People with chronic LBP display an altered movement pattern where the lumbar spine moves more readily than other joints that can contribute to the activity. The pattern is of particular clinical relevance because across multiple studies the magnitude of altered pattern is associated with LBP and function. One session of motor skill training (MST) during functional activities can improve the altered pattern. However, of the few studies investigating MST for people with LBP, none have examined the short-term or long-term effects of MST on the altered pattern. Urgency and frequency are common lower urinary tract symptoms (LUTS) in women and are accompanied by substantial activity and participation restrictions. Medical management begins with ruling out organ pathologies, typically followed by fluid intake and timed voiding often combined with pharmacologic management, and may progress to nerve stimulation, botulinum injections, or surgery where indicated. Despite abundant management options for urgency and frequency predominant LUTS (UF-LUTS) and high financial burden, patients often experience only partial relief from medical treatments for LUTS. This dissertation presents a series of studies into human reach and grasp, focusing on the neural systems and behaviors of upper-limb action that underly performance under varied sensory conditions: specifically, acting with and without visual feedback of the limb and under typical or impaired proprioceptive sensation (proprioceptive decline with aging and proprioceptive deficit following stroke). Under typical conditions, a combination of visual and non-visual (e.g., proprioception) sources of information are used to guide action. People with diabetes mellitus and peripheral neuropathy (DMPN) have midfoot and ankle musculoskeletal problems, including limited joint mobility and weakness and atrophy of foot intrinsic and calf muscles. Impaired foot structures and function could lead to midfoot and ankle movement dysfunction, measured by a heel rise task. A repeated movement dysfunction during weightbearing tasks (e.g., heel rise, walking) could cause excessive stress on the plantar tissue, which is a leading cause of plantar ulceration in people with DMPN. Parkinson disease (PD) is a neurodegenerative movement disorder characterized by motor complications such as gait deficits and is caused by the depletion of dopamine producing neurons in the basal ganglia (BG). Gait deficits, including decreased velocity and increased variability, are among the most debilitating symptoms of PD and lead to an increased risk of falls. Upper limb pain and injuries are prevalent among manual wheelchair users and can restrict their participation and daily activities. Due to the high repetition and force in wheelchair propulsion, chronic wheelchair propulsion has been linked to the risk of upper limb pain and injury. Prevention of upper limb pain and injury is a high priority in wheelchair-related research. Decades of research in wheelchair propulsion biomechanics have led to clinical practice guidelines (CPG). Stroke is the leading cause of long-term disability in the United States. Hemiparesis, or weakness on one side of the body, is a common impairment following a stroke. Approximately 80% of individuals with stroke will experience upper limb paresis, with only a small percentage regaining full functional use of their paretic upper limb. Individuals report ongoing difficulties with incorporating their paretic upper limb into routine activities after a stroke. Falls are a growing public health problem among the aging population. Structured exercise programs are effective in preventing falls, and many have been implemented for community-dwelling older adults in the U.S. However, low adherence rates for structured exercise are reported consistently for this population. The need to translate alternative evidence-based exercise programs the diverse older adult population is important. Parkinson disease (PD) is a neurodegenerative disease with multiple motor and non-motor symptoms, including postural instability, gait impairments, and cognitive deficits. More than 50% of individuals with PD experience a symptom called freezing of gait (FOG), described as a transient inability to take another step forward. Individuals with PD who experience FOG (freezers) have further postural, gait, and cognitive impairments compared to individuals with PD without FOG (non-freezers). 1-in-3 persons with type 2 diabetes (T2DM) develop chronic kidney disease (CKD), which is characterized by progressive renal dysfunction leading to end-stage renal disease. In response to elevated blood glucose and systemic inflammation of diabetes, a process of active thickening of the renal glomerular basement membrane ensues with concomitant damage to the structural supports (podocytes) of the kidneyճ filtration barrier. Prediabetes is a metabolic condition defined by elevated fasting (impaired fasting glucose (IFG)) and/or postprandial (impaired glucose tolerance (IGT)) glucose. Prediabetes affects nearly 86 million adults in the United States, with most (up to 70%) progressing to type 2 diabetes within as little as one year. Recent studies have indicated that elevated post-prandial glycemia and hypertriglyceridemia are early indicators of prediabetes, and are major risk factors for complications of prediabetes, including cardiovascular disease (CVD), stroke, elevated blood pressure, and obesity. The majority of persons living with chronic stroke experience some form of upper extremity motor impairment that affects their functional movement, performance of meaningful activities, and participation in the flow of daily life. Stroke survivors often compensate for these impairments by adapting their movement patterns to incorporate additional degrees of freedom at new joints and body segments. Executive function is central to our ability to learn and participate in everyday life activities and rehabilitation outcomes for individuals with executive dysfunction after neurological injury are poor. The impairments and performance challenges these individuals experience are typically not identified appropriately so they often do not receive adequate rehabilitation and can have significant challenges returning to complex everyday life activities.
