The Tennessee Division of Mental Retardation Services (DMRS) implemented the Employment First! initiative in 2002. The goal of Employment First was to make employment the first day service option for adults receiving supports funded by DMRS, Medicaid, or the state. Employment First set the standard that employment was the preferred service option for adults with mental retardation and developmental disabilities (MR/DD).

Over six years, regulations in Vermont gradually restricted and eventually prohibited the use of state funds for sheltered workshops or enclaves. Concurrent with the change in funding regulations, the state worked with providers to convert the remaining sheltered workshops in Vermont.

Vermont’s Division of Disability and Aging Services (DDAS) and Division of Vocational Rehabilitation (DVR) worked with a local service provider to convert its congregate day services to community employment. Implementation Between 1987 and 2002, DDAS and DVR worked with several providers to close down sheltered workshops and move people into community supports.

New Hampshire implemented an innovative technical assistance model that promoted organizational change to expand individual employment opportunities. This person-to-person change began at the micro level but “trickled up” through organizations across the state. In 2000, realizing that the state’s growth in integrated employment had stalled, the Bureau of Developmental Services invested aggressively in expanding its intervention strategy by recruiting a community provider to work directly through the bureau. The bureau hired John Vance-the director of ACCESS, a small individualized support organization in New Hampshire-half-time to provide individualized technical assistance to providers across the state. Washington’s Division of Developmental Disabilities (DDD) has recently issued a new policy which went into full effect on July 1, 2006. This policy “designates employment supports as the primary method of furnishing state-financed day services to adult participants.” Emphasizing community employment as the primary service option, the policy further states that: “services for persons under the age of 62 that do not emphasize the pursuit or maintenance of employment in integrated settings can be authorized only by exception to policy” (WA DHSH, DDD, “County Services for Working Age Adults” Policy 4.11). The Tennessee Employment Consortium (TEC) is a statewide organization focused on increasing the number of Tennesseans in integrated employment. The consortium comprises volunteers from the state’s Division of Mental Retardation Services (DMRS) and Division of Rehabilitation Services (DRS), the Tennessee Council on Developmental Disabilities, the ARC of Tennessee, the Center on Disability and Employment at the University of Tennessee, community rehabilitation providers (CRPs), family members, and other stakeholders. TEC’s ability to organize collaborative activities across state agencies, advocacy organizations, and CRPs has played an important role in increasing integrated employment outcomes.

According to the National Athletic Trainers’ Association, “In the last 10 years, college sports have flourished, with athletes required to train and compete year-round rather than seasonally . . .At the same time, athletes are getting bigger, stronger and more physical – which leads to a greater risk of injury.” Sports injury can be traumatic for many athletes because it is an important component of their self-identity. There are many reports about the associations between blood lipids, body mass index (BMI) and dietary cholesterol intakes both within the individual and between related individuals. The purpose of this descriptive research project was to investigate the relationships between LDL and HDL cholesterols, body mass index and dietary cholesterol intakes for a sample of African American (AA) mothers and their daughters and to attempt to separate the contribution of genetic versus environmental factors. Mother and daughter participants (n =42 and 66, respectively) were 12-14-hours fasted when blood samples were drawn, heights and weights measured, and 24 hour food recalls completed.

Beliefs and practices concerning death have changed throughout human history. In pre-modern times, death at a young age was common due to living conditions and medical practices. As medical science has advanced and helped humans live longer, attitudes and responses to death also have changed. In modern Western societies, death is often ignored or feared. Changes in lifestyles and improved medical science have depersonalized death and made it an encroachment on life instead of part of life. This has left many people ill equipped to deal with death when it touches their lives.

Child abuse is a very serious issue around the world. An estimated 906,000 children were determined to be victims of child abuse or neglect in the United States in 2003. Child fatalities are the most tragic consequence of maltreatment. Play therapy provides children with the ideal environment where they feel safe and they may experience their feelings without restriction.

This study is an examination of the professional socialization process of occupational therapy students from a role identity theoretical perspective. First-year students, second-year students, fieldwork students, and faculty at an occupational therapy educational program at a Midwestern institution volunteered to participate in the study. Data were collected through focus group interviews and surveys. The focus of the study was to determine factors associated with occupational therapy students’ identity salience, commitment, and role-person merger. Objective: The purpose of this study was to determine the effects on the Hoffman reflex and motor response with an analgesic (Biofreeze®). Design: The study was a 2X6 design. The independent variables in this study were treatment and time. The dependent variables were the H-reflex (volts) and the M response (volts). Treatment was randomly assigned to control for the effects of order. Setting: This study took place in the Exercise Physiology Laboratory located in the Health and Human Performance Building at Indiana State University. Children and adolescents with a traumatic brain injury (TBI) were the focus of this investigation using an extant data set. The 3 7 participants were between the ages of 5 and 15 years at the time of injury, and the majority had a severe TBI. Participants were evaluated soon after injury for a Baseline description of functioning on measures of cognition and memory and re-evaluated at one and/or two year return appointments. We examined congruence between chronic pain patients and their spouses on their reports of patient pain severity, patient disability, and spouse responses to pain. Patients reported that they were more physically and psychosocially disabled than their spouses reported them to be. However, spouses reported that the patients’ pain was more severe than patients reported. Depressive disorders in the patient and gender interacted with patient–spouse ratings. For physical and psychosocial disability, depressed patient couples reported significantly larger differences in disability ratings than nondepressed patient couples. This project is a close study of low back pain that women experience during pregnancy or postpartum. This study includes anatomy of the lower back, factors that affect Lumbar Lordosis, and physical and psychological changes during pregnancy. This project also includes an in depth analysis of lower back pain during pregnancy which includes symptoms, types, causes and risk factors. This paper demonstrates how physical therapy treats low back pain during pregnancy, labor and delivery, and postpartum and also includes a glossary, pictorial appendix and brochure.

Long QT syndrome (LQTS) is an independent risk factor for cardiovascular or ischemic disease mortality. Obesity as a cause for acquired LQTS in otherwise healthy individuals is rapidly gaining the attention of the scientific community. African American people have a higher incidence of obesity compared to Caucasians, yet race-specific information is not available for their prevalence of LQTS. Chronic physical activity can help reduce the incidence of obesity, yet little is known about the effects of chronic physical activity on acquired LQTS. Understanding the differences between the male compulsive gambler and the female compulsive gambler can play an important role in the healing process of the female compulsive gambler. This manuscript describes these differences and discusses treatment methods and issues specific to the female pathological gambler including the Twelve Step program for gamblers and cognitive treatment methods. Marital Functioning, Chronic Pain, And Psychological Distress, Annmarie Cano, Mazy Gillis, Wanda Heinz, Michael Geisser, Heather Foran

This study examined whether marital functioning variables related uniquely to psychological distress and diagnoses of depressive disorder independent of pain severity and physical disability. Participants were 110 chronic musculoskeletal pain patients. Hierarchical regression results showed that marital variables (i.e. marital satisfaction, negative spouse responses to pain) contributed significantly to depressive and anxiety symptoms over and above the effects of pain severity and physical disability. In contrast, marital variables were not significantly related to diagnoses of depressive disorder (i.e. major depression, dysthymia, or both) after controlling for pain variables. In multivariate analyses, physical disability and marital satisfaction were uniquely related to …

The purpose of this study was to determine the relationship between a set or psychosocial variables and a set of physiological variables for a cohort of female cardiac rehabilitation patients. The independent variable set included initial denial, inlcnwl locus of control, perceived mental health status, and perceived physical health status. Dependent variables included change scores for metabolic equivalents and resting heat rate. The sample consisted of76 women who were enrolled in a Phase II cardiac rehabilitation program. We hypothesized that Chinese Americans with disabilities may remain culturally attached to their ancestors' homeland and this cultural attachment may have influences on the concept of Quality of Life (QOL). That is, QOL may be perceived, by Chinese Americans with disabilities, not only as an individual's satisfaction with his or her life, but also the person's fulfillment of his or her responsibilities to his or her family and community. Play therapy is a current trend in school and mental health counseling that takes into account the importance of play for a child's self -expression. It is primarily used with children ranging from three to ten years in age (Knell, 1995).
