Consumers are interested in colored rice because its health benefits and its high fiber and protein content. Normally, the colored rice smell is generally not as strong as the jasmine rice. The first objective of this research was to investigate the proper preheated time and headspace incubation temperature to extract as many volatile compounds as possible from Thai-colored rice using the static headspace GC-MS approach. Metal nanomaterials can enhance the efficacy of current cancer therapies. Here, we show that Ti0.8O2 nanosheets cause cytotoxicity in several lung cancer cells but not in normal cells. The nanosheet-treated cells showed certain apoptosis characteristics. Protein analysis further indicated the activation of the p53-dependent death mechanism. Transmission electron microscopy (TEM) and scanning electron microscopy (SEM) analyses revealed the cellular uptake of the nanosheets and the induction of cell morphological change. Ti0.8O2 nanosheets induce apoptosis through a molecular mechanism involving peroxynitrite (ONOO−) generation. As peroxynitrite is known to be a potent inducer of S-nitrosylation, we further found that the nanosheets mediated …

การตรวจติดตามสภาวะแวดล้อมมีบทบาทสำคัญในการสร้างความมั่นใจต่อระดับความสะอาดของสภาพแวดล้อมในระหว่างกระบวนการผลิต โดยเฉพาะอย่างยิ่งในการผลิตผลิตภัณฑ์ประเภทยาฉีด โดยควรต้องกำหนดให้มีโปรแกรมการตรวจติดตามสภาวะแวดล้อมอย่างเป็นประจำ เพื่อลดความเสี่ยงของการปนเปื้อนจากเชื้อจุลินทรีย์และอนุภาคสิ่งแปลกปลอมอื่นๆ ในการศึกษาวิจัยนี้มีวัตถุประสงค์เพื่อพัฒนาโปรแกรมการตรวจติดตามสภาวะแวดล้อมตามแนวทางการประเมินความเสี่ยงในห้องสะอาดสำหรับยาฉีดของแผนกบรรจุผลิตภัณฑ์ ศูนย์ผลิตผลิตภัณฑ์จากพลาสมา สภากาชาดไทย จำนวน 13 ห้อง แนวทางการจัดการความเสี่ยงคุณภาพอ้างอิงตาม International Council for Harmonization Q9 โดยใช้เครื่องมือ Risk ranking and filtering ในการประเมินและจัดระดับความเสี่ยงเพื่อกำหนดความถี่และตำแหน่งในการตรวจติดตามสภาวะแวดล้อม สำหรับขั้นตอนการระบุความเสี่ยง มีการประเมินความเสี่ยงเพื่อค้นหาปัจจัยที่อาจทำให้เกิดการปนเปื้อนจากเชื้อจุลินทรีย์ด้วย Fishbone diagram ในการวิเคราะห์ความเสี่ยง ได้ทำการเลือกปัจจัยที่จะส่งกระทบผลต่อคุณภาพและความปลอดภัยของผลิตภัณฑ์ ในแง่ของความรุนแรงและโอกาสที่จะเกิดการปนเปื้อน ปัจจัยที่นำมาพิจารณา ได้แก่ กิจกรรมการผลิต, ระยะเวลาที่มีการปฏิบัติงาน, ขั้นตอนกระบวนการผลิต, ความยากง่ายต่อการทำความสะอาดอุปกรณ์, ความถี่ในการทำความสะอาดและกำจัดเชื้อโรค, ข้อมูลประวัติแนวโน้มของการตรวจติดตามสภาวะแวดล้อม, จำนวนบุคลากรที่ปฏิบัติงาน และการมีท่อเดรนหรือท่อน้ำในพื้นที่ ทั้งนี้เกณฑ์และคะแนนของแต่ละปัจจัยจะถูกกำหนดและประเมินโดยคณะทำงานประเมินความเสี่ยง เพื่อคำนวณค่าความเสี่ยงและจัดระดับได้เป็น 5 ระดับ แนวทางการประเมินความเสี่ยงนำไปสู่โปรแกรมการตรวจติดตามสภาวะแวดล้อมใหม่ ซึ่งมีความถี่ในการตรวจติดตามสภาวะแวดล้อมของแต่ละพื้นที่ได้เป็นสัปดาห์ละครั้ง จำนวน 5 ห้อง สองสัปดาห์ครั้ง จำนวน 1 ห้อง และสามเดือนครั้ง จำนวน 7 ห้อง สำหรับตำแหน่งและวิธีการทดสอบที่ประเมินได้ คือ Particle count 26 ตำแหน่ง, Volumetric air sampling 26 ตำแหน่ง, Settle plate 29 ตำแหน่ง และ Contact plate 33 ตำแหน่ง ในขั้นตอนการทบทวนความเสี่ยงพบว่าหลังจากเปลี่ยนโปรแกรมการตรวจติดตามสภาวะแวดล้อมใหม่ ผลการตรวจยังคงผ่านตามเกณฑ์ที่กำหนด และเมื่อพิจารณาจากการคำนวณ %Contamination recovery rate (CRR) พบว่ายังคงผ่านตามเกณฑ์ที่กำหนดเช่นเดียวกัน ดังนั้นจึงไม่ได้มีผลกระทบต่อคุณภาพของผลิตภัณฑ์หรือระดับการปนเปื้อนในห้องสะอาด โดยรวมแล้วความสำเร็จของโปรแกรมการตรวจติดตามสภาวะแวดล้อมจะขึ้นอยู่กับประสิทธิภาพของการประเมินความเสี่ยง การวิเคราะห์ข้อมูลที่เกี่ยวข้อง และวิธีในการทดสอบตรวจติดตามสภาวะแวดล้อม อย่างไรก็ตามสิ่งที่สำคัญหลังจากที่จัดทำหรือปรับปรุงโปรแกรมการตรวจติดตามสภาวะแวดล้อมแล้วจะต้องมีการประเมินและปรับปรุงโปรแกรมอย่างต่อเนื่องสม่ำเสมอ เพื่อให้มั่นใจว่าสภาวะแวดล้อมในพื้นที่สะอาดจะคงอยู่ในสถานะที่ควบคุมได้และไม่ส่งผลกระทบต่อคุณภาพของผลิตภัณฑ์

มะเร็งเต้านมเป็นปัญหาสาธารณสุขที่สำคัญ ผู้ป่วยที่เป็นมะเร็งเต้านมระยะแพร่กระจายเป็นปัจจัยพยากรณ์โรคที่ไม่ดี อย่างไรก็ตาม ยังมีข้อมูลเกี่ยวกับระยะเวลาการรอดชีวิตโดยรวม ปัจจัยที่มีผลต่อระยะเวลาการรอดชีวิต และ อัตราการรอดชีวิตที่ระยะเวลา 5 ปีของผู้ป่วยมะเร็งเต้านมระยะแพร่กระจายชาวไทยอยู่จำกัด การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีผลต่อระยะเวลาการรอดชีวิตของผู้ป่วยมะเร็งเต้านมระยะแพร่กระจาย โดยเก็บข้อมูลย้อนหลังในผู้ป่วยมะเร็งเต้านมระยะแพร่กระจายที่มารับการรักษาที่โรงพยาบาลศิริราชตั้งแต่วันที่ 1 มกราคม พ.ศ. 2555 ถึง 31 ธันวาคม พ.ศ. 2559 ในช่วงเวลาดังกล่าวมีผู้ป่วยที่มีข้อมูลครบถ้วนและสามารถประเมินข้อมูลระยะเวลาการรอดชีวิตได้จำนวน 790 ราย อายุเฉลี่ย 54+11.8 ปี ส่วนใหญ่ร้อยละ 50.5 เป็นมะเร็งเต้านมชนิด luminal A โรคมะเร็งมีการแพร่กระจายไปที่กระดูกมากที่สุดร้อยละ 70.8 รองลงมาคือที่ปอดและตับ ร้อยละ 55.7 และ 37.3 ตามลำดับ ค่ามัธยฐานของระยะเวลาการรอดชีวิตโดยรวมของผู้ป่วยคือ 21.9 เดือน อัตราการรอดชีวิตที่ 1, 3 และ 5 ปี คือร้อยละ 32.3, 14.4 และ 5.6 ตามลำดับ ผลการวิเคราะห์แบบหลายตัวแปรพบว่าปัจจัยที่มีผลต่อระยะเวลาการรอดชีวิตโดยรวมของผู้ป่วย ได้แก่ ประเภททางโมเลกุลของโรคมะเร็ง ระยะเวลาตั้งแต่ได้รับการวินิจฉัยมากกว่า 6 เดือนขึ้นไปและการแพร่กระจายของโรคมะเร็งไปที่ตับ และการได้รับยาฮอร์โมนหรือยามุ่งเป้า ขณะที่อายุ ดัชนีมวลกาย การได้รับยา chemotherapy หรือสูตรยาที่ได้รับ ไม่มีผลต่อระยะเวลาการรอดชีวิตโดยรวมของผู้ป่วยมะเร็งเต้านมระยะแพร่กระจาย ผู้ป่วยมะเร็งเต้านมระยะแพร่กระจายที่เป็นแบบตัวรับทั้งหมดเป็นบวกหรือแบบไม่มีตัวรับใดเป็นบวก, ได้รับการวินิจฉัยโรคตั้งแต่ 6 เดือนขึ้นไป, มีการแพร่กระจายของโรคมะเร็งไปที่ตับ และผู้ที่ไม่ได้รับยาฮอร์โมนหรือยามุ่งเป้ามีความเสี่ยงในการมีระยะเวลารอดชีวิตโดยรวมที่สั้นกว่า ควรได้รับการวางแผนการรักษาที่เข้มข้นอย่างเหมาะสมเพื่อเพิ่มระยะเวลาการรอดชีวิตโดยรวม

The objective of this study was to evaluate the effects of solid lipids: beeswax and cetyl alcohol, and surfactants: Tween 80®, soybean lecithin and poloxamer 188 on the characteristics of solid lipid microparticles containing asiatic acid (AASLM). The AASLM were prepared from 10% or 15% of beeswax or cetyl alcohol together with 3% of surfactant F1-F12) by melt dispersion cum freeze-drying technique to obtain a solid dosage form. Lung cancer is a prevalent cause of death, indicated by an elevated c-Myc level. We synthesized a new derivative of renieramycin T (RT), named DH_22, and tested it for anti-cancer activities in human lung cancer cells. In the cell viability and nuclear staining tests, DH_22 was discovered to be cytotoxic, with an IC50 of around 13 μM; and found to mediate apoptosis in the lung cancer cells. Meanwhile, in a mechanistic sense, DH_22 contributed to the activation of p53-dependent apoptosis and decreased the cellular level of c-Myc. Free radicals are unstable, highly reactive, molecules which can cause several degenerative diseases such as mutagenesis and carcinogenesis. This research aims to study the chemical constitutents of Dendrobium virgineum and their antioxidant activity in 3 different test models including DPPH radical scavenging assay, superoxide radical scavenging activity assay and oxygen radical absorbance capacity assay. Nine compounds were isolated and determined to be two new compounds named 2,8-dimethoxy-9,10-dihydro-4,5-phenanthrenediol and 2,5-dimethoxy-7-hydroxy-9,10-dihydro-1,4-phenanthrenequinone, together with seven known compounds, which are 2-methoxy-9,10-dihydro-4,5-phenanthrenediol, gigantol, 5-methoxy-7-hydroxy-9,10-dihydro-1,4-phenanthrenequinone, p-coumaric acid, tristin, 2,5,7-trihydroxy-4-methoxy-9,10-dihydrophenanthrene and 9,10-dihydro-2,4,7-phenanthrenetriol. Understanding the snakebite and antivenom market situation and burden of snakebite is crucial for developing evidence-based strategies to pursue the goal set by the World Health Organization (WHO) to halve morbidity and mortality of snakebite by 2030. Transcatheter pulmonary valve replacement (TPVR) was launched since 2000. It indicated for cardiac patients who have the right ventricular outflow tract obstruction (RVOT) and a prior surgery history. TPVR have a shorter hospital stay, fewer complications, and a less-invasive method. However, TPVR is more expensive than surgical pulmonary valve replacement (SPVR). This study aims to examine outpatient dispensing patterns in Thai hospitals, focusing on the current situation. The research explores the structures, processes, outputs, and environmental factors of alternative outpatient dispensing services. Surveys were conducted through phone interviews and in-depth interviews with pharmacy department heads using structured interviews with accurate content. Key findings from 197 participating hospitals revealed that 97.35% of interviewed hospitals offered at least one alternative outpatient dispensing service. The most popular method was home delivery (88.6%), followed by sub-district health promoting hospital medication pick-up service (45.1%), hospital refill service (44.7%), community pharmacy medication pick-up service (26.6%), and drive-thru …

Nanoparticles (NPs) have been used as an effective carrier for transmucosal delivery of drugs through their prolonged residence time and deposition, or the enhanced permeation through the mucosa. In this study, we aimed to design a nanocarrier using the combined mucoadhesive (M) properties of chitosan (CS) and alginate (ALG) polymers for the intranasal delivery of favipiravir (FVR) in treating SARS-CoV-2 infection. The optimized FVR-loaded mucoadhesive CS-coated ALG-NPs (FVR-MCS-ALG-NPs) rendered suitable size (233.5 nm), zeta potential (-21.6 mV), loading capacity (26.0%), and encapsulation efficiency (84.6%) for transmucosal delivery. Regeneration of damaged periodontal tissues becomes one of the biggest challenges that is hard to overcome, even without the presence of any pathogens. Tissue engineering was proposed as a possible technique for the lost tissues’ restoration. Cancer stem cells (CSCs) drive aggressiveness and metastasis by utilizing stem cell-related signals. In this study, 5-O-(N-Boc-L-alanine)-renieramycin T (OBA-RT) was demonstrated to suppress CSC signals and induce apoptosis. OBA-RT exerted cytotoxic effects with a half-maximal inhibitory concentration of approximately 7 µM and mediated apoptosis as detected by annexin V/PI and nuclear staining assays. Mechanistically, OBA-RT exerted dual roles, activating p53-dependent apoptosis and concomitantly suppressing CSC signals. A p53-dependent pathway was indicated by the induction of p53 and the depletion of anti-apoptotic Mcl-1 and Bcl-2 proteins. Cleaved PARP was detected in OBA-RT-treated cells. Quinuclidine derivatives (s)-T1, (s)-T2, and (s)-T6 are the ligands of α3β4 nicotinic acetylcholine receptor (nAChR). α3β4 nAChRs are highly expressed in medial habenula (MHb). The MHb connects to the brain rewarding pathway and mediates nicotine induce rewarding effect. การศึกษานี้เป็นการศึกษาเชิงสังเกตเพื่อหาปัจจัยที่สัมพันธ์กับการเกิดพิษต่อไตจากยาเคมีบำบัดกลุ่มแพลตินัมในผู้ป่วยมะเร็งชาวไทย เก็บข้อมูลจากเวชระเบียนผู้ป่วยที่อายุตั้งแต่ 18 ปีขึ้นไป ที่ได้รับสูตรยาเคมีบำบัดกลุ่มแพลตินัม ณ โรงพยาบาลจุฬาลงกรณ์ ระหว่างวันที่ 1 มกราคม พ.ศ. 2561 ถึง 31 ธันวาคม พ.ศ. 2561 โดยการประเมินพิษต่อไตประเมินจากระดับซีรัมครีเอทินีนที่เพิ่มขึ้นจากค่าพื้นฐานอย่างน้อยร้อยละ 25 ในการศึกษามีผู้ป่วย 240 คน อายุเฉลี่ย 55.6 ± 12.6 ปี เป็นเพศหญิงร้อยละ 55 ส่วนใหญ่เป็นมะเร็งระบบทางเดินอาหาร ร้อยละ 36.3 เป็นมะเร็งระยะแพร่กระจายร้อยละ 60 โดยส่วนใหญ่จะได้รับยาเคมีบำบัดกลุ่มแพลตินัมร่วมกับยาเคมีบำบัดชนิดอื่นในการรักษาร้อยละ 70.4 และได้รับรังสีรักษาร่วมด้วยร้อยละ 32.9 พบอุบัติการณ์การเกิดพิษต่อไต 63 คน (ร้อยละ 26.3) ผลการวิเคราะห์ถดถอยพหุคูณพบว่ามีปัจจัยที่สัมพันธ์กับการเกิดพิษต่อไตจากยาเคมีบำบัดกลุ่มแพลตินัมทั้งหมด 5 ปัจจัย โดยเป็นปัจจัยเสี่ยง 4 ปัจจัย ได้แก่ มีคะแนนสมรรถภาพร่างกาย ECOG PS 2 (ORadj = 11.447, 95%CI 1.84-71.40, p = 0.009) น้ำหนักตัวตั้งแต่ 60 กิโลกรัมขึ้นไป (ORadj = 2.200, 95%CI 1.05-4.63, p = 0.038) มีโรคประจำตัวตั้งแต่ 2 โรคขึ้นไป (ORadj = 2.884, 95%CI 1.30-6.38, p = 0.009) และโรคมะเร็งในระยะลุกลาม (ORadj = 4.595, 95%CI 1.30-16.28, p = 0.018) และปัจจัยป้องกัน 1 ปัจจัย คือ การได้รับคาร์โบพลาตินหรือออกซาลิพลาติน (ORadj = …

The antimalarial drug primaquine (PQ), which has minimal activity, has to be metabolized through the CYP2D6 enzyme for its most effective antimalarial activity. The stable form 5,6-orthoquinone-primaquine (5,6-PQ) was used as a representative marker for PQ active metabolite quantification. However, the relationship between CYP2D6 polymorphisms and the pharmacokinetic (PK) parameters of 5,6-PQ has not been reported. We assessed the relationship between CYP2D6 polymorphisms and PKs of both PQ and 5,6-PQ in the Thai army population on the Thailand-Cambodia border area. Alcoholic liver disease (ALD) is a significant public health problem worldwide. Numerous natural molecules, including curcumin, have been extensively studied as potential treatment agents for ALD. Tetrahydrocurcumin (THC), the reductive metabolite of curcumin, was previously reported to exert the potent antioxidant activity against chemical-induced oxidative stress suggesting the potential of THC to be used as a protective agent for ALD. Due to the limited aqueous solubility of FE, it may hamper the ocular bioavailability and results in low efficiency to treat such diseases. Folate receptors that are overexpressed in various cancer cells including breast cancers have become a promising strategy in targeted delivery of chemotherapeutics. In this study, the folic acid (FA)-conjugated chitosan/alginate nanocapsules loaded with turmeric oil (TO-FA-CS/Alg NC) were developed by ionotropic gelation technique to target folate receptors in MBA-MB-231 and MCF-7 breast cancer cell lines. The successful conjugation of FA with CS was confirmed with 1H-NMR and FT-IR, and Box-Behnken design was utilized in optimizing the formulation. การศึกษานี้มีวัตถุประสงค์เพื่อเปรียบเทียบระยะเวลาที่ผู้ป่วยได้รับยาต้านจุลชีพแบบคาดการณ์ครั้งแรกก่อนและหลังการเริ่มใช้ระบบดูแลผู้ป่วยที่มีภาวะพิษเหตุติดเชื้อของโรงพยาบาลรามาธิบดี (Ramathibodi Sepsis System : RSS) รวมไปถึงเพื่อศึกษาระยะเวลาในการได้รับยาต้านจุลชีพที่เหมาะสมกับผลเพาะเชื้ออัตราการเสียชีวิตในโรงพยาบาล อัตราการเสียชีวิตใน 28 วัน และระยะเวลาในการนอนโรงพยาบาล วิธีวิจัย : เป็นการวิจัยกึ่งทดลอง (Quasi-Experimental study) เปรียบเทียบช่วงก่อนและหลังการใช้ระบบ RSS ศึกษาในผู้ป่วยที่มีภาวะพิษเหตุติดเชื้อที่อายุมากกว่า 18 ปีและเข้ารับการรักษาในโรงพยาบาลรามาธิบดี ระหว่างวันที่ 29 มกราคม พ.ศ. 2564 – 30 เมษายน พ.ศ. 2565 ระบบ RSS เป็นการพัฒนาระบบที่จัดให้มีการทำงานร่วมกันของสหสาขาวิชาชีพประกอบด้วย แพทย์ เภสัชกร และพยาบาล พัฒนาการให้บริการแบบองค์รวมเพื่อการดูแลผู้ป่วยที่มีภาวะพิษเหตุติดเชื้อ โดยประกอบด้วย (1) การกำหนดแนวทางการวินิจฉัยผู้ป่วยที่มีภาวะพิษเหตุติดเชื้อที่เป็นรูปแบบมาตรฐาน เป็นระบบ รวดเร็ว และมีการระบุเวลาวินิจฉัยที่ชัดเจน (2) มีการกำหนดรายการยาต้านจุลชีพเพื่อบรรจุในเครื่องจ่ายยาอัตโนมัติซึ่งจัดตั้งอยู่ ณ บริเวณหอผู้ป่วย เพื่อให้ขั้นตอนการตรวจสอบคำสั่งใช้ยา การจ่ายยา รวดเร็ว (3) การติดตามตรวจสอบผลความไวของเชื้อต่อยาต้านจุลชีพที่ผู้ป่วยได้รับเพื่อให้ผู้ป่วยได้รับยาต้านจุลชีพที่เหมาะสมกับเชื้อก่อโรค ผลการวิจัย : ผู้ป่วยที่ผ่านเกณฑ์คัดเลือกเข้าร่วมการศึกษาจำนวน 137 คนแบ่งผู้ป่วยออกเป็น 2 กลุ่ม คือ กลุ่มก่อนใช้ระบบ RSS 50 คน (กลุ่มควบคุม) และกลุ่มหลังใช้ระบบ RSS 87 คน (กลุ่มศึกษา) พบว่ากลุ่มศึกษามีระยะเวลาในการบริหารยาต้านจุลชีพแบบคาดการณ์ครั้งแรกของผู้ป่วยเร็วกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ: 42 นาที (ค่าพิสัยระหว่างควอไทล์ 21.5, 57.5) เทียบกับ 76 นาที (ค่าพิสัยระหว่างควอไทล์ 35.0, 319.8; p < 0.001) และมีระยะเวลาในการบริหารยาต้านจุลชีพที่เหมาะสมตามผลเพาะเชื้อที่เร็วกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ: 48 ชั่วโมง (ค่าพิสัยระหว่างควอไทล์ 0.9, 84.0) เทียบกับ 96 ชั่วโมง (ค่าพิสัยระหว่างควอไทล์ 54.0,312.0 p = 0.009) นอกจากนั้นยังพบอัตราการเสียชีวิตในโรงพยาบาล เสียชีวิตใน 28 วัน และระยะเวลาในการนอนรักษาตัวในโรงพยาบาลลดลง ในกลุ่มศึกษาเมื่อเทียบกับกลุ่มควบคุมแต่ไม่มีนัยสำคัญทางสถิติที่ระดับ .05 สรุป : การใช้ระบบ RSS มาใช้ในการดูแลผู้ป่วยที่มีภาวะพิษเหตุติดเชื้อ ช่วยลดระยะเวลาในการบริหารยาต้านจุลชีพแบบคาดการณ์ครั้งแรก ลดระยะเวลาในการบริหารยาต้านจุลชีพที่เหมาะสมตามผลเพาะเชื้อ และ มีแนวโน้มส่งผลลดอัตราการเสียชีวิต

ปัจจัยที่มีผลต่อการเกิดภาวะเลือดออกในผู้ป่วยภาวะหัวใจเต้นผิดจังหวะที่ได้รับยาไรวาร็อกซาแบน, ภัทรพร สิริภัทรชัย

วัตถุประสงค์: เพื่อหาปัจจัยที่มีผลต่อการเกิดภาวะเลือดออกในผู้ป่วยภาวะหัวใจเต้นผิด จังหวะที่ได้รับยาไรวาร็อกซาแบน วิธีการ: งานวิจัยนี้เป็นการศึกษาเชิงวิเคราะห์ชนิดย้อนหลังจาก ผลไปหาเหตุแบบหลายศูนย์ รวบรวมข้อมูลผู้ป่วยที่มีภาวะหัวใจเต้นผิดจังหวะที่ได้รับยาไรวาร็อกซาแบน ณ โรงพยาบาลจุฬาลงกรณ์ และโรงพยาบาลรามาธิบดี ระหว่าง 1 ตุลาคม 2556 ถึง 30 กันยายน 2564 วิเคราะห์ปัจจัยที่มีผลต่อการเกิดภาวะเลือดออกด้วยสถิติการถดถอยโลจิสติค พหุคูณ ผลการวิจัย : กลุ่มตัวอย่าง 455 คน แบ่งเป็นกลุ่มศึกษาคือผู้ที่เกิดภาวะเลือดออก 91 คน ประกอบด้วยภาวะเลือดออกรุนแรง 20 คน ภาวะเลือดออกไม่รุนแรงที่มีความสำคัญทางคลินิก 45 คน และภาวะเลือดออกไม่รุนแรง 26 คน กลุ่มควบคุมคือผู้ที่ไม่เกิดภาวะเลือดออก 364 คน ผลการ วิเคราะห์พบปัจจัยที่มีผลต่อการเกิดภาวะเลือดออกอย่างมีนัยสำคัญทางสถิติ ได้แก่ ประวัติการเกิด ภาวะเลือดออก (adjusted OR: 20.29, p<0.001) ระดับอัลบูมินในเลือดต่ำ (adjusted OR: 2.19, p=0.020) ภาวะโลหิตจาง (adjusted OR: 2.03, p=0.026) และอันตรกิริยาระหว่างยา (adjusted OR: 2.40, p=0.007) เมื่อพิจารณาปัจจัยด้านอายุ น้ำหนัก ขนาดยาไรวาร็อกซาแบนต่อวัน การทำงานของไต ภาวะหัวใจล้มเหลว โรคหลอดเลือด และประวัติโรคหลอดเลือดสมองร่วมด้วย สรุป : ปัจจัยที่มีผลต่อการเกิดภาวะเลือดออกในผู้ป่วยภาวะหัวใจเต้นผิดจังหวะที่ได้รับยาไรวาร็อกซาแบน ได้แก่ ประวัติการเกิดภาวะเลือดออก ระดับอัลบูมินในเลือดต่ำ ภาวะโลหิตจาง อันตรกิริยาระหว่าง ยา ผู้ป่วยที่มีปัจจัยดังกล่าวจึงควรได้รับการดูแลติดตามอย่างใกล้ชิด เพื่อช่วยลดความเสี่ยงในการเกิดภาวะเลือดออกและเพิ่มความปลอดภัยจากการใช้ยาไรวาร็อกซาแบน
