This thesis proposal will investigate the impact of a comprehensive digital sexual health education program on the comfort levels of parents and teens when discussing sensitive topics such as pregnancy and sexually transmitted infections (STIs). BACKGROUND: Adherence to the standardized childhood immunization schedule put forth by the American Academy of Pediatrics is the gold standard for decreasing vaccine-preventable disease morbidity. The first step to improving adherence with the recommended immunization schedule is identifying children who are under-immunized. การศึกษาเชิงคุณภาพแบบบรรยายครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาประสบการณ์การดูแลบุตรของผู้ดูแลที่มีบุตรติดเชื้อไวรัสโคโรนา 2019 คัดเลือกผู้ให้ข้อมูลแบบเฉพาะเจาะจง คือ ผู้ดูแลซึ่งเป็นมารดาของผู้ป่วยเด็กวัยเรียนที่ติดเชื้อไวรัสโคโรนา 2019 ร่วมกับมีภาวะปอดอักเสบระดับปานกลางหรือรุนแรง เข้ารับการรักษาในห้องแยก เก็บข้อมูลด้วยการสัมภาษณ์เชิงลึก จนข้อมูลอิ่มตัวด้วยจำนวนผู้ให้ข้อมูล 7 ราย นำข้อมูลมาวิเคราะห์เนื้อหาเกิดเป็นประเด็นหลัก 3 ประเด็น คือ 1) ดูแลอย่างใกล้ชิด และเป็นคนทำทุกอย่าง 2) ความยากและลำบาก และ 3) ความรู้สึกขณะที่อยู่กับบุตรตลอดเวลาออกจากห้องแยกไม่ได้ ผลการศึกษาก่อให้เกิดความเข้าใจที่ชัดเจนมากยิ่งขึ้นเกี่ยวกับประสบการณ์การดูแลบุตรในห้องแยกของผู้ดูแลที่มีบุตรติดเชื้อไวรัสโคโรนา 2019 และสามารถนำข้อมูลการวิจัยนี้ไปใช้เป็นข้อมูลพื้นฐานในการดูแลผู้ป่วยเด็กได้อย่างเหมาะสม และตรงตามความต้องการของผู้ป่วยและผู้ดูแลอย่างแท้จริง รวมถึงเป็นข้อมูลเบื้องต้นสำหรับการจัดการองค์ความรู้เกี่ยวกับการดูแลผู้ป่วยเด็กโรคอุบัติใหม่ที่ต้องได้รับการรักษาภายในห้องแยก

การวิจัยเชิงคุณภาพแบบบรรยายนี้มีวัตถุประสงค์ในการบรรยายประสบการณ์การดูแลสุขภาพตนเองของวัยรุ่นที่มีอาการหลงเหลือหลังติดเชื้อไวรัสโคโรนา 2019 ผู้ให้ข้อมูลคือ วัยรุ่น ที่ได้รับทราบจากแพทย์ว่าเป็นผู้มีอาการหลงเหลือจากการติดเชื้อไวรัสโคโรนา 2019 และไม่มีโรคประจำตัวอื่น ๆ ใน โรงพยาบาลของจังหวัดอุบลราชธานี เก็บข้อมูลด้วยการสัมภาษณ์เชิงลึกไปพร้อมกับวิเคราะห์ข้อมูลแบบแก่นสาระ จนเกิดความอิ่มตัวของข้อมูล รวมผู้ให้ข้อมูลทั้งสิ้น 7 ราย ผลการศึกษาพบข้อสรุปดังนี้ 1) ชีวิตที่ต้องเผชิญกับผลพวงของโควิด-19 ประกอบด้วยประเด็นย่อย ได้แก่ 1.1 การเปลี่ยนแปลงในชีวิต และ 1.2 ผลกระทบต่อความรู้สึกทางอารมณ์และจิตใจ 2) การค้นหาวิธีจัดการกับอาการเรื้อรัง ประกอบด้วยประเด็นย่อย ได้แก่ 2.1. การแสวงหาวิธีการดูแลสุขภาพ และ 2.2. การดูแลอาการที่เกิดขึ้นด้วยตนเอง ข้อสรุปนี้ช่วยให้พยาบาลเด็กเข้าใจถึงประสบการณ์และความท้าทายที่วัยรุ่นกลุ่มนี้ต้องเผชิญ และสามารถนำข้อมูลนี้ไปใช้ในการให้คำปรึกษาและสนับสนุนทางอารมณ์แก่วัยรุ่นและครอบครัวที่ต้องเผชิญกับอาการหลงเหลือหลังติดเชื้อไวรัสโคโรนา 2019 การให้คำแนะนำวิธีการดูแลสุขภาพและเทคนิคการจัดการอาการเรื้อรังที่เหมาะสมกับแต่ละบุคคล รวมทั้งการพัฒนาแนวทางการดูแลสุขภาพที่เน้นการปรับตัวและการรับมือกับอาการหลงเหลือหลังติดเชื้อไวรัสโคโรนา 2019 เพื่อให้วัยรุ่นกลุ่มนี้มีคุณภาพชีวิตที่ดีขึ้น

การวิจัยนี้เป็นการวิจัยกึ่งทดลอง ศึกษาสองกลุ่มแบบวัดก่อนและหลังการทดลอง มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการส่งเสริมสุขภาพโดยใช้สมาร์ทโฟนแอปพลิเคชันไลน์ต่อพฤติกรรมป้องกันอาการกำเริบของโรคจมูกอักเสบภูมิแพ้ของผู้ดูแลเด็กวัยเรียนโรคจมูกอักเสบภูมิแพ้ อายุ 6-12 ปี ที่เข้ารับบริการที่คลินิกโรคภูมิแพ้ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย จำนวน 60 คน แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุมกลุ่มละ 30 คน กลุ่มทดลองได้รับโปรแกรมการส่งเสริมสุขภาพโดยใช้สมาร์ทโฟนแอปพลิเคชันไลน์ที่ออกแบบโดยใช้แนวคิดของเพนเดอร์ (2001) ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ กำกับการทดลองด้วยแบบสอบถามการรับรู้ประโยชน์ รับรู้อุปสรรค และรับรู้ความสามารถของตนต่อการปฏิบัติพฤติกรรม ประเมินพฤติกรรมป้องกันอาการกำเริบของโรคจมูกอักเสบภูมิแพ้ด้วยแบบสอบถามพฤติกรรมป้องกันอาการกำเริบของโรคจมูกอักเสบภูมิแพ้ เครื่องมือทุกฉบับมีค่าความตรงตามเนื้อหาและค่าความเที่ยงที่ยอมรับได้ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและสถิติทดสอบที (t-test) ผลการวิจัย พบว่า ผู้ดูแลเด็กโรคจมูกอักเสบภูมิแพ้กลุ่มที่ได้รับโปรแกรมการส่งเสริมสุขภาพโดยใช้สมาร์ทโฟนแอปพลิเคชันไลน์ มีพฤติกรรมป้องกันอาการกำเริบของโรคสูงกว่าก่อนได้รับโปรแกรมฯ อย่างมีนัยสำคัญทางสถิติ (t= 7.68, p< .05) และผู้ดูแลเด็กโรคจมูกอักเสบภูมิแพ้กลุ่มที่ได้รับโปรแกรมการส่งเสริมสุขภาพโดยใช้สมาร์ทโฟนแอปพลิเคชันไลน์ มีพฤติกรรมป้องกันอาการกำเริบของโรคจมูกอักเสบภูมิแพ้สูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติ (t= -4.25, p< .05)

การวิจัยกึ่งทดลองครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการดูแลตนเองที่เน้นการฝึกหายใจด้วยการเป่าลูกโป่งต่อความรุนแรงของการหยุดหายใจและความแข็งแรงของกล้ามเนื้อหายใจของเด็กวัยเรียนที่ได้รับการวินิจฉัยว่ามีภาวะหยุดหายใจขณะหลับจากการอุดกั้น ในระดับน้อยถึงปานกลาง ที่มารับการรักษาในแผนกผู้ป่วยนอก หู คอ จมูก โรงพยาบาลศิริราช จำนวน 30 คน แบ่งเป็นกลุ่มที่ได้รับโปรแกรมการดูแลตนเองที่เน้นการฝึกหายใจด้วยการเป่าลูกโป่ง ระยะเวลา 5 สัปดาห์ตามแนวคิดการพัฒนาความสามารถในการดูแลตนเองของโอเร็ม (2001) จำนวน 15 คน และ กลุ่มที่ได้รับการพยาบาลตามปกติ จำนวน 15 คน เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย แบบประเมินวัดความรุนแรงของการหยุดหายใจ แบบประเมินพฤติกรรมการดูแลตนเองของเด็กที่มีภาวะหยุดหายใจขณะหลับจากการอุดกั้น โดยเครื่องมือทั้งหมดมีค่าความตรงเชิงเนื้อหาเท่ากับ 0.95 และ 0.80 และค่าความเที่ยงเท่ากับ 0.83 และ 0.76 ตามลำดับ ประเมินความแข็งแรงของกล้ามเนื้อหายใจด้วยเครื่องวัดกำลังกล้ามเนื้อหายใจด้วยเครื่อง Respiratory Pressure Meter วิเคราะห์ข้อมูลโดยใช้สถิติการวิเคราะห์สถิติที ผลการวิจัยพบว่า ความรุนแรงของการหยุดหายใจของเด็กวัยเรียนที่มีภาวะหยุดหายใจขณะหลับจากการอุดกั้นหลังได้รับโปรแกรมการดูแลตนเองที่เน้นการฝึกหายใจด้วยการเป่าลูกโป่งลดลงเมื่อเทียบกับก่อนได้รับโปรแกรมฯ และน้อยกว่าเมื่อเทียบกับกลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 นอกจากนี้ยังพบว่าความแข็งแรงของกล้ามเนื้อหายใจของเด็กวัยเรียนที่มีภาวะหยุดหายใจขณะหลับจากการอุดกั้นหลังได้รับโปรแกรมฯ เพิ่มขึ้นเมื่อเทียบกับก่อนได้รับโปรแกรมฯ และดีกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 แต่ค่าแรงดันการหายใจเข้าสูงสุด(MIP) ของกลุ่มที่ได้รับโปรแกรมฯ กับกลุ่มที่ได้รับการพยาบาลตามปกติ ไม่แตกต่างกัน

การวิจัยกึ่งทดลองนี้มีวัตถุประสงค์เพื่อเปรียบเทียบโอกาสการพบโปรตีนในปัสสาวะของผู้ป่วยเด็กวัยเรียนกลุ่มอาการเนโฟรติกภายหลังได้รับการพยาบาลเพื่อส่งเสริมสมรรถนะแห่งตนในการปฏิบัติพฤติกรรมการรับประทานอาหารที่เหมาะสมและการพยาบาลตามปกติ ระยะหลังการทดลองทันที ระยะหลังการทดลอง 1 วัน และระยะหลังการทดลอง 2 วัน โดยการพยาบาลเพื่อส่งเสริมสมรรถนะแห่งตนนี้พัฒนามาจากทฤษฎีสมรรถนะแห่งตนของ Bandura (2001) กลุ่มตัวอย่าง คือ เด็กวัยเรียนอายุ 6-12 ปี กลุ่มอาการเนโฟรติกที่เข้ารับการรักษาในแผนกผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย จำนวน 40 คน แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 20 คนโดยจับคู่การใช้ยาสเตียรอยด์และยาควบคุมความดันโลหิตแล้วสุ่มเข้ากลุ่มโดยจับฉลาก เครื่องมือที่ใช้ในการทดลอง ได้แก่ การพยาบาลเพื่อส่งเสริมสมรรถนะแห่งตนในการปฏิบัติพฤติกรรมการรับประทานอาหารที่เหมาะสม แถบทดสอบปัสสาวะ และแบบสอบถามพฤติกรรมการรับประทานอาหารของเด็กกลุ่มอาการเนโฟรติก ซึ่งผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ และค่าความเที่ยงเท่ากับ 1.00 และ 0.80 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติการวิเคราะห์ถดถอยโลจิสติคทวิ ผลการวิจัยพบว่า กลุ่มผู้ป่วยเด็กกลุ่มอาการเนโฟรติกที่ได้รับการพยาบาลเพื่อส่งเสริมสมรรถนะแห่งตนในการปฏิบัติพฤติกรรมการรับประทานอาหารที่เหมาะสมมีโอกาสตรวจพบโปรตีนในปัสสาวะในระยะหลังการทดลอง 2 วัน น้อยกว่ากลุ่มที่ได้รับการพยาบาลตามปกติร้อยละ 92.00 อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

Background: Children in the preoperative area experience presurgical anxiety at rates of up to seventy-five percent. Increased pediatric anxiety is directly linked to increased anesthetic requirements, increased risk for development of emergence delirium, and increased time spent in the post anesthesia care unit. Numerous high-quality studies show that intervention by a certified child life specialist mitigates preoperative pediatric anxiety.
