MELAS (mitochondrial encephalomyopathy, lactic acidosis, and recurrent stroke-like episodes) syndrome is a mitochondrial disorder. Patients with mitochondrial myopathies typically have exercise-induced symptoms. Thus, patients are often advised to avoid exercise, which leads to deconditioning. Currently, the concept of aerobic exercise training as therapy for mitochondrial disease is not well established. We report the case of a patient with adult-onset MELAS, who responded positively to a low intensity rehabilitation program. Low intensity training was safe and significantly improved our patient's physical fitness. During the 20-month follow-up period, there was no significant reduction in muscle strength and function. Limb compression in an unattended comatous patient often results in peripheral nerve injuries; however, bilateral sciatic neuropathy in these patients has rarely been described in literature. A 37-year-old woman with bilateral numbness and paralysis of the lower limb after drug overdose with coma was reported. After onset of unconsciousness, she was in a seated position with trunk and hips flexed and knees nearly fully extended for approximately ten hours. In the department of emergency, she was found to have bilateral thighs and buttocks swollen as well as myoglobulinuria. The causes of cerebrospinal fluid leakage leading to intracranial hypotension are generally iatrogenic or due to trauma. Postural headache is usually a characteristic symptom of intracranial hypotension, although other presenting neurologic symptoms occur, such as cranial nerve injury, vomiting, dizziness, photophobia, ataxia, dementia, coma, or death. A spontaneous cause for intracranial hypotension is uncommon. Intracranial hypotension with ischemic stroke, as described herein, has not been previously reported.A 43-year-old man complained of intermittent headaches for approximately one year, but the symptoms remitted with bedrest. Rotator cuff tear is one of the most common causes of shoulder disability in middle-aged and elderly people. Although many patients with rotator cuff tear have pain and dysfunction, there is a great variability in the clinical manifestations. The management of rotator cuff tear includes non-operative and operative methods. Acute disseminated encephalomyelitis is a monophasic disorder of unknown pathogenesis, characterized by demyelination of the central nervous system. The possible T-cell immune-mediated inflammatory process, affecting mainly children and young adults, typically follows a recent infection or immunization. Meningeal symptoms are common early in the course and often begin 1-3 weeks after infection. Abnormalities of motor, sensory, gait, visual, and cognitive function are variable and depend on the location (cerebral white matter, brainstem, cerebellum, or spinal cord) of the most severely damaged areas of the nervous system. Setting: Inpatient rehabilitation unit at a university hospital

Patient: 37-year-old male with diabetic myonecrosis.

Case Description: The patient had a long-standing history of uncontrolled diabetes mellitus with multiple comorbidities, including end-stage renal disease on dialysis and diabetic myonecrosis of the left biceps femoris diagnosed by biopsy and magnetic resonance imaging (MRI.) On this admission, he presented with right leg pain and swelling, found to be a reoccurrence of diabetic myonecrosis in the vastus lateralis, medialis, and intermedius, diagnosed by MRI only. Prior to admission, he lived alone in a wheelchair inaccessible duplex and required minimal assistance with housekeeping.

Recent findings on motor lateralization have revealed consistent differences in the control strategies of the dominant and nondominant hemisphere/limb systems that could have implications for hemiplegic stroke patients. Studies in stroke patients have demonstrated deficiencies in the ipsilesional arm that reflect these distinctions; patients with right-hemisphere damage tend to show deficits in positional accuracy, and patients with left-hemisphere damage show deficits in trajectory control. Such deficits have been shown to impede functional performance; yet patients with severe dominant-side hemiplegia must often use the nondominant arm as the primary manipulator for activities of daily living. Word-retrieval treatment studies in aphasia have reported the greatest influences on picture naming for trained words. To increase treatment effects to untrained words and sentence contexts, we investigated a sentence-reading treatment hierarchy that moves from errorless to generative production of sentences incorporating target nouns and verbs. In an individual with nonfluent aphasia, treatment resulted in improved picture naming for nouns and verbs and generalized increases in numbers of grammatical sentences and content words following noun therapy. A second individual with fluent aphasia improved little in picture-naming and sentence-generation tasks for both nouns and verbs. This sentence-based word-retrieval training, in which …

This thesis investigated the experiences of men diagnosed with localised prostate cancer, as they reconstructed their lifeworlds during the first post-diagnostic year. With the exception of health related quality of life, a review of the psychosocial research literature revealed few studies that, explored the psychosocial experience of men diagnosed with localised prostate cancer. Furthermore, the review uncovered no studies that explored the process of Iifeworld reconstruction, and only three studies that considered the role of masculinity in the responses of men to the prostate cancer experience. Churg-Strauss syndrome (CSS) is a rare systemic vasculitis of unknown cause, occurring in patients with asthma and accompanied by hypereosinophilia. Pathologically, CSS is characterized by necrotising eosinophilic vasculitis which may involve multiple organ systems, commonly including the lungs, skin, and peripheral nervous system. Corticosteroids are the first-line therapy, with cyclophosphamide as adjuvant therapy. The overall prognosis is good.We report a case of CSS with the clinical pattern of asthma, hypereosinophlilia (>10%), pulmonary infiltration, and mononeuritis multiplex. Progressive weakness and numbness in the bilateral hands and lower limbs, with associated walking inability, remained a concern despite steroid treatment. The patient underwent …

Cerebellar stroke is uncommon in young patients. We report the case of a 22-year-old man with a delayed cerebellar infarction secondary to vertebral artery dissection caused by trauma. While riding a bicycle as part of an extreme sport, his left clavicle struck a bicycle handle. Evaluation in the emergency room led to a diagnosis of left shoulder contusion without fracture of clavicle or cervical spine. Headache, severe vomiting, and weakness of the right-sided limbs developed three days later, and he presented to emergency staff again. Objectives: Muscle stiffness increases during muscle contraction.The purpose of this study was to determine the strength of the correlation between myotonometric measurements of muscle stiffness and surface electromyography (sEMG) measurements during various levels of voluntary isometric contractions of the biceps brachii muscle. Subjects: Eight subjects (four female; four male), with mean age of 30:6 ± 8:23 years, volunteered to participate in this study. The report discusses views and perspectives of American Indialaska Native people with disabilities, tribal leaders, and federal agency representatives identified as productive in meeting the needs of people with disabilities residing in tribal lands. Research objectives. One source of falls in the elderly may be an inability to sufficiently adjust to transient postural perturbations or slips. Identifying useful predictors of fall potential, as well as factors that affect the ability of an individual to detect a movement of the standing support surface may provide insight into postural stability and methods to increase stability in elders. To do this, acceleration thresholds to short, precise, lateral platform translations and the resultant psychophysical responses of adults with early Type 2 diabetes to age-matched controls and young adults were measured.

The purpose of this study was to assess the reliability and validity of a mobility obstacle course for women with mobility impairments. Participants included 72 adult women with permanent physical disabilities including arthritis, orthopedic conditions, paraplegia, and others. The 60-m course consisted of carpeted runways and turns, ramps, a doorway, a transfer, and object manipulation. Participants completed two trials, walking or wheeling through the course as quickly as possible, safely and without running. Total course time and peak heart rate data were correlated with SF-36 health survey subscales. Overall, peak heart rate was significantly (p < .05) correlated with physical functioning (r = -.328), limitations due to physical functioning (r = .261), and pain (r = .296). Total course time was significantly correlated with physical functioning. These findings indicate very high reliability and preliminary evidence of validity.

Comparison between successful and failed sit-to-stand trials of a patient after traumatic brain injury.

Objective: To compare the peak whole-body center of mass (COM) velocities and joint angular contributions in successful and unsuccessful sit-to-stand (STS) trials in a subject with traumatic brain injury (TBI).

Design: Single-case study.

Setting: Motion research laboratory.

Participant: A 24-year-old man who was 3.5 years post-TBI.

Interventions: Not applicable.

Main Outcome Measures: Peak horizontal and vertical velocities of the whole-body COM and peak angular velocities of the ankle, knee, hip, and shoulder joints.

The author became interested in spirituality and rehabilitation during her summer 1999 employment at the Pennsylvania State Correctional Institute-Graterford and as a legal intern in the law firm of Woody and Falkenbach, which specializes in criminal defense work, during the summers of 1991 to 1998. The article focuses on the role of spirituality in rehabilitation processes in correctional settings. It pays special attention to the sources offaith and inner strength, the nature of spiritual guidance, the roles of values, beliefs, and moral commitments, and the effects of cultural, social, political, and economic forces.

Jogging is commonly performed in the latter stages of the rehabilitation program (2½-3 months) following ACL reconstruction. However, its cyclic loading effect on the ACL graft, 4-6 months after surgery has not been examined. Therefore, the purpose of this study was to examine changes in anterior knee laxity before and after 15 minutes of jogging on the treadmill. Subjects (N=l 1) were 8 males and 3 females (X age= 30.27 ± 4.76 years; heights= 175.7 ± 10.7 cm; weights= 81.1 ± 15.9 kg) with unilateral ACL reconstruction (central 1/3 patellar tendon graft, postoperation range = 4-6 months). OBJECTIVE: This article describes the methods occupational therapists use to teach bathing and dressing device use to older patients in rehabilitation programs. The relationship of three patient characteristics to five aspects of assistive device instruction was examined.

METHOD: The study sample included 86 patients and 19 occupational therapists who provided the assistive device training. Patients were 55 years of age or older and in rehabilitation for an orthopedic deficit, cerebrovascular accident, or lower limb amputation. Therapists recorded information on teaching methods, perceptions of patient knowledge, and expectations for future device use after each treatment session.

The development of occupational therapy is rooted in early 20th century medical reform. During the early 1910s, several members of the medical profession, human service workers, and the larger American society were increasingly disturbed by medical practices that did not consider the individual's personal experience of disability. Occupational therapy was developed, in part, out of this desire to provide persons with treatment that helped them to function in their communities despite their disability. The purpose of this study was to compare medical quality of department of rehabilitation in Northern Taiwan area by using a new evaluation form designed by quality-oriented model for hospital accreditation. In this study, the questionnaire was mailed to twenty-six teaching hospitals in this area and was responsed from twenty-three hospitals including seven medical centers, eleven regional teaching hospitals and five district teaching hospitals (responsible rate 88.5%).
