Perioperative hyperglycemia is reported in 20-40% of patients undergoing general surgery and is an independent risk factor for negative surgical outcomes. Patients with diabetes presenting to a local community hospital are at risk for negative surgical outcomes due to the lack of strategies to prevent perioperative hyperglycemia. This project aimed to implement and evaluate the usefulness of succinct diabetes education for preoperative total joint surgical patients with diabetes. The intervention was a nurse-led, face-to-face diabetes education session provided to total joint surgical patients with diabetes during a preoperative evaluation appointment, compared to usual care (no preoperative diabetes education). Background: Parents caring for a child with an acquired disability often report a negative impact on physical, psychological, emotional, and spiritual health associated with caregiver burden related to their child’s complex health needs. This is further complicated by additional challenges related to the loss of a previously healthy child. Traditional patterns of loss follow linear, time-bound stages that have been observed to be unrelated to the experiences of these specific parents and their families. In 2000, the United Nations (UN) developed the Millennium Development Goals (MDGs) as an international framework to address poverty, promote sustainable development, and improve global health. The framework was translated into 8 goals with goals 4 and 5 targeting the reduction of child mortality and the improvement of maternal health. The UN and the World Health Organization (WHO) encouraged nurses and midwives to participate in the implementation process of the MDGs, and each country had the responsibility of formulating its own implementation policies and plans. Occupational stress and burnout are commonly experienced by nurses in the intensive care unit and contribute to poor nursing practice. A study in the United Kingdom revealed that approximately one-third of the intensive care unit team members were at a ‘high risk’ for burnout syndrome. Research suggests that multiple factors contribute to occupational stress and burnout, including nursing shortages, staff turnover, high workloads, workplace conflicts, and bullying. Research suggests that the wellbeing of nurses is imperative in ensuring the best delivery of care to patients, which further results in positive health outcomes for patients.

Background: Continuous renal replacement therapy (CRRT) is a complex, life-preserving treatment for unstable patients who require hemodialysis in the intensive care unit (ICU). Nurses responsible for managing the CRRT machine in this large academic medical center’s cardiovascular ICU (CVICU) complete a one-hour basic training course focused on setting up the machine, responding to basic alarms, and changing out the CRRT circuit when necessary. Background: Patients have an increased risk of developing phlebitis when amiodarone is delivered through a peripheral intravenous catheter. In a 32-bed cardiac surgery step-down unit, patients experienced phlebitis at a rate of 28.57% despite following manufacturer recommendations. Individual interventions targeted at amiodarone-related phlebitis have presented mixed results, however, when an amiodarone infusion practice guideline was established, the rate and severity of phlebitis consistently decreased across studies. The challenge of postoperative pain control following a thoracotomy is well noted among anesthesia providers. The intraoperative utilization of low-dose ketamine in several patient populations, including those undergoing a thoracotomy, has been deemed a viable intervention in the multi-modal analgesia plan at not only reducing postoperative pain scores for patients, but also reducing opioid consumption. This project sought not only to improve the communication and interunit transitions of care between emergency department providers (EDPs) and inpatient providers (IPs) using a standardized handoff tool, but also to reduce the Emergency Room (ED) wait time for patients admitted to the community hospital. The project employed a quantitative quasi-experimental pre-test/post-test design to explore provider satisfaction with an evidence-based standardized handoff tools and wait time. Forty-eight providers completed the survey before the I-PASS implementation, and 43 providers completed the survey post I-PASS implementation. Compassion Fatigue And Resilience In Long-Term Care Nurses, Dierdre C. De Gravina

The purpose of this study was to investigate the association between CF and resilience in LTC nurses. Watson’s caring theory was the theoretical foundation for the study. A survey containing demographic questions and items from the Professional Quality of Life Measure and the Connor-Davidson Resilience Scale survey was administered online to 111 LTC nurses from 10 states across the Eastern Seaboard. A simple linear regression analysis was performed to identify if there was an association between CF and resilience. The results indicated that the model was significant (p<.001). Results revealed an inverse relationship that with higher resilience scores, CF scores were lower and when CF was high, resilience was lower. The study may promote positive social change by highlighting the need for LTC facility managers to identify strategies that foster increased LTC nurse resilience such as improving work environments and developing programs that promote mental and emotional health of LTC nurses. Implementation of these strategies may reduce CF and promote nurse job retention and ultimately improve patient care. Recommendations for future research include quantitative studies to consider the effect of age, length of time in practice, and education level on resiliency and CF and qualitative studies on strategies for increasing resiliency among nurses in LTC facilities.

Most of these patients initiate hemodialysis with a tunneled central venous catheter, with some depending on its use for long-term treatment due to poor vascularity. Catheter-related bloodstream infections in hemodialysis patients are a complication of hemodialysis catheter use with a high morbidity and mortality rate.
