Over the last few years, there has been an exponential increase in morbidity and mortality associated with heroin abuse. The current rise in heroin abuse and overdose is attributed to widespread use and abuse of prescription opioids, which can produce significant euphoric effects in humans. In fact, reports suggest that heroin abusers initially become addicted to prescription opioids but subsequently switch to heroin because it is cheaper and more easily available than prescription opioids. Over the years, the purity of heroin available for illicit use has been on the decline. Smugglers and heroin vendors have started mixing heroin with other …

Human breast milk is known to provide the most complete nutrition to growing infants. There may be more gained from breastfeeding than simply nutritional fulfillment-recent research has uncovered benefits related to the development and health of infants. Breastfeeding has been shown to stimulate immune system development by exposing the infant to bacteria and antibodies from the mother. Breastfeeding may also benefit nursing mothers by improving insulin sensitivity and glucose tolerance as well as decreasing postpartum weight gain. In place of breastfeeding, formulas are able to provide an infant with necessary nutrition. Many infant formulas have been enriched with probiotics and …

Zika virus is a public health emergency of international concern. Zika virus (ZIKV) is a mosquito-borne pathogen that is carried by the Aedes genus. Zika is spread through direct bite and nonvector transmission. Most individuals infected with Zika will be asymptomatic, but some may present nonspecific viral symptoms. A rising number of neurological disorders in newborns whose mothers were infected with ZIKV during pregnancy have been reported due to recent outbreaks. Neurological disorders affect both the central and peripheral nervous systems and can result from bacterial, viral, fungal or parasitic infections of the nervous system or from the immune response …

The Zika virus has become a growing concern as a global pandemic. Since being identified in Uganda in 1947, the virus has spread around the world, recently emerging in South America with a great impact on Brazil. As of 2016, Zika has made its appearance in the United States and is now actively being transmitted in Florida. The virus is a mosquito-vectored flavivirus primarily transmitted by the Aedes mosquito which infects a human through biting. Transmission through sexual intercourse is also possible as well as transmission from a pregnant woman to her fetus. This latter form of transmission presents the …

Non-responsive subpopulation of tumor cells, and acquired resistance in initially responsive cells are major challenges for cancer therapy with molecularly-targeted drugs. While point mutations are considered the major contributing factor to acquired resistance, in this study we explored the role of heterogeneity and plasticity of selected human breast cancer cell lines (MDA-MB-231, MDA-MB-468, and AU565) in their initial and adjusted response, respectively, to ruxolitinib, everolimus, and erlotinib. After determination of lethal concentration for 50% cell death (LC50), cells were exposed to selected drugs using three different approaches: single exposure to 4 × LC50 and collection of surviving cells, multiple exposures …

The Role Of A Home Health Care Pharmacist-Medication Management For Patients With Feeding Tubes, Lydia Suchecki, Sabrina Hamman, Rachel Muhlenkamp, Eyob D. Adane
Pharmacy and Wellness Review
Home health care is a method of medical care that patients receive inside their home under the supervision of a collaborative team of physicians, nurses, pharmacists and sometimes other health care professionals. Home health provides patients with the same standard of care that they would be receiving in a nursing home or hospital. However, the treatment and continued monitoring at home reduces health care costs and makes the patient feel more comfortable. Pharmacists analyze, resolve and prevent medication-related problems in home health care in order to minimize hospitalizations and improve patient quality of life. Pharmacist involvement with other health care …

Pharmacy and Wellness Review
Within the current health care system, preconception care is often a misunderstood topic and, in many cases, a missed opportunity to improve women's health and decrease adverse pregnancy outcomes. It is important that preconception care is delivered to all women of childbearing age, regardless of pregnancy intentions, as the interventions associated with preconception care can help improve a woman's health overall. In 2006, the Centers for Disease Control and Prevention (CDC) released four goals, 10 recommendations, and 14 evidence-based interventions regarding preconception care. Pharmacists can have a significant role in ensuring preconception care for all women through the application of …

Vitamin D has a significant role in bone health, calcium homeostasis, immune function and other biological functions. In our bodies, the main source of vitamin D is linked to our skin's exposure to sunlight. It can also be obtained through foods that contain vitamin D. Despite these two sources, vitamin D supplementation is often necessary. It is available in two forms, cholecalciferol and ergocalciferol as over-the-counter (OTC) products, as well as calcitriol as prescription only. Reasons for vitamin D deficiency include lack of sunlight, poor diet, malabsorption of vitamin D, liver and/or kidney disease. Vitamin D deficiencies lead to diseases …

Neonatal Abstinence Syndrome: A Targeted Review For Pharmacists, Andrew Skouby, Gabi Gegenheimer, Kelsey Lindsley, Sarah Kradel, Michael Rush
Pharmacy and Wellness Review
Neonatal abstinence syndrome (NAS) is a disease that impacts drug-exposed infants and describes an array of issues that arise in newborns just hours after birth. Patient presentation and disease symptomatology vary widely based upon the specific substance utilized by the mother while pregnant and duration of exposure. Treatment is dependent on which symptoms are present and, assuming an opioid-derived abstinence syndrome, is based primarily on opioid supplementation to prevent symptoms of withdrawal. Treatment of non-opioid derived abstinence syndrome is often slightly more complex and involves the use of different agents depending on the drug of exposure. Due to the intricate …

Pertussis is an acute infectious disease caused by the organism Bordetella pertussis and characterized by a "whooping cough." Incidence of the disease had declined since the development of a vaccine, but is now increasing in reported cases. This increase has been attributed to both an increased awareness but also surmised to be related to a decrease in vaccinations. The pertussis vaccine is given in conjunction with tetanus and diphtheria vaccines to children before the age of 6 in five separate injections over the course of four to six years. A booster is now recommended for the older child and adults …

Prescription drug abuse is the fastest growing drug problem in America. Among the different prescription drugs being abused, analgesics are the most commonly abused group of drugs. In the last few years, there is increasing evidence of abuse of tramadol, which is an atypical, centrally acting opioid analgesic. The increasing abuse of tramadol has prompted regulatory authorities to strengthen the product labeling of tramadol with respect to its abuse potential. Furthermore, several states have added tramadol to their controlled substances list. In this article, we will review the pharmacology of tramadol and some of the preclinical and clinical studies that …

Endometriosis is a gynecological condition that occurs in women between the ages of 15 and 49 years, in which endometrial cells grow outside the uterus. Normally, endometrial cells form the endometrium and respond to hormones throughout the menstrual cycle, but when endometrial cells are located outside the endometrium, painful bleeding and other side effects may occur. Treatment of endometriosis is usually symptomatic relief, including the use of oral contraceptives and other hormone replacement options, as well as non-steroidal anti-inflammatory drugs. To ensure that patients receive the appropriate treatment for this condition, pharmacists should be able to recognize the signs and …

There are many misconceptions among the general public regarding the administration of vaccinations during pregnancy. It is imperative for pharmacists to be current regarding guidelines and updates about recommended vaccines. Data from the Centers for Disease Control and Prevention (CDC) indicate that while some vaccines are highly recommended during pregnancy, others have limited safety data or are contraindicated. As new data emerges on the safety and efficacy of immunizations during pregnancy, pharmacists should continue to review the literature to stay up-to-date on vaccination recommendations. The CDC also has information available for pharmacists and other health care professionals regarding the use …

The severe side effects of chemotherapy negatively affect quality of life and may limit the amount of life-saving drug delivered to patients with cancer. These adverse events can be difficult to manage and evidence-based guidelines are lacking. Insufficient supportive care can amplify common side effects, such as chemotherapy-induced nausea and vomiting (CINV), myelosuppression, alopecia, gastrointestinal effects and neuropathy. Therefore, it is important to recognize the most commonly dispensed chemotherapy agents and the side effects that accompany them. Community pharmacists, as easily accessible health care professionals, can provide valuable supportive care to help manage potentially debilitating side effects. However, a major …

Human Chorionic Gonadotropin (hCG) has recently experienced a resurgence in popular media. Late night television commercials and Internet advertisements have suggested that it is an essential, unequivocal means to losing weight fast. ls hCG really a miracle cure to help patients shed unwanted pounds? In 1954, A.T.W. Simeons claimed that hCG impacts weight loss by decreasing hunger, increasing fat redistribution, and increasing overall mood. Knowing that weight loss cannot be directly attributed to hCG use, Simeons developed a very low calorie diet (VLCD) to which the success of the therapy can be attributed. He saw hCG as a means to …

Oral anticoagulants are a critical component of stroke prevention, but carry a risk of brain hemorrhage. These hemorrhagic complications tend to occur in elderly individuals, especially those with predisposing conditions such as cerebral amyloid angiopathy (CAA). Clinical evidence suggests that non-vitamin K antagonist oral anticoagulants are safer than traditional oral anticoagulants. We analyzed whether the anticoagulant dabigatran produces cerebral microhemorrhage (the pathological substrate of MRI-demonstrable cerebral microbleeds) or intracerebral hemorrhage in aged mice with and without hemorrhage-predisposing angiopathy. We studied aged (22 months old) Tg2576 (a model of CAA) and wild-type (WT) littermate mice. Mice received either dabigatran etexilate (DE) …

Pharmacy and Wellness Review
As substance abuse continues to be problematic on college campuses across the United States, pharmacy colleges in particular are faced with the challenge of deterring drug abuse among their students. Many pharmacy schools are considering the adoption of student drug -screening policies in hopes of discouraging abuse, directing impaired students to treatment, and, ultimately, protecting the public. However, these policies are being met with apprehension in regards to added costs, Fourth Amendment rights violations, and privacy concerns. Drug-screening policies continue to be an issue of deliberation for pharmacy colleges, requiring much consideration and care.
