This study was conducted to investigate the antioxidant and antimutagenic activity of Zanthoxylum limonella seed extract and that of Z. limonella pericarp extract. The total phenolic contents and antioxidant activity assay including DPPH assay and FRAP assay were performed. The results showed that the antioxidant activity of pericarp extract was higher than that of the seed extract. The direct mutagenicity assay was determined, with and without nitrite treatment using the Ames test towards Salmonella typhimurium strains TA98 and TA100. The purposes of this study were to establish a database of the carbohydrate content of medications and investigate carbohydrate content of medications in epileptic children treated with ketogenic diet (KD) at King Chulalongkorn Memorial Hospital. One hundred sixty-nine KD order forms in 3 events (KD initiation, follow-up visit, and hospital re-admission) for epileptic children whose aged younger than 18 years old during 2009-2017 were selected. Clinical and nutritional data were obtained from medical records and KD order forms from the pediatric nutrition unit. Powder mixing is one of an important unit operation in pharmaceutical manufacturing of solid dosage forms. Continuous mixing was developed as an alternative method to reduce disadvantages of traditional batch mixing processes. This process was originally developed using horizontal cylindrical mixer equipped with Process Analytical Technology (PAT) tools for real-time monitoring. The objectives of this study were to prepare saccharide fiber via centrifugal spinning and further investigate its stability under stress condition. Different saccharides were chosen and spun via centrifugal force to form thread like matter. The results indicated that sucrose (SC) and trehalose (TH) could be fabricated as fiber whereas other saccharides were not able to form. The formation of fiber mainly involved with glass transition temperature (Tg), molten ability including stability of saccharide after melting. A total of 50 endophytic actinomycetes isolated from fourteen plant species collected from six provinces of Thailand including Bangkok, Chachoengsao, Chiang Mai, Nakhon Pathom, Prachinburi and Suphan Buri were taxonomically studied relied on their phenotypic and chemotaxonomic characteristics, and the 16S rRNA gene sequence analysis. The isolates were identified as members of the family Streptomycetaceae involving Streptomyces (22 isolates), and the family Micromonosporaceae including Micromonospora (25 isolates), Plantactinospora (1 isolate) and Polymorphospora (1 isolate). The objective of this research was to study the phytochemical composition of the Okra seed extract and its in vitro anticancer activity for future pharmaceutical use. We detected active flavonoids compounds such as isoquercitrin, at an amount of 2.89 ± 1.64 % w/w. Furthermore, antioxidation testing by DPPH assay and ABTS assay, showed values of IC50 = 55.80 ug/ml and 49.04 ug/ml, respectively. The Okra seed extract’s anti-cancer activity was evaluated on cancer cell lines, indicating anticancer properties for certain forms of cancer such as cervical cancer (HeLa), liver cancer (HepG2), and most significantly for breast cancer (MCF-7). Phytochemical study of the methanol extract of the whole plant of Dendrobium scabrilingue Lindl. (Orchidaceae) resulted in the isolation of a new phenanthrene and a new bisbibenzyl namely dendroscabrols A and B, respectively, along with eight known compounds, identified as (Z)-ferulic acid tetracosyl ester, (E)-ferulic acid tetracosyl ester, gigantol, batatasin III, coelonin, aloifol I, lusianthridin and RF-3192C. The structures of these isolated compounds were determined mainly by spectroscopic data. All isolates were examined for alpha-glucosidase inhibitory activity. Previous studies reported various pharmacological effects of, ECa233, especially in the central nervous system disorders. The present study aimed to investigate the neuroprotective effects and protective mechanisms of ECa233 against rotenone-induced neurotoxicity in rats and in neuroblastoma SH-SY5Y cells. การศึกษาแบบเก็บข้อมูลย้อนหลังนี้มีวัตถุประสงค์เพื่อสร้างแบบจำลองทางเภสัชจลนศาสตร์และหาปัจจัยที่มีผลต่อค่าพารามิเตอร์ทางเภสัชจลนศาสตร์ของยาแวนโคมัยซินในผู้ป่วยเด็กชาวไทย ค่าความเข้มข้นของ ยาแวนโคมัยซินในซีรั่มจานวน 348 ตัวอย่าง จากผู้ป่วยเด็กอายุระหว่าง 1 เดือน ถึง 18 ปี ที่เข้ารับการรักษา ณ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ระหว่างปี พ.ศ. 2555-2560 จานวน 212 คน วิเคราะห์ข้อมูลเภสัช จลนศาสตร์ประชากรด้วยโปรแกรม NONMEM® โดยวิธี first-order conditional estimation with interaction ลักษณะทางเภสัชศาสตร์ของยาแวนโคมัยซินอธิบายได้ด้วยแบบจำลองชนิดหนึ่งห้องที่มีการกำจัดยา แบบปฏิกิริยาอันดับหนึ่ง ค่าเฉลี่ยอัตราการกำจัดยาแวนโคมัยซิน (CL) มีค่า 0.13 ลิตร/ชั่วโมง/กิโลกรัม และ ค่าเฉลี่ยปริมาตรการกระจายยา (Vd) มีค่า 0.88 ลิตร/กิโลกรัม ปัจจัยที่มีผลต่อค่าพารามิเตอร์ทางเภสัชศาสตร์ ได้แก่ น้ำหนัก (กิโลกรัม) และอัตราการกรองของไต (มิลลิลิตร/นาที/1.73 ตารางเมตร) ดังแสดงในสมการ CL (ลิตร/ชั่วโมง) = 1.66*(น้ำหนัก/14)[superscript 0.75]*(อัตราการกรองของไต/108.9) และ V (ลิตร) = 12.7*(น้ำหนัก/14) ค่า ความผันแปรระหว่างบุคคลของ CL เท่ากับร้อยละ 34.8 ค่าความผันแปรระหว่างบุคคลของ V เท่ากับร้อยละ 39.6 และแบบจำลองสุดท้ายถูกตรวจสอบความสมเหตุสมผลด้วยวิธี bootstrap และ visual predictive check (VPC) ค่าพารามิเตอร์จากแบบจำลองสุดท้ายมีค่าใกล้เคียงกับค่าพารามิเตอร์ที่ได้จากการทำ bootstrap 1,000 ครั้ง และอยู่ในช่วงความเชื่อมั่นที่ร้อยละ 95 ผลการจำลองข้อมูลด้วยวิธี VPC 1,000 ครั้ง พบค่าความเข้มข้นของ ยาแวนโคมัยซินร้อยละ 6 ที่อยู่นอกช่วงความเชื่อมั่นที่ร้อยละ 90 แสดงถึงความเหมาะสมของแบบจำลองดังกล่าว ในการอธิบายเภสัชจลนศาสตร์ของยาแวนโคมัยซิน

สาร Oxyresveratrol เป็นสารสำคัญในแก่นของต้นมะหาด โดยได้มีการศึกษาถึงฤทธิ์ทางเภสัชวิทยาของสาร Oxyresveratrol ในหลายด้าน แต่อย่างไรก็ตามสาร Oxyresveratrol เป็นสารที่มีค่าชีวปริมาณออกฤทธิ์ต่ำจึงเป็นอุปสรรคที่สำคัญในการนำมาพัฒนาเป็นยารับประทาน ดังนั้นจุดประสงค์ในการศึกษานี้ เพื่อศึกษาเภสัชจลนศาสตร์ของสาร Oxyresveratrol ที่เปลี่ยนไปเมื่อให้สาร Piperine เป็นสารเพิ่มชีวปริมาณออกฤทธิ์ในหนูแรท ซึ่งขนาดที่ให้แบ่งเป็นสาร Oxyresveratrol เดี่ยวขนาด 10 mg/kg หรือ สาร Oxyresveratrol ขนาด 10 mg/kg ให้ร่วมกับสาร Piperine 1 mg/kg โดยการบริหารยาทางหลอดเลือดดำ และ สาร Oxyresveratrol เดี่ยวขนาด 100 mg/kg หรือ สาร Oxyresveratrol ขนาด 100 mg/kg ให้ร่วมกับสาร Piperine 10 mg/kg โดยการบริหารยาทางปากจากนั้นเก็บตัวอย่างเลือด, อวัยวะ, ปัสสาวะ และ อุจจาระ ตามเวลาที่กำหนดนำไปวิเคราะห์ด้วยเครื่อง LC-MS/MS เพื่อวิเคราะห์ข้อมูลทางเภสัชจลนศาสตร์ จากผลการศึกษาพบว่าสาร Piperine สามารถเพิ่มค่าชีวปริมาณออกฤทธิ์ของสาร Oxyresveratrol ขึ้นประมาณ 2 เท่า รวมถึงสามารถช่วยเพิ่มค่าความเข้มข้นของสาร Oxyresveratrol สูงสุดในพลาสมาโดยการบริหารทางปากและ เมื่อคำนวณอัตราส่วนปริมาณยาในอวัยวะต่อพลาสมามีค่าประมาณ 10-100 เท่า ซึ่งสอดคล้องกับค่าปริมาณการกระจายยาที่สูง ประมาณ 50 L/kg ในกระบวนการเปลี่ยนแปลงสภาพของสารเมื่อได้รับร่วมกับสาร Piperine มีค่า Conversion ratio ลดลงในทั้งสองรูปแบบการบริหาร โดยสรุปสาร Piperine สามารถปรับปรุงค่าทางเภสัชจลนศาสตร์ของสาร Oxyresveratrol ได้ทั้งในทั้งสองรูปแบบการบริหารยา การศึกษาทางเภสัชจลนศาสตร์นี้จึงสามารถนำมาเป็นข้อมูลในการพัฒนาสาร Oxyresveratrol เพื่อใช้ประโยชน์ต่อไปได้

ผู้ป่วยมะเร็งเต้านมที่มีการแสดงออกของตัวรับเอสโตรเจนในระยะลุกลามร้อยละ 40 สามารถกลับเป็นมะเร็งซ้ำเนื่องจากมีการดื้อยา tamoxifen แพทย์จึงเปลี่ยนไปใช้ยาต้านฮอร์โมนกลุ่มอื่น แต่ยังเกิดการดื้อยาและมีผลข้างเคียงที่รุนแรง ดังนั้นการนำสารจากธรรมชาติมาพัฒนาเป็นยาที่มีฤทธิ์ยับยั้งการเจริญของเซลล์มะเร็งจึงเป็นแนวทางที่อาจลดปัญหาดื้อยาได้ สารสกัดจากต้นจันทน์หอม (Mansonia Gagei Drumm) ยับยั้งการเจริญของเซลล์มะเร็งได้หลายชนิด โดย mansonone G (MG) สารสำคัญที่แยกจากสารสกัดสามารถแย่งจับกับตัวรับเอสโตรเจนได้ ในการศึกษานี้จึงทำการทดสอบฤทธิ์อนุพันธ์ MG คือ Ethoxy mansonone G (EMG) สารกึ่งสังเคราะห์ในการยับยั้งการเจริญ การลุกลามและการยับยั้งการเจริญแบบไร้การยึดเกาะ พบว่าเมื่อเพิ่มความเข้มข้นและระยะเวลาในการทดสอบ EMG สามารถยับยั้งการเจริญในเซลล์มะเร็งเต้านมชนิดที่ไม่ดื้อยา (MCF-7) และเซลล์มะเร็งเต้านมที่ดื้อต่อยาต้านฮอร์โมน (MCF-7/LCC2, MCF-7/LCC9) อย่างมีนัยสำคัญทางสถิติและมีความเป็นพิษน้อยกว่ายาเคมีบำบัดในเซลล์ไฟโปรบลาสต์ และพบว่ากลไกการออกฤทธิ์ของ EMG ในการยับยั้งการเจริญของเซลล์ MCF-7 ส่วนหนึ่งผ่านการลดการแสดงออกของยีนที่เป็นเป้าหมายของตัวรับเอสโตรเจนและสาร EMG ยังสามารถลดการแสดงออกของยีนที่เกี่ยวกับการดื้อต่อยา tamoxifen ใน MCF-7/LCC2 และ MCF-7/LCC9 นอกจากนี้ EMG ยังยับยั้งการลุกลามของ MCF-7/LCC2 และ MCF-7/LCC9 โดยลดการแสดงออกของ MMP-9 และ uPA ทั้งยังสามารถลดการเจริญแบบไร้การยึดเกาะผ่านการลดจำนวน ขนาดของ colony ใน MCF-7/LCC2 และ MCF-7/LCC9 สรุปว่าสาร EMG มีคุณสมบัติในการต้านมะเร็งเต้านมชนิดไม่ดื้อยาและชนิดที่ดื้อต่อยาต้านฮอร์โมน ต้านการลุกลาม รวมไปถึงลดการเจริญแบบไร้การยึดเกาะของ MCF-7/LCC2 และ MCF-7/LCC9 ซึ่งอาจนำไปพัฒนาเป็นยาชนิดใหม่ในการรักษาผู้ป่วยมะเร็งเต้านมที่ดื้อต่อยาต้านฮอร์โมน

ยาฟิแนสเทอไรด์ (finasteride) เป็นยาในกลุ่ม 5-alpha reductase inhibitor ใช้รักษาโรคผมร่วงทางพันธุกรรมในเพศชาย โดยยาฟิแนสเทอไรด์อาจทำให้เกิดผลข้างเคียงหลายประการได้แก่ การเสื่อมสมรรถภาพทางเพศ ปริมาณน้ำอสุจิลดลง ดังนั้นควรมีการถูกใช้ในรูปแบบยาทาหนังศีรษะเพื่อลดผลข้างเคียงดังกล่าว โดยการศึกษานี้มีวัตถุประสงค์เพื่อศึกษาระดับยาฟิแนสเทอไรด์ในเลือดและในชั้นผิวหนัง หลังการให้ยาความแรง 0.5% ด้วยการทาผิวหนังเพียงครั้งเดียว (single dose) และแบบทาผิวหนังหลายครั้ง (repeated dose) บนพื้นที่ 4 ตารางเซนติเมตรของหนูแรทเพศผู้สายพันธุ์ Sprage-Dawley โดยมีการเก็บพลาสมาและผิวหนังภายใน 24 ชั่วโมง หลังการทายาครั้งสุดท้ายและสกัดยาออกมาเพื่อตรวจวัดระดับยาฟิแนสเทอไรด์ด้วยเครื่อง LC-MS/MS ผลการศึกษาแสดงให้เห็นว่า ระดับยาฟิแนสเทอไรด์ในพลาสมาหลังการให้ยาด้วยการทาผิวหนังเพียงครั้งเดียวมีแนวโน้มการดูดซึมเข้ากระแสเลือดเพิ่มขึ้นตามปริมาตรยาที่ทา โดยเริ่มตรวจพบระดับยาที่เวลา 4 ชั่วโมง หลังทายาในปริมาตร 300 ไมโครลิตร เพียงครั้งเดียว แต่ไม่พบระดับยาในพลาสมาของกลุ่มที่ได้รับยาปริมาตร 75 หรือ 150 ไมโครลิตร นอกจากนี้การทายาซ้ำวันละ 1 ครั้งและทายาซ้ำวันละ 2 ครั้ง ติดต่อกัน 7 วัน ไม่มีนัยสำคัญต่อระดับยาในพลาสมา ซึ่งเห็นได้ว่ายาฟิแนสเทอไรด์ถูกดูดซึมเข้ากระแสเลือดได้น้อยหลังการให้ยาด้วยการทาผิวหนัง สำหรับอัตราส่วนระหว่างระดับยาในผิวหนังและระดับยาในพลาสมาที่เวลา 24 ชั่วโมง หลังได้รับยาทางปากเพียงครั้งเดียวมีค่าประมาณ 2.6 เท่า โดยพบอัตราส่วนระหว่างระดับยาในผิวหนังบริเวณที่ไม่สัมผัสยา (control sites) และระดับยาในพลาสมาเกินกว่า 2.6 เท่า ในหนูแรทกลุ่มที่ได้รับยาทางผิวหนังทุกกลุ่ม ทั้งนี้ค่าที่ตรวจพบระดับยาในพลาสมามีความแปรปรวนสูงมาก อย่างไรก็ตามผลการศึกษานี้แสดงให้เห็นว่า ยาฟิแนสเทอไรด์สามารถสะสมอยู่ในผิวหนังชั้นหนังแท้ (dermis) และถูกดูดซึมเข้าสู่กระแสเลือดได้น้อยหลังให้ยาด้วยการทาผิวหนัง ทั้งนี้จำนวนครั้งการทาทั้งการทาซ้ำวันละ 1 ครั้งและการทาซ้ำวันละ 2 ครั้งอาจไม่มีผลอย่างมีนัยสำคัญต่อการดูดซึมยาเข้าสู่กระแสเลือด

Generally, inactivated whole-cell and subunit vaccines are often less antigenic than live-attenuated vaccines, then they require the inclusion of adjuvants to improve immunostimulating activities. In the present study, microemulsion (ME) and nanoemulsion adjuvants were developed to evoke a safety and effective immune response of vaccine antigens. The emulsion systems including vegetable oil and nonionic surfactants have been formulated with or without cationic surfactant, cetyltrimethylammonium bromide (CTAB) and then sterilized by autoclaving. วัตถุประสงค์ : เพื่อศึกษาอิทธิพลร่วมกันของภาวะพหุสัณฐาน POR*28 และ CYP3A5 ต่อสัดส่วนของระดับความเข้มข้นต่ำสุดต่อขนาดยา tacrolimus (C0/D) ในผู้ปลูกถ่ายไตชาวไทย วิธีการดำเนินการวิจัย : รูปแบบการศึกษาวิเคราะห์แบบย้อนหลัง, ศึกษาในผู้ปลูกถ่ายไตจำนวน 230 คน โดยผู้ปลูกถ่ายไตทุกคนจะได้รับยากดภูมิคุ้มกัน 3 รายการ ประกอบด้วย tacrolimus, mycophenolate mofetil หรือ sodium, และ steroids ข้อมูลพื้นฐานและข้อมูลทางห้องปฏิบัติการเก็บรวบรวม ณ วันที่ 3, 7, 90, และ 180 หลังการปลูกถ่ายไต ใช้สถิติ two-way ANCOVA ในการวิเคราะห์อิทธิพลร่วมกันของภาวะพหุสัณฐาน POR*28 และ CYP3A5 ต่อ C0/D ของยา tacrolimus โดยควบคุมปัจจัยอายุ, ฮีโมโกลบิน, อัลบูมิน, และขนาดยา steroids ผลการศึกษา : ไม่พบอิทธิพลร่วมกันของภาวะพหุสัณฐาน POR*28 และ CYP3A5 ต่อ Log C0/D ของยาtacrolimus (ณ วันที่ 3, 7, 90, และ 180 หลังการปลูกถ่ายไต, P>0.05) แต่อย่างไรก็ตามพบว่า ภาวะพหุสัณฐาน CYP3A5 มีอิทธิพลต่อ Log C0/D ของยา tacrolimus ทุกช่วงเวลาที่ทำการศึกษา (P<0.001) โดยผู้ที่มีภาวะพหุสัณฐาน CYP3A5 expressers จะมีค่าเฉลี่ยของ Log C0/D ของยา tacrolimus ต่ำกว่า CYP3A5 non-expressers สำหรับภาวะพหุสัณฐาน POR*28 พบว่ามีอิทธิพลต่อ Log C0/D ของยา tacrolimus เฉพาะวันที่ 7 หลังการปลูกถ่ายไต (P=0.047) และพบว่าผู้ที่มีภาวะพหุสัณฐาน POR*28 T carriers จะมีค่าเฉลี่ย Log C0/D ของยา tacrolimus ต่ำกว่า POR*28 CC อีกทั้งยังพบว่าอายุ, ฮีโมโกลบิน, และขนาดยา steroids มีอิทธิพลต่อ Log C0/D ของยา tacrolimus สรุปผลการศึกษา : ณ วันที่ 3, 7, 90, และ 180 หลังการปลูกถ่ายไต ใม่พบอิทธิพลร่วมกันระหว่างภาวะพหุสัณฐาน POR*28 และ CYP3A5 ต่อสัดส่วนของ Log C0/D ของยา tacrolimus และพบว่าภาวะพหุสัณฐาน CYP3A5 เป็นปัจจัยที่มีอิทธิพลต่อเภสัชจลนศาสตร์ของยา tacrolimus มากที่สุด

วัตถุประสงค์: เปรียบเทียบอุบัติการณ์การเกิดแผลติดเชื้อในผู้ที่ได้รับยา amoxicillin และ amoxicillin-clavulanic acid เพื่อป้องการการติดเชื้อจากสุนัขและแมวกัด/ข่วน และศึกษาปัจจัยที่เกี่ยวข้องกับการเกิดแผลติดเชื้อ
วิธีการดำเนินการวิจัย: การวิจัยเชิงทดลองแท้จริงและใช้วิธีการปกปิดแบบทางเดียว ศึกษาในผู้ป่วยที่เกิดแผลจากสุนัขหรือแมวกัด/ข่วนที่มีระดับการสัมผัสโรคพิษสุนัขบ้าระดับ 3 และเข้ารับการรักษาภายใน 24 ชั่วโมง ณ แผนกฉุกเฉิน โรงพยาบาลบ้านบึง จังหวัดชลบุรี ระหว่างเดือนกุมภาพันธ์ พ.ศ. 2561 ถึงเดือนพฤษภาคม พ.ศ. 2561 โดยการสุ่มแบบบล็อกให้ผู้ป่วยได้รับยา amoxicillin หรือยา amoxicillin-clavulanic acid เป็นระยะเวลา 5 วัน และประเมินผลการเกิดแผลติดเชื้อในวันที่ 3 และ 7 หลังจากได้รับยาปฏิชีวนะ
ผลการวิจัย: ผู้ป่วยทั้งหมด 236 คน เข้ารับบริการที่โรงพยาบาลด้วยสาเหตุสุนัขกัด แมวกัดและแมวข่วนร้อยละ 75, 18.2 และ 6.8 ตามลำดับ อุบัติการณ์การเกิดแผลติดเชื้อระหว่างกลุ่มที่ได้รับยา amoxicillin (ร้อยละ 1.7) กับยา amoxicillin-clavulanic acid (ร้อยละ 4.2) ไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p=0.447) ผู้ป่วยที่ได้รับยา amoxicillin-clavulanic acid เกิดอาการไม่พึงประสงค์มากกว่ากลุ่มที่ได้รับยา amoxicillin ร้อยละ 17.5 และ 10.3 ตามลำดับ (p=0.113) แผลบริเวณเท้าจากสุนัขกัดและแผลเจาะจากแมวกัดมีความสัมพันธ์กับการติดเชื้ออย่างมีนัยสำคัญทางสถิติ (p=0.048 และ p=0.029 ตามลำดับ)
สรุปผลการวิจัย : ยา amoxicillin สามารถใช้เป็นยาปฏิชีวนะเพื่อป้องกันการเกิดแผลติดเชื้อจากสุนัขและแมวกัดได้ โดยไม่มีความแตกต่างของอุบัติการณ์การเกิดแผลติดเชื้อระหว่างกลุ่มที่ได้รับยา amoxicillin กับยา amoxicillin-clavulanic acid โดยการเกิดแผลบริเวณเท้าจากสุนัขกัดและการมีแผลเจาะจากแมวกัดอาจเพิ่มความเสี่ยงต่อการเกิดแผลติดเชื้อ

To assess effects of these nanoparticles on human immune system, this study aimed to assess toxicity of AgNPs, AgCuNPs and AgSnNPs on human monocytes, THP-1 cells, and THP-1 differentiated macrophage cells. The nanoparticles were characterized and assessed for cytotoxicity and generation of intracellular ROS.
