Can you imagine not sleeping well for weeks or months at a time? Insomnia impacts approximately one-third of adults and is considered the most prevalent of all sleep disorders. With a lack of consistent, quality sleep, insomnia has significant negative consequences that impact a person’s overall mental well-being, like increased anxiety, depression, chronic pain, and daily coping. Cognitive Behavioral Therapy for Insomnia (CBT-I) allows patients to reduce or eliminate insomnia or related symptoms without needing medication to obtain quality and consistent sleep. March 2020 changed the healthcare field entirely. When the COVID-19 pandemic swept through the United States in early 2020, it was recommended that surgical departments nationwide shutdown altogether or at least decrease their workloads to exclusively emergent procedures. This step was taken both to save surgical equipment for more critical patients and to offload those staff members from surgical departments to more critical areas in the hospital systems.

Purpose and rationale: Deep Sternal Wound Infections (DSWI) are associated with morbidity rates and mortality rates as high as 10% -15% as well as significantly increased hospital costs. The purpose of this project is to decrease current rates of DSWIs in adults undergoing cardiac surgery at a large medical institution.

Synthesis of evidence: The literature shows a strong relationship between elevated HbA1C and DSWI. All articles reviewed for this proposal which implement a sternal vest intervention support its use to prevent DSWI.

Ketorolac is a readily available, inexpensive, moderate to severe pain management adjunct. As opioids lost favor, ketorolac gained popularity, as an opioid-sparing alternative (Corsini et al., 2021). Anecdotally, surgeons have been resistant to intra-operative intravenous ketorolac due to perceived bleeding risks. As such, this review seeks to evaluate the literature to determine whether ketorolac administration is associated with clinically significant bleeding in adult breast surgery patients. The results demonstrated conflicting evidence for ketorolac use. No literature has established a causal relationship between ketorolac and clinically significant bleeding. Surgical site infections are an avoidable complication in surgical patients and one of the most common hospital-acquired infections. Adverse effects of surgical site infections include increased hospital length of stay, increased costs to patient and healthcare system, disability, morbidity, and mortality. The purpose of this quality improvement project was to design a protocol with the objective of decreasing surgical site infections in abdominal and spinal surgery patients. High blood pressure (HBP), also known as Hypertension (HTN), is a common diagnosis for primary care office visits in the United States; however, a persistent gap remains between the stated public health targets and achieved blood pressure (BP) control rates in the United States (US). In a small sample of patients with essential HTN (ICD 10: I10) receiving primary care at a mid-western Federally Qualified Health Center (FQHC), a chart audit tool revealed 10 out of 10 patients achieved BP control during the measurement period (defined as a BP reading <140/90). Quality improvement (QI) analysis revealed the bigger picture: Achieved BP control rates of patients does not reflect the 2017 American College of Cardiology/American Heart Association HBP guidelines. Healthcare providers have inadvertently fueled the epidemic by overprescribing opioid medications contributing to 40% of the opioid overdose-related deaths. One in every 48 patients who receives an opioid medication for the first time will become a chronic user of these medications. In 2019, 57% of Adolescents who have substance or alcohol abuse disorders experienced at least one ACE, and 25 % experienced two or more ACEs; this means that over half of the patients admitted to an adolescent substance and alcohol abuse program have experienced childhood trauma (Gomez et al., 2017).

This study investigated ACE awareness amongst staff working with a population known to have high ACE scores. The intended purpose was to evaluate staff understanding of ACEs and their impact on patient satisfaction scores.
