วิจัยนี้เป็นวิจัยแบบกึ่งทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบคุณภาพชีวิตด้านสุขภาพในผู้ป่วยมะเร็งท่อน้ำดีระยะลุกลาม ก่อนได้รับโปรแกรมและภายหลังได้รับโปรแกรมในสัปดาห์ที่ 1 สัปดาห์ที่ 2 และสัปดาห์ที่ 6 กลุ่มตัวอย่างคือ ผู้ป่วยมะเร็งท่อน้ำดีระยะลุกลาม ซึ่งมีคุณสมบัติตามเกณฑ์ที่กำหนดจำนวน 24 คน ได้รับโปรแกรมเป็นเวลา 2 สัปดาห์ เครื่องมือในการวิจัย คือ โปรแกรมการดูแลวิถีพุทธ และ แบบประเมิน Functional Assessment of Chronic Illness Therapy–Palliative Care (FACIT-Pal) ฉบับภาษาไทย วิเคราะห์ค่าดัชนีความตรงตามเนื้อหาของแบบประเมิน FACIT-Pal ได้ 0.89 และค่าสัมประสิทธิ์แอลฟาครอนบาคได้ 0.94 วิเคราะห์ผลด้วยสถิติการวิเคราะห์ความแปรปรวนทางเดียวแบบวัดซ้ำ (One-way repeated ANOVA) ผลวิจัยสรุปได้ดังนี้ คุณภาพชีวิตด้านสุขภาพโดยรวมของผู้ป่วยมะเร็งท่อน้ำดีระยะลุกลามหลังได้รับโปรแกรมสัปดาห์ที่ 1 และสัปดาห์ที่ 2 มากกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่ .05 เมื่อจำแนกรายมิติพบว่ามิติความผาสุกด้านจิตอารมณ์หลังได้รับโปรแกรม สัปดาห์ที่1 สัปดาห์ที่ 2 และ สัปดาห์ที่ 6 มากกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่.05 สรุปได้ว่า โปรแกรมการดูแลวิถีพุทธสามารถส่งเสริมคุณภาพชีวิตด้านสุขภาพของผู้ป่วยมะเร็งท่อน้ำดีระยะลุกลามได้ แม้ว่าในสัปดาห์ที่ 6 คุณภาพชีวิตด้านสุขภาพมีแนวโน้มลดลง แต่ผลลัพธ์ของโปรแกรมยังคงสามารถส่งเสริมความผาสุกด้านจิตอารมณ์ของผู้ป่วยมะเร็งท่อน้ำดีระยะลุกลามได้ต่อเนื่อง

การศึกษาครั้งนี้เป็นการวิจัยเชิงหาความสัมพันธ์ เพื่อศึกษาคุณภาพการนอนหลับในผู้ป่วยโรคหัวใจขาดเลือดเฉียบพลัน และศึกษาความสัมพันธ์ระหว่างปัจจัยคัดสรร ได้แก่ ความรุนแรงของอาการเจ็บหน้าอก ภาวะซึมเศร้า ความวิตกกังวล สถานะการสูบบุหรี่ อาการเหนื่อยล้า กับคุณภาพการนอนหลับ กลุ่มตัวอย่าง คือ ผู้ป่วยโรคหัวใจขาดเลือดเฉียบพลันอายุ 18 ปีขึ้นไป จำนวน 152 คน ซึ่งคัดเลือกแบบเฉพาะเจาะจงจากโรงพยาบาลสองแห่งในกรุงเทพมหานคร ตามเกณฑ์การคัดเข้า เครื่องมือที่ใช้ ได้แก่ แบบประเมินความรุนแรงของอาการเจ็บหน้าอก แบบสอบถามภาวะซึมเศร้า แบบประเมินความวิตกกังวล แบบประเมินอาการเหนื่อยล้า และแบบวัดคุณภาพการนอนหลับ ซึ่งแบบสอบถามมีค่าสัมประสิทธิ์สหสัมพันธ์แอลฟาครอนบาค มากกว่าเท่ากับ 0.70 วิเคราะห์ข้อมูลด้วยสถิติเชิงบรรยายและทดสอบความสัมพันธ์ด้วยสถิติสหสัมพันธ์เพียร์สัน ผลการวิจัยพบว่า กลุ่มตัวอย่างส่วนใหญ่ (ร้อยละ 84.9) มีคุณภาพการนอนหลับดี ความวิตกกังวล สถานะการสูบบุหรี่ อาการเหนื่อยล้า และ ภาวะซึมเศร้ามีความสัมพันธ์ทางลบกับคุณภาพการนอนหลับ (r= -.197, -.175, -.440, -.528; p< .05) ส่วนความรุนแรงของอาการเจ็บหน้าอกไม่มีความสัมพันธ์กับคุณภาพการนอนหลับ

การศึกษาครั้งนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการจัดการตนเองที่เน้นการออกกำลังกายแบบแกว่งแขนต่อระดับฮีโมโกลบินเอวันซี ของผู้ป่วยโรคเบาหวานเข้าจอประสาทตา กลุ่มตัวอย่างได้แก่ผู้ป่วยโรคเบาหวานเข้าจอประสาทตาที่มารับการรักษาที่แผนกผู้ป่วยนอกจักษุโรงพยาบาลเมตตาประชารักษ์ (วัดไร่ขิง) จังหวัดนครปฐม เป็นผู้ป่วยโรคเบาหวานเข้าจอประสาทตามีอายุมากกว่าหรือเท่ากับ 30 ปีขึ้นไป ได้รับการวินิจฉัยว่าเป็นโรคเบาหวานเข้าจอประสาทตา เลือกตัวอย่างแบบเฉพาะเจาะจง ตามเกณฑ์ที่กำหนด จำนวน 44 คน แบ่งเป็นกลุ่มทดลอง 22 คนและกลุ่มควบคุม 22 คน จับคู่โดยคำนึงถึง ระยะเวลาการเป็นโรคเบาหวาน ชนิดของยาที่ใช้ และค่าดัชนีมวลกาย กลุ่มทดลองได้รับโปรแกรมการจัดการตนเองที่เน้นการออกกำลังกายแบบแกว่งแขน และกลุ่มควบคุมได้รับการพยาบาลตามปกติ ระยะเวลา 12 สัปดาห์ เครื่องมือที่ใช้ในการวิจัย ได้แก่ โปรแกรมการจัดการตนเองที่เน้นการออกกำลังกายแบบแกว่งแขน เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล คือแบบบันทึกข้อมูลส่วนบุคคล และเครื่องตรวจระดับฮีโมโกลบินเอวันซี วิเคราะห์ข้อมูลโดยใช้ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติการทดสอบที ผลการวิจัยพบว่า 1..ค่าฮีโมโกลบินเอวันซีของผู้ป่วยโรคเบาหวานเข้าจอประสาทตาในกลุ่มทดลองหลังได้รับโปรแกรม ( x = 8.30 ± 1.55 ) มีค่าต่ำกว่าก่อนการทดลอง ( x = 9.14 ± 1.46) อย่างมีนัยสำคัญทางสถิติ ที่ระดับ .05 (t= 3.23, p <05) 2. ผลต่างของฮีโมโกลบินเอวันซีก่อนและหลังเข้าร่วมวิจัยของกลุ่มทดลอง ( d = 0.84 ± 1.21) มากกว่ากลุ่มควบคุม ( d = 0.13 ± 0.85) อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (t= 2.24 , p < .05 )

การวิจัยนี้เป็นการวิจัยกึ่งทดลอง เพื่อศึกษาผลของโปรแกรมสนับสนุนข้อมูลผ่านไลน์แอพพลิเคชั่นร่วมกับการบริหารกาย-จิตแบบชี่กงวิถีไทยต่อความวิตกกังวลก่อนการผ่าตัดในผู้ป่วยมะเร็งเต้านม กลุ่มตัวอย่างคือผู้ป่วยมะเร็งเต้านมที่ได้รับการนัดหมายเพื่อผ่าตัด Mastectomy และ Breast Conserving Surgery ในโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย คัดเลือกแบบเฉพาะเจาะจงคุณสมบัติตามที่กำหนด แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 22 คน ได้รับการจับคู่ด้วย อายุ วิธีการผ่าตัดและการได้รับยาเคมีบำบัดก่อนผ่าตัด กลุ่มควบคุมได้รับการพยาบาลตามปกติและกลุ่มทดลองได้รับโปรแกรมสนับสนุนข้อมูลผ่านไลน์แอพพลิเคชั่นร่วมกับการบริหารกาย-จิตแบบชี่กงวิถีไทยเป็นเวลา 2 สัปดาห์ก่อนผ่าตัด เครื่องมือที่ใช้ในการวิจัยมี 3 ชุด คือ 1) เครื่องมือเก็บรวบรวมข้อมูล ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล, แบบประเมินความวิตกกังวลขณะเผชิญ STAI Form Y-I 2) เครื่องมือทดลอง ได้แก่ โปรแกรมสนับสนุนข้อมูลผ่านไลน์แอพพลิเคชั่นร่วมกับการบริหารกาย-จิตแบบชี่กงวิถีไทย 3) เครื่องมือกำกับการทดลอง ได้แก่ แบบสอบถามความรู้เรื่องโรคมะเร็งเต้านมและการผ่าตัด, แบบสอบถามความรู้เรื่องการเตรียมสุขภาพก่อนการผ่าตัดและประโยชน์ของชี่กง และแบบบันทึกการทำชี่กง ตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ 5 ท่าน ทดสอบค่าความเที่ยงของแบบประเมินความวิตกกังวลขณะเผชิญโดยใช้สูตรสัมประสิทธิ์แอลฟาคอนบาค ได้ค่าความเที่ยงเท่ากับ 0.88 วิเคราะห์ข้อมูลโดยใช้สถิติร้อยละ ค่าเฉลี่ย และสถิติทดสอบค่าที (t-test) ผลการวิจัยสรุปได้ดังนี้ 1. ความวิตกกังวลก่อนผ่าตัดของผู้ป่วยมะเร็งเต้านมในกลุ่มทดลองภายหลังได้รับโปรแกรมต่ำกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. ความวิตกกังวลก่อนผ่าตัดภายหลังการทดลองของผู้ป่วยมะเร็งเต้านมในกลุ่มทดลองต่ำกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

Background: Filial piety plays a vital role in combating loneliness among older people. Older adults have a high tendency to feel lonely when affection toward and from their children decreases. This study aimed to determine the association between filial piety expectations and loneliness among older people in Kuantan, Pahang.
Methods: A cross-sectional study was conducted on 198 older people in Kuantan, Pahang. Filial piety expectation and loneliness were measured by the Filial Piety Expectation Scale and UCLA (University of California, Los Angeles) Loneliness Scale questionnaire. Background: With the increasing needs of the aging population and the increasing prevalence of non-communicable diseases, assessment of the equity and accessibility of health care among the elderly has become important.
Methods: A cross-sectional descriptive study was conducted from December 2018 to December 2019 among 225 elderly to describe the factors influencing health service utilization. Multistage sampling was applied, and structured questionnaire was used to interview the elderly. Chi-squared test and multiple logistic regression were performed in data analysis.

The purpose of this dissertation was to explore the strength of the relationship between unavoidable Hospital Acquired Pressure Injury (HAPI) and increased risk of one-year mortality for patients age 55 and older who were admitted to a hospital from 2015 to 2018. A quantitative, retrospective case-control study was designed to compare demographics, comorbid conditions, pressure injury risk, and living situation of those patients that developed unavoidable HAPI and hospitalized adults with similar physiological burdens that remained injury-free. Abstract
Background. Ineffective patient handoff can result in poor nurse communication, increasing the likelihood of adverse events including medication and documentation errors.
Context/Problem. In one 20-bed ICU unit in a northern California community hospital, 48 patient handoffs were observed over 2 weeks. Only 29% occurred at the patient’s bedside; 39.5% used a standardized handoff tool; and 54% included the patient and/or family. These findings indicate significant quality gaps in the unit’s ICU patient handoff processes.

The novel SARS-CoV-2 strain (COVID-19) was identified in Wuhan, China in December 2019 (Eddy et. al, 2020). The resulting US outbreak has affected the Kaiser Permanente (KP) Oakland health care system. With the threat of COVID-19 highest among immunosuppressed and elderly clients, KP Oakland home health and hospice (HH/HO) department is rapidly mobilizing to provide community and in-home testing for eligible clients. Problem: Catheter-associated urinary tract infection (CAUTI) remains the leading cause of hospital-acquired infection (HAI) despite being preventable. CAUTI increases the length of stay, morbidity, mortality, readmissions, and costs. There is also increasing antimicrobial resistance in pathogens causing CAUTI. Thus, reducing HAIs such as CAUTI should be a priority for every institution.
Context: The microsystem for this CAUTI quality improvement project is a 24-bed adult medical-telemetry unit in an acute care teaching hospital with excessive CAUTI. This paper summarizes the quality improvement falls prevention project on a Medical-Surgical unit with an Enhanced Fall Algorithm incorporated in the Electronic Medical Record conducted by two Clinical Nurse Leader students of the University of San Francisco.
This project addresses the patient falls events in a San Francisco-Bay Area Hospital on a Medical-Surgical unit. The purposed evidence-based practice (EBP) project focuses on a nursing intervention (team huddles) to increase safety and reduce inpatient fall rates. Negative outcomes to consider from an inpatient fall are harm and injury to the patient along with increased health care cost for the patient as well as hospital facility. The purpose was to examine the effect of a self-care educational intervention on nursing student resilience and thus the potential for compassion fatigue, depersonalization, burnout, depression, and inadequate self-care. A one-group pretest-posttest research design was applied to a convenience sample of 104 nursing students near the end of their last semester in a baccalaureate nursing program. The measurements were demographics, a psychometric resilience scale, program evaluation, and reflection question. Physician-assisted suicide, or PAS, is end-of-life option in which patients self-administer physician-prescribed lethal medication to end their life. Nurses' own interpretations and understandings of the legalities, ethical dilemmas, and factors that influence their attitudes can be analyzed to determine the nurses’ role in the support or opposition of the practice. Although nurses do not have an active role in PAS, they play a pivotal role in patient advocacy and education. This paper analyzes the factors that contribute to nurses’ attitudes toward the practice, along with the benefits of education of this topic going forth.

Background and Objectives: Scholarly research has established the role of altruism in facilitating human cooperation and prosocial behaviors and highlighted its contribution to psychological well-being. Given the health significance of altruistic attitudes and orientations, we developed a valid and reliable measure of this construct that is suitable for use with older adults. Background: Chronic lower respiratory disease, including asthma, is the fourth leading cause of death in the U.S. Financially, asthma accounts for approximately 54 billion dollars in healthcare spending annually. The National Heart Lung and Blood Institute [NHLBI] established guidelines to reduce impairment and to reduce future asthma attacks.
Purpose: This Quality Improvement (QI) pilot implemented the Asthma Control Test (ACT) and the Asthma Action Plan (AAP) per NHLBI guidelines. This project aimed to increase provider use of the ACT and AAP to 50% in patients with a diagnosis of asthma over one month.

Objective: To explore the feasibility of Auricular Point Acupressure (APA) to self-manage pain among patients with Alzheimer’s disease and related disorders (ADRD) and their caregivers (patient-caregiver dyads). Methods: A two-phase study design was used. Phase 1 was designed to explore the experiences of the dyads in using APA to manage chronic pain. In Phase 2, we examined the revised intervention protocol to manage pain. All of the study activities were conducted at the participants’ homes. BACKGROUND: It is crucial that nursing homes have adequate personal protective equipment (PPE) and staff to protect residents and staff from COVID-19. Some states have taken actions to mitigate shortages of PPE and staffing in nursing homes, including creating dedicated long-term care (LTC) teams and supporting staffing capacity.
OBJECTIVE: To examine whether state actions and nursing home characteristics are associated with shortages of PPE and staffing.

Patients suffer unintentional harm from their medications. Many organizations have proposed that healthcare providers implement strategies to decrease medication related harm for patients. This study implemented a brown bag review along with medication safety education within a clinic that caters to the poor and uninsured. It was notable that the majority of the participants educational levels were equivalent to a high school graduate or beyond. Background: Every year in the United States, hundreds of thousands of patients fall in hospitals with 30 to 50 percent resulting in injury. In Texas, the fall rate in adult patients is 33.9 percent, and in one teaching hospital in South Texas, patient fall rates have been above the national benchmark for two years (2017-2019), despite increased use of sitters for patient safety and multiple fall prevention strategies. The annual direct care cost of all fall events in the United States for individuals more than 65 years old is about $34 billion.

Practice Problem:There was a report of a high rate of medication errors from inaccurate medication reconciliation during admission to a local hospital in South Texas. The medication error rate was 14.88% on 20 reviewed charts, and 85% of all evaluated charts contained at least one medication discrepancy.

People’s expectations of aging are shaped by various sources and experiences. These aging expectations subsequently influence a person’s overall health and well-being. I intended to gain a deeper understanding of the various sources by which Filipino older adults attribute and reshape negative and positive expectations regarding aging.
