This dissertation is comprised of three manuscripts exploring the concept of social capital and hospital readmission of older adults. The first manuscript provides a review of the literature evaluating studies that measure the relationship between social capital and health, healthy behaviors, and access to and utilization of healthcare resources. Further research focusing on testing various types of individual social capital and their relationship to key outcomes including hospital readmission is needed.

Workplace incivility (WPI) affects many healthcare providers, including Certified Registered Nurse Anesthetists. This project was an exploration of the prevalence, nature, and sources of WPI affecting CRNAs. The project’s intervention was an in-service education on WPI for CRNAs. The purposes of the project were to measure the effectiveness of the in-service education in increasing awareness of CRNAs about WPI; and to offer a practice change proposal in terms of a policy against WPI in the anesthesia department.

One of the most stressful times in the life of a student registered nurse anesthetist (SRNA) is during the integration of didactic work with that of clinical anesthesia practice (Chipas et al., 2012). One method that has been proven effective in other avenues of student nursing education is the use of procedural simulation labs. These simulation labs allow SRNAs to experience what it is like to be in the operating room setting, while also showing distinct differences within each case, treatment, and the importance to be familiar with all aspects of anesthesia. Approximately 50% of US nursing home residents have a diagnosis of dementia. Abilities Care Approach® was created by occupational therapists (OTs) to promote quality dementia care for residents in skilled nursing facilities (SNFs). Certified nursing assistants (CNAs) are often the primary caregiver for residents with dementia. To ensure carryover of ACA principles, ACE dementia training was targeted towards CNAs. The purpose of this study was to evaluate Abilities Care Experts®(ACE) training program which provides CNAs with an occupation-based dementia care approach by focusing on the importance of occupational engagement and knowledge of dementia stages. This study focuses on the 2017 nurses’ strike and how the crippling of the public health system impacted maternal care in Kisumu, Kenya. Kenya has one of the highest maternal mortality ratios in the world, and there have been many efforts to reduce this, such as delivery with a Skilled Birth Attendant and attending antenatal care visits, all of which are centered around a functioning health system. The research team used a combination of interviews with key populations and analysis of service delivery data at local health centers to evaluate the effects of the strike on maternal care. Effectiveness Of Breastfeeding Shift Champions On Exclusive Breastfeeding, Comfort Obi

Purpose. This project evaluated the effect of breastfeeding support on breastfeeding and exclusive breastfeeding rates among postpartum mothers in immediate postpartum period. The breastfeeding support was provided by breastfeeding shift champions (BFSC) following childbirth.

Design. A convenience sample of BFSCs, aged between 18 and 44 years, was utilized.

Method. Pre-and post-intervention questionnaires were used to evaluate breastfeeding knowledge of BFSC. Breastfeeding and exclusive breastfeeding rates were extracted, pre- and post- intervention, from electronic medical records of newborns and their mothers.

Effect Of Intimate Partner Violence Education On Nurses Readiness To Screen, Felisha Dixon-Brazier

Doctor of Nursing Practice (DNP) Translational and Clinical Research Projects

This quality improvement translational project was intended to increase the confidence of nurses screening for intimate partner violence, through education. The project utilized a focus group and the Physicians Readiness to Manage Intimate Partner Violence Survey to gather needs, knowledge gaps, and prior education to develop IPV education for a group of nurses at a public health department. Nurses were found to have a low level of IPV knowledge, with a mean score of 42.44 (SD= 15.13), on a 100-point scale. Six of the nurses also reported less than one hour of previous IPV training in the past. Nurses reported …

Implementing The Use Of Intracuff Alkalinized Lidocaine Among Certified Registered Nurse Anesthetists: A Practice Change Proposal, Shawn Taylor

Patients today are experiencing comorbidities predisposing them to increased risk under general anesthesia. Emergence cough reflex is a response that can lead to hemodynamic alterations occurring during emergence and the postoperative period for many patients requiring general endotracheal tube anesthesia (GETA). It is proposed that 38% to 96% of patients receiving GETA will experience coughing during emergence (As, Iqbal, & Ali, 2009; Watkins, Lee, White Jr, & Mundy, 2012).

The use of intracuff alkalinized lidocaine is an intervention shown to be effective in previous studies at blunting this response during anesthesia emergence. Through verbal communication with CRNAs, it was discovered …

In the pursuit of graduate level education, student registered nurse anesthetists (SRNAs) undertake curricula requiring an immense investment of time attending didactic classes and clinical training, studying, and completing assignments. The required time commitment combined with the stress accompanying these responsibilities have the potential to leave the SRNA with inadequate time for an optimal amount of sleep, contribute to sleep disturbances, and ultimately result in sleep-related fatigue (SRF). SRF in the SRNA population is a problem because it is imperative for both patient and student safety that SRNAs attain adequate sleep to allow for optimal cognitive function.

Suicide has been the 2nd leading cause of death for 18-24-year-olds in the US since 2011. The stress experienced by undergraduate college students has the potential to increase ones’ risk for suicide. Resilience theory was used as a theoretical framework to examine the interplay between risk and protective factors. A cross-sectional and correlational design was used to assess the mediating effects of positive thinking and/or social support on suicide resilience in 131 college students 18-24 years old who completed an online survey. An indirect effect of self-esteem on suicide resilience was found through positive thinking and social support indicating that …

Using data from a longitudinal study of breastfeeding in Hispanics, this study evaluated the influence of early skin-to-skin contact (SSC) on initiation and sustained exclusive breastfeeding (EBF) at 1 month postpartum. Two-thirds of the women in the sample participated in early SSC. At discharge, over half of the women were EBF; this proportion decreased to one-third at 1 month postpartum. Controlling for demographic and clinical variables in the model, participation in early SSC was associated with a greater than sevenfold increase in the odds of EBF at discharge (p = .005) but was not predictive of EBF at 1 …

Communication and team-based care are at the heart of patient safety. As anesthesia professionals, we witness this at its very best and worst when transferring patients to and from the operating room (see article by Dr. Lorinc on types of transfers in this issue). In response, we have an opportunity to take a leading role in redesigning the most ubiquitous teaming event in hospitals in a manner that promotes team-based behaviors. Respiratory depression is a serious complication after surgery. Early detection is a major patient safety concern for recovery room personnel as patients recover from anesthesia and experience pain. The opioids used for pain management may contribute to over-sedation effects and respiratory depression. Vital signs and pulse oximetry are standard postoperative monitoring procedures. End-tidal capnography and arterial blood gases may augment the standard-of-care procedures but may not be effective at detecting early respiratory depression.
