Background and Rationale: High Deductible Health Plans (HDHPs) are designed to reduce overall healthcare spending by shifting more upfront costs to patients to discourage overutilization of care, yet HDHPs often increase financial burden, particularly in outpatient mental health care. This Doctor of Nursing Practice (DNP) project explores the impact of implementing a Virtual Discharge Nurse (VDN) on patient length of stay (LOS) and discharge timing at Paoli Hospital within the Main Line Health system. Hospital discharges are often complex and can negatively impact patient flow and outcomes. This study highlights the urgent need for innovative discharge practices in healthcare and supports the potential benefits of integrating technology into nursing roles to improve patient care and operational efficiency. Objective: To utilize the Gallup Q12+ survey to assess baseline staff engagement among nurses and nursing assistants at a critical access hospital, implement direct education intervention, and evaluate its impact on staff engagement rates, with the goal of identifying micro and macro-problems.

Methods: This quality improved project was conducted at Littleton Regional Hospital, a level Ⅲ trauma center and critical access hospital. The PDSA cycle started on March 1 and ended May 10. งานวิจัยมีวัตถุประสงค์เพื่อ 1) ศึกษาความตั้งใจคงอยู่ในองค์การของพยาบาลวิชาชีพเจเนอเรชั่นแซด โรงพยาบาลระดับตติยภูมิ สังกัดมหาวิทยาลัย ภาคตะวันออกเฉียงเหนือ และ 2) ศึกษาความสัมพันธ์ระหว่างภาวะผู้นำสอดคล้องของหัวหน้าหอผู้ป่วย คุณภาพชีวิตในการทำงาน ความเข้มแข็งทางจิตใจกับความตั้งใจคงอยู่ในองค์การของพยาบาลวิชาชีพเจเนอเรชั่นแซด กลุ่มตัวอย่าง คือ พยาบาลวิชาชีพอายุ 22-29 ปี จำนวน 189 คน จากโรงพยาบาลระดับตติยภูมิ สังกัดมหาวิทยาลัย ภาคตะวันออกเฉียงเหนือ 3 แห่ง โดยใช้แบบสอบถาม Google form จำนวน 1 ชุด 5 ตอน ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามภาวะผู้นำสอดคล้องของหัวหน้าหอผู้ป่วย แบบสอบถามคุณภาพชีวิตในการทำงาน แบบสอบถามความเข้มแข็งทางจิตใจ แบบสอบถามความตั้งใจคงอยู่ในองค์การ มีค่าความเที่ยงเท่ากับ .98, .94, .89, 1.00 วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาหาค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และหาความสัมพันธ์โดยใช้สถิติ Chi-square และรายงานขนาดความสัมพันธ์ด้วย Phi coefficient ผลการวิจัยพบว่า 1) ความตั้งใจคงอยู่ในองค์การของพยาบาลวิชาชีพเจเนอเรชั่นแซด จำนวน 189 คน ส่วนใหญ่มีความตั้งใจที่จะคงอยู่ในองค์การ คิดเป็นร้อยละ 70.40 และกลุ่มที่ตั้งใจจะลาออกจากองค์การคิดเป็นร้อยละ 29.60 2) ภาวะผู้นำสอดคล้องของหัวหน้าหอผู้ป่วย คุณภาพชีวิตในการทำงาน ความเข้มแข็งทางจิตใจมีความสัมพันธ์กับความตั้งใจคงอยู่ในองค์การของพยาบาลวิชาชีพเจเนอเรชั่นแซดอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (χ² = 59.93, χ² = 46.95, χ² = 14.87) ตามลำดับ โดยภาวะผู้นำสอดคล้องของหัวหน้าหอผู้ป่วย คุณภาพชีวิตในการทำงานมีความสัมพันธ์ค่อนข้างสูง (Phi = .56, Phi = .50) และความเข้มแข็งทางจิตใจมีความสัมพันธ์ปานกลาง (Phi = .28)

การศึกษานี้เป็นการวิจัยเชิงพรรณนา มีวัตถุประสงค์เพื่อศึกษาสมรรถนะวิสัญญีพยาบาล โรงพยาบาลตติยภูมิ กระทรวงสาธารณสุข โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลคือ ผู้เชี่ยวชาญด้านวิสัญญีวิทยา จำนวน 19 คน ประกอบด้วย วิสัญญีแพทย์จากราชวิทยาลัยวิสัญญีแพทย์แห่งประเทศไทย จำนวน 4 คน คณะกรรมการชมรมวิสัญญีแห่งประเทศไทย จำนวน 3 คน คณะกรรมการการฝึกอบรมและสอบความรู้ความชำนาญเฉพาะทางทางการพยาบาลและการผดุงครรภ์ด้านการให้ยาระงับความรู้สึก จำนวน 2 คน หัวหน้างานวิสัญญีพยาบาล จำนวน 4 คน วิสัญญีพยาบาลผู้ปฏิบัติการพยาบาลวิสัญญี จำนวน 6 คน เครื่องมือที่ใช้ในการวิจัย ประกอบด้วยแบบสอบถามแบบปลายเปิด และแบบสอบถามมาตราส่วนประมาณค่า 5 ระดับ การดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ 1) สัมภาษณ์ผู้เชี่ยวชาญเพื่อรวบรวมความคิดเห็นเกี่ยวกับสมรรถนะวิสัญญีพยาบาล 2) นำข้อมูลที่ได้จากการสัมภาษณ์มาวิเคราะห์เนื้อหาและนำมาสร้างแบบสอบถามเพื่อให้ผู้เชี่ยวชาญประมาณค่าระดับความสำคัญ 3) นำข้อมูลที่ได้จากการตอบแบบสอบถามประมาณค่าความสำคัญมาคำนวณค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์ และส่งแบบสอบถามให้ผู้เชี่ยวชาญยืนยันความคิดเห็นอีกครั้ง ผลการวิจัย พบว่า สมรรถนะวิสัญญีพยาบาล โรงพยาบาลตติยภูมิ กระทรวงสาธารณสุข ประกอบด้วย 7 ด้าน ดังนี้ 1) ด้านการปฏิบัติการพยาบาลวิสัญญี จำนวน 27 ข้อ 2) ด้านการบริหารยาทางวิสัญญี จำนวน 10 ข้อ 3) ด้านการจัดการความปลอดภัยและการบริหารความสี่ยงทางวิสัญญี จำนวน 14 ข้อ 4) ด้านการสื่อสารและการประสานงานเพื่อการดูแลผู้ป่วย จำนวน 7 ข้อ 5) ด้านภาวะผู้นำและการใช้เทคโนโลยี จำนวน 12 ข้อ 6) ด้านการพัฒนาคุณภาพการพยาบาลวิสัญญี จำนวน 8 ข้อ และ 7) ด้านจรรยาบรรณ และกฎหมาย จำนวน 5 ข้อ

การศึกษานี้เป็นการวิจัยเชิงพรรณนา เพื่อศึกษาบทบาทพยาบาลหัวหน้าเวร หอผู้ป่วยวิกฤตโรคหัวใจและหลอดเลือด โรงพยาบาลตติยภูมิ โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลประกอบด้วยผู้เชี่ยวชาญจำนวน 19 คน วิธีดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ ขั้นตอนที่ 1 สัมภาษณ์ผู้เชี่ยวชาญเกี่ยวกับบทบาทพยาบาลหัวหน้าเวร หอผู้ป่วยวิกฤตโรคหัวใจและหลอดเลือด โรงพยาบาลตติยภูมิ ขั้นตอนที่ 2 นำข้อมูลที่ได้จากการสัมภาษณ์มาวิเคราะห์เนื้อหาแล้วสร้างเป็นแบบสอบถามให้ผู้เชี่ยวชาญประมาณค่าแนวโน้มความสำคัญของบทบาทพยาบาลหัวหน้าเวร หอผู้ป่วยวิกฤตโรคหัวใจและหลอดเลือด โรงพยาบาลตติยภูมิ และขั้นตอนที่ 3 นำข้อมูลที่ได้จากรอบที่ 2 มาคำนวณหาค่ามัธยฐาน ค่าพิสัยระหว่างควอไทล์ และส่งแบบสอบถามให้ผู้เชี่ยวชาญกลุ่มเดิมยืนยันคำตอบ จากนั้นจึงนำข้อมูลที่ได้มาคำนวณค่ามัธยฐาน และค่าพิสัยควอไทล์อีกครั้ง เพื่อสรุปเป็นบทบาทพยาบาลหัวหน้าเวร หอผู้ป่วยวิกฤตโรคหัวใจและหลอดเลือด โรงพยาบาลตติยภูมิ ผลการวิจัยพบว่า บทบาทพยาบาลหัวหน้าเวร หอผู้ป่วยวิกฤตโรคหัวใจและหลอดเลือด โรงพยาบาลตติยภูมิ ประกอบด้วย 9 บทบาท จำนวน 82 ข้อรายการ ผู้เชี่ยวชาญให้ความสำคัญระดับมากที่สุด จำนวน 81 ข้อรายการ (MD : 4.61-4.76, IR : 0.25-0.87 ) และความสำคัญระดับมาก จำนวน 1 ข้อ (MD : 4.36, IR : 0.83) ดังนี้ 1) ผู้นำทีมพยาบาลและพัฒนาคุณภาพ จำนวน 11 ข้อ 2) ผู้ประสานงานและสร้างเครือข่ายการดูแลสหวิชาชีพ จำนวน 11 ข้อ 3) ผู้เชี่ยวชาญการพยาบาลเฉพาะทางผู้ป่วยวิกฤตหัวใจและหลอดเลือด จำนวน 10 ข้อ 4) ผู้พิทักษ์สิทธิและให้การพยาบาลแบบองค์รวม จำนวน 10 ข้อ 5) ผู้จัดการสถานการณ์ฉุกเฉิน จำนวน 9 ข้อ 6) ผู้ส่งเสริมการใช้หลักฐานเชิงประจักษ์และนวัตกรรม จำนวน 9 ข้อ 7) ผู้ประยุกต์ใช้เทคโนโลยีและสารสนเทศ จำนวน 8 …

การศึกษานี้เป็นการวิจัยเชิงพรรณนา มีวัตถุประสงค์เพื่อศึกษาบทบาทพยาบาลปริศัลยกรรมในการผ่าตัดที่ใช้หุ่นยนต์ โดยใช้เทคนิคการวิจัยแบบเดลฟาย เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย แบบสอบถามปลายเปิด และแบบสอบถามประมาณค่า 5 ระดับ การดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ ขั้นตอนที่ 1 สัมภาษณ์ผู้เชี่ยวชาญเกี่ยวกับบทบาทพยาบาล ปริศัลยกรรมในการผ่าตัดที่ใช้หุ่นยนต์ ขั้นตอนที่ 2 นำข้อมูลที่ได้จากการสัมภาษณ์มาวิเคราะห์เนื้อหาแล้วสร้างเป็นแบบสอบถามให้ผู้เชี่ยวชาญประมาณค่าบทบาทพยาบาลปริศัลยกรรมในการผ่าตัดที่ใช้หุ่นยนต์ และขั้นตอนที่ 3 นำข้อมูลที่ได้จากรอบที่ 2 มาคำนวณหาค่ามัธยฐาน ค่าพิสัยระหว่างควอไทล์ และส่งแบบสอบถามให้ผู้เชี่ยวชาญกลุ่มเดิมยืนยันคำตอบ จากนั้นนำข้อมูลที่ได้มาคำนวณค่ามัธยฐาน และค่าพิสัยระหว่างควอไทล์อีกครั้ง เพื่อสรุปเป็นบทบาทพยาบาลปริศัลยกรรมในการผ่าตัดที่ใช้หุ่นยนต์ ผลการวิจัยพบว่า บทบาทพยาบาลปริศัลยกรรมในการผ่าตัดที่ใช้หุ่นยนต์ ประกอบด้วย 6 บทบาท ดังนี้ 1) ด้านผู้ปฏิบัติการพยาบาลปริศัลยกรรมในการผ่าตัดที่ใช้หุ่นยนต์ จำนวน 31 ข้อ 2) ด้านผู้จัดการทรัพยากร จำนวน 9 ข้อ 3) ด้านผู้พัฒนาคุณภาพและงานวิจัย จำนวน 17 ข้อ 4) ด้านผู้ประสานงานและการสื่อสาร จำนวน 10 ข้อ 5) ด้านผู้จัดการความปลอดภัยและบริหารความเสี่ยง จำนวน 7 ข้อ และ 6) ด้านผู้สอนและให้คำปรึกษา จำนวน 5 ข้อ

การวิจัยเชิงพรรณนานี้ มีวัตถุประสงค์เพื่อศึกษาสมรรถนะตามบันไดวิชาชีพของพยาบาลหน่วยงานไฟไหม้น้ำร้อนลวก โรงพยาบาลตติยภูมิ โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลคือ กลุ่มผู้เชี่ยวชาญจำนวน 20 คน วิธีดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ 1) สัมภาษณ์ผู้เชี่ยวชาญเกี่ยวกับสมรรถนะตามบันไดวิชาชีพของพยาบาลหน่วยงานไฟไหม้น้ำร้อนลวก 2) วิเคราะห์ข้อมูล และสร้างแบบสอบถามเพื่อให้ผู้เชี่ยวชาญประเมินระดับความสำคัญ และ3) คำนวณค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์เพื่อสรุปผล ผลการวิจัยพบว่า สมรรถนะของพยาบาลหน่วยงานไฟไหม้น้ำร้อนลวก โรงพยาบาลตติยภูมิ ประกอบด้วย 9 ด้าน รวมจำนวน 143 ข้อรายการสมรรถนะ ดังนี้ 1) ด้านการพยาบาลผู้ป่วยแผลไฟไหม้น้ำร้อนลวกในภาวะวิกฤต จำนวน 24 ข้อ 2) ด้านการควบคุมการติดเชื้อและภาวะแทรกซ้อน จำนวน 14 ข้อ 3) ด้านการดูแลบาดแผลไฟไหม้น้ำร้อนลวก จำนวน 18 ข้อ 4) ด้านการบริหารจัดการความปวดจำนวน 17 ข้อ 5) ด้านการฟื้นฟูสภาพร่างกายและจิตใจ จำนวน 12 ข้อ 6) ด้านการจัดการโภชนาการ จำนวน 9 ข้อ 7) ด้านการวางแผนจำหน่ายและการดูแลระยะท้าย จำนวน 17 ข้อ 8) การสื่อสารและการทำงานร่วมกับทีมสหวิชาชีพ จำนวน 17 ข้อ และ9) การใช้เทคโนโลยีและอุปกรณ์ทางการแพทย์ จำนวน 15 ข้อ ซึ่งพยาบาลที่ปฏิบัติงานในหอผู้ป่วยแต่ละระดับมีสมรรถนะทั้ง 9 ด้าน แต่สมรรถนะย่อยมีความแตกต่างกัน การศึกษานี้สามารถใช้เป็นแนวทางในการประเมินสมรรถนะและมอบหมายงานตามความสามารถของพยาบาลแต่ละระดับ

Background: Moral distress, burnout, and compassion fatigue in acute care nurses has increased significantly since the Covid-19 pandemic and has been associated with high levels of nursing turnover and increased costs to healthcare systems. Burnout among advanced practice providers (APPs) remains a significant concern nationally and within the University of Kentucky Healthcare system, with post-COVID evidence showing persistently elevated stress and emotional exhaustion. This quality improvement (QI) project implemented and evaluated a brief, app-based mindfulness intervention using the Headspace “Learn the Basics of Meditation” program for work-related stress, job satisfaction, mindfulness, and retention intentions among APPs. A one-group pre-post design was implemented, with 45 APPs completing the pre-intervention survey and four completing both the pre- and post-intervention surveys. A newly established pediatric outpatient infusion center in Los Angeles does not have immediate access to a Code Blue Team or a Rapid Response Team, and instead relies on the Staff Assist protocol to manage medical emergencies. Via direct interviews, registered nurses at the infusion center report feeling underprepared and lacking confidence to respond to medical emergencies that must be escalated to code blue. Diabetes affects 37 million people annually in the United States, with direct medical costs exceeding $237 billion per year (American Diabetes Association, 2018; Xu et al., 2018). Complications account for up to 64% of diabetes-related expenses, emphasizing the importance of glycemic stability in preventing costly and debilitating outcomes (Dieleman et al., 2016).
