"First, Do No Harm": Ethical Failures And Preventive Solutions In The United States Pharmaceutical System - A Narrative Review, Rebekah R. Bailey

The United States pharmaceutical system is highly advanced but faces growing ethical concerns related to drug development and marketing, overprescription, and high drug costs. This thesis examines how pharmaceutical marketing, regulatory practices, and prescribing behaviors may conflict with the ethical principle of “first, do no harm.”

Using a narrative review of interdisciplinary literature in public health, medical ethics, and applied ethics, this thesis analyzes patterns in drug development, marketing strategies, and regulatory oversight. The findings suggest that ethical issues are systemic, driven by commercial incentives, inconsistent regulatory standards, and an increasing reliance on pharmaceutical solutions. One of the most daunting challenges of the industrial world is air pollution. Within this broad spectrum, there is an inhalable particle group named PM2.5, which contains black carbon. Hepatitis C virus (HCV) remains a significant public health concern and is a leading cause of cirrhosis, hepatocellular carcinoma, and liver transplantation when left untreated. Although direct-acting antiviral therapies have made HCV curable for most patients, many underserved populations continue to experience barriers to diagnosis, linkage-to-care, treatment completion, and sustained virologic response (SVR). This study evaluated how patient navigation programs influence progression through the HCV care continuum among underserved populations. Community pharmacists are among the most accessible healthcare providers in the United States and play an increasingly important role in improving patient outcomes. This study observes whether geographical distance and demographic differences influence patient healthcare needs within community pharmacy settings. Communities in Wisconsin and Mississippi, approximately 800 miles apart, were compared due to differences in population demographics, socioeconomic factors, and disease burden.

A survey was distributed to community pharmacy employees, including pharmacists, pharmacy interns, and pharmacy technicians, to assess common disease states, medication patterns, pharmacy workload, and patients' barriers to care. Twelve responses were collected from pharmacy employees.

Telehealth has improved patient access by reducing barriers to convenient, cost-effective, and quality health care. Telehealth mobile applications allow individuals to remotely access services through smartphone-based platforms. The purpose of this study was to evaluate leading telehealth applications available in the United States, analyzing key factors that influence platform selection, including cost, prescribing and diagnostic capabilities, user feedback, and data privacy practices. This study looks at the trends in over-the-counter medication usage and self-efficacy among undergraduate college students in the United States. Over-the-counter (OTC) medications serve as the most easily accessible and commonly purchased medications and can be used as a basis to determine this demographic’s attitudes and behaviors towards self-medication. Long lines at the pharmacy can frustrate patients. Consequently, patients becoming frustrated can add stress to an already overworked pharmacy. Without knowing what patients understand about the often complicated prescription filling process at community pharmacies, it can be difficult for pharmacists and pharmacy technicians to communicate the reason behind any delays in patients receiving their medication. Use of medical cannabis in Mississippi was legalized in February 2022 following many years of cannabis research at the University of Mississippi. Issues immediately became apparent, beginning with the lack of education about medical cannabis among providers. A 2017 study showed that the majority of physicians are not exposed to information about medical cannabis despite its legalization in 37 states. Another issue with Mississippi’s medical cannabis program is the cost of the product itself. The legalization of medical cannabis in Mississippi was a debated topic among all citizens. This thesis specifically explored pharmacists’ perceptions and knowledge as medical professionals who will be involved with patients certified to use medical cannabis. Information was gathered using a voluntary online survey on the Qualtrics platform to produce a cross-sectional, descriptive study.

Background: Vaccinations are successful, cost-effective tools to prevent the spread of certain infectious diseases. Many colleges conduct vaccination campaigns on their campuses for various vaccinepreventable diseases, including measles, mumps, influenza, HPV, and most recently, for SARS-CoV-2, the virus responsible for COVID-19. Implementing these campaigns requires substantial effort and understanding their effectiveness is an important factor in justifying these programs.

In the first chapter, I discussed three of the major issues in a community pharmacy which are rude customers, phones, and shorter hours. Technicians have to deal with very opinionated customers who can come off as insolent. To fill the gap in existing literature related to immune-related adverse events (irAEs) induced by immune checkpoint inhibitors (ICIs) in non-small cell lung cancer (NSCLC), this dissertation focused on: 1) assessing the incidence, spectrum and risk factors for the development of irAEs; 2) assessing the clinical impact of irAE occurrence on survival and treatment discontinuation; 3) analyzing and estimating the incremental economic burden of irAEs among an older population in the United States (US). Objective: The principal objective in this study is to identify the contextual factors predicting the nonmedical use of prescription stimulants among college students who nonmedically use prescription stimulants (NMUPS) for academic reasons.

Participants: 470 college-aged students from varying undergraduate classes

Methods: This study utilized an observational design, which consisted of repeated surveys administered to a sample of undergraduate students at the University of Mississippi. Descriptive statistics were used to assess frequencies of contextual factors regarding the background and exam surveys.
