PURPOSE: The purpose of this study is to examine whether inpatients with liver disease at University of Kentucky Hospital receiving lactulose are meeting the protocol of 2-4 bowel movements per day. Research shows that lactulose is the medication of choice used to treat and prevent hepatic encephalopathy (HE). Additional lactulose interventions, a patient’s MELD score, and GCS score will be examined in this study as well. การวิจัยแบบบรรยายเชิงความสัมพันธ์นี้ มีวัตถุประสงค์เพื่อศึกษาการทำหน้าที่ทางเพศของผู้ป่วยโรคกล้ามเนื้อหัวใจตายหลังได้รับการขยายหลอดเลือดหัวใจ (PCI) และปัจจัยคัดสรรที่สัมพันธ์กับการทำหน้าที่ทางเพศของผู้ป่วยโรคกล้ามเนื้อหัวใจตายหลังการทำ PCI ได้แก่ เพศ ความรุนแรงของโรค ความรู้เรื่องเพศสัมพันธ์ ความวิตกกังวลทางเพศ การรับรู้สมรรถนะแห่งตนทางเพศ ภาวะซึมเศร้าทางเพศ และการสนับสนุนทางสังคม กลุ่มตัวอย่าง คือ ผู้ป่วยโรคกล้ามเนื้อหัวใจตายที่ได้รับการรักษาด้วยการทำ PCI เพศชายและหญิงอายุ 18 - 59 ปี ที่เข้ามารับบริการ ณ แผนกผู้ป่วยนอกอายุรกรรมทั่วไป และคลินิกโรคหัวใจ โรงพยาบาลสมเด็จพระปิ่นเกล้า และโรงพยาบาลธรรมศาสตร์เฉลิมพระเกียรติ จำนวน 124 คน ใช้วิธีเลือกกลุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือที่ใช้ในการเก็บข้อมูล ได้แก่ 1) แบบสอบถามข้อมูลทั่วไป 2) แบบวัดการทำหน้าที่ทางเพศ 3) แบบประเมินความรุนแรงของโรค 4) แบบวัดความรู้เรื่องเพศสัมพันธ์ 5)แบบสอบถามความวิตกกังวลทางเพศ 6) แบบสอบถามการรับรู้สมรรถนะแห่งตนทางเพศ 7) แบบสอบถามภาวะซึมเศร้าทางเพศ 8) แบบสอบถามการสนับสนุนทางสังคม ผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน ได้ค่าดัชนีความตรงตามเนื้อหา ในชุดที่ 2 - 8 เท่ากับ .93, 1.00, .98, .97, .91, 1.00, .98 ตามลำดับ และค่าความเที่ยงชุดที่ 2, 4 - 8 เท่ากับ .82, .77, .91, .91, .85, และ .81 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติบรรยาย สถิติสหสัมพันธ์เพียร์สัน และสถิติสัมประสิทธิ์สหสัมพันธ์พอยท์ไบซีเรียล ผลการวิจัย พบว่า 1. การทำหน้าที่ทางเพศอยู่ในระดับปานกลาง ( x = 45.61, SD = 4.38) 2. เพศชาย และความรุนแรงของโรคมาก มีความสัมพันธ์กับการทำหน้าที่ทางเพศในผู้ป่วยโรคกล้ามเนื้อหัวใจตายหลังการทำ PCI อย่างมีนัยสำคัญทางสถิติที่ระดับ …

การวิจัยกึ่งทดลองครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมแบบบูรณาการที่เน้นการออกกำลังกายขณะฟอกเลือดต่ออาการเหนื่อยล้าในผู้ป่วยโรคไตเรื้อรังที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม กลุ่มตัวอย่าง ได้แก่ ผู้ป่วยโรคไตเรื้อรังที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม อายุ 18-60 ปี แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลองกลุ่มละ 25 คน กลุ่มควบคุมได้รับการพยาบาลตามปกติ ในขณะที่กลุ่มทดลองเข้าร่วมโปรแกรมแบบบูรณาการที่เน้นการออกกำลังกายขณะฟอกเลือด โดยออกกำลังกายแบบเพิ่มพิสัยการเคลื่อนไหวของข้อ ภายใน 2 ชั่วโมงแรกของการฟอกเลือด ครั้งละ 20 นาที 3 ครั้งต่อสัปดาห์ นาน 8 สัปดาห์ เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลได้แก่ แบบบันทึกข้อมูลทั่วไป แบบประเมินอาการเหนื่อยล้าซึ่งมีค่าความเที่ยง (reliability) เท่ากับ 0.85 วิเคราะห์ข้อมูลด้วยสถิติพรรณา วิเคราะห์ความแปรปรวนแบบวัดซ้ำ และการทดสอบที ผลการวิจัยสรุปได้ดังนี้ 1. คะแนนเฉลี่ยอาการเหนื่อยล้าของผู้ป่วยโรคไตเรื้อรังที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียมหลังเข้าร่วมโปรแกรมแบบบูรณาการที่เน้นการออกกำลังกายขณะฟอกเลือดน้อยกว่าก่อนเข้าร่วมโปรแกรมฯอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยอาการเหนื่อยล้าที่สัปดาห์ที่ 5 และสัปดาห์ที่ 9 ของผู้ป่วยโรคไตเรื้อรังที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียมกลุ่มทดลองน้อยกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การศึกษาวิจัยเชิงคุณภาพครั้งนี้มีวัตถุประสงค์เพื่อบรรยายประสบการณ์การเป็นพยาบาลผู้ประกอบการคลินิกการพยาบาลและการผดุงครรภ์ของพยาบาลวิชาชีพ โดยใช้ระเบียบวิธีวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาของ Heidegger โดยผู้ให้ข้อมูลเป็นพยาบาลวิชาชีพที่เป็นผู้ประกอบการคลินิกการพยาบาลและการผดุงครรภ์ จำนวน 10 คน โดยการคัดเลือกผู้ให้ข้อมูลแบบเจาะจง ซึ่งเก็บรวบรวมข้อมูลด้วยการสัมภาษณ์เชิงลึก การสังเกต และเอกสารต่างๆข้อมูลทั้งหมดนำมาวิเคราะห์เนื้อหาตามวิธีการของ van Manen ผลการศึกษาประสบการณ์การเป็นพยาบาลผู้ประกอบการคลินิกการพยาบาลและการผดุงครรภ์ สามารถแบ่งเป็น 5 ประเด็นหลักและประเด็นย่อยดังนี้ 1. เหตุผลที่เลือกเป็นผู้ประกอบการคลินิกการพยาบาลและการผดุงครรภ์ ประกอบด้วย 5 ประเด็นย่อย ได้แก่ 1.1) ต้องการรายได้เสริมเพื่อเป็นค่าใช้จ่ายในครอบครัว 1.2) เติมเต็มช่องว่างการให้บริการสุขภาพแก่ชุมชน1.3) อยากมีเวลาดูแลครอบครัว 1.4) มีอิสระในการทำงาน และ 1.5) เป็นงานที่รองรับให้มีรายได้หลังเกษียณจากงานราชการ 2. เตรียมพร้อมสู่การเป็นผู้ประกอบการคลินิกการพยาบาลและการผดุงครรภ์ ประกอบด้วย 7 ประเด็นย่อย ได้แก่ 2.1) สอบถามข้อมูลการเปิดสถานพยาบาลจากแหล่งข้อมูล 2.2) เงินทุนเตรียมให้พร้อม 2.3) หาทำเลที่ตั้งสถานพยาบาลในแหล่งชุมชน 2.4) เตรียมอุปกรณ์ ยา และเวชภัณฑ์ ในสถานพยาบาล 2.5) เอกสารหลักฐานสำคัญ ต้องเตรียมให้พร้อม 2.6) เตรียมพร้อมรับการตรวจประเมินสถานพยาบาล และ 2.7) ประชาสัมพันธ์สถานพยาบาล 3. ให้บริการตามขอบเขตวิชาชีพ ประกอบด้วย 4 ประเด็นย่อย ได้แก่ 3.1) รักษาโรคเบื้องต้น 3.2) วางแผนครอบครัว 3.3) การตรวจและรับฝากครรภ์ และ 3.4) การให้ภูมิคุ้มกันโรค 4. สิ่งที่ทำให้ธุรกิจอยู่รอด ประกอบด้วย 4 ประเด็นย่อย ได้แก่ 4.1) บริการดี เข้าถึงและเข้าใจความต้องการของผู้ป่วย 4.2) เรียนรู้จากข้อผิดพลาดเป็นโอกาสให้ปรับการบริการ 4.3) เปิดให้บริการตรงเวลา หากติดภารกิจอื่นติดป้ายแจ้งเป็นลายลักษณ์อักษร และ 4.4) พัฒนาองค์ความรู้ให้ทันสมัย 5. สิ่งที่ได้รับจากการเป็นผู้ประกอบการคลินิกการพยาบาลและการผดุงครรภ์ ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 5.1) ได้บุญ ได้ช่วยเหลือผู้อื่น และ …

การวิจัยกึ่งทดลองแบบสองกลุ่มวัดสองครั้งก่อนและหลังการทดลองในครั้งนี้ มีวัตถุประสงค์1) เพื่อเปรียบเทียบภาวะกดดันด้านจิตใจของผู้ดูแลผู้ป่วยจิตเภทก่อนและหลังได้รับโปรแกรมเสริมสร้างทักษะการเผชิญความเครียดและ 2) เพื่อเปรียบเทียบภาวะกดดันด้านจิตใจของผู้ดูแลผู้ป่วยจิตเภทกลุ่มที่ได้รับโปรแกรมเสริมสร้างทักษะการเผชิญความเครียดกับกลุ่มที่ได้รับการดูแลตามปกติ กลุ่มตัวอย่าง คือผู้ดูแลผู้ป่วยจิตเภทที่มารับบริการที่แผนกจิตเวชผู้ป่วยนอกโรงพยาบาลจิตเวชนครราชสีมาราชนครินทร์ จังหวัดนครราชสีมา ซึ่งมีคุณสมบัติตามเกณฑ์จำนวน 40 คน ได้รับการจับคู่ให้มีคุณสมบัติใกล้เคียงกันในเรื่องเพศและระยะเวลาการเจ็บป่วยของผู้ป่วย แล้วถูกสุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 20 คน ผู้ดูแลในกลุ่มทดลองได้รับโปรแกรมเสริมสร้างทักษะการเผชิญความเครียด ส่วนผู้ดูแลในกลุ่มควบคุมได้รับการดูแลตามปกติ เครื่องมือที่ใช้ในการวิจัย คือ 1) โปรแกรมเสริมสร้างทักษะการเผชิญความเครียด 2) แบบประเมินภาวะกดดันด้านจิตใจของ Kessler 3) แบบวัดการเผชิญความเครียดของผู้ดูแลผู้ป่วยจิตเภท ซึ่งได้รับการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิจำนวน 5 ท่าน เครื่องมือชุดที่ 2 และ 3 มีค่าความเที่ยงสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ .89 และ .80 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐานและสถิติทดสอบที (t-test) ผลการวิจัย สรุปได้ดังนี้ 1. ภาวะกดดันด้านจิตใจของผู้ดูแลผู้ป่วยจิตเภทหลังได้รับโปรแกรมเสริมสร้างทักษะการเผชิญความเครียดต่ำกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. ภาวะกดดันด้านจิตใจของผู้ดูแลผู้ป่วยจิตเภทกลุ่มที่ได้รับโปรแกรมเสริมสร้างทักษะการเผชิญความเครียดต่ำกว่ากลุ่มที่ได้รับการดูแลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยกึ่งทดลองมีวัตถุประสงค์ เพื่อศึกษาผลของโปรแกรมการวางแผนการจำหน่ายต่อพฤติกรรมการดูแลที่บ้านของผู้ดูแลเด็กโรคหัวใจพิการแต่กำเนิดหลังได้รับการผ่าตัด กลุ่มตัวอย่างคือผู้ดูแลเด็กโรคหัวใจพิการแต่กำเนิดหลังได้รับการผ่าตัดครั้งแรกอายุ 1-5 ปี และเข้ารับการรักษาที่แผนกผู้ป่วยในโรงพยาบาลตติยภูมิ 2 แห่ง จำนวน 44 ราย แบ่งเป็นกลุ่มทดลองและควบคุม กลุ่มละ 22 ราย โดยการจับคู่ให้มีอายุ ระดับการศึกษา และรายได้ในครอบครัวอยู่ในระดับเดียวกัน และมีจำนวนเท่ากันในทั้ง 2 โรงพยาบาล กลุ่มควบคุมได้รับการพยาบาลตามปกติ กลุ่มทดลองได้รับโปรแกรมการวางแผนการจำหน่ายเด็กโรคหัวใจพิการแต่กำเนิดหลังได้รับการผ่าตัด เครื่องมือวิจัยประกอบด้วย โปรแกรมการวางแผนการจำหน่ายเด็กโรคหัวใจพิการแต่กำเนิดหลังได้รับการผ่าตัด, สื่อวิดิทัศน์, คู่มือการดูแลเด็กโรคหัวใจพิการแต่กำเนิดหลังได้รับการผ่าตัดที่บ้าน แบบสอบถามพฤติกรรมการดูแลที่บ้านของผู้ดูแลเด็กโรคหัวใจพิการแต่กำเนิดและแบบวัดความรู้เรื่องการดูแลเด็กโรคหัวใจพิการแต่กำเนิดหลังได้รับการผ่าตัดเมื่อกลับไปอยู่ที่บ้าน วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาและสถิติทดสอบค่าที ผลการวิจัยพบว่าค่าเฉลี่ยของคะแนนพฤติกรรมการดูแลที่บ้านของผู้ดูแลเด็กโรคหัวใจพิการแต่กำเนิดหลังได้รับการผ่าตัด กลุ่มทดลองสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ผลการวิจัยให้นวัตกรรมการพยาบาลในการวางแผนการจำหน่าย สำหรับการเตรียมความพร้อมก่อนการจำหน่ายกลับบ้านให้แก่ผู้ดูแลเด็กโรคหัวใจพิการแต่กำเนิดหลังได้รับการผ่าตัดได้ เพื่อส่งเสริมความสามารถของผู้ดูแลและป้องกันภาวะแทรกซ้อนที่อาจเกิดขึ้นที่บ้าน

Cañadas‐De la Fuente et al. performed a meta‐analysis of the prevalence of emotional exhaustion, depersonalization, and diminished personal accomplishment—the three definitional components of the burnout syndrome—among nursing professionals working in oncology units. All in all, their meta‐analysis was inconclusive because of the very state of burnout research. Because there is mounting evidence that burnout is a depressive condition, we recommend that investigators focus on depression, rather than burnout, in occupational health research and practice.

Background:

In the past decade, little research has been conducted regarding individuals' stories about not initially being successful on the National Council Licensure Examination for Registered Nurses. An unsuccessful licensure examination attempt may affect the individual personally and profoundly in ways that could influence their future success.

Method:

Individual in-depth interviews were conducted once with 15 RNs who were initially unsuccessful on the licensure examination and subsequently passed. Data analyses were conducted using both manual and electronic coding through thematic identification. Rigor was ensured by meeting quality criteria for qualitative research.
