While several studies have reported a decrement in performance by older adults while walking and concurrently performing a dual task on even surfaces, to date the effects of dual tasking while walking on uneven surfaces commonly found in the community has received less attention. Thus, we sought to test the hypothesis that an incremental decrement in gait parameters will be observed, when walking on an uneven versus an even surface and furthermore, that this decrement would be dependent upon the concurrent performance of a secondary cognitive and/or motor task in functionally independent-living-community older adults.

Background. One important objective for clinical trialists in rehabilitation is determining efficacy of interventions to enhance motor behavior. In part, limitation in the precision of measurement presents a challenge. The few valid, low-cost observational tools available to assess motor behavior cannot escape the variability inherent in test administration and scoring. This is especially true when there are multiple evaluators and raters, as in the case of multisite randomized controlled trials (RCTs). Falls remain a challenge for ageing societies. Strong evidence indicates that a previous fall is the strongest single screening indicator for a subsequent fall and the need for assessing fall risk without accounting for fall history is therefore imperative. Testing in three functional domains (using a total 92 measures) were completed in 84 older women (60–85 years of age), including muscular control, standing balance, and mean and variability of gait. One of the leading causes of unintentional death worldwide is drowning. From 2005-2009, an estimated 3, 107 people in the United States over the age of fifteen died from drowning (Peden, Oyegbite, & Ozanne-Smith, 2008). Research has found higher amounts of drowning deaths to be males than females and African-American than Caucasian (Saluja et al., 2005). However, no significant differences have been found in real or perceived water competency between gender or age groups (Moran et al., 2012). Negative results, cynicism, the piecing together of puzzles and integrating physiology with experimental approaches are all part of the inspiring and meandering journey on the path to understand ATP-mediated vasodilatation. While it is satisfying to be "right" as scientists, we know sometimes the most important progress occurs when data are surprising or contrary to our original hypothesis. The pursuit of explanations for unexpected findings often leads to the best advancements. The story of how our laboratory came to investigate the underlying vasodilatator pathways of adenosine triphosphate (ATP) is an example of such a path to our current understanding.

Fatigue in lower extremity musculature is associated with decline in postural stability, motor performance and alters normal walking patterns in human subjects. Automated recognition of lower extremity muscle fatigue condition may be advantageous in early detection of fall and injury risks. Supervised machine learning methods such as Support Vector Machines (SVM) have been previously used for classifying healthy and pathological gait patterns and also for separating old and young gait patterns. The purpose of this quality improvement project was to increase physical activity among postoperative adult liver transplant patients, improve documentation of daily activity, and ultimately influence quality of life (QOL). Quality of life has been shown to improve dramatically after liver transplant, as patients enter transplant severely debilitated with limitations on their physical activity which carry over following transplant. The literature supports that liver transplant patients should engage in physical activity which may improve QOL. Background: Evaluation of task related outcomes within geriatric and fall-prone populations is essential not only for identification of neuromuscular deficits, but also for effective implementation of fall prevention strategies. The purpose of this article is to provide a brief review of the principles of motor control and learning. Different models of motor control from historical to contemporary are presented with emphasis on the Systems model. Concepts of motor learning including skill acquisition, measurement of learning, and methods to promote skill acquisition by examining the many facets of practice scheduling and use of feedback are provided. A fictional client case is introduced and threaded throughout the article to facilitate understanding of these concepts and how they can be applied to clinical practice.

Many women who have experienced breast cancer (BC) report continued impairments in upper extremity (UE) function beyond the time required for normal healing after surgical treatment. Most research supporting this has not made comparisons between survivors of breast cancer (BCS) to a sample of healthy women. This lack of comparison to a healthy cohort prevents an understanding of whether continued deficits in UE function are due to normal aging or the BC treatment.

Background and Purpose: During intramuscular manual therapy (IMT), an acupuncture needle is inserted into the skin and muscle. The direct mechanical stimulation may interrupt the pathogenic mechanisms of myofascial trigger points (MTrPs). The purpose of this study was to demonstrate the application and efficacy of IMT on a patient suffering from right chronic elbow lateral epicondylalgia. Methods: A case study of a 26-year-old male presenting with a 6-month history of right elbow pain who failed 11 conservative physical therapy sessions and previous site-specific acupuncture. Fluctuations during isometric force production tasks occur due to the inability of musculature to generate purely constant submaximal forces and are considered to be an estimation of neuromuscular noise. The human sensori-motor system regulates complex interactions between multiple afferent and efferent systems, which results in variability during functional task performance. Since muscles are the only active component of the motor system, it therefore seems reasonable that neuromuscular noise plays a key role in governing variability during both standing and walking. Correct muscular activation of the body segments during an overhand throw is achieved when movement originates in the larger and more proximal legs and trunk and moves sequentially to the smaller, distal segments of the shoulder and arm. This sequence permits angular velocity to transfer progressively through the throw as part of an open kinetic chain. Fingertip force scaling for lifting objects frequently occurs in anticipation of finger contact. An ongoing question concerns the types of memories that are used to inform predictive control. Object-specific information such as weight may be stored and retrieved when previously encountered objects are lifted again. Alternatively, visual size and shape cues may provide estimates of object density each time objects are encountered. We reasoned that differences in performance with familiar versus novel objects would provide support for the former possibility. Recently, researchers have demonstrated that Whole Body Vibration results in significant increases in skin blood flow. No study has determined if there is an optimal frequency or duration for optimizing this response. In addition, there is no known mechanism for the increase in skin blood flow. Recent findings on motor lateralization have revealed consistent differences in the control strategies of the dominant and nondominant hemisphere/limb systems that could have implications for hemiplegic stroke patients. Studies in stroke patients have demonstrated deficiencies in the ipsilesional arm that reflect these distinctions; patients with right-hemisphere damage tend to show deficits in positional accuracy, and patients with left-hemisphere damage show deficits in trajectory control. Such deficits have been shown to impede functional performance; yet patients with severe dominant-side hemiplegia must often use the nondominant arm as the primary manipulator for activities of daily living. There are many reports about the associations between blood lipids, body mass index (BMI) and dietary cholesterol intakes both within the individual and between related individuals. The purpose of this descriptive research project was to investigate the relationships between LDL and HDL cholesterols, body mass index and dietary cholesterol intakes for a sample of African American (AA) mothers and their daughters and to attempt to separate the contribution of genetic versus environmental factors. Mother and daughter participants (n =42 and 66, respectively) were 12-14-hours fasted when blood samples were drawn, heights and weights measured, and 24 hour food recalls completed.

Young adults exhibit a longer time to task failure when performing a submaximal isometric contraction by pushing against a force transducer (force task) than when supporting an equivalent inertial load (position task). The purpose of this study was to compare the time to failure for old adults when they performed a force task and a position task with the elbow flexor muscles. Eighteen old adults (72 ± 4 years) performed the force and position tasks at 20% maximal voluntary contraction (MVC) force until task failure. Long QT syndrome (LQTS) is an independent risk factor for cardiovascular or ischemic disease mortality. Obesity as a cause for acquired LQTS in otherwise healthy individuals is rapidly gaining the attention of the scientific community. African American people have a higher incidence of obesity compared to Caucasians, yet race-specific information is not available for their prevalence of LQTS. Background and Purpose. Fear-avoidance behavior contributes to chronic low back pain. The purpose of this study was to determine whether patient education on fear-avoidance behavior added to conventional physical therapy would have significant effects on return to work status in contrast to a comparison group with only physical therapy intervention. Return to work status was measured by the length of time from the initial injury to the subject’s return to work on regular duty.

Injuries to the ankle joint are among the most common of all sport-related injuries. Figures range from 10% to 30% of all injuries and from 5% to 20% of all time-loss injuries.1-9 Athletes participating in sports such as football, soccer, basketball, and volleyball are especially at risk for this type of injury, largely because of the running and jumping involved. In 1977, Garrick3 reported that during any single year of a 6-year period at the University of Washington, at least 16% of all time-loss injuries for all sports involved ankle sprains.
