Purpose: Rapid clinical assessment of newly arriving direct admission veteran patients to the Department of Veterans Affairs Health Care System (VAHCS) Lexington is of the utmost importance to determine current patient condition, alteration from previously known condition at time of provider acceptance or time of report, and appropriate level of care for these veterans. การศึกษานี้เป็นการวิจัยเชิงบรรยาย มีวัตถุประสงค์เพื่อศึกษาบทบาทพยาบาลเวชศาสตร์การบิน โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลคือ ผู้เชี่ยวชาญด้านเวชศาสตร์การบิน จำนวน 19 คน ประกอบด้วย ผู้อำนวยการหรือผู้บริหารด้านเวชศาสตร์การบิน จำนวน 3 คน แพทย์เวชศาสตร์การบิน จำนวน 3 คน คณะกรรมการหลักสูตรหรืออาจารย์ผู้สอนในหลักสูตรที่เกี่ยวข้องกับงานด้านเวชศาสตร์การบิน จำนวน 3 คน หัวหน้าพยาบาลเวชศาสตร์การบินหรือลำเลียงผู้ป่วยทางอากาศ จำนวน 4 คน และพยาบาลเวชศาสตร์การบินหรือลำเลียงผู้ป่วยทางอากาศ จำนวน 6 คน เครื่องมือที่ใช้ในการวิจัย ประกอบด้วยแบบสอบถามแบบปลายเปิด และแบบสอบถามมาตราส่วนประมาณค่า 5 ระดับ การดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ 1) สัมภาษณ์ผู้เชี่ยวชาญเพื่อรวบรวมความคิดเห็นเกี่ยวกับบทบาทพยาบาลเวชศาสตร์การบิน 2) นำข้อมูลที่ได้จากการสัมภาษณ์มาวิเคราะห์เนื้อหาและนำมาสร้างแบบสอบถามเพื่อให้ผู้เชี่ยวชาญประมาณค่าระดับความสำคัญ 3) นำข้อมูลที่ได้จากการตอบแบบสอบถามประมาณค่าความสำคัญมาคำนวณค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์ และส่งแบบสอบถามให้ผู้เชี่ยวชาญกลุ่มเดิมยืนยันความคิดเห็นอีกครั้ง ผลการวิจัย พบว่า บทบาทพยาบาลเวชศาสตร์การบิน ประกอบด้วย 6 บทบาท ดังนี้ 1) ด้านผู้ปฏิบัติการพยาบาลเวชศาสตร์การบินจำนวน 16 ข้อ 2) ด้านผู้จัดการนิรภัยการบินและนิรภัยภาคพื้น จำนวน 8 ข้อ 3) ด้านผู้จัดการความปลอดภัยและจัดการความเสี่ยง จำนวน 16 ข้อ 4) ด้านผู้ประสานงานและสื่อสาร จำนวน 11 ข้อ 5) ด้านผู้นำการทำงานเป็นทีม การจัดการ และการให้คำปรึกษา จำนวน 18 ข้อ และ 6) ด้านผู้พัฒนาคุณภาพและมาตรฐานการบริการ จำนวน 11 ข้อ

การศึกษานี้เป็นการศึกษาเชิงบรรยาย เพื่อศึกษาบทบาทพยาบาลผู้เชี่ยวชาญไตเทียม โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลคือผู้เชี่ยวชาญจำนวน 18 คน วิธีดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ ขั้นตอนที่ 1 สัมภาษณ์ผู้เชี่ยวชาญเกี่ยวกับบทบาทพยาบาลผู้เชี่ยวชาญไตเทียม ขั้นตอนที่ 2 นำข้อมูลที่ได้จากการสัมภาษณ์มาวิเคราะห์เนื้อหาแล้วสร้างเป็นแบบสอบถามให้ผู้เชี่ยวชาญประมาณค่าแนวโน้มความสำคัญของบทบาทพยาบาลผู้เชี่ยวชาญไตเทียม และขั้นที่ 3 นำข้อมูลที่ได้จากรอบที่ 2 มาคำนวณหาค่ามัธยฐาน ค่าพิสัยระหว่างควอไทล์ และส่งแบบสอบถามให้ผู้เชี่ยวชาญกลุ่มเดิมยืนยันคำตอบ จากนั้นจึงนำข้อมูลที่ได้มาคำนวณค่ามัธยฐาน และค่าพิสัยควอไทล์อีกครั้ง เพื่อสรุปเป็นบทบาทพยาบาลผู้เชี่ยวชาญไตเทียม ผลการวิจัยพบว่า บทบาทพยาบาลผู้เชี่ยวชาญไตเทียม ประกอบด้วย 6 บทบาท จำนวน 87 ข้อรายการ ดังนี้ 1) ด้านผู้ปฏิบัติการพยาบาลผู้เชี่ยวชาญไตเทียม จำนวน 40 ข้อ 2) ด้านผู้สอนและให้คำปรึกษา จำนวน 11 ข้อ 3) ด้านผู้ประสานงานและสื่อสาร จำนวน 7 ข้อ 4) ด้านผู้จัดการความปลอดภัยและบริหารความเสี่ยง จำนวน 10 ข้อ 5) ด้านผู้พัฒนาคุณภาพการพยาบาลการดูแลผู้ป่วยไตเทียม จำนวน 12 ข้อ และ6) ด้านผู้สร้างและใช้นวัตกรรมและเทคโนโลยี จำนวน 7 ข้อ

Urgent action is needed to strengthen mental health support for nurses experiencing burnout. This thesis examines the stigma surrounding burnout in the nursing community and implements holistic approaches to alleviate burnout of those affected. Integrating mental health awareness, mindfulness-based interventions, personal self-care, and supportive hospital practice can decrease nurse burnout. By first coming to terms with nurse turnover and further prioritizing nurses' physiological, emotional, and social welfare, leadership and organization can implement holistic strategies to address these diverse challenges nurses face. การวิจัยนี้เป็นการวิจัยแบบกึ่งทดลองเพื่อศึกษาเปรียบเทียบ ความสามารถในการดูแลผู้ป่วยวิกฤตของพยาบาลวิชาชีพหอผู้ป่วยวิกฤตศัลยกรรมที่ได้รับโปรแกรมการนิเทศทางการพยาบาลร่วมกับการใช้ไลน์แอปพลิเคชันในระยะก่อน - หลัง และระยะติดตามผลโดยใช้แนวคิดการนิเทศของพรอคเตอร์และการ บรูณาการการประเมินการดูแลผู้ป่วยวิกฤตของวินเซนต์และคณะร่วมกับไลน์แอปพลิเคชัน โดยศึกษาในพยาบาลวิชาชีพหอผู้ป่วยวิกฤตศัลยกรรมผู้มีประสบการณ์การปฏิบัติงานในหอผู้ป่วยวิกฤตศัลยกรรม 3 ถึง 5 ปี จำนวน 12 คน ตามแบบแผนการวิจัยแบบศึกษากลุ่มเดียว วัดก่อนและหลังการทดลอง เครื่องมือที่ใช้ในงานวิจัยประกอบด้วย1) โปรแกรมการนิเทศทางการพยาบาลร่วมกับไลน์แอปพลิเคชัน 2)คู่มือการนิเทศตามโปรแกรม 3)แบบสอบถามประเมินความสามารถในการดูแลผู้ป่วยวิกฤตศัลยกรรม มีค่าความตรง 0.99 4)แบบวัดความรู้เรื่องการประยุกต์ใช้แนวคิดการนิเทศทางการพยาบาลของพยาบาลผู้นิเทศ มีค่าความตรง 0.98 เครื่องมือทั้งหมดผ่านการตรวจสอบคุณภาพเครื่องมือจากผู้ทรงคุณวุฒิ 5 คน และวิเคราะห์หาความเที่ยงรายด้านคือ ด้านความสามารถด้านการปฏิบัติทางการพยาบาล 0.94 ความสามารถในการจัดการป้องกันความเสี่ยงในด้านการติดเชื้อ 0.84 ความสามารถในการบริหารยาความเสี่ยงสูง 0.81โดยใช้แบบประเมินเป็นแบบสอบถามประเมินความสามารถในการดูแลผู้ป่วยวิกฤตศัลยกรรมเก็บข้อมูลก่อน-หลังการได้รับโปรแกรม และระยะติดตามหลังได้รับโปรแกรมการนิเทศ 4 สัปดาห์ วิเคราะห์ข้อมูลโดยแจกแจงความถี่จำนวนร้อยละ ค่าเฉลี่ยส่วนเบี่ยงเบนมาตรฐาน และใช้สถิติทดสอบWilcoxon signed-rank test ผลการวิจัยพบว่าความสามารถในการดูแลผู้ป่วยวิกฤตของพยาบาลวิชาชีพหอผู้ป่วยวิกฤตศัลยกรรมภายหลังการได้รับโปรแกรมการนิเทศทางการพยาบาลร่วมกับไลน์แอปพลิเคชันสูงกว่าก่อนได้รับโปรแกรมการนิเทศทางการพยาบาลร่วมกับไลน์แอปพลิเคชัน อย่างมีนัยสำคัญทางสถิติที่ระดับ.05

การวิจัยครั้งนี้เป็นลักษณะกึ่งทดลอง มีวัตถุประสงค์เพื่อพัฒนาโปรแกรมการให้บริการผู้ป่วยที่เข้ารับการผ่าตัดต้อกระจกโดยแนวคิดลีนร่วมกับโมบายแอปพลิเคชันและเพื่อเปรียบเทียบความพึงพอใจในบริการพยาบาลของผู้ป่วย ระหว่างกลุ่มผู้ป่วยที่ได้รับการบริการผ่าตัดต้อกระจกตามปกติ กับกลุ่มผู้ป่วยที่ได้รับโปรแกรมการให้บริการผู้ป่วยที่เข้ารับการผ่าตัดต้อกระจกโดยแนวคิดลีนร่วมกับโมบายแอปพลิเคชัน ห้องตรวจโรคจักษุกรรม โรงพยาบาลพระมงกุฎเกล้า กลุ่มตัวอย่าง คือกลุ่มผู้ป่วยที่ได้รับการวินิจฉัยว่าเป็นโรคต้อกระจกทั้งเพศหญิงและเพศชายที่เข้ารับการบริการผ่าตัดต้อกระจกตามคุณสมบัติที่ผู้วิจัยกำหนด จำนวน 40 คน โดยแบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม จำนวนกลุ่มละ 20 คน จัดเข้ากลุ่มโดยวิธีการจับคู่ (Matched pair) ในด้านเพศ อายุ ระดับการศึกษา และจำนวนครั้งในการผ่าตัดต้อกระจก เพื่อให้มีคุณสมบัติใกล้เคียงกัน เครื่องมือที่ใช้ในการวิจัยประกอบด้วย 1) โปรแกรมการให้บริการผู้ป่วยที่เข้ารับการผ่าตัดต้อกระจกโดยแนวคิดลีนร่วมกับโมบายแอปพลิเคชัน 2) แบบประเมินความพึงพอใจในบริการพยาบาล เครื่องมือทั้งหมดผ่านการตรวจสอบคุณภาพเครื่องมือจากผู้ทรงคุณวุฒิจำนวน 5 ท่าน แบบประเมินความพึงพอใจได้ค่าความตรง .86 และตรวจสอบความเที่ยง ได้ค่าสัมประสิทธิ์แอลฟ่าครอนบาค .98 วิเคราะห์ข้อมูลโดยคำนวณค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติทดสอบที (Independent t-test statistic) ผลการวิจัยสรุปได้ว่า ความพึงพอใจในบริการพยาบาลของผู้ป่วยที่เข้ารับโปรแกรมการให้บริการผ่าตัดต้อกระจกโดยแนวคิดลีนร่วมกับโมบายแอปพลิเคชัน สูงกว่ากลุ่มที่ได้รับการบริการผ่าตัดต้อกระจกตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05
