Exploring The Value Of Group And Traditional Obstetrical Appointments To Reduce Health Disparity, Helene M. Holbrook

Disparity in health outcomes is influenced by socioeconomic factors that may include access to important healthcare information in a culturally sensitive way. Can a group appointment model like the CenteringPregnancy model provide a more effective means for engaging Spanish-speaking pregnant women? Research studies using the group prenatal appointment model show increased pregnancy knowledge, readiness for labor and higher satisfaction compared with individual prenatal appointments. This paper discusses the importance of reducing disparities in birth outcomes using a group appointment model conducted in partial fulfillment for the Doctor of Nursing Practice degree.

Earnings Of Nurses In Non-Nursing Occupations: Evidence Of Significant Nursing Dissatisfaction?, Stephen D. Rubb

In examining the nursing shortage, many scholars note the high correlation between job dissatisfaction and "intentions" to leave. This study compares the earnings of individuals with nurse training in non-nursing occupations to those in nursing occupations. Nurses exiting the occupation while remaining employed appear to occur if alternative occupations pay a premium. This premium is higher for males and increases with education. Earnings of nurses in non-nursing occupations do not provide evidence of significant nursing dissatisfaction.

The purpose of this study is to evaluate the role of simulation in preparing senior nursing students for their first home visit and to determine comfort and confidence levels of these students as they prepare for their first home visits. This exploratory study used a convenience sample of public health nursing students (n= 115). A pretest posttest design evaluated the effectiveness of the simulation using a 5-point Likert scale to detect changes in student confidence. Significant differences were noted on the majority of mean scores (p <.001) post-simulation. This approach could also be used to prepare newly hired home …

The American Pain Foundation reported in 2006 that 32% of military men and women with chronic pain reported that military culture is a barrier to pain care and receiving pain care jeopardized their career. Pain management required a series of understandings about pain in the military that motivated enlisted women's pain perceptions, pain behaviors, and pain relief strategies. The seen and unseen factors were ever-present in military life and contributed to the chronic pain experiences of enlisted women but to what degree was largely unknown. A unique program that partners community health workers with nursing students was developed in response to community-based participatory research data and integrated into a public health practicum at a baccalaureate nursing program in West Michigan. The goal of the program is to promote the health of 3 underserved urban neighborhoods while educating nursing students in population health and public health nursing. The purpose of this paper is to describe program conception and implementation and discuss evaluation of educational outcomes as a public health learning experience.

Communication competence and medical communication competence served as the theoretical framework for this research that seeks to identify specific communication behaviors associated with what nurses say constitute a communicatively competent patient handoff at the nursing change of shift. Data collected from 286 nurses responding to an online modified Medical Communication Competence Scale posted at www.allnurses.com supported the hypotheses that information exchange (information giving, seeking and verifying) and socioemotional communication behaviors are rated more highly in the best patient handoffs than in the worst ones. Assisting smokers to quit and stay quit is the most important intervention clinicians can undertake to improve the length and quality of life of patients who use tobacco. The chronic, relapsing nature of tobacco dependence complicates tobacco treatment. Tobacco treatment counseling programs provide on-going support to help patients avoid relapse. Assistance with a referral increases the likelihood that patients will participate in counseling, but few clinicians regularly assist with referrals to tobacco treatment programs. This dissertation examined health system processes and clinician attitudes that influence the likelihood that clinicians will refer their patients for tobacco treatment counseling.

The transition from mechanical ventilation (MV) to spontaneous ventilation during weaning is associated with hemodynamic alterations and autonomic nervous system (ANS) alterations (reflected by heart rate variability [HRV]). Although cardiac dysrhythmias are an important manifestation of hemodynamic alterations, development of dysrhythmias during MV and weaning and subsequent impact on length of MV has received little attention.

The purposes of this dissertation were to 1) evaluate the relationship of heart rate variability (HRV) during weaning to the development of cardiac dysrhythmias and 2) determine the relationship of cardiac dysrhythmias to length of MV.

The CDC estimates that one in 20 patients admitted to the hospital is a carrier of methicillin-resistant Staphylococcus aureus (MRSA). Staphylococci are commonly found on the skin and mucous membranes within the anterior nares, which provides the principle reservoir for this organism. These organisms can go on to cause surgical site infections in hospitalized patients. Mupirocin is an effective topical medication used to eliminate nasal carriage of Staphylococcus aureus (S. aureus). Based on Level A evidence, the 2007 Society of Thoracic Surgeons has made a Class I recommendation for the use of mupirocin for all patients undergoing cardiac surgery in …

This study examined the rates and risks of premature neonates >2500grams developing intraoperative non-euglycemia events (IONEE). A retrospective chart review of 26 premature neonates >2500 grams who underwent surgical procedures between January 1 and December 31, 2009 was conducted. Statistical analysis was done using Chi square and t-tests. Ten of the 26 subjects (38%) experienced an IONEE. Hyperglycemia was the primary IONEE that was noted in the neonates. (Mean: 143.19; sd: 56.041) Length of surgery was significantly longer in those premature neonates with IONEE than those with euglycemia (71.7 0± 27.03 vs. 45.62 ± 17.98 minutes). Type 2 diabetes is a common chronic metabolic disease affecting 23.6 million people (7.8%) in the U.S. and is the 6th leading cause of death among adults (CDC, 2007). Mexican-Americans, the largest group within the Hispanic U.S population, have double the rate of type 2 diabetes.
