This evidence brief presents results of a study aimed to assess the acceptability and feasibility of two proposed solutions for strengthening the content and delivery of in-school sexual and reproductive health (SRH) programs in Ghana. The study was conducted in Nima, a suburb of Accra, where stakeholders agreed there was a need for enhanced SRH services in school. The study explored providing comprehensive in-school SRH education to adolescents using trained psychologists and health workers to deliver and explain comprehensive sexuality education to adolescents and link them as needed to outside services. Establishing and maintaining well-functioning supply chains that meet the needs of public and private sectors, as well as health professionals at all levels in family planning programs, will play a critical role in reducing unmet need for modern contraception in low and middle income countries. Meeting family planning (FP) needs in humanitarian emergencies is challenging but feasible, and could present opportunities for reaching marginalized, remote, or otherwise underserved populations. Although high-quality evidence on family planning in emergencies is needed to improve access to voluntary FP services in humanitarian settings, this evidence brief identifies several promising, evidence-based interventions that can help improve uptake: provide comprehensive SRH services in line with global norms, provide a full range of family planning methods via mobile clinics, train mobile health workers to provide short-acting methods, second refugee providers to health facilities to provide FP in humanitarian settings, train community health …

Through close cooperation with the Ministry of Health and Family Welfare of Bangladesh, and other partners, STEP UP generated strong evidence and cultivated ongoing collaboration that contributed to policy changes and program expansions to increase access to menstrual regulation with medication (MRM). Study results demonstrate that MRM is acceptable and effective in Bangladesh, and has thus been legalized and folded into the national FP program and scaled up nationwide. However, ongoing observation and studies are still needed to understand whether the efficacy and acceptability of MRM remains the case when services are delivered at scale on a national level. Despite the significant progress that the government of Bangladesh has made toward introducing menstrual regulation with medication (MRM) into the national health program, there has been no systematic documentation on the introduction of MRM in Bangladesh, and on the approval process of local manufacturing of the mifepristone-misoprostol combination for menstrual regulation. This Research Report presents findings from the stakeholder analysis which was initiated to document the entire process of introducing MRM in the country. In April 2016, the Population Council, the World Health Organization (WHO), and the International Consortium for Emergency Contraception (ICEC) convened a three-day regional technical meeting aimed at helping participating countries meet their obligations under the Maputo Protocol to protect and promote the reproductive health rights of women and girls, with a special emphasis on survivors of sexual and intimate partner violence. By 2015, at the conclusion of the Millennium Development Goals, Bangladesh had achieved a reduction in maternal deaths, however, despite the progress, there are still between 5,000 and 6,000 maternal deaths every year, with 20 percent the result of pre-eclampsia and eclampsia (PE/E). To fully understand the key challenges, gaps, and interventions related to the prevention and treatment of PE/E at the national level, Ending Eclampsia conducted a systematic review of papers in Bangladesh published between 2000 and 2015 specifically looking at issues around the quality of care, gaps in the evidence, and barriers to accessing PE/E services. With high rates of early marriage, especially among girls, a significant proportion of adolescents in Bangladesh need sexual and reproductive health services (SRH), including contraceptive information. In 2015, the Population Council in conjunction with UNFPA conducted a study that drew on data from the 2013–14 Zambia Demographic and Health Survey and the 2010 Census of Population and Housing to identify where adolescent pregnancy is most likely to occur in Zambia. Using a literature review, key informant interviews among organizations working with adolescents, and in-depth interviews with adolescent girls, the study also identified key factors that lead to adolescent pregnancy. Study findings reaffirmed that adolescent pregnancies are high in Zambia, potentially undermining girls’ human rights and compromising their opportunity to fully realize their socioeconomic development potential. The health of Bangladesh’s 29.5 million adolescents, who make up nearly one-fifth of the country’s total population, is critical to the country’s future, but issues surrounding adolescent sexual and reproductive health (ASRH) remain taboo. ASRH initiatives have been implemented by both the Government of Bangladesh and nongovernmental organizations, but with limited coordination, documentation, or evaluation, making it difficult to know what worked well and what did not. The Population Council conducted a landscape analysis in Pakistan to assess the knowledge and practices of trained community midwives in Tando Allahyar, a rural district of Sindh, India on the prevention, detection, and management of pre-eclampsia, severe pre-eclampsia, and eclampsia, as well as gaps and challenges around the prevention and treatment at the provincial and district levels. This brief shares the findings of that assessment, and provides recommendations that would minimize the gaps in maternal health services. Results from Ending Eclampsia’s landscape analysis of seven states in Nigeria served as the catalyst for a training with Community Health Extension Workers (CHEWs) in Nigeria. Researchers conducted observations of client–provider interactions during antenatal care (ANC) visits and assessed healthcare providers’ knowledge of proven risk factors for pre-eclampsia, including chronic hypertension, previous history of PE/E, obesity, pre-existing diabetes or diabetes in pregnancy, advanced maternal age, sickle cell diseases, and connective tissue diseases. The training’s objective was to teach CHEWs how ANC providers can prevent pregnancy-related complications like pre-eclampsia and eclampsia (PE/E). In 2015, following the 2013–14 Zambia Demographic and Health Survey, the Population Council and UNFPA conducted a study to establish the areas where child marriage is most likely to occur in Zambia. Findings reaffirm that child marriage is very high in Zambia and that child marriage affects more girls than boys. Protecting girls from child marriage is a national priority in protecting girls’ rights and achieving progress in national development. The brief recommends that government efforts be accelerated and effectively supported to reduce and eventually eradicate child marriage. This study builds on previous interventions by the Population Council and the Zambia Police Service to mitigate unintended pregnancy and HIV among survivors of sexual violence by involving trained police in task-sharing interventions. These previous efforts have involved police providing emergency contraceptive pills and HIV post-exposure prophylaxis to survivors reporting to police stations, in addition to referrals to health facilities. There is a shortage of qualified and skilled health professionals providing cost-effective maternal, newborn, and child health (MNCH) services in Cross River State, Nigeria. Frontline health workers (FLHWs) have the potential to learn new skills as part of a task-shifting and sharing (TSS) policy. The Population Council, under its USAID-funded Evidence Project, partnered with Bangladesh’s Business for Social Responsibility program to conduct operational research to evaluate the effectiveness of the HERhealth model for improving female factory workers’ health, and to find ways to optimize program inputs and processes to support future scale-up of the intervention. The Business for Social Responsibility's (BSR) HERproject is a collaborative initiative that strives to empower low-income women working in global supply chains in 14 countries worldwide. One of the HERproject’s three pillars is HERhealth, which seeks to improve the health-related knowledge and behaviors, and access to health services and products, of low-income working women. In Bangladesh, HERhealth specifically addresses reproductive health and family planning needs. BSR asked the Evidence Project/Population Council to conduct an implementation science study assessing the effectiveness of BSR’s HERhealth model in Bangladesh.
