การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาอุบัติการณ์และปัจจัยที่มีความสัมพันธ์กับการเกิดภาวะไตวายเฉียบพลันในผู้สูงอายุที่ได้รับการรักษาด้วย vancomycin เป็นการวิจัยภาคตัดขวาง กลุ่มตัวอย่าง คือ ผู้ป่วยอายุตั้งแต่ 60 ปีขึ้นไปที่ได้รับ vancomycin รูปแบบฉีดเข้าหลอดเลือดดำ เก็บข้อมูลจากเวชระเบียนผู้ป่วยใน โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ตั้งแต่เดือนมกราคม พ.ศ. 2560 ถึงมิถุนายน พ.ศ. 2562 ประเมินการเกิดภาวะไตวายเฉียบพลันตามเกณฑ์ KDIGO และวิเคราะห์หาปัจจัยที่มีความสัมพันธ์กับการเกิดภาวะไตวายเฉียบพลันโดยการวิเคราะห์ Multiple logistic regression ผลการวิจัย กลุ่มตัวอย่างจำนวน 244 ราย เกิดภาวะไตวายเฉียบพลัน 64 ราย คิดเป็นอุบัติการณ์ 10.49 รายต่อ 100 ประชากรต่อปี ผลการวิเคราะห์ Multiple logistic regression พบว่า ปัจจัยที่มีความสัมพันธ์กับการเกิดภาวะไตวายเฉียบพลันในผู้สูงอายุที่ได้รับ vancomycin อย่างมีนัยสำคัญทางสถิติ ได้แก่ ค่าเฉลี่ยระดับ vancomycin ในพลาสมา ณ จุดต่ำสุด (Cave, trough) และจำนวนยาที่เป็นพิษต่อไตเมื่อใช้ร่วมกันตั้งแต่ 2 ชนิดขึ้นไป โดยกลุ่มที่มี Cave, trough ที่ 15.0-19.9, 20.0-24.9, 25.0-29.9 และ 30.0 มิลลิกรัมต่อลิตรขึ้นไปมีความเสี่ยงต่อการเกิดภาวะไตวายเฉียบพลันสูงกว่ากลุ่มที่มี Cave, trough ที่น้อยกว่า 15.0 มิลลิกรัมต่อลิตร Adjusted odd ratio (ORadj) = 3.98 (95% CI: 1.24-12.78), 4.06 (95% CI: 1.24-13.35), 9.88 (95% CI: 2.77-35.19) และ 17.86 (95% CI: 4.78-66.67) ตามลำดับ ส่วนกลุ่มที่ได้รับจำนวนยาที่เป็นพิษต่อไตเมื่อใช้ร่วมกันตั้งแต่ 2 ชนิดขึ้นไป มีความเสี่ยงต่อการเกิดภาวะไตวายเฉียบพลันสูงกว่ากลุ่มที่มีจำนวนยาที่เป็นพิษน้อยกว่า 2 ชนิด …

วัตถุประสงค์: เพื่อศึกษาอัตราความสำเร็จในการรักษาผู้ป่วยวัณโรคปอดในผู้ใหญ่ และปัจจัยที่มีความสัมพันธ์ต่ออัตราความสำเร็จในการรักษาผู้ป่วยวัณโรคปอดในผู้ใหญ่ ณ คลินิกควบคุมการติดเชื้อทางเดินหายใจ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย วิธีการวิจัย: เป็นการศึกษาย้อนหลังเชิงพรรณนา ทำในผู้ป่วยวัณโรคปอดที่เข้ารับการรักษา ณ คลินิกควบคุมการติดเชื้อทางเดินหายใจ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ตั้งแต่เดือนมกราคม พ.ศ. 2561 ถึงเดือนธันวาคม พ.ศ. 2564 โดยเก็บข้อมูลจากเวชระเบียนผู้ป่วยนอก แบบเก็บข้อมูลคลินิกควบคุมการติดเชื้อทางเดินหายใจ สมุดลงทะเบียนผู้ป่วย TB-03 และโปรแกรม KCMS_HIS วิเคราะห์ข้อมูล univariate และ multivariate ด้วยสถิติ Binary logistics regression หาปัจจัยที่มีความสัมพันธ์ต่ออัตราความสำเร็จในการรักษาวัณโรค ผลการวิจัย: ผู้ป่วยวัณโรคปอดเข้าร่วมการศึกษาทั้งสิ้น 538 ราย ส่วนใหญ่เป็นเพศชาย 325 ราย (ร้อยละ 60.4) อายุและน้ำหนักตัวเฉลี่ย คือ 52.0 ± 31.0 ปี และ 53.0 ± 14.8 กิโลกรัม ตามลำดับ ส่วนใหญ่เป็นผู้ป่วยวัณโรคที่ไวต่อยา (ร้อยละ 87.2) ไม่มีประวัติการรักษาวัณโรค (ร้อยละ 86.2) และไม่มีการติดเชื้อเอชไอวีร่วมด้วย (ร้อยละ 93.3) สำหรับปัจจัยที่มีความสัมพันธ์กับอัตราความสำเร็จในการรักษา วัณโรคปอดในผู้ใหญ่ ได้แก่ อายุ น้ำหนักตัว การติดเชื้อเอชไอวี และผลตรวจเสมหะ โดยผู้ป่วยวัณโรคที่มีอายุน้อยกว่า 60 ปี มีโอกาสประสบความสำเร็จในการรักษามากกว่าผู้ป่วยวัณโรคที่มีอายุมากกว่าหรือเท่ากับ 60 ปี 2.11 เท่า (ORadj = 2.11, 95%CI: 1.26 – 3.56) ผู้ป่วยวัณโรคที่มีน้ำหนักตัวมากกว่า 40 กิโลกรัม มีโอกาสประสบความสำเร็จในการรักษามากกว่าผู้ป่วยวัณโรคที่มีน้ำหนักตัวไม่เกิน 40 กิโลกรัม 2.46 เท่า (ORadj = 2.46, 95%CI: 1.27 …

ผลของการบริบาลทางเภสัชกรรมต่อผลลัพธ์ทางคลินิก และผลลัพธ์แบบองค์รวมในผู้ป่วยจิตเภท, วาสนันท์ พลไพรินทร์

วัตถุประสงค์: ผลของการบริบาลทางเภสัชกรรมต่อผลลัพธ์ทางคลินิก และผลลัพธ์แบบองค์รวมในผู้ป่วยจิตเภท วิธีการวิจัย: การวิจัยแบบสุ่มมีกลุ่มควบคุมชนิดปกปิดฝ่ายเดียว ทำการศึกษาในผู้ป่วยจิตเภทได้รับการรักษาในโรงพยาบาลศรีธัญญา ระหว่างเดือนมกราคม ถึงเดือนมิถุนายน 2564 จำนวน 130 ราย สุ่มแบ่งออกเป็น 2 กลุ่ม คือ กลุ่มควบคุมได้รับการรักษาตามมาตรฐาน และกลุ่มศึกษาได้รับการบริบาลทางเภสัชกรรมร่วมกับการรักษาตามมาตรฐาน ติดตามผลลัพธ์ตั้งแต่การรักษาเป็นผู้ป่วยใน จนกระทั่งเปลี่ยนสถานะเป็นผู้ป่วยนอก ประเมินผลลัพธ์ทางคลินิกจากสัดส่วนของจำนวนอาการไม่พึงประสงค์ที่เปลี่ยนแปลงดีขึ้น ประเมินผลลัพธ์แบบองค์รวมด้านพฤติกรรมการใช้ยาและคุณภาพชีวิตด้านการใช้ยาด้วยเครื่องมือ MTB-Thai และ PROMPT ฉบับย่อ ตามลำดับ ผลการวิจัย: เมื่อสิ้นสุดการศึกษา กลุ่มศึกษามีผลลัพธ์ทางคลินิกและผลลัพธ์แบบองค์รวมที่ดีขึ้นเมื่อเทียบกับกลุ่มควบคุม โดยพบสัดส่วนของจำนวนอาการไม่พึงประสงค์ที่เปลี่ยนแปลงดีขึ้นอย่างมีนัยสำคัญทางสถิติ (89.30% และ 39.10 %; p<0.001) ส่วนผลลัพธ์แบบองค์รวม กลุ่มศึกษามีคะแนนเฉลี่ยพฤติกรรมการใช้ยามากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (22.81 ± 1.61 และ 16.85 ± 3.00; p < 0.001) และกลุ่มศึกษามีคะแนนเฉลี่ยคุณภาพชีวิตด้านการใช้ยามากกว่าอย่างมีนัยสำคัญทางสถิติเมื่อเทียบกับกลุ่มควบคุม ได้แก่ มิติที่ 1 การได้รับข้อมูลโรคและยา (p = 0.015) มิติที่ 3 ผลกระทบจากอาการข้างเคียงของยา (p = 0.003) มิติที่ 5 ความสะดวกในการใช้ยา (p = 0.023) และมิติที่ 7 ความสัมพันธ์ทางด้านการรักษา (p = 0.041) สรุปผล: ผลของการบริบาลทางเภสัชกรรมในผู้ป่วยจิตเภท ส่งผลให้ผู้ป่วยมีความปลอดภัยจากการใช้ยาต้านโรคจิต และมีแนวโน้มเปลี่ยนแปลงพฤติกรรมการใช้ยาและคุณภาพชีวิตด้านการใช้ยาที่ดีขึ้น

Alzheimer’s disease (AD) is a neurodegenerative disease that results in impaired cognition, disorientation, confusion, poor judgement, and behavioral changes. The mitochondrial cascade hypothesis (MCH), which postulates that accumulating oxidative stress and mitochondrial dysfunction plays an integral role in the development of the disease, potentially upstream, in conjunction with or independent of Aβ and tau. The literature supports the critical role of mitochondrial dysfunction in the progression and development of AD, but is inconclusive about what precise role, as initiator or delegate, it performs. Regardless, the impact that oxidative stress and mitochondrial dysfunction have on AD deserves further study and investigation. …

Metastases are the leading cause of cancer-related deaths, and the lungs are one of the primary sites of metastases from several malignancies. Existing therapies fail to entirely eradicate tumors that disseminate and reside in lungs; therefore, patient prognosis significantly worsens. Conventional therapies utilized to treat lung tumors such as chemotherapy are administered systemically and typically exhibit poor lung biodistribution profiles, leading to many undesirable off target effects and suboptimal drug concentration in the tumor, which may promote resistance, further compromising the quality of life of secondary lung cancer patients. Emphysema, a major manifestation of chronic obstructive pulmonary disease (COPD), affects millions of patients in the United States; yet, no present treatment is able to directly modify the disease process. In the recent past, a pathobiologic hypothesis surrounding the deficiency of vascular endothelial growth factor (VEGF), has surfaced as a cause of the alveolar structural destruction and loss in emphysema. Additionally, sirtuin 1 (SIRT1), a regulator of anti-oxidative proteins and cellular homeostasis, has been found to be reduced in the lungs of emphysematous patients. Chronic pain affects between 20 and 30 percent of the adult population in western countries and represents a wide array of specific etiologies (Berge, 2011). Neuropathic pain secondary to traumatic nerve injury, chemotherapeutic toxicity, or diseases (e.g., diabetes mellitus) is often refractory to conventional analgesics, with patients receiving less than 50% pain relief compared to placebo (Finnerup et al. 2010). The dopamine transporter (DAT) is responsible for the removal of the neurotransmitter from the synaptic gap and a therapeutic target for a multitude of drugs. While the ortholog Drosophila melanogaster dopamine transporter (dDAT) and human serotonin transporter (hSERT) have resolved structures, the human dopamine transporter (hDAT) does not. A 3-D computational homology model of hDAT was constructed for the study of molecular interactions with agents within the central binding site (S1) of the transporter.

Methylation is a fundamental mechanism used in the biological system to modify the structure and function of biomolecules such as proteins, DNA, RNA, and metabolites.1 Methyl groups are installed by a large and diverse class of S-adenosyl-L-methionine (SAM)-dependent methyltransferases (MTases), which transfer the sulfonium methyl group of SAM to either carbon, nitrogen, oxygen, or other heteroatoms on biomolecules.2 Dysregulated MTase activity contributes to numerous diseases, including cancer, metabolic disorders, neurodegenerative diseases. …

The overall aim of this research project was to evaluate effects of spray drying parameters and storage conditions on solid-state characteristics and aerosol performance of excipient enhanced growth (EEG) powders and to formulate a stable and highly dispersible tobramycin EEG powder to be delivered to pediatric cystic fibrosis subjects using a novel positive pressure dry powder inhaler.
Micrometer-sized albuterol sulfate EEG powders were produced using a medium-sized nebulizer and optimized spray drying conditions. Using trileucine as the dispersion enhancer improved the aerosol performance but showed increased moisture absorption compared to the L-leucine formulation.

Combining vibrating mesh nebulizers with additional new technologies leads to substantial improvements in pharmaceutical aerosol delivery to the lungs across therapeutic administration methods. In this dissertation, streamlined components, aerosol administration synchronization, and/or Excipient Enhanced Growth (EEG) technologies were utilized to develop and test several novel devices and aerosol delivery systems. Today’s opioid crisis is one of the most challenging public health emergency. Two million Americans suffer opioid addiction. Over 20 million adults are affected by daily pain without non-addictive analgesics.
Kappa opioid receptor (KOR) is one of the most studied new pain targets void of abuse liability. KOR agonists have shown potent analgesia and also potential in alleviating opioid addiction. The only approved KOR agonist in clinic is nalfurafine (NFU). Though not leading to addiction, the analgesic effects of NFU are accompanied by sedative side effects. To design novel inhibitors of the SARS-CoV-2 main protease (Mpro), we investigated the binding mode of the recently reported α-ketoamide inhibitors of this enzyme. Following, we utilized in-silico screening to identify 168 peptidomimetic and non-peptidomimetic compounds that are high probability Mpro binding candidates. The compounds were synthesized in 5 to 10 mg for initial screening for their potential inhibition of Mpro using Fluorescence Resonance Energy Transfer (FRET) assay. Cerebral ischemia reperfusion injury, a common type of stroke, is a neurodegenerative disorder which is the leading cause of permanent disability in the United States. The underlying pathophysiology of ischemic stroke involves an occlusion in the vasculature supplying oxygen and glucose to the brain tissue, leading to infarction and eventual necrosis in the brain. This essay examines the main characteristics and shortcomings of mainstream social media responses to vaccine misinformation and disinformation. Parts I and II contextualize the recent expansion of vaccine information and disinformation in the online environment. Part III provides a survey and taxonomy of ongoing responses to vaccine misinformation adopted by mainstream social media. It further notes the limitations of current self-regulatory modes and illustrates these limitations by presenting a short case study on Facebook—the largest social media vehicle for vaccine-specific misinformation, currently estimated to harbor approximately half of the social media accounts linked to vaccine misinformation. Background
Apixaban pharmacokinetic properties and some clinical reports suggest cessation 48 hours prior to surgery is safe, but this has not been demonstrated in a naturalistic setting. We sought to measure the residual apixaban exposure in patients who had apixaban held as part of standard of care peri-operative management.
Methods
This was a prospective, observational study of patients in whom apixaban plasma concentration and anti-Xa activity were measured while at steady state apixaban dosing and again immediately prior to surgery. Clinical management of cessation and resumption of apixaban was at the discretion of the treating physician.
