip strength, 10-s chair stand test for frail older adults (FCS-10), one-legged stance test (OLST), and timed-up-and-go (TUG) test. Introduction: Interpretations of patient-reported outcome measures following knee arthroplasty lack context and typically do not account for costs, risks and benefits compared to an alternative treatment. The primary purpose of our paper is to estimate expectations patients have for pain and function destination outcome, six-months following surgery relative to the outcome expected if knee arthroplasty was not done. Secondary purposes were to determine if statistically significant changes in the smallest worthwhile six-month outcome occurred following an interactive discussion and to assess the construct validity of the expected six-month outcome obtained at baseline.

Blood flow restriction training utilizes a tourniquet system to occlude arterial and venous flow while one performs exercises. This training strategy allows one to exercise using loads at a lower percentage of 1RM while experiencing physiological change similar to those achieved when training at a higher percentage of 1RM.

Background Information Numerous disabilities have been shown to benefit with aquatic exercise by research. There is a dearth of research on the health effects of aquatic exercise for individuals with visual impairments. Adults with visual impairments generally have a lower health-related quality of life (HRQoL) than their peers without visual impairments. Background and Purpose: Maintaining functional mobility in an acute care setting helps prevent physiologic deconditioning and prolonged hospital stays. However, not all patients qualify for skilled physical therapy (PT). This case report explores the application of a mobility program before and after a unique gastrointestinal (GI) surgery.

Case Description: A 52-year-old man required PT services on day one following surgery. He experienced severe pain post-operatively and required complex pain management. PT interventions were implemented once per day for two weeks including strength, balance, functional mobility, endurance, pain management, and safety training. Falls account for >$50 billion of medical spending in 2015 (CDC). Real-time haptic feedback shows promise at reducing static standing COM sway (Meszaros, 2019). Pilot data variability suggests that some haptic-wearers ‘over-correct’ (unnecessarily reducing available degrees of freedom), when haptic correction is perceived as ‘a mistake. Does over-correction reduce multi-segment control and impair sway stability (Hsu, Scholz, et al. 2007)?

Challenging older adults to execute peak effort functional tests is not common. A peak effort test pushes patients to the point of not sustaining the task. The Wingate test is a peak effort test that measures the amount of revolutions per second a participant can perform over 30s. Older adults with knee osteoarthritis (OA) may regulate their effort during the Wingate. Background: Neck pain is a common condition that is often difficult to diagnose. Previous literature has investigated diagnostic accuracy of examination measures, but the strength and clinical applicability are limited. This overview of systematic reviews aimed to investigate clinical features for diagnosing neck pain and its associated disorders.

Methods: An overview of systematic reviews was conducted searching four electronic databases for systematic reviews evaluating diagnostic criteria for neck pain. Quality and risk of bias were assessed using the AMSTAR 2 and ROBIS. Clinical features for neck pain were investigated for diagnostic utility.

Doctor of Physical Therapy (DPT) program faculty often enter academia as expert clinicians or researchers, but rarely as expert educators. Recommendations for faculty development exist, yet many physical therapy faculty still enter academia with little or no formal preparation for teaching and limited understanding of academe. Navigating The Layers Of Concern For Safe Dry Needling Of The Rectus Abdominis: A Cadaveric Study, Ryan C. Mcconnell, Christi L. Williams

Objectives: To assess needle placement accuracy in the rectus abdominis (RA) muscle during dry needling (DN) without entering the peritoneum.

Methods: Two physical therapists performed DN on a cadaver, making 10 attempts each to needle the RA without entering the peritoneum. Techniques followed two common DN approaches. Ultrasound verified needle depth and safety.

Results: Seventy percent of attempts were recorded as safe needle placement, while 30% were unsafe. Accurate RA needle placement without peritoneal entry occurred in 55% of attempts.

Discussion/Conclusion: Inadvertent peritoneal needle placement during RA DN poses risks regardless of experience. Ultrasound guidance may enhance safety and precision …

Background: Cortisol plays a central role in both muscle breakdown and growth. These effects are highly dependent on dose and interactions with other hormones. It is known that men and women show considerable differences in their muscle structure and physiology, which are largely driven by hormonal differences. The human hand is a powerful tool essential for performing a range of tasks from simple to complex, and it plays a crucial role in social interactions and environmental engagement. Upper limb (UL) loss disrupts these functions and significantly impacts daily life. Individuals with unilateral lower limb amputation (LLA) frequently experience intact limb knee pain, dysfunction, and osteoarthritis. These problems can worsen pre-existing reductions in mobility and quality of life that this population regularly experiences. It is widely accepted that knee joint loading is implicated in the development of knee joint issues, and intact limb knee joint load alterations are thought to be widespread in individuals with LLA. Gait training in physical therapy is a common standard of practice for new lower limb prosthetic users. Progress is typically assessed through functional outcome measures such as the 10-meter or 6-minute walk tests1. While these tests measure walking speed and endurance, they fall short of capturing gait quality or quantifying gait parameters which literature shows to be of therapeutic value 2,3. The growing need for collaborative healthcare teams to meet complex health challenges has led to physical therapists (PTs) being embedded in adult primary care settings for many years now. However, this model of care has not been found in pediatrics. This qualitative study sought to gain insights from pediatricians on the potential of embedding pediatric PTs in primary care. Participants were nine pediatricians practicing in both urban and rural, hospital-based and private settings. Semi-structured interviews were recorded, transcribed, and analyzed via thematic analysis per published methods, assuring trustworthiness. Background: Although beneficial to patient outcomes, inpatient ambulation is not a prioritized care intervention, and a task often deferred to physical therapy. Patient immobility is a complex problem that requires a multifaceted approach.

Local Problem: In a rural, critical access hospital, on an 18-bed medical surgical unit, nursing staff lacked ownership of patient mobility. There was no standardized process to document ambulation and mobility tasks were not scheduled, falling to the end of the nursing work list. Background: Older adult falls are a severe concern in terms of public health worldwide, with many assessment tools predominantly in English, pointing to the need to perform cross-cultural adaptation and validation to use the different instruments in various cultures.
