Purpose: The purpose of this sequential explanatory mixed methods study was to identify and describe the leadership strategies that exemplary chief nurse executives use in inpatient acute care hospitals located in Southern California to create an organizational culture of inclusiveness using Kennedy’s (2008) 5 leadership qualities of cultural differences.
Methodology: A sequential mixed methods research design was selected to answer this study’s research questions. The researcher was a part of a thematic group of 10 peer researchers and 7 faculty advisors. Medical coding and billing errors are avoidable problems that have afflicted practitioners for decades. Correct coding and billing are required for reimbursement of healthcare services. Fraud, waste and abuse increase healthcare costs, reduce the quality of care provided and directly impact costs to patients. This pilot study was intended to evaluate the impact of an educational webinar on medical billing and coding, fraud, waste and abuse on nurse practitioner (NP) knowledge using a pretest posttest design. Mean scores increased from 58.4% to 76.4%. Test reliability was low at 0.477 (pretest) and 0.142 (posttest) with two questions that were unable to …

Nursing shortages are reaching crisis level in the United States. Many of the nurses who fill these needs will be millennials. In order to address this crisis, nurse leaders must develop strategies to reduce turnover and retain nurses on their units. This research investigates how leader-member relationship affects turnover intention of millennial nurses. A nonexperimental correlational design was used to examine the relationship between nurse turnover intention and quality of relationships with leaders in millennial nurses. Community health workers provide case management and care coordination services to high-risk members enrolled in Louisiana Department of Health’s Medicaid program. BACKGROUND: Emergency Department boarding is a well-documented systemic problem across the country. ED-2b, the time from decision to admit a patient to Emergency Department departure, is specified by the Joint Commission as a quality measure for Emergency Department boarding. ED-2b metrics have been a longstanding challenge at this community hospital outside the nation’s capital. The aim of this study was to reduce median ED-2b times by 10% compared to fiscal year 2020 (FY20). Background: Stress levels associated with the nursing career can be amplified by hostile social environments in the workplace, horizontal aggression, lack of leadership support, and poor communication. This can lead to poor retention rates and increased burnout.

Effective nurse-patient communication, also described as therapeutic communication, is essential for providing high quality nursing care. It increases patient satisfaction and health, decreases patient anxiety and disease symptoms, and increases patient cooperation and compliance to treatment. Some therapeutic communication techniques include attentive listening, empathy, silence, focusing, open-ended questions, clarification, exploring, clarifying, and summarizing. การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาสมรรถนะของพยาบาลผู้ประกอบการคลินิกพยาบาลชุมชนอบอุ่น โดยใช้เทคนิค EDFR กลุ่มตัวอย่าง คือผู้เชี่ยวชาญ จำนวน 19 คน ประกอบด้วย พยาบาลผู้ประกอบการคลินิกพยาบาลชุมชนอบอุ่นจำนวน 7 คน คณะทำงานโครงการคลินิกพยาบาลชุมชนอบอุ่นจำนวน 5 คน นักวิชาการด้านสุขภาพจำนวน 4 คน และนักกฎหมายวิชาชีพการพยาบาลและการบริการสุขภาพ จำนวน 3 คน วิธีดำเนินการวิจัยแบ่งเป็น 3 รอบ คือ รอบที่ 1 การสัมภาษณ์เกี่ยวกับสมรรถนะของพยาบาลผู้ประกอบการคลินิกพยาบาลชุมชนอบอุ่น รอบที่ 2 นำข้อมูลที่ได้มาวิเคราะห์เนื้อหาแล้วนำมาสร้างแบบสอบถามให้ผู้เชี่ยวชาญประมาณค่าแนวโน้มความสำคัญสมรรถนะของพยาบาลผู้ประกอบการคลินิกพยาบาลชุมชนอบอุ่น และรอบที่ 3 นำข้อมูลมาคำนวณหาค่ามัธยฐาน ค่าพิสัยระหว่างควอไทล์ และจัดทำแบบสอบถามให้ผู้เชี่ยวชาญยืนยันความคิดเห็นเพื่อสรุปผลการวิจัย ผลการวิจัยพบว่า สมรรถนะของพยาบาลผู้ประกอบการคลินิกพยาบาลชุมชนอบอุ่น ประกอบด้วย 7 ด้าน ดังนี้ 1) ด้านบริการพยาบาล ประกอบด้วย การรักษา การส่งเสริมและป้องกันสุขภาพ การฟื้นฟูสุขภาพและการเข้าถึงชุมชน 2) ด้านการบริหารจัดการธุรกิจ ประกอบด้วย การพัฒนากลยุทธ์การดำเนินธุรกิจ การตลาดและการเงิน-การบัญชี 3) ด้านการจัดการความปลอดภัยและความเสี่ยงของคลินิก ประกอบด้วย ความเสี่ยงในการดูแลรักษา สิ่งแวดล้อม การร้องเรียนและสิทธิผู้ป่วย และเทคโนโลยี 4) ด้านการพัฒนาคุณภาพบริการ ประกอบด้วย พัฒนาคุณภาพอย่างต่อเนื่อง ตอบสนองความต้องการของผู้รับบริการอย่างรวดเร็วและทันท่วงที และนำเทคโนโลยีที่ทันสมัยมาสนับสนุนการให้บริการ5) ด้านการจัดทำระบบจัดเก็บอุปกรณ์ทางการแพทย์ ประกอบด้วย การจัดทำระบบบัญชียาและวัสดุอุปกรณ์ทางการแพทย์อย่างเป็นระบบ 6) ด้านกฎหมายและจริยธรรม ประกอบด้วย การตัดสินใจเชิงจริยธรรมการบริหารจัดการคลินิกและปฏิบัติตามกฎหมายที่เกี่ยวข้อง และ 7) ด้านการสื่อสาร ประกอบด้วย การสร้างสัมพันธภาพ และการสื่อสารที่มีประสิทธิภาพ

เชื้อโควิด 19 โดยใช้ระเบียบวิธีวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาของ Heidegger โดยผู้ให้ข้อมูลเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในหอผู้ป่วยผู้ป่วยติดเชื้อโควิด 19 และมีความยินดีเข้าร่วมการวิจัย โดยคัดเลือกผู้ให้ข้อมูลแบบเจาะจง จำนวน 15 คนเก็บรวบรวมข้อมูลด้วยการสัมภาษณ์เชิงลึกและการบันทึกเทป ร่วมกับการบันทึกภาคสนาม และนำมาข้อมูลที่ได้มาถอดเทปแบบคำต่อคำ และวิเคราะห์ข้อมูลโดยวิธีการของ van Manen ผลการศึกษา ประสบการณ์การปฏิบัติงานของพยาบาลในหอผู้ป่วยติดเชื้อโควิด 19 แบ่งเป็น 6 ประเด็นหลักและประเด็นย่อย ดังนี้ 1. ปฏิบัติตามแนวทางการดูแลผู้ป่วยโควิด ประกอบด้วย 4 ประเด็นย่อย1.1) สวมอุปกรณ์ป้องกันก่อนเข้าห้องผู้ป่วย 1.2) วางแผนให้การพยาบาลแบบเบ็ดเสร็จตามรอบเวลา1.3) เสร็จสิ้นการทำงาน ถอดชุดป้องกันอย่างถูกวิธี และ 1.4) ทำความสะอาดร่างกาย เปลี่ยนชุดใหม่ก่อนลงเวรหรือกลับบ้าน 2. ดูแลด้วยใจ เอาใจใส่ให้ความช่วยเหลือทุกเรื่อง ประกอบด้วย 7 ประเด็นย่อย2.1) ดูแลด้านร่างกายให้สุขสบาย 2.2) ติดตามอาการที่อาจเปลี่ยนแปลงได้ตลอดเวลา 2.3) ให้กำลังใจในการรักษา 2.4) สิ่งใดที่ปรารถนา เต็มใจจัดหามาให้ 2.5) ใส่ใจตรวจสอบและประสานสิทธิ์การรักษา 2.6) เมื่อผู้ป่วยมีปัญหา ติดต่อประสานหน่วยงานที่เกี่ยวข้อง และ 2.7) จัดการช่วยเหลือผู้ป่วยจนถึงวาระสุดท้าย 3. มีปัญหาต้องแก้ไข เพื่อให้ทำงานได้อย่างต่อเนื่อง ประกอบด้วย 4 ประเด็นย่อย 3.1) บริหารจัดการเตียงให้เพียงพอกับผู้ป่วยโควิดแต่ละประเภท 3.2) อัตรากำลังพยาบาลไม่เพียงพอ ขออัตรากำลังเสริมเข้าช่วย 3.3) ต่างชาติต่างภาษา เจรจาวุ่นวาย หาตัวช่วยคลี่คลาย สื่อสารจนเข้าใจ และ 3.4) อึดอัดใจ ทีมไม่เข้าดูคนไข้ เจรจาให้เข้าใจทำงานกันได้ราบรื่น 4. ขวัญและกำลังใจในการปฏิบัติหน้าที่ ประกอบด้วย 5 ประเด็นย่อย 4.1 ผู้บริหารเห็นความสำคัญ สร้างขวัญและกำลังใจผู้ปฏิบัติงาน 4.2 เพื่อนร่วมงานให้กำลังใจ ช่วยเหลือกันเป็นอย่างดี 4.3 ผู้ป่วยและญาติ ให้กำลังใจและให้การสนับสนุนอุปกรณ์การแพทย์ 4.4 บุคคลภายนอกให้กำลังใจในรูปแบบที่แตกต่างจากเดิม และ 4.5 การได้บรรจุเป็นข้าราชการและเงินค่าเสี่ยงภัย เป็นขวัญกำลังใจที่สำคัญ …

การวิจัยนี้มีวัตถุประสงค์เพื่อบรรยายประสบการณ์ของผู้บริหารทางการพยาบาลในการบริหารจัดการพยาบาลผู้ป่วยโควิด 19 โดยใช้การวิจัยเชิงคุณภาพ ผู้ให้ข้อมูล คือ ผู้บริหารทางการพยาบาลที่มีส่วนร่วมในการบริหารจัดการพยาบาลผู้ป่วยโควิด 19 ทั้งหมด 25 คน เก็บรวบรวมข้อมูลด้วยวิธีการสัมภาษณ์เชิงลึก การบันทึกเทป การสังเกต และการบันทึกภาคสนาม นำข้อมูลทั้งหมดมาวิเคราะห์เนื้อหาตามวิธีการของ Colaizzi ผลการศึกษาประสบการณ์ของผู้บริหารทางการพยาบาลในการบริหารจัดการการดูแลผู้ป่วยโควิด 19 ประกอบด้วยประเด็นใหญ่และย่อยดังนี้ 1. การเตรียมความพร้อมเพื่อรองรับผู้ป่วยโควิด 19 ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 1.1) จัดหาพื้นที่และปรับโครงสร้างหน่วยงานสำหรับรองรับผู้ป่วยโควิด 19 1.2) จัดหาบุคลากรพยาบาลในการดูแลผู้ป่วยโควิด 19 และ 1.3) เตรียมความพร้อมของพยาบาลและจัดหาอุปกรณ์การป้องกัน 2. บริหารจัดการขับเคลื่อนงานดูแลผู้ป่วยโควิด 19 ประกอบด้วย 4 ประเด็นย่อย ได้แก่ 2.1) กำหนดนโยบาย สร้างแนวปฏิบัติ ควบคุมกำกับ ติดตามทีม 2.2) เปิดใจกว้างรับฟังความคิดเห็นของทุกคน 2.3) สื่อสารเรื่องราว บอกเล่าความจริง ให้บุคลากรรับทราบ และ 2.4) จัดพี่เลี้ยงสอนงานให้กับพยาบาลที่มาช่วยงานในหอผู้ป่วยโควิด 19 3. จัดการกับปัญหานานาประการเพื่อให้งานดำเนินต่อไป ประกอบด้วย 5 ประเด็นย่อย ได้แก่ 3.1) จัดการกับอารมณ์และความรู้สึกทางลบของพยาบาล 3.2) จัดการกับความเครียดจากการปฏิบัติงานของตนเอง 3.3) เพิ่มทักษะการดูแลผู้ป่วยโควิด 19ในภาวะวิกฤต 3.4) อุปกรณ์ป้องกันขาดแคลน ต้องมีแผนการแก้ไข และ 3.5) อัตรากำลังพยาบาลไม่พอ ต้องขอกำลังเสริมจากทุกหน่วยงาน 4. สร้างขวัญและกำลังใจในการปฏิบัติงานช่วงสถานการณ์โควิด ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 4.1) ผู้บริหารลงหน้างาน ให้คำปรึกษา และร่วมแก้ไขปัญหา และ 4.2) ผลักดันการเพิ่มค่าตอบแทนและสวัสดิการอื่นๆให้ผู้ปฏิบัติงาน 5. ผลลัพธ์การบริหารจัดการในสถานการณ์โควิด 19 ประกอบก้วย 2 ประเด็นย่อย 5.1) เกิดความร่วมแรงร่วมใจ จึงควบคุมสถานการณ์โควิด 19 …

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาการจัดบริการพยาบาลของคลินิกพยาบาลชุมชนอบอุ่น โดยใช้เทคนิค EDFR กลุ่มตัวอย่าง คือ ผู้เชี่ยวชาญ จำนวน 17 คน 4 กลุ่ม ได้แก่ 1) กลุ่มคณะทำงานโครงการพัฒนารูปแบบคลินิกการพยาบาลและการผดุงครรภ์เป็นหน่วยบริการร่วมให้บริการในระบบหลักประกันสุขภาพแห่งชาติ 5 คน 2) กลุ่มผู้ประกอบการคลินิกพยาบาลชุมชนอบอุ่น 5 คน 3) กลุ่มนิติกรที่เชี่ยวชาญกฎหมายด้านสาธารณสุข 3 คน และ 4) กลุ่มอาจารย์พยาบาล/นักวิชาการที่มีความเชี่ยวชาญด้านการจัดบริการพยาบาล 4 คน สัมภาษณ์เฉพาะรอบที่ 1 วิธีดำเนินการวิจัยประกอบด้วยการเก็บข้อมูล 3 รอบ คือ รอบที่ 1 การสัมภาษณ์กลุ่มผู้เชี่ยวชาญ จำนวน 17 คน รอบที่ 2 นำข้อมูลที่ได้จากการสัมภาษณ์มาวิเคราะห์เนื้อหาแล้วนำมาสร้างเป็นแบบสอบถาม ส่งให้ผู้เชี่ยวชาญ จำนวน 17 คน ประมาณค่าแนวโน้มความสำคัญ และรอบที่ 3 นำข้อมูลจากรอบที่ 2 มาคำนวณหาค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์ จากนั้นนำแบบสอบถามส่งให้ผู้เชี่ยวชาญยืนยันความคิดเห็น เพื่อสรุปผลการศึกษาการจัดบริการพยาบาลคลินิกพยาบาลชุมชนอบอุ่น ผลการวิจัยพบว่าการจัดบริการพยาบาลคลินิกพยาบาลชุมชนอบอุ่น ประกอบด้วย 5 ด้าน ดังนี้ 1) ด้านการให้บริการในคลินิก ประกอบด้วย กิจกรรมส่งเสริมสุขภาพ ตามกลุ่มวัย ป้องกันโรค ตามความเสี่ยงกลุ่มวัย รักษาพยาบาลเบื้องต้น และบริการฟื้นฟูสภาพ 2) ด้านการให้บริการในชุมชน ประกอบด้วย กิจกรรมการเยี่ยมบ้านผู้รับบริการกลุ่มต่างๆ 3) ด้านบริการส่งต่อผู้ป่วย ประกอบด้วยการประสานงานกับหน่วยบริการเครือข่ายระดับต่างๆ เพื่อส่งต่อไปรับการบำบัด 4) ด้านการให้บริการผ่านเทคโนโลยีสารสนเทศ ประกอบด้วย การนำ application มาช่วยในกิจกรรมการให้คำปรึกษาด้านสุขภาพและการติดตามเยี่ยมบ้าน การส่งข้อมูลความรู้ด้านสุขภาพแก่ผู้รับบริการ และ บันทึกข้อมูลการให้บริการด้วยระบบเทคโนโลยีสารสนเทศ 5) ด้านการให้บริการป้องกันและควบคุมการแพร่กระจายเชื้อที่บ้านและชุมชนของผู้รับบริการ ประกอบด้วยการวางแผนป้องกันและควบคุมการติดเชื้อตามปัญหาของผู้รับบริการ ติดตามประเมินผลการปฏิบัติของผู้รับบริการ และรายงานสถานการณ์การระบาด ผลการวิจัยนี้ทำให้ทราบถึงการจัดบริการพยาบาลคลินิกพยาบาลชุมชนอบอุ่นที่มีความชัดเจน เพื่อเป็นพื้นฐานในการจัดบริการพยาบาลที่มีมาตรฐานและตอบสนองความต้องการของผู้รับบริการ

Problem. In a 21-hospital region of a 39-hospital integrated health system, CNE turnover peaked at 63% (12 CNEs) in 2015. Interviews were conducted in 2019 with 12 CNEs across the region to understand potential issues related to CNE job satisfaction. Responses revealed concerns regarding empowerment, alignment, work-life balance, information transparency/sharing, and recognition. While identifying a solution to address CNE concerns, the organization experienced successive crisis events during a 12-month period that included a record-setting wildfire, multiple labor union strikes, and a novel pandemic.
