Youth aged 15–24 represent 20 percent of the Indian population. This cohort faces significant risks related to sexual and reproductive health (SRH), and many lack the knowledge and power to make informed SRH choices. Information on intimate partnerships among youth, female and male, married and unmarried, is sparse and evidence is practically nonexistent that identifies the factors that protect young people's ability to ensure safe SRH and their autonomy to make informed decisions. This brief was commissioned by the Population Council to complement its two previous publications on the TRY project. The two previous publications contain data and information essential for understanding the project and its results, although they do not analyze the microfinance component in depth. In contrast, this brief was written by microfinance practitioners for a microfinance audience. In 1998, the Population Council and K-Rep (a Kenyan microfinance institution) developed a microfinance approach to address livelihood-strategy constraints for adolescent girls at risk for HIV/AIDS in an urban slum in Kenya. Measuring Women's Work: A Methodological Exploration, Ray Langsten, Rania Salem

In this working paper, the authors contrast two approaches to the measurement of women’s work applied to the same population of ever-married women. These women were interviewed on two occasions—first during the 2003 Interim Egypt DHS, and again during the Slow Fertility Transition (SFT) survey conducted in 2004. The DHS uses a standard keyword question to measure work, while the SFT employs an activities list question format. The authors argue that the widely used keyword approaches to measuring women’s work underestimate the level of female labor force activity. The 7.6-magnitude earthquake on October 8, 2005, caused a large number of deaths, and left more than 100,000 people injured and an estimated 3 million people displaced or homeless. To assist the Government of Pakistan in the preparation of a rehabilitation plan, the first step was to identify the vulnerable people living in the tent camps. The Population Council completed an assessment survey of the camps in the earthquake area. This assessment is distinctive from the surveys carried out previously, since it collects socio-demographic and vulnerability indicators about each individual. Tap and Reposition Youth (TRY) was a multiphase initiative undertaken by the Population Council and K-Rep Development Agency (KDA), the oldest and largest microfinance institution in Kenya. The overall aim of the project was to reduce adolescents’ vulnerabilities to adverse social and reproductive health outcomes, including HIV infection, by improving their livelihoods options. The project was launched in low-income and slum areas of Nairobi, Kenya, where rates of HIV infection are alarming and where young women are disproportionately affected.

Nairobi, Kenya is home to one of the largest slum populations in sub-Saharan Africa. Adolescents living in these slums, especially girls, are cut off from the potentially protective environments of school and family and lack safe economic opportunities. Unemployment is a major concern for adolescent girls, and desperate circumstances force them to survive however they can: 21 percent of sexually active girls aged 15–19 report exchanging sex for money or gifts. This heightened vulnerability is reflected in rates of HIV infection. The Self Employed Women’s Association (SEWA), a key Indian micro-finance nongovernmental organization, in partnership with the Population Council, undertook a pilot livelihoods skill-building program to support adolescent girls aged 13–19 years (mostly unmarried, in school and out of school). Like other such programs, this livelihoods program focused on exposing girls to new ideas, building social networks, exposing them to the world around them, and providing the technical skills needed to enhance their access to income generating opportunities. This working paper examines the distribution of household wealth and tests whether associations exist between wealth inequality and health outcomes among older adults living in one of the world’s poorest regions, rural Cambodia. Results confirm difficult economic conditions among most elderly in rural Cambodia. This study suggests there is some validity to generalizing the relationship between wealth inequality and health to extremely poor populations and that a very small difference in wealth makes a relatively large difference in regard to the association with health among those living in impoverished surroundings.

Systematic screening is a strategy to integrate reproductive health services at the provider level. Integration is the proactive provision of multiple reproductive health services in the same facility at the same time. Systematic screening is a simple intervention to increase the number of services received at a single client visit. The Somali community living in Kenya has practiced the severest form of female genital mutilation/cutting (FGM/C), infibulation, for centuries. To understand the context within which the practice takes place, and how its complications are managed, FRONTIERS undertook a diagnostic study in North Eastern Province and in the Eastleigh area of Nairobi. The study found that the health system is ill equipped to serve women who have been cut, and particularly infibulated women who are pregnant and delivering, stemming from an overall weakness in the availability and quality of maternal and neonatal heath services. This article explores efforts to encourage abandonment of female genital cutting (FGC) in Senegal, Burkina Faso, and Mali. The strategy of encouraging traditional practitioners to stop performing FGC is not effective as community-level support for the practice remains high. Training health staff significantly improved healthcare providers' levels of knowledge about FGC and its complications. Consequently, it was recommended that such training be incorporated into preservice medical training. The use of community-based strategies like the Village Education Program (VEP) showed the most promise, as their multifaceted approach addresses knowledge, attitudes, actions, and communal support in an integrated manner. The Population Council’s Frontiers in Reproductive Health (FRONTIERS) program, in collaboration with the Bangladesh Ministry of Health and Family Welfare, the Urban Family Health Partnership, and two nongovernmental service delivery partners, carried out the Global Youth project in northwestern Bangladesh from 1999–2003. The important lesson learned from that project was that reproductive health education could increase reproductive health knowledge in adolescents, particularly in areas related to reproductive biology, family planning, pregnancy, sexually transmitted infections, HIV, and AIDS. The Christian Health Association of Ghana (CHAG) is a large faith-based NGO which currently serves an estimated 35 percent of the Ghanaian population, mainly in remote rural areas. This study built capacity within the CHAG secretariat to calculate the economic cost and cost recovery levels of selected reproductive health services in four CHAG-affiliated hospitals. Techniques learned in the study are applicable to most costing problems, not just to reproductive health. Information obtained in the study forms the basis for negotiating reimbursement under the Ghana National Health Insurance Scheme, and for setting cost recovery and containment policies. The main objective of this study, undertaken by Noguchi Memorial Institute for Medical Research in collaboration with the Ghana Health Service and FRONTIERS, with USAID funding, was to examine the extent to which adaptation of the WHO focused antenatal care (ANC) package influenced quality of care received by pregnant women and its acceptability to both providers and clients. The results indicate that national-level support for focused ANC is high. The objectives of this study were to assess the Kenyan Ministry of Health’s capacity to adapt focused antenatal care (ANC) at the service delivery level so that it can be sustained, and to examine the extent to which adaptation of the package has increased coverage and quality of key ANC services and the overall quality of care received by pregnant women. The organization faces a number of challenges related to sustainability: declining donor support (especially for reproductive health services), low cost recovery levels, and increasing poverty levels among its clientele. This project contributed significantly to the capacity-building of regional professionals in planning, implementing, and monitoring of reproductive health programs in South Asia. During 2001–05, professionals from 17 countries received training in various aspects of reproductive health in nine workshops, including operations research, economic evaluation, qualitative research methods, proposal writing, and process documentation and enhancing the utilization of research findings in reproductive health programs. Forty-three percent of workshop participants were program managers from government health programs and nongovernmental organizations.
