Background: Inadequate communication can lead to poor quality of care and patient outcomes. Physician-nurse dyad partnerships have been shown to increase effective communication but there is a lack of data to show the impact these dyads have on dyad members’ perceptions about patient safety, work engagement and quality patient outcome measures.

Background: Shared governance programs are a popular way to improve Registered Nurse (RN) engagement and retention. These programs are notoriously difficult to garner and maintain participation. However, successful shared governance can improve patient outcomes, patient satisfaction and RN engagement.

Conceptual Framework: The theory of planned behavior was used as the conceptual framework for this study. This theory posits that attitudes, subjective norms, perceived behavioral control and intentions towards a behavior can predict behavioral achievement.

BACKGROUND: Relational aggression (RA) in nursing is widespread. One group often at risk for exposure to RA is the newly licensed nurse. RA can lead to a negative work environment which can cause poor patient outcomes and high nurse turnover.

PURPOSE: The purpose of this study was to evaluate the effects of an educational program on RA in the emergency department with newly licensed registered nurses. This was conducted using one pre and two post surveys. The surveys evaluated the nurses’ knowledge of RA self-awareness before and six weeks after an educational session.

การศึกษาวิจัยเชิงคุณภาพครั้งนี้มีวัตถุประสงค์เพื่อบรรยายประสบการณ์การเป็นพยาบาลที่ปฏิบัติงานในหน่วยงานอุบัติเหตุและฉุกเฉินยุคใหม่โรงพยาบาลทั่วไปแห่งหนึ่ง โดยใช้ระเบียบวิธีวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาของ Heidegger โดยผู้ให้ข้อมูลเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในหน่วยงานอุบัติเหตุและฉุกเฉิน และมีความยินดีเข้าร่วมการวิจัย จำนวน 12 คน เก็บรวบรวมข้อมูลด้วยการสัมภาษณ์เชิงลึกและการบันทึกเทป ร่วมกับการบันทึกภาคสนาม และนำข้อมูลที่ได้มาถอดเทปแบบคำต่อคำ และวิเคราะห์ข้อมูลโดยวิธีของ Crist and Tanner ผลการศึกษา ประสบการณ์การเป็นพยาบาลที่ปฏิบัติงานในหน่วยงานอุบัติเหตุและฉุกเฉินยุคใหม่โรงพยาบาลทั่วไปแห่งหนึ่ง แบ่งเป็น 4 ประเด็นหลัก ดังนี้ 1. ภูมิหลังและการเตรียมความพร้อมในการปฏิบัติงานใน ER ประกอบด้วยประเด็นย่อยดังนี้ 1.1) หลากหลายเหตุผลที่เข้ามาทำงานใน ER 1.2) เริ่มทำงาน เกิดความไม่มั่นใจต้องใช้เวลาในการปรับตัว และ 1.3) การเตรียมความรู้ในการดูแลผู้ป่วย 2. การปฏิบัติงานของพยาบาลในหน่วยอุบัติเหตุและฉุกเฉิน ประกอบด้วยประเด็นย่อยดังนี้ 2.1) คัดกรองผู้ป่วยด้วยทักษะที่หลากหลาย 2.2) ดูแลผู้ป่วยในห้องสังเกตการ 2.3) ช่วยฟื้นคืนชีวิตโดยปฏิบัติตามเทคนิคที่ถูกต้อง 2.4) บริการรับและส่งต่อผู้ป่วย 2.5) ปฏิบัติงานการแพทย์ฉุกเฉิน 2.6) บริการงานสาธารณภัย พร้อมโรคอุบัติใหม่ที่เกิดขึ้นและ 2.7) บริการความรู้แก่สังคมเพื่อประโยชน์แห่งตนและผู้อื่น 3. ปัญหาหน้างาน บริหารจัดการโดยหัวหน้าเวร ประกอบด้วยประเด็นย่อยดังนี้ 3.1) บริหารอัตรากำลังให้เหมาะสม หากมีผู้ป่วยจำนวนมาก 3.2) สื่อสารให้ผู้ป่วยและญาติเข้าใจหากต้องรอตรวจนาน 3.3) ป้องกันความผิดพลาดการดูแล จึงต้องมีการตรวจสอบและทวนซ้ำและ 3.4) ระงับเหตุการณ์วุ่นวาย ต้องอาศัยหลายฝ่ายร่วมกัน 4. ผลของการปฏิบัติงานใน ER ประกอบด้วยประเด็นย่อยดังนี้ 4.1) เครียดจากการบริหารจัดการในงานยุคใหม่ 4.2) ขาดสมดุลระหว่างชีวิตกับงานที่ต้องปฏิบัติ และ 4.3) ความสุขเกิดขึ้นได้จากผู้ร่วมงานที่ดีและเต็มที่กับการดูแลผู้ป่วยจนปลอดภัย การวิจัยนี้ทำให้เข้าใจการเป็นพยาบาลหน่วยอุบัติและฉุกเฉินที่ให้ความสำคัญกับการดูแลผู้ป่วยและการบริหารจัดการ ซึ่งผู้บริหารทางการพยาบาลสามารถนำไปใช้เป็นแนวทางในการพัฒนาคนและพัฒนางานของหน่วยอุบัติและฉุกเฉินให้มีประสิทธิภาพมากขึ้น

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาการจัดบริการทางการพยาบาลสำหรับผู้ป่วยที่ติดเชื้อโคโรนาไวรัส 2019 ที่เข้ารับการรักษาในหอผู้ป่วยเฉพาะกิจ โดยใช้เทคนิคเดลฟาย ในการรวบรวมฉันทามติของกลุ่มผู้เชี่ยวชาญ จำนวน 20 คน เก็บรวบรวมข้อมูล จำนวน 3 รอบ วิเคราะห์ข้อมูลโดยวิเคราะห์เนื้อหา และใช้สถิติหาค่ามัธยฐาน และค่าพิสัยระหว่างควอไทล์ ผลการวิจัยพบว่า ประกอบด้วย 7 ด้าน ดังนี้ 1) ด้านการวางแผนการจัดบริการ หัวหน้าพยาบาลเป็นผู้กำหนดแผนการจัดบริการทางการพยาบาลและถ่ายทอดนโยบายมาสู่พยาบาลควบคุมการติดเชื้อในโรงพยาบาลและผู้จัดการพยาบาล ในการคัดเลือกสถานที่ จัดเตรียมเอกสาร เวชภัณฑ์ และอุปกรณ์ทางการแพทย์ 2) ด้านการจัดการโครงสร้าง หัวหน้าพยาบาลเป็นผู้จัดโครงสร้างขององค์การ โดยแบ่งหน้าที่ความรับผิดชอบ และจัดอัตรากำลังโดยผู้จัดการพยาบาล รวมถึงมีการจัดโครงสร้างของหอผู้ป่วยเฉพาะกิจโดยพยาบาลควบคุมการติดเชื้อในโรงพยาบาล 3) ด้านการบริหารจัดการบุคลากรผู้ปฏิบัติงาน ประกอบด้วย การให้ความรู้และฝึกอบรม การมอบหมายหน้าที่ การปฐมนิเทศ และการจัดสวัสดิการและจูงใจการทำงานให้แก่บุคลากรทางการพยาบาล 4) ด้านการควบคุมกำกับการปฏิบัติงาน ประกอบด้วย การควบคุมกำกับการบริหารจัดการความเสี่ยงโดยหัวหน้าทีมการพยาบาล การควบคุมกำกับและป้องกันการติดเชื้อในการปฏิบัติงานโดยผู้จัดการพยาบาล และการควบคุมกำกับการปฏิบัติงานตามมาตรฐานการพยาบาลโดยหัวหน้าทีมการพยาบาล 5) ด้านการติดต่อสื่อสารและประสานงาน ประกอบด้วย การติดต่อภายใน และภายนอกหอผู้ป่วยเฉพาะกิจ เพื่อให้การพยาบาลผู้ป่วยอย่างต่อเนื่อง 6) ด้านการรายงาน ผู้จัดการพยาบาลเป็นผู้รายงานสถานการณ์ปัญหาและยอดผู้ป่วยในแต่ละวันให้หัวหน้าพยาบาลและผู้ที่มีส่วนเกี่ยวข้องทราบ 7) ด้านงบประมาณ หัวหน้าพยาบาลมีการบริหารจัดการให้มีค่าเสี่ยงภัย และจัดสรรเวชภัณฑ์และอุปกรณ์ทางการแพทย์ให้เพียงพอและพร้อมใช้

การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อบรรยายประสบการณ์ชีวิตของพยาบาลวิชาชีพที่ติดเชื้อไวรัสโคโรนา 2019 โดยใช้ระเบียบวิธีการวิจัยเชิงคุณภาพ ผู้ให้ข้อมูล คือ พยาบาลวิชาชีพที่ติดเชื้อไวรัสโคโรนา 2019 จากการปฏิบัติงาน โดยคัดเลือกผู้ให้ข้อมูลแบบเจาะจง จำนวน 15 ราย เก็บรวบรวมข้อมูลด้วยวิธีการสัมภาษณ์เชิงลึก การบันทึกเทป และการบันทึกภาคสนาม นำข้อมูลทั้งหมดมาวิเคราะห์เนื้อหาตามวิธีการของ Colaizzi ผลการศึกษาประสบการณ์ชีวิตของพยาบาลวิชาชีพที่ติดเชื้อไวรัสโคโรนา 2019 ประกอบด้วยประเด็นหลัก และประเด็นย่อยดังต่อไปนี้ 1. สภาพการทำงานที่มีภาวะเสี่ยง ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 1.1) มีโอกาสสัมผัสจากเชื้อโรค ไม่ได้มีการป้องกันทุกขั้นตอน และ 1.2) ละเลยการใช้อุปกรณ์การป้องกัน 2. เริ่มมีอาการ แจ้งหน่วยงานเข้าระบบการรักษา ประกอบด้วย 4 ประเด็นย่อย ได้แก่ 2.1) มีอาการนำก่อนมาโรงพยาบาล 2.2) เข้ารับการตรวจตามระบบเพื่อยืนยันการติดเชื้อโควิด 2.3) เลือกสถานที่เข้ารับการรักษา และ 2.4) ได้รับการรักษาตามอาการ 3. ความรู้สึกหลากหลายในช่วงเวลากักตัวและรับการรักษา ประกอบด้วย 4 ประเด็นย่อย ได้แก่ 3.1) กลัวและกังวลใจจะทำให้คนอื่นติดเชื้อโควิด 3.2) เครียด กลัว กังวล สุขภาพของตนในระยะยาว 3.3) เบื่อกับการอยู่ในพื้นที่จำกัดทำกิจวัตรซ้ำๆ เดิม และ 3.4) รู้สึกดีที่ได้รับกำลังใจจากคนรอบข้าง 4. อาการที่หลงเหลืออยู่หลังการรักษา ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 4.1) อาการเหนื่อยง่าย หายใจไม่เต็มอิ่ม และ 4.2) ไอ มีน้ำมูกและเจ็บคอ 5. การทำงานของพยาบาลมีความเสี่ยงตลอดเวลา ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 5.1) ป้องกันการติดเชื้อ เพื่อไม่ให้มีการติดซ้ำ และ 5.2) ใส่ใจสุขภาพ ใช้ชีวิตอย่างระมัดระวัง ผลการวิจัยนี้ ผู้บริหารทางการพยาบาลสามารถใช้เป็นข้อมูลพื้นฐานในการวางมาตรการเฝ้าระวัง ป้องกัน และควบคุมการแพร่กระจายเชื้อไวรัสโคโรนา 2019 …

การวิจัยเชิงคุณภาพนี้ มีวัตถุประสงค์เพื่อศึกษาประสบการณ์ของพยาบาลวิชาชีพที่ดูแลผู้ที่ติดเชื้อไวรัสโคโรนา 2019 ในหอผู้ป่วยวิกฤต โดยการสัมภาษณ์แบบเจาะลึก ร่วมกับการบันทึกเทป การสังเกต การจดบันทึกภาคสนาม จำนวน 15 คน คัดเลือกแบบเฉพาะเจาะจง วิเคราะห์เนื้อหาตาม van Manen (1990) ผลการศึกษา ประสบการณ์ของพยาบาลในการดูแลผู้ที่ติดเชื้อไวรัสโคโรนา 2019 ในหอผู้ป่วยวิกฤต พบมี 5 ประเด็นหลัก ดังนี้ 1) การดูแลผู้ที่ติดเชื้อด้วยความรู้สึกหลากหลายเพราะโควิดเป็นโรคอุบัติใหม่ ประกอบด้วยประเด็นย่อย ดังนี้ 1.1) เหมือนอยู่ในสมรภูมิรบ 1.2) เป็นความเครียด มีความกดดัน เพราะเป็นการดูแลที่ไม่คุ้นเคย 1.3) กลัวติดเชื้อจากผู้ป่วย 1.4) รู้สึกหดหู่ใจเพราะผู้ป่วยตายทุกวัน 1.5) ตื่นเต้น วุ่นวายในการช่วยชีวิตผู้ป่วย 1.6) เหนื่อยกายหลังให้การดูแลแต่มีใจสู้ต่อ และ 1.7) ภูมิใจในตนเองที่สามารถให้การดูแลผู้ป่วยกลุ่มนี้ได้ 2) การดูแลด้วยประสบการณ์เดิมผสานกับความรู้ใหม่ ประกอบด้วยประเด็นย่อย ดังนี้ 2.1) การดูแลผู้ป่วยใส่เครื่องช่วยหายใจในท่านอนคว่ำ 2.2) เฝ้าระวังอาการและภาวะการหายใจ หากเปลี่ยนไปรายงานแพทย์ทันที 2.3) ป้องกันข้อต่อเครื่องช่วยหายใจไม่ให้เลื่อนหลุด 2.4) สังเกตน้ำยาล้างไตและสารน้ำที่ให้ผ่านเครื่อง Monitor และ 2.5) ใช้ Defibrillator แทนการ CPR เมื่อผู้ป่วยใส่ ECMO 3) การดูแลเอาใจใส่ด้วยหัวใจความเป็นมนุษย์ ประกอบด้วยประเด็นย่อย ดังนี้ 3.1) การดูแลด้วยความเข้าใจ/ใช้สติ ให้กำลังใจและถามไถ่ความรู้สึก 3.2) เป็นสะพานเชื่อมระหว่างผู้ป่วยและสมาชิกครอบครัว และ 3.3) วาระสุดท้ายต้องดูแลให้ได้ตามมาตรฐานและตอบสนองความต้องการของครอบครัว 4) ปัญหาและอุปสรรคของการดูแลผู้ป่วย ประกอบด้วยประเด็นย่อย ดังนี้ 4.1) ข้อจำกัดด้านการสื่อสารในทีมสุขภาพ และ 4.2) ข้อจำกัดด้านการสื่อสารระหว่างผู้ป่วยและครอบครัว 5) บทเรียนรู้ที่ได้รับจากการดูแลผู้ป่วย ประกอบด้วยประเด็นย่อย ดังนี้ 5.1) เกิดการเรียนรู้นำสู่การปรับปรุงตน 5.2) ผลของงานเกิดจากความทุ่มเทของทีม 5.3) ความรับผิดชอบในวิชาชีพและคุณค่าของการเป็นพยาบาลวิกฤติ และ …

Postgraduate programs for PAs provide formal postgraduate training for clinical specialty areas. These programs are intended to provide intense specialty training in various fields, as well as to standardize education beyond the entry level. The purpose of this descriptive study was to determine if there are consistent trends or clear differences in program length, approach to training, validation of learning, and accreditation through the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) for postgraduate programs with a surgical focus in the United States.

Background: James Madison University and Cayman Islands Health Services Authority (HSA) entered into a collaborative agreement the summer of 2020. There was a lack of organizational structure surrounding implementation and systemization of evidence-based practice (EBP) models and a disconnect between nurse manager knowledge and EBP implementation within HSA.

Methods: The population for this evidence-based project was 21 HSA nurse managers from the inpatient setting. Pre-intervention survey results informed organizational readiness, nurse manager knowledge and ability to implement, and guided the intervention that included the creation and implementation of a website toolkit for EBP. Problem: Organizations have taken measures to reduce the length of stay by addressing logistical issues such as care coordination across the continuum. This becomes especially important for patients being discharged to skilled nursing facilities (SNF), where placement can be impeded due to complex care needs. In March 2021, the facility accounted for 20% of all extended length of stay (ELOS) patients across the organization. A large healthcare organization’s 26-bed, medical-surgical, telemetry, COVID-19 overflow unit was significantly impacted. Many healthcare organizations have adopted advocacy for attaining joy in work as an extra dimension of the Institute for Health Improvement’s Triple Aim.
