Anecdotal reports can be found in the literature of athletes who suffer from an injury, yet continue to participate in their sport with little or no pain. These reports have resulted in an interest in the possible role of stress, including exercise, in analgesia. Exercise-induced hypoalgesia (EIH), a decrease in pain perception following exercise, has been found to occur in healthy adults. In the 1970’s opioid peptides with analgesic properties were discovered (Dalayeun, Nores & Bergal, 1993). Children with hearing deficits frequently have delayed motor development. The purpose of this study was to evaluate saccular function in children with hearing impairments using the Vestibular Evoked Myogenic Potential (VEMP). The impact of the saccular hypofunction on the timely maturation of normal balance strategies was examined using the Movement Assessment Battery for Children (Movement ABC). Thirty-three children with bilateral severe/profound hearing impairment between 4 and 7 years of age were recruited from a three-state area. Approximately half of the sample had one or bilateral cochlear implants, one used bilateral hearing aids, and the rest used no amplification. Parents reported …

No head-to-head comparisons of different orthoses for patients with stage II posterior tibial tendon dysfunction (PTTD) have been performed to date. Additionally, the cost of orthoses varies considerably, thus choosing an effective orthosis that is affordable to the patient is largely a trial-and-error process. Case Description: A 77-year-old woman was seen with complaints of abnormal foot posture (“my foot is out”), minimal medial foot and ankle pain, and a 3-year history of conservatively managed stage II PTTD. Background and Purpose. This case study describes the evaluation and treatment of a patient with sacroiliac dysfunction and low back pain. The purpose of this case study is to explore the effects of pelvic clocks used as muscle energy techniques in the treatment of sacroiliac dysfunction and low back pain.
Description. The patient was a 62-year-old woman with a 5-month history of SI pain who was seen for outpatient physical therapy over a 5-week period. Background and Purpose: This case report describes the use of two different conservative treatment approaches on a patient suffering from chronic nonspecific low back pain. The case report illustrates the difference between aquatic therapy/core stabilization, which focuses on the biological aspect of the injury, and multi-disciplinary treatment, which treats the injury on a biopsychosocial level.

Objectives: To compare the effectiveness of a digital video disc (DVD) with that of a written workbook delivering falls prevention education to older hospital patients on self-perceived risk of falls, perception of falls epidemiology, knowledge of prevention strategies, and motivation and confidence to engage in self-protective strategies. To compare the effect of receiving either education approach versus no education on patients' perception of falls epidemiology.
Design: Randomized trial (DVD vs workbook) with additional quasi-experimental control group.

This handbook is the culmination of a multiphase, multidisciplinary research project that used grounded theory and participatory action research to illuminate ways that healthcare providers can work sensitively (in a trauma-informed way) with adult survivors of childhood sexual abuse.
The research identified nine Principles of Sensitive Practice: respect, rapport, taking time, sharing information and control, respecting boundaries, fostering mutual learning, understanding non-linear healing and demonstrating an understanding of trauma to patients. Specific guidelines were developed for a wide variety of issues pertinent to clinical practice such as, removal of clothing, touch, responding to disclosures of abuse, managing triggers among others. …

Background and Purpose: This case report follows a 20 year old male through his five week out patient rehabilitation for an open reduction internal fixation (ORIF) pinning of his right lunate and subsequent hardware removal following a motor vehicle accident. The patient presented with pain, decreased strength and range of motion of his right wrist. A deficit was also identified in his right shoulder stemming from decreased use of his right upper extremity. The reason for this case study is to look into benefits of physical therapy interventions as well as their functional outcomes.

Background/Purpose: This case study describes a 28-year-old woman’s six physical therapy visits over a period of four weeks for treatment of right pes anserine bursitis. The purpose of this case study is to describe the treatment of an individual with pes anserine bursitis using hamstring stretching, quadriceps strengthening, and iontophoresis and to describe the outcomes.
Case Description: Following a fall down eight steps in her home with her right knee underneath her. The patient presented with increased pain, swelling, loss of range of motion, and tenderness at the pes anserine. Treatment consisted of iontophoresis, hamstring stretches, and quadriceps strengthening.

Case Description: This 45-year-old patient went through a right rotator cuff repair surgery followed by physical therapy to regain functional use of her shoulder. Her therapy was interrupted twice; first by her mother’s heart condition and then by her own neurological condition.

The purpose of this case report is to present a return to running program for a postpartum client who had delivered via cesarean section. The client, a 29-year-old female health care professional who was 8 months postcesarean section, was referred to physical therapy for a return to running exercise program. Background: Individuals with either knee or hip osteoarthritis are at risk of experiencing pain and developing functional limitations (Bennell 2005). In many cases, to alleviate pain and improve one's quality of life, an individual may require a total joint replacement. Despite advances in surgical technique and joint replacement hardware, surgical procedures are not without risk. Conservative treatments, including therapeutic exercise programmes, may improve a patient's status while avoiding the potential risks associated with surgery (pain, medication utilisation, long bouts of rehabilitation, and/ or death). Objectives: To investigate the effect of inflation of the air bladder component of the AirLift PTTD brace on relative foot kinematics in subjects with stage II posterior tibial tendon dysfunction (PTTD).
Background: Orthotic devices are commonly recommended in the conservative management of stage II PTTD to improve foot kinematics.

Objective: Wrestling is a popular sport in the United States at both the high school and collegiate levels. Traditionally a men’s sport, participation by female athletes in wrestling is increasing. There exists a paucity of literature regarding injury incidence in women’s wrestling. This lack of information challenges the ability of sports medicine and strength training professionals to design optimal injury prevention programs, training routines, and rehabilitation strategies. Nonlinear time series analysis was used to estimate maximal Lyapunov exponents of select ankle and knee kinematics during three different conditions of treadmill walking: independent, side by side, and side by side with forced synchronization of stepping. Stride to stride variability was significantly increased for the condition in which individuals walked side by side and synchronized unintentionally when compared to the conditions of forced synchronization and independent walking. In addition, standard deviations of three kinematic variables of lower extremity movement were significantly increased during the condition in which unintentional synchronization occurred. This purpose of this study was twofold: to determine whether ultrasound transducer velocity affected tissue temperature increase, and whether heating was uniform across the treatment site. Thermocouples were inserted 2.5cm below the skin surface at the center, edge of the ERA, and the edge of the treatment site that was the size of two times the soundhead. Each subject received three 10-minute treatments at each speed, letting tissue temperature return to baseline between treatments.
Repeated measures factorial ANOVA revealed no significant differences in the speed of application and no interaction between speed and location. However, the location of the thermocouples …

Walking speed is "almost the perfect measure." This review summarizes the strong psychometric properties of walking speed and robust evidence for using this clinical measurement. Walking speed is easily measurable, clinically interpretable, and a potentially modifiable risk factor. For these reasons, using WS as the sixth vital sign is both pragmatic and essential.

It is not known whether a clinical prediction rule or a whiplash associated disorder (WAD) classification may be applied effectively in a second traumatic event to predict the outcome in a patient recovering from cord trauma. Therefore, the purpose of this case was to apply a decision rule and a whiplash classification to predict the outcome of a second traumatic event in a recovering incomplete spinal cord injured (SCI) patient who was ambulatory.

The purpose of this study was to compare the time to task failure, postcontraction hyperemia, and vascular conductance of young men and women for a submaximal intermittent fatiguing contraction performed with the handgrip muscles. Twenty men and 20 women (mean SD: 22 4 years) performed an isometric contraction at 50% of maximal voluntary contraction (MVC) (6-s contraction, 4-s rest) until task failure. Forearm venous occlusion plethysmography was used to estimate the peak blood flow (after 10-min occlusion) and blood flow at rest after 6-s submaximal contractions of varying intensities, and during an intermittent fatiguing contraction at 1-min intervals and task …

Female athletes are at greater risk for certain injuries and conditions than are their male counterparts due to unique anatomic, physiologic, and psychological factors.1-4 Injury to the female high school student athlete may significantly impact her physical and mental well-being, disrupt the athlete’s family life with medical appointments, and affect her team’s success.5 Athletic trainers and therapists (ATs) should consider these injury risk factors when developing injury prevention programs for female student athletes.

Focus of attention manipulation has been shown to have an effect on learning and performance of movement tasks. The purpose of this experiment was to determine whether inducing an external or internal focus of attention while learning a Pilates roll-up had an effect on movement technique acquisition. Movement form, movement time, and distance reached served as dependent variables. Objective: To assess the level of burnout in ATEPDs and to determine the relationship between burnout and various demographics of ATEPDs.
Design: Cross-sectional study.
Setting: Public and private colleges and universities nationwide.
Patients or Other Participants: Two hundred forty-nine ATEPDs of undergraduate athletic training education programs accredited by the Commission on Accreditation of Athletic Training Education.
Intervention(s): We administered the Maslach Burnout Inventory (MBI) to all participants.

There are widespread chemosensitive areas in the brain with varying effects on breathing. In the awake goat, microdialyzing (MD) 50% CO(2) at multiple sites within the medullary raphe increases pulmonary ventilation (Vi), blood pressure, heart rate, and metabolic rate (Vo(2)) (11), while MD in the rostral and caudal cerebellar fastigial nucleus has a stimulating and depressant effect, respectively, on these variables (17). In the anesthetized cat, the pre-Bötzinger complex (preBötzC), a hypothesized respiratory rhythm generator, increases phrenic nerve activity after an acetazolamide-induced acidosis (31, 32).
