Abstract

Background: Fetal alcohol spectrum disorders (FASD) are the most common yet completely preventable cause of mental disability in the western world.

Objective: The purpose of this integrative literature review is to examine and identify the common elements among varying interventions for FASD prevention that have demonstrated effectiveness for reducing the risk of FASD to better understand what defining features may contribute to the efficacy of the interventions.

Background: Previous studies have reported associations between prenatal maternal distress and negative health outcomes for the unborn. There are gaps within this body of knowledge and inconsistencies in the research which make the relevant relationships unclear. If changes in maternal cortisol cause alterations in fetal development, it is important to gain an adequate understanding of how, and if, maternal psychological distress predicts maternal cortisol levels. The link between life course adversity and adverse health outcomes is well established, particularly early life adversity (ELA). There is also evidence that the physiologic adaptations associated with stress, depression and ELA can be transmitted intergenerationally via long-term set-point changes within the maternal hypothalamic-pituitary-adrenal axis (HPA). It is unknown how the type and timing of maternal stress and adversity influences HPA regulation during pregnancy and whether maternal coping attenuates this relationship. Postpartum depression is recognized as the most common complication of childbearing; however, its etiology remains fairly undetermined. Many different influences have been hypothesized as to what may cause postpartum depression, including changes in levels of various hormones (such as estrogen and progesterone), a decrease in serotonin, low levels of vitamin D, social factors, and iron deficiency. The purpose of this study was to construct the explanatory models of postpartum depressive symptomatology (PPDS) from the perspective of rural Nebraska women and to compare these models with the medical model of PPDS. A sample of 20 rural Nebraska women were interviewed in a one-on-one qualitative descriptive telephone interview using questions based on Kleinman’s (1980) explanatory model of illness. This study used feminist pragmatism as a guiding philosophical paradigm. A study using qualitative descriptive methodology was done to learn of the decision pathways that mothers experiencing six types of perinatal loss recall making during their experiences of loss. Five participants describe their experience as they recall decisions within their journeys of miscarriage, stillbirth, fetal malformation; one carrying to term by choice (perinatal palliative care), the other terminating, a neonatal death and a SIDS death. Objective: To measure the effectiveness of a perinatal loss workshop on death anxiety of nursing students. Design: Simple, pretest-posttest. Setting: A classroom at a State University in California. Participants: 20 junior students in a baccalaureate nursing program in a maternal-child rotation. Intervention: A 60-minute perinatal loss workshop including Worden's tasks of mourning, therapeutic and non-therapeutic nursing interventions, photographs, poetry, and coping skills . Main Outcome Measure: Anxiety towards perinatal death was measured by Speilberger's State Trait Anxiety Inventory (STAI Form Y-1).
