Hospital-acquired delirium affects older adult patients, across the healthcare continuum. The incidence in frail older adults is high, costly, and often deemed preventable. Hospital-acquired delirium is associated with a multitude of short- and long-term complications, resulting in an increased rate of morbidity and mortality. The challenges of early recognition and management are prevalent in healthcare literature. Catheter-associated urinary tract infections (CAUTIs) significantly contribute to morbidity, prolonged hospitalization, and elevated healthcare costs, underscoring the crucial need for prevention to ensure patient safety. Despite ongoing mitigation efforts, CAUTI prevalence remained a concern in our facility at the time of this study. Catheter insertion and maintenance bundles, a recognized framework utilizing teamwork, collaboration, and focused interventions, were implemented to reduce CAUTIs and improve patient safety. This study evaluated the effectiveness of these bundles in reducing CAUTIs and catheter utilization to identify best practices for enhanced catheter management and care quality.

Older adults taking multiple medications are at high risk for experiencing adverse drug events that result in hospitalization. Nurses are one of the first points of contact for patients and serve as patient educators. Many factors influence medication adherence, such as the patient-provider relationship and patient self-efficacy. This study aims to assess what impact patient self-efficacy has on the interaction between the patient-provider relationship and medication adherence.

This thesis investigates the potential for improving the Eden Alternative Model, a person-centered approach to elder care that emphasizes creating a homelike environment and meaningful relationships, using WELL Standards, a building certification system focused on enhancing human health and well-being through improvements in environmental quality. Within the healthcare industry, the geriatric population holds a unique set of patient-care needs due to a myriad of physical and cognitive challenges. The presentation of these challenges can negatively impact the quality of a patient’s life and overall independence. One specific challenge providers must account for when providing care to geriatric patients is a decreased range of mobility as age increases. Delirium is an acute disorder yielding cognitive deficits, such as disruption in attention and awareness. It impacts approximately 50% of individuals over the age of 65 who are admitted to the hospital and is prevalent with patients in the intensive care unit (ICU). ICU delirium is associated with adverse outcomes, including higher morbidity, prolonged hospital stays, and chronic neurocognitive deficits. This research looks into the implementation of music therapy for dementia patients in several different clinical settings. The framework for the research takes a look at person-centered nursing care which focuses on the patient as an individual with specific wants and needs.
After reviewing the research articles, aspects of changing patient care like adding implementations of personal speakers and playing music in group settings were brought up as recommendations for facilities caring for dementia patients.

Background: Advance Directives (ADs) support patient-centered care by documenting preferences for future medical treatment. While the Centers for Medicare & Medicaid Services (CMS) incentivizes AD documentation through billing codes, national adoption remains low.
Local Problem: A rural Northern Illinois primary care practice had electronic referral capabilities for ACP; however, referrals remained low and inconsistent during Medicare Annual Wellness Visits (AWVs), limiting opportunities for AD completion.

Implementation of the Nurse-Driven Palliative Care Referral Protocol
Background and Purpose: Palliative care enhances quality of life, symptom management, and patient–family communication for individuals with serious illness. Despite strong evidence and national recommendations for early integration, referrals remain inconsistent and often occur late. At Advocate Sherman Hospital, only 17.9% of eligible oncology inpatients received palliative care consults. The absence of standardized referral criteria, reliance on provider discretion, and misconceptions equating palliative care with hospice contributed to delayed or missed consults. Background: Older Spanish-speaking Hispanics are a rapidly growing population in the United States that face disproportionally high rates of diabetes and inadequate health literacy. Familism is valued among Hispanics and caregivers may play an important role in overcoming the social determinants of health facing this vulnerable population. Methods: This descriptive correlation study sought to explore correlations between the caregiver’s health literacy and the patient’s control of diabetes. Findings: Mixed effects model found a significant correlation between the caregiver’s print literacy and the patient’s hemoglobin A1c. Background: Despite the ease of receiving OCT, patients’ OCT adherence is low. With this low prevalence rate, patients are not likely obtaining the full life-promoting effects of OCT. Since medication adherence is often related to lived experiences and standardized medication adherence instruments may not capture these experiences completely, the purpose of this study was to explore how the lived experiences of older adults with cancer influence their adherence to OCT.

Introduction: The lack of education and use of the Cognitive Assessment and Care Planning Services (CACPS) visit creates a gap in care for Medicare patients over the age of 65 with cognitive impairment.
Methods: Recommendations were created from a robust collection of graded, evidence-based literature. The recommendations were presented to a set of stakeholders who rated each recommendation on a Likert scale from 1 (poor) to 4 (excellent).
Results: The twelve stakeholders agreed with the recommendations to support the education and use of the CACPS visit for Medicare patients 65 and older.

Clostridium Difficle Infection (CDI) is a significant healthcare threat due to its ability to be transmitted rapidly in healthcare settings. Patient engagement and participation in infection prevention initiatives at admission will lead to early implementation of isolation precautions, diagnosis of infectious pathogens, and treatment. A quality improvement project was implemented at a south-central Texas acute care hospital to engage alert and oriented adult patients, using a bundled approach to screen for the early detection of CDI within 48 hours of hospital admission, thereby reaching the goal of reducing hospital-acquired CDIs. This study investigated the lived experiences of individuals living with HIV (PLWH) in the Dallas-Fort Worth (DFW) area of Texas during the COVID-19 pandemic, using an Interpretative Phenomenological Analysis (IPA) framework. The research examined the intersection of health, social dynamics, and psychological resilience among PLWH, who faced compounded vulnerabilities due to systemic inequities and the dual public health crises of HIV and COVID-19. Through semi-structured interviews with ten participants from diverse backgrounds, the study captured the profound impacts of disrupted healthcare systems, heightened stigma, and social isolation on this marginalized population. Loneliness among older adults is a significant public health concern associated with adverse physical, emotional, and cognitive outcomes. Despite widespread recognition of the issue, effective interventions remain limited in clinical practice. This Doctor of Nursing Practice (DNP) project implemented and evaluated a Health Promotion Home Visit (HPHV) intervention designed to reduce loneliness in community-dwelling adults aged 65 and older. The purpose of this research proposal is to explore the impact of early mental health screenings, comprehensive education, and support in alleviating caregiver burden and anxiety among hospice immigrant family caregivers.
Methods
This research proposal will evaluate the effectiveness of caregiver education and mental health screenings. Baseline data will be collected using the Zarit Burden Scale and the State-Trait Anxiety Inventory (STAI). After interventions are implemented, a post survey will be conducted to evaluate the effectiveness.

Background: Traumatic injuries in the geriatric (> 65) patient population can have devastating effects. Only a small percentage of these patients return to independent ambulation and living. As the population continues to age and life expectancy continues to increase, the incidence of traumatic injuries in geriatric patients will continue to increase. Geriatric friendly hospital wards in several disciplines have been shown to reduce length of stay and improve patient outcomes. การศึกษาครั้งนี้เป็นการวิจัยแบบบรรยายเชิงทำนาย เพื่อศึกษาความสามารถของปัจจัยคัดสรรในการทำนายพฤติกรรมการป้องกันวัณโรคในผู้สัมผัสวัณโรคร่วมบ้าน โดยปัจจัย ประกอบด้วย 1) ความรู้เกี่ยวกับวัณโรค 2) ทัศนคติต่อวัณโรค 3) แรงจูงใจในการป้องกันวัณโรค 4) การรับรู้ความสามารถของตนเองในการป้องกันวัณโรค และ 5) การรับรู้อุปสรรคเกี่ยวกับการป้องกันวัณโรค กลุ่มตัวอย่างเป็นผู้สัมผัสวัณโรคร่วมบ้าน จำนวน 111 คน ซึ่งเลือกอย่างเจาะจงจากโรงพยาบาล 3 แห่งในเขตกรุงเทพมหานคร เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามชนิดตอบด้วยตนเอง วิเคราะห์ด้วยสถิติเชิงพรรณนาและการถดถอยพหุคูณ ผลการวิจัย พบว่า พฤติกรรมการป้องกันวัณโรคในผู้สัมผัสวัณโรคร่วมบ้านโดยรวม อยู่ในระดับดี (Mean = 44.2, SD = 6.5) ปัจจัยที่สามารถทำนายพฤติกรรมการป้องกันวัณโรคอย่างมีนัยสำคัญทางสถิติ ได้แก่ ทัศนคติต่อวัณโรค (b = .374, p<.05) การรับรู ้ความสามารถของตนเอง ในการป้องกันวัณโรค (b = .244, p <.05) และ การรับรู ้อุปสรรคเกี ่ยวกับการป้องกันวัณโรค (b = .166, p <.05) ปัจจัยดังกล่าวสามารถร่วมทำนายพฤติกรรมการป้องกันวัณโรคได้ร้อยละ 32.8 อย่างมีนัยสำคัญทางสถิติ (R2 = .328, p <.05) อย่างไรก็ตามในการศึกษานี้ ความรู ้เกี ่ยวกับวัณโรค และแรงจูงใจในการป้องกันวัณโรค ไม่สามารถทำนายพฤติกรรมการป้องกันวัณโรคในผู ้สัมผัสวัณ โรคร่วมบ้าน

การวิจัยแบบกึ่งทดลองนี้มีวัตถุประสงค์ เพื่อศึกษาผลของโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันต่อพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม เบาหวาน ความดันโลหิตสูง ไขมันในเลือด 1 โรคขึ้นไป ที่มีอายุตั้งแต่ 35 – 80 ปี คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 23 คน จับคู่ทั้งสองกลุ่มให้มีลักษณะคล้ายคลึงกันในเรื่องอายุ จำนวนโรคร่วม จำนวนกลุ่มยาที่ใช้รักษาโรค เปรียบเทียบข้อมูลพื้นฐานด้วยสถิติ Chi-square test และ Fisher’s Exact Test กลุ่มตัวอย่างทั้งสองกลุ่มมีคุณลักษณะพื้นฐานที่ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติในทุกตัวแปร (p > .05) กลุ่มควบคุมจะได้รับการพยาบาลปกติ ในขณะที่กลุ่มทดลองได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชัน เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารของผู้ป่วยที่มีภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม และแบบสอบถามประเมินความรอบรู้ด้านสุขภาพเกี่ยวกับพฤติกรรมการจัดการตนเองด้านการบริโภคอาหาร ทดสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน ได้ค่าดัชนีความตรงตามเนื้อหาเท่ากับ 0.92 และ 1.00 และค่าความเที่ยงเท่ากับ 0.91 และ 0.73 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิตทดสอบค่าที ผลการวิจัยสรุปได้ดังนี้ 1. คะแนนเฉลี่ยพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม กลุ่มทดลองหลังได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันสูงกว่าก่อนได้รับโปรแกรม ฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม ที่ได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง เพื่อศึกษาผลของโปรแกรมสนับสนุนการให้ข้อมูลแบบรูปธรรม-ปรนัยต่อความวิตกกังวลของสมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองครั้งแรกในห้องฉุกเฉิน โดยกลุ่มตัวอย่างในการศึกษาครั้งนี้คือ สมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองครั้งแรกที่นำส่งผู้ป่วยมารับบริการที่ห้องฉุกเฉินของโรงพยาบาลสรรพสิทธิประสงค์ แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 30 คน โดยคัดเลือกตามแบบเฉพาะเจาะจงตามเกณฑ์ที่กำหนด โปรแกรมรูปธรรม-ปรนัย และแบบสอบถาม ผ่านผู้ทรงคุณวุฒิ 5 ท่าน ได้ค่าความเที่ยง 0.84 โปรแกรมการสนับสนุนการให้ข้อมูลแบบรูปธรรม–ปรนัยต่อความวิตกกังวล มีเครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ประกอบด้วย ข้อมูลทั่วไปของผู้ป่วยโรคหลอดเลือดสมอง และแบบประเมินความวิตกกังวลขณะเผชิญ (The State Anxiety Inventory (STAI) Form Y) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติทดสอบ Independent t-test และสถิติ paired t-test ผลการวิจัย: ผลการวิจัยพบว่าสมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองที่ห้องฉุกเฉิน ในกลุ่มทดลอง ภายหลังได้รับโปรแกรมคะแนนความวิตกกังวลตํ่ากว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติ (p < .05) และคะแนนความวิตกกังวลของสมาชิกครอบครัวของผู้ป่วยในกลุ่มทดลองภายหลังได้รับโปรแกรมตํ่ากว่าในสมาชิกครอบครัวของผู้ป่วยได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติ (p < .05)

Chronic neck and shoulder myofascial pain has long been challenging patients and medical professionals due to its limited response to traditional therapies on top of high recurrence. Physical therapy, oral analgesics, and trigger point injections have only provided partial or short-term relief. As a result, botulinum toxin type A injections have been explored as an alternative treatment. Previous studies have shown promising short-term outcomes, but concerns regarding long-term safety, cost-effectiveness, and potential adverse effects, particularly in older adults, have remained unresolved. การวิจัยครั้งนี้เป็นการศึกษาเชิงกึ่งทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบการฟื้นตัวด้านการทำหน้าที่ของลำไส้หลังผ่าตัดในผู้ป่วยมะเร็งลำไส้ใหญ่และทวารหนักที่ได้รับการผ่าตัดแบบเปิดช่องท้องระหว่างกลุ่มที่ได้รับโปรแกรมการนวดกดจุดร่วมกับการจัดการอาการ กับกลุ่มที่ได้รับการพยาบาลตามปกติ ที่เข้ารับการรักษาในหอผู้ป่วยวิกฤต ในวันที่ 1 ถึง 5 หลังผ่าตัด กลุ่มตัวอย่างคือผู้ป่วยมะเร็งลำไส้ใหญ่ และทวารหนักที่ได้รับการผ่าตัดแบบเปิดช่องท้อง เข้ารับการรักษาตัวที่หอผู้ป่วยวิกฤตศัลยกรรมในโรงพยาบาลธรรมศาสตร์เฉลิมพระเกียรติ ทั้งเพศชายและหญิงที่มีอายุมากกว่า 20 ปี จำนวน 44 ราย แบ่งเป็นกลุ่มควบคุม 22 ราย และกลุ่มทดลอง 22 ราย จับคู่ ให้กลุ่มตัวอย่างทั้งสองกลุ่มมีลักษณะใกล้เคียงกันในเรื่องของเพศ ดัชนีมวลกาย และระดับโพแทสเซียมในเลือดหลังผ่าตัด โดยกลุ่มทดลองได้รับโปรแกรมการจัดการอาการร่วมกับการนวดกดจุด โดยการเคลื่อนไหวร่างกายวันละ 1 ครั้ง ครั้งละ 10-15 นาที ร่วมกับการนวดกดจุดบริเวณจุด Zusanli (ST-36) วันละ 2 ครั้ง ครั้งละ 10 นาที ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือวิจัยประกอบด้วยโปรแกรมการนวดกดจุดร่วมกับการจัดการอาการที่ออกแบบเพื่อส่งเสริมการฟื้นตัวด้านการทำหน้าที่ของลำไส้หลังผ่าตัด และแบบประเมินการฟื้นตัวด้านการทำหน้าที่ของลำไส้ ซึ่งผ่านการทดสอบความเที่ยง โดยมีค่าสัมประสิทธิ์แอลฟาครอนบาคเท่ากับ 0.87 การวิเคราะห์ข้อมูลใช้สถิติเชิงพรรณนา Mann–Whitney U test และ Friedman test ผลการศึกษาพบว่า คะแนนเฉลี่ยการฟื้นตัวด้านการทำหน้าที่ของลำไส้ในช่วงวันที่ 1 ถึง 5 หลังผ่าตัดของผู้ป่วยที่ได้รับโปรแกรมการจัดการอาการร่วมกับการนวดกดจุดสูงกว่าผู้ป่วยที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 (p < 0.05) (x² = 82.13, p < 0.001 และ x² = 5.62, p < 0.001 ตามลำดับ)

การวิจัยแบบกึ่งทดลองครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมส่งเสริมความรอบรู้ด้านสุขภาพผ่านแอปพลิเคชันต่อพฤติกรรมการบริโภคอาหารของผู้ป่วยหลังผ่าตัดนิ่วในถุงน้ำดี กลุ่มตัวอย่างเป็นผู้ป่วยหลังผ่าตัดนิ่วในถุงน้ำดี เพศชายและเพศหญิง อายุ 20 - 75 ปี คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงตามคุณสมบัติที่กำหนด โดยจับคู่ให้มีลักษณะคล้ายคลึงกันในเรื่องเพศ อายุ ระดับการศึกษา แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 30 คน เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามพฤติกรรมการบริโภคอาหารหลังผ่าตัดนิ่วในถุงน้ำดี และแบบประเมินความรอบรู้ด้านสุขภาพพฤติกรรมการบริโภคอาหารของผู้ป่วยหลังผ่าตัดนิ่วในถุงน้ำดี ได้ค่าดัชนีความตรงตามเนื้อหาเท่ากับ .94, 1 และ 1 และค่าความเชื่อมั่นโดยแบบสอบถามพฤติกรรมการบริโภคอาหารหลังผ่าตัดนิ่วในถุงน้ำดี และแบบประเมินความรอบรู้ด้านสุขภาพพฤติกรรมการบริโภคอาหารหลังผ่าตัดนิ่วในถุงน้ำดี .744 และ .719 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและเปรียบเทียบความแตกต่างด้วยสถิติทดสอบที ผลการวิจัยสรุปได้ดังนี้ 1. พฤติกรรมการบริโภคอาหารของผู้ป่วยหลังผ่าตัดนิ่วในถุงน้ำดีในกลุ่มทดลองหลังได้รับโปรแกรมส่งเสริมความรอบรู้ด้านสุขภาพผ่านแอปพลิเคชันสูงกว่าก่อนได้รับโปรแกรมส่งเสริมความรอบรู้ด้านสุขภาพผ่านแอปพลิเคชันอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. พฤติกรรมการบริโภคอาหารของผู้ป่วยหลังผ่าตัดนิ่วในถุงน้ำดีที่ได้รับโปรแกรมส่งเสริมความรอบรู้ด้านสุขภาพผ่านแอปพลิเคชันสูงกว่ากลุ่มที่ได้รับการพยาบาลปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05
