Ghana’s population of 24 million is projected to reach 31.8 million in 2025. This population growth is coupled with rapid urbanization. Urban adolescent females experience a rapid decline in school enrollment, often because they need to provide economic support to families. Girls living apart from families and out of school often lack social networks and have few opportunities to build the critical skills needed for adulthood. In the past decade, there have been expanding resources to address the underlying gender dimensions of HIV. This has been particularly urgent in sub-Saharan Africa as the female-to-male infection ratios in young populations has reached 3 to 1 and sometimes above. The phrase “gender and HIV” has become commonplace yet does not provide any specific guidance as to target audiences, content, or measurable results. It can include everything from microcredit programs for HIV-positive women, to workplace programs seeking to change negative male norms, to efforts to increase respect for diverse sexual and gender identities. This report is a midterm evaluation and documentation of the process and approach of the FGM-Free Village Model implemented in Egypt by the National Council of Childhood and Motherhood with assistance from the Population Council. In a comparison of responses from women and men in intervention groups to those in nonintervention (control) villages, data analysis shows that the program has been successful in changing views and attitudes toward female genital mutilation (FGM). FGM is an entrenched generational practice, and eradicating it in a community requires concerted effort over an extended period of time. This evaluation strongly recommends that efforts be …

The program presented in this case study, the Association for the Struggle Against Violence Against Women (ALVF, from its name in French) in Cameroon, provides counseling, language and literacy training, economic support, and empowerment activities for girls who have fled (or been thrown out of) early or forced marriages to help them overcome their adversities. ALVF also seeks to bring about broader social change by highlighting in the public sphere—as well as among parents and husbands—the plight of girls who were married early and/or by force. Responding to a dearth of data on youth in Egypt, the Population Council conducted a comprehensive situation analysis of Egyptian adolescents and young people: the Survey of Young People in Egypt (SYPE) 2009, which covered a nationally representative sample of 15,029 young people aged 10-29. SYPE collected data on the five key life transitions of education, work, family formation; health; and civic and political participation. Education on sexuality and HIV can help adolescents develop the capacity for healthy, respectful relationships and protect themselves from unwanted and unsafe sex, unintended pregnancy, and sexually transmitted infections. A 2015 Population Council study found that sexuality and HIV education programs that address gender and power in intimate relationships are five times more likely to be effective than programs that do not. In Ethiopia, the Ethiopia Ministry of Youth and Sports, the Amhara Regional Bureau of Youth and Sports, and the Population Council implemented programs for vulnerable girls. As these programs increased in popularity, demand for support programs arose from husbands. Qualitative, in-depth interviews were undertaken among adolescents, men, and women in the Amhara Region, exploring the roles of men and women, risk-taking behavior, and patterns of HIV and reproductive health vulnerability. In 2001, UNICEF initiated a pilot intervention to test whether livelihoods opportunities could ameliorate early marriage and other adverse outcomes for girls in rural Bangladesh. Kishori Abhijan (“Adolescent Girls’ Adventure”) aimed to lower school dropout rates, increase girls’ independent economic activity, and raise the age at which girls marry. Life-skills training consisted of enhancing self-esteem and leadership skills and providing education related to gender roles and discrimination, health and nutrition, and legislation and legal rights, particularly early marriage and girls’ and women’s rights. Almost 1 billion people around the globe live in urban informal settlements, or “slums.” In Kenya, 60 percent of Nairobi’s nearly 3 million inhabitants live in slum areas characterized by high levels of poverty and HIV. Kibera is the largest of these slums. Life is particularly perilous for adolescent girls in slums, as they make their transition to adulthood. Girls are several times more likely than their male age-mates to become infected with HIV, and sexual initiation for girls often occurs in the context of violence and coercion. Education, particularly at the primary level, has long been regarded as an essential input of social and economic development. In addition to its role in increasing the cognitive abilities of students, schooling is recognized as a facilitator of labor force participation, female empowerment, improved health outcomes, and greater civic engagement. As one of the early pioneers in sub-Saharan Africa of free primary education, Malawi’s enrollment figures compare favorably with those of its neighbors in southern Africa. This Population Council working paper describes the opportunity cost of women’s participation in the labor market in Egypt, a critical determinant of not only participating in the labor market, but also of continuing in it. The paper observes career mobility and job-to-job turnover by gender; it also looks at career development over a period of time and analyzes the impact of different factors on career development. The importance of the results increases with the privatization trend in the Egyptian economy. This study evaluates the difference between male and female job search behavior in the Egyptian labor market and the changes in this behavior between 1998 and 2006, in order to examine the effect of transition toward a market-oriented economy on job search activity. The paper also investigates the determinants of women’s job search behavior. In Bangladesh, maternal health programs are not yet reaching the desired level of facility-based obstetric care service. Most deliveries are conducted by untrained persons at home, demonstrating inequity in access to recommended maternal health care services and underutilization of existing obstetric and newborn care services. Suboptimal performance by providers is a key barrier to improving availability and quality of maternal, newborn, and child health (MNCH) services. To increase use of facility-based services, an operations research study of “pay-for-performance” (P4P) incentives for providers and subsidization of consumer costs was initiated in 2010. To improve access to, and use of, facility-based obstetric and newborn care services in rural areas, the Government of Bangladesh is implementing two innovative performance-based financing programs: demand-side financing (DSF) and pay-for-performance (P4P). Both programs have contributed to the increase in institutional deliveries, yet not enough women receive the recommended care during pregnancy and delivery. DSF and P4P are implemented in parallel and have their own merits and limitations. In Bangladesh, first-level fixed-facility services—provided at the union level through Health and Family Welfare Centers (HFWCs)—are designed to improve maternal and child health by making services available in rural areas. However, most HFWCs do not have the capacity to provide normal delivery services. HFWCs could help women receive free normal delivery services within a convenient distance from their home, if they were strengthened with necessary human resources, infrastructure, and equipment. For a long time, the reduction of the maternal mortality ratio has constituted a major challenge for the health, nutrition, and population sector of Bangladesh. The objectives of this Population Council study is to estimate the costs of the additional inputs required for strengthening Health and Family Welfare Centers (HFWC) for providing normal delivery and newborn care services, the cost-effectiveness, and the financial implications of the addition of capacity. Results clearly show that the benefit-cost ratio for the intervention will be quite high. Some additional measures on the demand side should thus adopted. To improve access to and use of facility-based obstetric and newborn care, the Government of Bangladesh is implementing two innovative performance-based financing programs, namely demand-side financing (DSF) and pay-for-performance (P4P). With the purpose of identifying the lessons learned, limitations of the P4P and DSF models, and scopes for cross learning, a two-day workshop was organized in Dhaka. This workshop report, prepared by the Population Council, resulted in several recommendations to modify DSF and P4P schemes. In partnership with a number of local stakeholders, the Population Council Viet Nam is working to improve the evidence base available to health authorities and to use that evidence to improve health in Thai Nguyen Province. Building on cooperation between the Population Council and the Thai Nguyen Health Department, the project detailed in this report has three main elements: 1) developing a computerized information system to generate regular reports; 2) supporting research in the province to better understand health problems, health-seeking behavior, and health services; and 3) increasing the capability of managers and providers to better use the evidence generated. …
