ปัจจุบันอุบัติการณ์การเกิดภาวะซึมเศร้ามีแนวโน้มเพิ่มมากขึ้น โดยเฉพาะภาวะซึมเศร้าที่เกิดขึ้น ในกลุ่มสตรีที่กำลังอยู่ในระยะตั้งครรภ์ ระยะคลอด และระยะหลังคลอด ซึ่งภาวะซึมเศร้าที่เกิดขึ้น ส่งผลกระทบที่รุนแรงทั้งด้านร่างกาย ด้านจิตใจของผู้ที่เป็นมารดา ส่งผลกระทบต่อทารกในครรภ์ ส่งผลกระทบต่อการปรับบทบาทในการเป็นมารดาและสัมพันธภาพระหว่างมารดาและทารก รวมทั้ง ส่งผลกระทบต่อสัมพันธภาพระหว่างคู่สมรสและสัมพันธภาพในครอบครัว ดังนั้นพยาบาลจึงควรมีความรู้ความเข้าใจและให้ความสำคัญกับการพยาบาลสตรีที่มีภาวะซึมเศร้าตั้งแต่ในระยะตั้งครรภ์ ระยะคลอด และระยะหลังคลอด โดยป้องกันผลกระทบที่อาจจะเกิดขึ้นกับสตรีที่มีภาวะซึมเศร้าตั้งแต่ในระยะตั้งครรภ์ ระยะคลอด และระยะหลังคลอดเพื่อให้สตรีที่มีภาวะซึมเศร้าสามารถใช้ชีวิตในสังคมและดำรงบทบาท การเป็นมารดาได้อย่างมีความสุข

วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมการฟื้นคืนสู่สุขภาวะต่ออาการซึมเศร้าของผู้สูงอายุโรคซึมเศร้าในชุมชน

รูปแบบการวิจัย: งานวิจัยกึ่งทดลอง

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้สูงอายุโรคซึมเศร้าในชุมชนที่มีอายุ 60 ปีขึ้นไป ที่ได้รับการวินิจฉัยว่าเป็นโรคซึมเศร้า มีคุณสมบัติตามเกณฑ์ โดยได้รับการสุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 21 คู่ กลุ่มทดลองได้รับโปรแกรมการฟื้นคืนสู่สุขภาวะผู้สูงอายุโรคซึมเศร้าเป็นระยะเวลา 3 สัปดาห์ ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการวิจัย คือ 1) โปรแกรมการฟื้นคืนสู่สุขภาวะผู้สูงอายุโรคซึมเศร้า และ 2) แบบประเมินภาวะซึมเศร้าของเบคฉบับภาษาไทย มีค่าดัชนีความตรงเชิงเนื้อหา เท่ากับ .81 และมีค่าความเที่ยงสัมประสิทธิ์อัลฟ่าของครอนบาค เท่ากับ .80 วิเคราะห์ข้อมูลโดยใช้สถิติทดสอบที

ผลการวิจัย: 1) คะแนนเฉลี่ยอาการซึมเศร้าของผู้สูงอายุโรคซึมเศร้าในชุมชนที่ได้โปรแกรมการฟื้นคืนสู่สุขภาวะน้อยกว่าก่อนเข้าร่วมโปรแกรม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และ 2) คะแนนเฉลี่ยอาการซึมเศร้าของผู้สูงอายุโรคซึมเศร้าในชุมชนกลุ่มที่ได้รับโปรแกรมการฟื้นคืนสู่สุขภาวะน้อยกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

วัตถุประสงค์:  เพื่อศึกษาความสัมพันธ์ระหว่างภาวะซึมเศร้า การรับรู้สมรรถนะแห่งตน การรับรู้ความเจ็บป่วย การสนับสนุนทางสังคม ระดับการศึกษาทางโลก ระดับการศึกษาทางธรรม และดัชนีมวลกาย กับความกระตือรือร้นในการดูแลตนเอง

รูปแบบการวิจัย: เชิงพรรณนาสหสัมพันธ์

วิธีการดำเนินการวิจัย: กลุ่มตัวอย่างคือ พระสงฆ์โรคความดันโลหิตสูงอายุ 60 ปีขึ้นไป ที่มารับบริการแผนกผู้ป่วยนอก โรงพยาบาลสงฆ์ 84 รูป เครื่องมือที่ใช้ คือ แบบสอบถามข้อมูลส่วนบุคคล แบบประเมิน ภาวะซึมเศร้า แบบสอบถามการรับรู้สมรรถนะแห่งตน แบบสอบถามการรับรู้ความเจ็บป่วย แบบสอบถามการสนับสนุนทางสังคม แบบสอบถามความกระตือรือร้นในการดูแลตนเอง ค่าสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ .84, .81, .86, .87, .88 ตามลำดับ วิเคราะห์ข้อมูลโดยการหาค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่าสัมประสิทธิ์สหสัมพันธ์แบบเพียร์สันและพอยท์ไบซีเรียล

ผลการศึกษา:  ประมาณ 1 ใน 3 ของพระสงฆ์ฯ เท่านั้น (ร้อยละ 38.1) ที่มีความกระตือรือร้นในการดูแลตนเองระดับ 4 คือ มีการปรับเปลี่ยนพฤติกรรมใหม่ และคงไว้ได้ แม้อยู่ในสภาวะกดดัน โดยปัจจัยที่มีความสัมพันธ์ทางบวกกับความกระตือรือร้นในการดูแลตนเอง ได้แก่ การรับรู้สมรรถนะแห่งตน การรับรู้ความเจ็บป่วย การสนับสนุนทางสังคม และระดับการศึกษาทางโลก (r = .736, r = .659, r = .386, r = .295 ตามลำดับ) ส่วนภาวะซึมเศร้ามีความสัมพันธ์ทางลบกับความกระตือรือร้นในการดูแลตนเอง (r = -.765) อย่างมีนัยสำคัญทางสถิติ .05

สรุป: การส่งเสริมความกระตือรือร้นในการดูแลตนเองของพระสงฆ์ อาจพิจารณานำปัจจัยที่มีความสัมพันธ์จากการค้นพบในการศึกษานี้ เป็นแนวทางในการสร้างโปรแกรมส่งเสริมความกระตือรือร้นในการดูแลตนเองของพระสงฆ์สูงอายุอาพาธโรคความดันโลหิตสูงต่อไป

วัตถุประสงค์: เพื่อศึกษาความเหนื่อยหน่ายของคู่ชีวิตของผู้ป่วยโรคจิตเภท และปัจจัยสัมพันธ์ระหว่าง เพศ ระยะเวลาในการดูแลผู้ป่วย ความเครียด การสนับสนุนทางสังคม สัมพันธภาพผู้ป่วยกับคู่ชีวิต ภาระการดูแลผู้ป่วย การรับรู้การถูกตีตรา อาการทางบวก และอาการทางลบ กับความเหนื่อยหน่ายของคู่ชีวิตของผู้ป่วยโรคจิตเภท

รูปแบบการวิจัย: การวิจัยเชิงบรรยายหาความสัมพันธ์

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ คู่ชีวิตของผู้ป่วยโรคจิตเภทที่เข้ารับการรักษาในโรงพยาบาลจิตเวชและโรงพยาบาลที่มีแผนกผู้ป่วยนอกจิตเวช ในเขตกรุงเทพมหานคร จำนวน 180 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล แบบประเมินความเหนื่อยหน่าย แบบวัดความเครียด แบบสอบถามการสนับสนุนทางสังคม แบบประเมินสัมพันธภาพผู้ป่วยกับคู่ชีวิต แบบประเมินภาระการดูแลผู้ป่วย แบบประเมินการรับรู้การถูกตีตรา แบบประเมินอาการทางบวก และแบบประเมินอาการทางลบผู้ป่วยโรคจิตเภท วิเคราะห์ข้อมูลโดยสถิติเชิงพรรณาและสถิติสัมประสิทธิ์สหสัมพันธ์เพียร์สัน

ผลการวิจัย: ระดับความเหนื่อยหน่ายทั้ง 3 ด้าน พบว่า ด้านความอ่อนล้าทางอารมณ์อยู่ในระดับต่ำ ( = 11.02, SD = 10.83) ด้านการมองข้ามความเป็นบุคคลของผู้ป่วยและผู้อื่นอยู่ในระดับปานกลาง( = 6.36, SD = 8.06) และด้านความไม่สมหวังในผลสำเร็จของผู้ดูแลอยู่ในระดับสูง ( = 13.45, SD=9.64) การศึกษาความสัมพันธ์ระหว่างปัจจัยต่าง ๆ กับความเหนื่อยหน่ายมีค่านัยสำคัญทางสถิติที่ระดับ .05 ดังนี้ สัมพันธภาพผู้ป่วยกับคู่ชีวิต และการสนับสนุนทางสังคมมีความสัมพันธ์ทางลบ (r = -.378 และ -.154 ตามลำดับ) อาการทางลบของผู้ป่วยโรคจิตเภทมีความสัมพันธ์ทางบวก (r = .305) อาการทางบวกของผู้ป่วยโรคจิตเภทมีความสัมพันธ์ทางบวก (r = .509) ภาระการดูแลผู้ป่วยและการรับรู้การถูกตีตราของคู่ชีวิตมีความสัมพันธ์ทางบวก (r = .779 และ .711 ตามลำดับ) ความเครียดมีความสัมพันธ์ทางบวก (r = .819) เพศและระยะเวลาในการดูแลผู้ป่วย ไม่มีความสัมพันธ์

สรุป: ผลการวิจัยสามารถนำไปวางแผนการจัดกิจกรรมเพื่อลดปัจจัยที่ส่งผลต่อความเหนื่อยหน่ายของคู่ชีวิตของผู้ป่วยโรคจิตเภทต่อไป

ภาวะเซพซิส คือ ภาวะคุกคามต่อชีวิต ต้องได้รับการดูแลในหอผู้ป่วยวิฤต โดยเฉพาะอย่างยิ่งในผู้สูงอายุที่มีแนวโน้มเกิดภาวะแทรกซ้อนร้ายแรง ใช้เวลาในการรักษาตัวนาน และมีอัตราการเสียชีวิตสูงเมื่อเทียบกับผู้ป่วยที่มีอายุน้อยกว่า ประกอบกับปัจจัยทางด้านกระบวนการสูงอายุ ได้แก่ ระบบภูมิคุ้มกันลดลงและระบบร่างกายมีการเสื่อมถอย และลักษณะสำคัญที่พบได้ในผู้สูงอายุคือ อาการและอาการแสดงไม่เฉพาะเจาะจง เช่น ไม่มีไข้ ภาวะสับสนเฉียบพลัน รับประทานอาหารได้ลดลง และอ่อนเพลีย เป็นต้น ส่งผลให้เกิดปัญหาสุขภาพที่ซับซ้อนและรุนแรง การคัดกรองเพื่อให้ผู้สูงอายุได้รับการรักษาอย่างถูกต้องและรวดเร็วจะสามารถช่วยลดภาวะอวัยวะในร่างกายล้มเหลวหลายระบบและอัตราการเสียชีวิต ดังนั้นพยาบาลวิกฤติต้องมีความรู้ ความเข้าใจ และความสามารถในการดูแลผู้สูงอายุที่มีภาวะเซพซิส ซึ่งต้องได้รับการดูแลจากพยาบาลที่มีความเชี่ยวชาญ เพื่อให้ได้รับการคัดกรองอย่างรวดเร็ว วินิจฉัยที่ถูกต้อง และได้รับการรักษาที่มีมาตรฐาน อีกทั้งทำให้ผู้ป่วยปลอดภัยและไม่เกิดภาวะแทรกซ้อนรุนแรง

An ostomy is a surgically created opening and can be performed to divert faecal effluent by exteriorisation of a segment of the small bowel otherwise known as the ilium, or from the large bowel otherwise known as the colon. A urinary diversion of effluent generally involves exteriorisation of a segment of ileum into which the ureters have been anastomosed. Abstract

Background: Minimally invasive esophagectomy surgery is commonly performed by thoracic surgeons on patients with esophageal cancer. The post-operative care of these patients is meticulous and follows a protocol developed by the thoracic surgeons at the University of Kentucky. At UK Chandler Medical Center, concerns have been raised regarding nurses’ lack of education of the protocol, and therefore decreased implementation. Increasing nursing knowledge pertaining to this protocol by performing educational in-services periodically and standardizing and implementing formal education as part of nursing orientation will decrease the knowledge gap and improve patient outcomes and increase utilization of the post-operative protocol.

Background: Electroconvulsive Therapy (ECT) is a robust, versatile treatment option for many psychiatric disorders and diseases, but systemic low levels of knowledge and negative attitudes exist towards the procedure among healthcare workers, specifically among psychiatric RNs. The negative attitudes surrounding ECT are most attributable to a misunderstanding of its uses, efficacy, procedures, and safety.
