การควบคุมอาการชักและคุณภาพชีวิตในผู้ป่วยโรคลมชักวัยสูงอายุ, อัจฉราภรณ์ ทองเย็น

การศึกษาแบบ unmatched case-control study นี้มีวัตถุประสงค์เพื่อเปรียบเทียบการควบคุมอาการชัก เหตุการณ์ไม่พึงประสงค์ และคุณภาพชีวิตระหว่างผู้ป่วยโรคลมชักวัยสูงอายุ (60 ปีขึ้นไป) และวัยผู้ใหญ่ (18-59 ปี) ศึกษาในผู้ป่วยที่มารับการรักษาต่อเนื่องเป็นเวลาอย่างน้อย 18 เดือน ณ ศูนย์ความเป็นเลิศโรคลมชักครบวงจร โรงพยาบาลจุฬาลงกรณ์ ช่วงระหว่างเดือนกุมภาพันธ์ พ.ศ. 2560 ถึงมิถุนายน พ.ศ. 2560 โดยเก็บข้อมูลทั่วไป ข้อมูลเกี่ยวกับโรคและการรักษา และให้ผู้ป่วยประเมินเหตุการณ์ไม่พึงประสงค์ในช่วง 1 เดือนที่ผ่านมา และคุณภาพชีวิตของผู้ป่วยโรคลมชัก (QOLIE-31 ฉบับภาษาไทย) ด้วยตนเอง ผลการศึกษาพบว่า มีผู้ป่วยเข้าร่วมการวิจัยจำนวน 120 คน แบ่งเป็นผู้ป่วยวัยสูงอายุและวัยผู้ใหญ่กลุ่มละ 60 คน ผู้ป่วยวัยสูงอายุมีสัดส่วนของผู้ป่วยที่ปลอดจากอาการชักในช่วง 12 เดือนมากกว่าวัยผู้ใหญ่ (ร้อยละ 65 และ 45 ตามลำดับ, P=0.028) เมื่อประเมินเหตุการณ์ไม่พึงประสงค์ในช่วง 1 เดือนที่ผ่านมา พบว่า ผู้ป่วยวัยสูงอายุรายงานการเกิดเหตุการณ์ไม่พึงประสงค์ในหัวข้อรู้สึกไม่มั่นคงด้านการเคลื่อนไหวมากกว่าผู้ป่วยวัยผู้ใหญ่ (P<0.05) ในขณะที่ผู้ป่วยวัยผู้ใหญ่รายงานการเกิดเหตุการณ์ไม่พึงประสงค์ในหัวข้อมีปัญหาเกี่ยวกับผิวหนัง, น้ำหนักเพิ่ม และง่วงนอนมากกว่าวัยสูงอายุ (P<0.05) ผู้ป่วยโรคลมชักวัยสูงอายุมีคะแนนคุณภาพชีวิตมากกว่าวัยผู้ใหญ่ในมิติด้านความวิตกกังวลเกี่ยวกับอาการชัก ด้านคุณภาพชีวิตโดยรวม ด้านบทบาทหน้าที่ทางสังคม และคะแนนเฉลี่ยรวม (P<0.05) การวิเคราะห์ถดถอยโลจิสติก พบปัจจัยที่มีผลต่อการควบคุมอาการชักในผู้ป่วยวัยสูงอายุเพียงปัจจัยเดียวคือ จำนวนยากันชักที่เคยได้รับ โดยผู้ป่วยที่มีจำนวนยากันชักที่เคยได้รับตั้งแต่ 2 รายการขึ้นไปจะสามารถควบคุมอาการชักได้แย่กว่าผู้ป่วยที่เคยได้รับยากันชัก 1 รายการ (OR=0.154, 95% CI 0.045-0.528) โดยสรุปผู้ป่วยโรคลมชักวัยสูงอายุตอบสนองต่อยากันชักและสามารถควบคุมอาการชักได้ดีกว่า มีคุณภาพชีวิตดีกว่า รวมทั้งรายงานการเกิดเหตุการณ์ไม่พึงประสงค์น้อยกว่าเมื่อเปรียบเทียบกับผู้ป่วยวัยผู้ใหญ่

ส้มโอ (Citrus grandis L. Osbeck) เป็นผลไม้ประเภท citrus ด้วยความคล้ายคลึงกันทางพฤกษศาสตร์กับเกรปฟรุต (Citrus paradisi) ที่มีรายงานว่าก่อให้เกิดการรบกวนเภสัชจลนศาสตร์ของยาหลายชนิดรวมถึงยา simvastatin ที่ใช้รักษาโรคภาวะไขมันในเลือดสูงแต่มีผลข้างเคียงที่เป็นอันตรายได้โดยเฉพาะต่อกล้ามเนื้อ งานวิจัยนี้จึงศึกษาผลของน้ำส้มโอต่อเภสัชจลนศาสตร์ของยา simvastatin และผลต่อการทำงานของเอนไซม์ไซโทโครมพี3a2 (CYP3a2) และการแสดงออกของเอ็มอาร์เอ็นเอ Mdr1a, Mdr1b และ Slc21a5 ในหนูแรทเพศผู้ โดยแบ่งหนูแรทออกเป็น 4 กลุ่มคือ กลุ่มควบคุม กลุ่มที่ได้รับน้ำส้มโอ (2ml/kg) เท่านั้นวันละ 2 ครั้งเป็นเวลา 7 วัน กลุ่มที่ได้รับยา simvastatin เท่านั้น (ขนาดยา 20 mg/kg) และกลุ่มที่ได้น้ำส้มโอ (2ml/kg) วันละ 2 ครั้งเป็นเวลา 7 วันร่วมกับยา simvastatin (20mg/kg) โดยทำการเก็บตัวอย่างเลือดในหนูกลุ่มที่ได้ยา simvastatin เป็นเวลาทั้งหมด 12 ชั่วโมงเพื่อวัดระดับ simvastatin และ simvastatin acid ด้วยเทคนิค LC-MS/MS และศึกษาการแสดงออกของเอ็มอาร์เอ็นเอ Mdr1a, Mdr1b และ Slc21a5 ในเซลล์ตับและลำไส้เล็กของหนูแรทด้วยเทคนิค real-time polymerase chain reaction (RT-PCR) รวมถึงศึกษาการทำงานของเอนไซม์ไซโทโครมพี3เอ2 ด้วยเทคนิค HPLC ผลการทดลองพบว่าหนูกลุ่มที่ได้รับน้ำส้มโอร่วมกับยา simvastatin มีค่า AUC0-∞ ของ simvastatin เพิ่มขึ้นเป็น 4 เท่า (p<0.01) และ simvastatin acid เป็น 3 เท่า (p<0.01) เมื่อเทียบกับหนูกลุ่มที่ได้รับยา simvastatin อย่างเดียว ค่าความเข้มข้นสูงสุดเฉลี่ยของยา (Cmax) เพิ่มขึ้น 3.9 เท่า (p<0.01) และ simvastatin acid เพิ่ม 3.6 เท่า (p<0.01) เมื่อเทียบกับหนูกลุ่มที่ได้รับยา simvastatin อย่างเดียว ส่วนค่า Kel, t1/2 และ Tmax ไม่แตกต่างกันในทั้งสองกลุ่ม พบการแสดงออกของเอ็มอาร์เอ็นเอ Mdr1a ในเซลล์ลำไส้เล็กในกลุ่มที่ได้รับน้ำส้มโอเท่านั้นและกลุ่มที่ได้รับน้ำส้มโอร่วมกับยา simvastatin ลดลงอย่างมีนัยสำคัญทางสถิติ 79.8% (p<0.01) และ 84.3% (p<0.01) ตามลำดับเมื่อเทียบกับกลุ่มควบคุม เช่นเดียวกับการแสดงออกของเอ็มอาร์เอ็นเอ Mdr1b ในลำไส้ของหนูทั้งสองกลุ่มลดลง 52.6% (p<0.05) และ 56.9% (p<0.05) ตามลำดับ ส่วนกลุ่มที่ได้รับยา simvastatin เท่านั้นมีการแสดงออกของเอ็มอาร์เอ็นเอ Mdr1a และ Mdr1b ในเซลล์ลำไส้เล็กไม่แตกต่างอย่างมีนัยสำคัญจากกลุ่มควบคุม ขณะที่การแสดงออกของเอ็มอาร์เอ็นเอ Slc21a5 ในลำไส้เล็กของหนูทุกกลุ่มไม่แตกต่างจากกลุ่มควบคุม ในเซลล์ตับของหนูแรทกลุ่มที่ได้รับน้ำส้มโอเท่านั้นและกลุ่มที่ได้รับน้ำส้มโอร่วมกับยา simvastatin พบว่ามีการแสดงออกของเอ็มอาร์เอ็นเอ Slc21a5 ลดลงอย่างมีนัยสำคัญทางสถิติ (p<0.05) เมื่อเทียบกับกลุ่มควบคุม แต่กลุ่มที่ได้รับยา simvastatin อย่างเดียวไม่แตกต่างจากกลุ่มควบคุม ในขณะที่การแสดงออกของเอ็มอาร์เอ็ เอ Mdr1a และ Mdr1b ในเซลล์ตับของหนูแรททุกกลุ่มไม่แตกต่างจากกลุ่มควบคุม ผลการศึกษาการทำงานของเอนไซม์ไซโทโครมพี3เอ2 พบว่าหนูกลุ่มที่ได้รับน้ำส้มโอเท่านั้น และกลุ่มที่ได้รับน้ำส้มโอร่วมกับยา simvastatin มีการทำงานของเอนไซม์ไซโทโครมพี3เอ2 ลดลงถึง 51.77% (p<0.01) และ 49.17% (p<0.01) ตามลำดับ แต่ในหนูที่ได้รับยา simvastatin เท่านั้นไม่แตกต่างจากกลุ่มควบคุม โดยสรุปน้ำส้มโอมีผลต่อการเพิ่มระดับยา simvastatin และ simvastatin acid ในพลาสมาของหนูแรท และมีผลลดการแสดงออกของเอ็มอาร์เอ็นเอ Slc21a5 ในตับและลดการแสดงออกของเอ็มอาร์เอ็นเอ Mdr1a และ Mdr1b ในลำไส้เล็กของหนูแรท รวมถึงมีผลยับยั้งการทำงานของเอนไซม์ไซโทโครมพี3เอ2 ดังนั้นการศึกษาครั้งนี้แสดงให้เห็นความเสี่ยงของการเกิดอันตรกิริยาระหว่างน้ำส้มโอกับยา simvastatin

มะเร็งปอดเป็นสาเหตุการเสียชีวิตจากโรคมะเร็งอันดับต้นๆ ของผู้ป่วยจากทั่วโลกและยังคงมีผู้ป่วยเสียชีวิตมากขึ้นในทุกปี สาเหตุเนื่องมาจากการแพร่กระจายของเซลล์มะเร็ง การแพร่กระจายของเซลล์มะเร็งเป็นกระบวนการหลายขั้นตอนที่ส่งเสริมให้เซลล์มะเร็งหลุดออกจากอวัยวะต้นกำเนิดไปยังส่วนต่างๆ ของร่างกาย ซึ่งกลไกที่สำคัญของการแพร่กระจายคือ กระบวนการเปลี่ยนแปลงจากเซลล์เยื่อบุผิวไปเป็นเซลล์มีเซนไคม์ การรักษาโรคมะเร็งในปัจจุบันยังไม่สามารถยับยั้งกระบวนการเปลี่ยนแปลงจากเซลล์เยื่อบุผิวไปเป็นเซลล์มีเซนไคม์ได้อย่างมีประสิทธิภาพ ดังนั้นการศึกษาพัฒนายาใหม่โดยมีเป้าหมายในการยับยั้งกระบวนการนี้ จึงเป็นส่วนสำคัญที่ช่วยในการรักษามะเร็งให้เกิดประสิทธิภาพ ในผลการศึกษาครั้งนี้พบว่าสารไซพริพีดินซึ่งเป็นสารเคมีในกลุ่มฟีแนนทรีนควิโนน สกัดได้จากกล้วยไม้สกุล Dendrobium densiflorum สามารถยับยั้งการแพร่กระจายของเซลล์มะเร็งปอดชนิดเอช 460 และเอช 23 ได้ โดยลดจำนวนของเส้นใยแอคตินสเตรสไฟเบอร์ และ โฟคอลแอดฮีชัน อย่างมีนัยสำคัญทางสถิติ และยับยั้งการเจริญเติบโตของเซลล์ภายใต้สภาวะไร้การยึดเกาะได้ การศึกษาในเชิงกลไกแสดงให้เห็นว่าสารไซพริพีดินยับยั้บกระบวนการเปลี่ยนแปลงจากเซลล์เยื่อบุผิวไปเป็นเซลล์มีเซนไคม์ และลดการแสดงออกของโปรตีนบ่งชี้ ได้แก่ สลัก เอ็น-แคดฮีริน และไวเมนติน ผ่านการยับยั้งการทำงานของวิถีเอเคที ซึ่งส่งผลต่อการกระตุ้นการทำงานของจีเอสเค-3เบตาในการเพิ่มการทำลายโปรตีนสลัก งานวิจัยนี้แสดงให้เห็นถึงผลของสารไซพริพีดินสามารถยับยั้งการเคลื่อนที่ของเซลล์มะเร็งปอดโดยผ่านการยับยั้งกระบวนการเปลี่ยนแปลงจากเซลล์เยื่อบุผิวไปเป็นเซลล์มีเซนไคม์ ซึ่งจะเป็นประโยชน์ต่อการวิจัยและพัฒนาสารดังกล่าวเพื่อใช้ในการรักษาโรคมะเร็งปอดต่อไปในอนาคต

Older adults are regular consumers of over-the-counter (OTC) medications. OTC medications are generally considered safe, and convenient to use without requiring a prescription. However, the safety of an OTC medication and the final health outcome depends in part upon consumers’ perceptions, beliefs, and their decision-making about OTC medication use. This dissertation aimed at developing an inhibitor strategy to improve the oral bioavailability (Foral) and systemic exposure (AUC∞) of buprenorphine (BUP) as well as reduce the variability associated with them. Twenty-seven generally recognized as safe (GRAS) compounds or dietary substances were evaluated for their potential to inhibit the oxidative and conjugative metabolism of BUP, using pooled human intestinal and liver microsomes. In both the organs, oxidation appeared to be the major metabolic pathway with a 6 fold (intestine) and 4 fold (liver) higher intrinsic clearance than glucuronidation. Today, more Americans die each year because of drug overdoses than are killed in motor vehicle accidents. In fact, in 2015, more than 33,000 individuals died due to an overdose of heroin or prescription opioids. Sadly, 40-60 % of patients on current opioid addiction treatment medications relapse. Studies have shown that the addiction/abuse liability of opioids are abolished in mu opioid receptor (MOR) knock-out mice; this indicates that the addiction and abuse liability of opioids are mainly mediated through MOR. There has been a desire to accurately interpret the inhaled pharmacokinetic (PK) profiles of drugs in humans to aid successful inhaled drug and product developments. The rise of blockbuster pharmaceutical acquisitions has prompted fears that unprecedented market concentration will weaken competition. Two of the most prominent concerns focus on the upstream and downstream ends of the pharmaceutical industry: (1) the concern that these mergers will concentrate the market for discovery and will therefore lead to fewer discoveries; and (2) the concern that merging large marketing, sales, and distribution forces will strengthen the hands of select pharmaceutical manufacturers and weaken downstream competition. The purpose of this scholarly project was to evaluate an evidence-based quality improvement program implemented in 2016 in a clinical practice setting to curtail overprescribing of opioids for noncancer pain management. In 2001, the National Pharmaceutical Council and The Joint Commission on Accreditation and Hospital Accreditation initiated a standard of practice for opioid use in noncancer pain management that resulted in opioid overprescribing and a 200% increase in opioid-related deaths and incalculable societal costs. Uncaria tomentosa, also known as cat’s claw, is an indigenous herb which originates from the Amazon River basin and has proven to be highly effective via case report and limited epidemiological evaluation in the pallative treatment of a number of inflammatory conditions. The herb is particularly preferred on the American market due to its various uses for healing, which provides the impetus for this research. Opioid analgesic tolerance remains a considerable drawback to chronic pain management. The finding that concomitant administration of delta opioid receptor (DOR) antagonists attenuates the development of tolerance to mu opioid receptor (MOR) agonists has led to interest in producing bifunctional MOR agonist/DOR antagonist ligands. Herein, we present 7-benzylideneoxymorphone (6, UMB 246) displaying MOR partial agonist/DOR antagonist activity, representing a new lead for designing bifunctional MOR/DOR ligands.

Components Of A Measure To Describe Organizational Culture In Academic Pharmacy, Shane P. Desselle, Meagen Rosenthal, Erin R. Holmes, Brienna Andrews, Julia Lui, Leela Raja

Objective. To develop a measure of organizational culture in academic pharmacy and identify characteristics of an academic pharmacy program that would be impactful for internal (eg, students, employees) and external (eg, preceptors, practitioners) clients of the program.
Methods. A three-round Delphi procedure of 24 panelists from pharmacy schools in the U.S. and Canada generated items based on the Organizational Culture Profile (OCP), which were then evaluated and refined for inclusion in subsequent rounds. Items were assessed for appropriateness and impact.
Results. The panel produced 35 items across six domains that measured organizational culture in academic pharmacy: competitiveness, …
