Background: Substance use disorder (SUD) is a disparity affecting nearly 24 million individuals in the U.S., nearly 10% of the total population. Oftentimes, the emergency department (ED) is the only medical care this population receives. There is often a lack of screening for SUD in ED’s, therefore referred care may be limited for these individuals. The National Institute on Drug Abuse Modified Assist (NMASSIST) is an evidence-based tool developed to educate and screen for SUD. This tool can be completed in a timely manner and aid in the recommended referral of care for this select population.

Purpose: Sedation protocols have been positively correlated with improved patient outcomes in the intensive care unit (ICU). Therefore, healthcare leaders should direct efforts to improving protocol compliance through evidence-based strategies. The purpose of this study was to evaluate the impact of a multifaceted intervention, consisting of educational outreach, point of care (POC) reminders, and audit and feedback (A&F), on nurse compliance with an ICU sedation protocol. A secondary data analysis was performed to evaluate the impact of the intervention on patient outcomes.

BACKGROUND: Relational aggression (RA) in nursing is widespread. One group often at risk for exposure to RA is the newly licensed nurse. RA can lead to a negative work environment which can cause poor patient outcomes and high nurse turnover.

PURPOSE: The purpose of this study was to evaluate the effects of an educational program on RA in the emergency department with newly licensed registered nurses. BACKGROUND: Primary care providers (PCPs) are poised across healthcare settings to reduce the leading cause of preventable morbidity and mortality, cigarette smoking. Background: Fluid overload in patients requiring intensive care leads to increased costs for hospitals and patients, increased length of intensive care unit (ICU) and total hospital stay, ventilator days, acute kidney injuries, and mortality rates. Multiple studies have indicated that aggressive de-resuscitation with diuretics can decrease length of stay, ventilator days, organ injury, and mortality rates. การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อบรรยายการแสดงตัวตนของมารดาในหอผู้ป่วยกุมารเวชบำบัดวิกฤตระหว่างการเข้ารับการรักษาของผู้ป่วยเด็กที่มีภาวะการหายใจล้มเหลว โดยใช้วิธีการวิจัยเชิงคุณภาพตามแนวคิดปรากฏการณ์วิทยาของ Husserl ผู้ให้ข้อมูลหลักคือมารดาที่ดูแลผู้ป่วยเด็กอย่างต่อเนื่องตามเกณฑ์การคัดเลือกแบบเฉพาะเจาะจงจำนวน 6 ราย เก็บข้อมูลโดยวิธีการสังเกตแบบไม่มีส่วนร่วมและการสัมภาษณ์เชิงลึก วิเคราะห์ข้อมูลตามวิธีการของ Colaizzi ผลการศึกษาวิจัยทำให้สามารถอธิบายประสบการณ์การการแสดงตัวตนของมารดา ได้เป็น 2 ประเด็นหลัก คือ 1. “การดูแลด้วยหัวใจความเป็นมารดา” ประกอบไปด้วย 2 ประเด็นย่อยคือ 1) การดูแลอย่างใส่ใจเป็นพิเศษด้วยความรักจากหัวใจมารดา และ 2) การดูแลพร้อมความหวังและกำลังใจให้ลูกหายป่วย 2. “ความรู้สึกผลักดันและคงไว้ซึ่งการดูแลของมารดาที่ตั้งใจทำให้ลูก” ประกอบไปด้วย 2 ประเด็นย่อย คือ 1) หัวใจของการเป็นแม่กับอารมณ์ความรู้สึกที่มีต่อลูกในภาวะวิกฤต และ 2) พยายามอยู่ใกล้ลูกให้เหมือนเดิมเพื่อถ่ายทอดความอบอุ่นจากแม่ ผลการวิจัยสะท้อนถึงการแสดงตัวตนของมารดาที่มีความสัมพันธ์กันระหว่าง การดูแลด้วยหัวใจความเป็นมารดาที่เกิดจาก ความรัก ความใส่ใจในการดูแลอย่างเป็นพิเศษเพื่อบุตรของตนเองฟื้นหายจากความเจ็บป่วย ส่งผลให้เกิดความรู้สึกผลักดันที่ทำให้มารดาคงไว้ซึ่งการดูแลบุตรแม้อยู่ในภาวะวิกฤตเพื่อให้บุตรได้รับความรักความอบอุ่นจากมารดาเหมือนเดิมก่อนการเจ็บป่วย ข้อค้นพบเหล่านี้สามารถนำไปใช้เป็นข้อมูลพื้นฐานในการวางแผนการพยาบาลเพื่อส่งเสริมการแสดงตัวตนของมารดา รวมถึงการวิจัยในอนาคตต่อไป

การศึกษาวิจัยเชิงคุณภาพครั้งนี้มีวัตถุประสงค์เพื่อบรรยายประสบการณ์การเป็นพยาบาลที่ปฏิบัติงานในหน่วยงานอุบัติเหตุและฉุกเฉินยุคใหม่โรงพยาบาลทั่วไปแห่งหนึ่ง โดยใช้ระเบียบวิธีวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาของ Heidegger โดยผู้ให้ข้อมูลเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในหน่วยงานอุบัติเหตุและฉุกเฉิน และมีความยินดีเข้าร่วมการวิจัย จำนวน 12 คน เก็บรวบรวมข้อมูลด้วยการสัมภาษณ์เชิงลึกและการบันทึกเทป ร่วมกับการบันทึกภาคสนาม และนำข้อมูลที่ได้มาถอดเทปแบบคำต่อคำ และวิเคราะห์ข้อมูลโดยวิธีของ Crist and Tanner ผลการศึกษา ประสบการณ์การเป็นพยาบาลที่ปฏิบัติงานในหน่วยงานอุบัติเหตุและฉุกเฉินยุคใหม่โรงพยาบาลทั่วไปแห่งหนึ่ง แบ่งเป็น 4 ประเด็นหลัก ดังนี้ 1. ภูมิหลังและการเตรียมความพร้อมในการปฏิบัติงานใน ER ประกอบด้วยประเด็นย่อยดังนี้ 1.1) หลากหลายเหตุผลที่เข้ามาทำงานใน ER 1.2) เริ่มทำงาน เกิดความไม่มั่นใจต้องใช้เวลาในการปรับตัว และ 1.3) การเตรียมความรู้ในการดูแลผู้ป่วย 2. การปฏิบัติงานของพยาบาลในหน่วยอุบัติเหตุและฉุกเฉิน ประกอบด้วยประเด็นย่อยดังนี้ 2.1) คัดกรองผู้ป่วยด้วยทักษะที่หลากหลาย 2.2) ดูแลผู้ป่วยในห้องสังเกตการ 2.3) ช่วยฟื้นคืนชีวิตโดยปฏิบัติตามเทคนิคที่ถูกต้อง 2.4) บริการรับและส่งต่อผู้ป่วย 2.5) ปฏิบัติงานการแพทย์ฉุกเฉิน 2.6) บริการงานสาธารณภัย พร้อมโรคอุบัติใหม่ที่เกิดขึ้นและ 2.7) บริการความรู้แก่สังคมเพื่อประโยชน์แห่งตนและผู้อื่น 3. ปัญหาหน้างาน บริหารจัดการโดยหัวหน้าเวร ประกอบด้วยประเด็นย่อยดังนี้ 3.1) บริหารอัตรากำลังให้เหมาะสม หากมีผู้ป่วยจำนวนมาก 3.2) สื่อสารให้ผู้ป่วยและญาติเข้าใจหากต้องรอตรวจนาน 3.3) ป้องกันความผิดพลาดการดูแล จึงต้องมีการตรวจสอบและทวนซ้ำและ 3.4) ระงับเหตุการณ์วุ่นวาย ต้องอาศัยหลายฝ่ายร่วมกัน 4. ผลของการปฏิบัติงานใน ER ประกอบด้วยประเด็นย่อยดังนี้ 4.1) เครียดจากการบริหารจัดการในงานยุคใหม่ 4.2) ขาดสมดุลระหว่างชีวิตกับงานที่ต้องปฏิบัติ และ 4.3) ความสุขเกิดขึ้นได้จากผู้ร่วมงานที่ดีและเต็มที่กับการดูแลผู้ป่วยจนปลอดภัย การวิจัยนี้ทำให้เข้าใจการเป็นพยาบาลหน่วยอุบัติและฉุกเฉินที่ให้ความสำคัญกับการดูแลผู้ป่วยและการบริหารจัดการ ซึ่งผู้บริหารทางการพยาบาลสามารถนำไปใช้เป็นแนวทางในการพัฒนาคนและพัฒนางานของหน่วยอุบัติและฉุกเฉินให้มีประสิทธิภาพมากขึ้น

This correlational study aimed to 1) investigate mobility and 2) examine direct and indirect paths of relationships among comorbidity, cognitive function, social support, pain, fatigue, and sleep quality on mobility among persons with hip fracture after surgery. The hypothesized model was constructed based on the theory of unpleasant symptoms and the literature reviewed. A three-stage random sampling approach was utilized to recruit 260 persons with hip fracture after surgery aged 50 years old and older who visited four hospitals in three health regions of Thailand. การวิจัยครั้งนี้เป็นการศึกษาความสัมพันธ์เชิงบรรยายเพื่อศึกษาปัจจัยคัดสรรที่สัมพันธ์กับพฤติกรรมการรับประทานอาหารของผู้ป่วยกลุ่มอาการภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลัน กลุ่มตัวอย่าง คือ ผู้ป่วยกลุ่มอาการภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลัน จำนวน 195 คน คัดเลือกกลุ่มตัวอย่างด้วยวิธีการสุ่มแบบหลายขั้นตอน จากผู้ป่วยที่มารับบริการ ณ แผนกผู้ป่วยนอกระบบหัวใจและหลอดเลือด โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย โรงพยาบาลตำรวจ และโรงพยาบาลศิริราช เก็บข้อมูลโดยใช้แบบสอบถามทั้งหมด 7 ส่วน คือ 1) แบบบันทึกข้อมูลส่วนบุคคล 2) แบบสอบถามความรู้เรื่องโรคหัวใจขาดเลือด 3) แบบสอบถามการรับรู้การเจ็บป่วย 4) แบบสอบถามแรงจูงใจ 5) แบบสอบถามการสนับสนุนทางสังคม 6) แบบสอบถามการรับรู้สมรรถนะแห่งตน และ 7) แบบสอบถามพฤติกรรมการรับประทานอาหาร ตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ 0.94, 1.0, 1.0, 0.97, 1.0 และ 0.95 ตามลำดับ และตรวจสอบความเที่ยงของเครื่องมือเท่ากับ .76, .80, .73, .91, .96 และ .87 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้วิธีการทดสอบค่าสัมประสิทธิ์สหสัมพันธ์เพียร์สัน ผลการวิจัยสรุปได้ดังนี้ 1. ผู้ป่วยกลุ่มอาการภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันมีคะแนนเฉลี่ยพฤติกรรมการรับประทานอาหารเฉพาะโรค ความรู้ แรงจูงใจ การสนับสนุนทางสังคม และการรับรู้สมรรถนะแห่งตนอยู่ในระดับสูง (Mean = 100.98 SD = 12.39, Mean = 16.48 SD = 2.16, Mean = 76.03 SD = 8.63, Mean = 67.43 SD = 9.09 และ Mean = 25.73 SD = 3.79 ตามลำดับ) และมีคะแนนการรับรู้การเจ็บป่วยระดับปานกลาง (Mean = 49.81 SD …

การวิจัยกึ่งทดลองครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการช่วยเลิกบุหรี่ต่อพฤติกรรมการเลิกบุหรี่ในผู้ที่เป็นวัณโรคปอด กลุ่มตัวอย่างคือ ผู้ป่วยวัณโรคปอดที่สูบบุหรี่อย่างน้อย 1 มวนในช่วง 1 สัปดาห์ที่ผ่านมา เข้ารับการรักษาที่คลินิกวัณโรค โรงพยาบาลระดับตติยภูมิแห่งหนึ่ง กรุงเทพมหานคร คัดเลือกกลุ่มตัวอย่างแบบเจาะจงตามคุณสมบัติที่กำหนด จำนวน 60 คน กำหนดให้ 30 คนแรกเป็นกลุ่มควบคุม และ 30 คนหลังเป็นกลุ่มทดลอง จับคู่กลุ่มตัวอย่างให้มีลักษณะคล้ายคลึงกันในเรื่องอายุและระดับการติดนิโคติน กลุ่มควบคุมได้รับการพยาบาลตามปกติ ในขณะที่กลุ่มทดลอง ได้รับการพยาบาลตามปกติและโปรแกรมการช่วยเลิกบุหรี่แบบสั้น เป็นเวลา 8 สัปดาห์ เก็บรวบรวมข้อมูลโดยใช้แบบประเมินพฤติกรรมการเลิกบุหรี่ และแบบสอบถามสถานะการสูบบุหรี่ ประเมินการเลิกบุหรี่จากการเลิกสูบบุหรี่ได้ติดต่อกันในช่วง 7 วันก่อนระยะเวลาประเมินผลที่ 2 เดือน วิเคราะห์ข้อมูลโดยใช้ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติทดสอบไคสแควร์ สถิติทีและสถิติซี ผลการทดลองพบว่า 1. คะแนนเฉลี่ยพฤติกรรมการเลิกบุหรี่ของผู้ป่วยวัณโรคปอดที่สูบบุหรี่ภายหลังการเข้าร่วมโปรแกรมการช่วยเลิกบุหรี่แบบสั้น สูงกว่าก่อนเข้าร่วมโปรแกรม อย่างมีนัยสําคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยพฤติกรรมการเลิกบุหรี่ของผู้ป่วยวัณโรคปอดที่สูบบุหรี่ภายหลังการเข้าร่วมโปรแกรมการช่วยเลิกบุหรี่แบบสั้น สูงกว่ากลุ่มควบคุมอย่างมีนัยสําคัญทางสถิติ (p<.05) 3. เมื่อประเมินที่ระยะเวลา 2 เดือนหลังการทดลอง พบว่า ผู้ป่วยวัณโรคปอดที่สูบบุหรี่ในกลุ่มทดลอง เลิกบุหรี่ได้ 12 คน ส่วนกลุ่มควบคุม เลิกบุหรี่ได้ 3 คน อัตราการเลิกบุหรี่ในช่วง 7 วันก่อนประเมินผล ในกลุ่มทดลองสูงกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติ (p<.05)

Background

Nurse duty of care, the balance between nursing occupational obligations to provide care, the personal costs for providing such care, and the reward for providing care, has been significantly altered by the COVID-19 pandemic. ICU nurses are increasingly burdened with higher personal costs to fulfill their jobs, but little additional reward for continuing to provide care.

Objectives

The purpose of this study was to examine the impact of the COVID-19 pandemic on the duty of care balance among ICU nurses who manage COVID-19 patients.

Opioid effects on lower gastrointestinal motility are well documented, and increasing attention is being paid to their effect on esophageal motility. Naloxegol is a µ-opioid receptor antagonist that is used for opioid-induced constipation, but its impact on esophageal motility has not been well documented. We report a case series of 3 patients with coexisting esophageal dysmotility and constipation on chronic opioids that improved both symptoms after starting naloxegol. Based on these observational studies, we propose that formal studies be conducted to assess the effect of naloxegol on opioid-induced esophageal dysmotility.

Background

Caesarean section (CS) is an intervention to reduce maternal and perinatal mortality, for complicated pregnancy and labour. We analysed trends in the prevalence of birth by CS in Ghana from 1998 to 2014.

Methods

Using the World Health Organization’s (WHO) Health Equity Assessment Toolkit (HEAT) software, data from the 1998-2014 Ghana Demographic and Health Surveys (GDHS) were analysed with respect of inequality in birth by CS. Introduction

Studies in Southeast Asia found that bullying commonly occurred among students, and it has a detrimental impact on their school attendance. However, there is a paucity of literature in Timor-Leste on the association between bullying and truancy. Therefore, this study examined the association between bullying and truancy among Timor-Leste school-going adolescents.

We used the 2015 Timor-Leste Global School-based Student Health Survey (GSHS) dataset to examine our hypothesis in logistic regression models for both full adolescents (N = 3609) and gender stratified samples. Background and Aims

Previous large studies on the epidemiology and clinical profile of chronic pancreatitis have suggested significant differences in presentation and management in the eastern and western hemispheres. The aim of this systematic review was to compare large multicenter studies across different geographic regions of the world to detect meaningful differences in the presentation and management of this poorly understood disease.

Methods

Background

Breast milk feeding has numerous benefits for women and infants. Positive maternal experiences with breast milk feeding impacts exclusivity, duration, and maternal mental health. Most research focuses on women feeding directly at the breast. Some women elect to feed exclusively expressed milk to their healthy, term infants rather than feed directly at the breast.
