Fibrinogen; A Predictor Of Injury Severity And Mortality Among Patients With Traumatic Brain Injury In Sub-Saharan Africa: A Cross-Sectional Observational Study, Michael Devereaux, Martha Sajatovic

Studies show that fibrinogen concentrations <2 g/L in patients with traumatic brain injury (TBI) is associated with increased mortality. However, little is known regarding fibrinogen levels and TBI severity as well as mortality in sub-Saharan Africa despite shouldering a high burden of TBI. We therefore set out to determine whether fibrinogen levels are associated with TBI severity and outcome. To determine the sensitivity and specificity of fibrinogen levels and the association with severity and mortality among TBI patients at Mulago Hospital. We prospectively enrolled 213 patients with TBI aged between 13 and 60 years of age and presenting within 24 hours of injury. Patients with preexisting coagulopathy, concurrent use of anticoagulant or antiplatelet agents, preexisting hepatic insufficiency, diabetes mellitus and who were pregnant were excluded. Fibrinogen levels were determined using the Clauss fibrinogen assay. Logistic regression analyses were conducted to identify the association between fibrinogen level and 7-day outcomes. Majority of the patients were male (88.7%) and nearly half were aged 30 or less (48.8%). Fibrinogen levels <2 g/L were observed in 35.1% of the study participants. The average time spent in the study was 3.7 ± 2.4 days. The sensitivity and specificity using fibrinogen <2 g/L was 56.5% and 72.9% respectively. Fibrinogen levels predict TBI severity with an AUC = 0.656 (95% CI 0.58-0.73: P = .000) Fibrinogen levels <2 g/L (hypofibrinogenemia) were independently associated with severe TBI. (Adjusted odds ratio 2.87 CI, 1.34-6.14: P = .007). Levels above 4.5 g/L were also independently associated with injury severity (adjusted odds ratio 2.89, CI 1.12-7.48: P < .05) Fibrinogen levels more than 4.5 g/L were independently associated with mortality (OR 4.5, CI; 1.47-13.61, P < .05). The fibrinogen level is a useful tool in predicting severity including mortality of TBI. Fibrinogen levels may be used as an additional tool to screen TBI patients for injury severity especially among patients with Glasgow coma scale scores of <14.

Problem: Depression is a prevailing global health issue and a typical complication of dialysis patients. This quality improvement project's purpose was to evaluate mindfulness on depression in dialysis patients, aged 18-93, who are on dialysis and participated in a mindfulness program over three months.

Methods: A descriptive, prospective, and retrospective design was used to assess depression pre- and post-implementation of a mindfulness telephone line over three months. Introduction: Healthcare provider burnout, an indicator of wellbeing, impacts patient safety, provider distress, and employee turnover. In this mixed methods, multi-site quality improvement study conductedpandemic, we assessed employee wellbeing in a large clinical department.

Methods: Wellbeing surveys were sent electronically to Department of Medicine clinicians, researchers, administrators, and staff from August-September 2019 assessing perceptions of Burnout, Emotional Thriving (ET), and Emotional Recovery (ER). Qualitative responses were reviewed for themes using mixed inductive-deductive analysis. The initial coding was done by small teams with consensus obtained through large group discussions. Cleft palate (CP) is one of the most common craniofacial birth defects; however, there are relatively few established genetic risk factors associated with its occurrence despite high heritability. Historically, CP has been studied as a single phenotype, although it manifests across a spectrum of defects involving the hard and/or soft palate. This motivated our team to implement changes in our residency program’s inpatient service. We approached resident wellness in a comprehensive manner. The changes we implemented at our site had crucial benefits for our inpatient teams. Background: Leukocytosis relates to infections, malignancies, stress response, and multiple intoxicants. Like other intoxicants, cannabis smoking can result in a persistent leukocytosis. Here we report a case of cannabis-induced leukocytosis.

Case Presentation: A 20-year-old Hispanic female with a history of anxiety disorder presented with 3-days of nausea, vomiting, and colicky abdominal pain. She endorsed heavy marijuana smoking as self-medication and compulsive hot showers for symptom relief. She was admitted for oral intake intolerance, concerning for cannabis hyperemesis syndrome. OBJECTIVES: Mexico has a rapidly aging population at risk for cognitive impairment. Social and leisure activities may protect against cognitive decline in older adults. The benefits of these behaviors may vary by patterns of cognitive impairment. The objectives of this study were to identify latent states of cognitive functioning, model the incidence of transitions between these states, and investigate how social and leisure activities were associated with state transitions over a 6-year period in Mexican adults aged 60 and older.

Many studies have reported that autism spectrum disorder (ASD) is associated with atypical structural and functional connectivity. However, we know relatively little about the development of these differences in infancy.

Methods

We used a high-density electroencephalogram (EEG) dataset pooled from two independent infant sibling cohorts, to characterize such neurodevelopmental deviations during the first years of life. EEG was recorded at 6 and 12 months of age in infants at typical (N = 92) or elevated likelihood for ASD (N = 90), determined by the presence of an older sibling with ASD. Mental health and suicide are significant issues in Alabama, a state that ranks 46th out of the 50 states and Washington D.C. in access to mental health services. Alabama’s suicide rate is 59% higher than its homicide rate of 8.3%. This crisis is particularly acute among Caucasian males aged 45-54, who account for 71% of suicides; this demographic is four times more likely to commit suicide than females and three times more likely than African Americans. The PROSPER initiative is a collaborative effort of Healthy Acadia and its regional healthcare and nonprofit partners designed to support pregnant women, mothers, and families with Substance Use Disorder (SUD). Utah's opioid crisis presents unique challenges as progress was hindered by the COVID-19 pandemic. This pilot study assessed the effectiveness of a brief webinar-based professional development program aimed at improving knowledge and confidence among health and behavioral healthcare professionals. Results indicated positive changes in participants' confidence and knowledge regarding opioid treatment options, misuse, and connecting individuals to resources.

The Relationship Between Organizational Culture And Working Parent Burnout, Ryan S. Monroe

The purpose of this non-experimental quantitative study was to explore working parent burnout (WPB) and the relationships between organizational culture and WPB. Data were collected from working parents living with children <18 years in the United States (N=284) and analyzed using descriptive and statistical techniques to answer the research questions. The study had excellent internal reliability and yielded several key findings. The perceptions of study participants’ WPB was statistically significant. Gender, ethnicity, work schedule, work shift, income level, and marital status had statistically significant effects on WPB. Organizational culture types reflected statistically significant response effects for perceptions of WPB, with two reflecting very large response effects (clan and hierarchy) and two reflecting large response effects (adhocracy market). The results suggest that WPB was significant upon sample population and varied by different demographic variables. In addition, WPB appears to be significantly related to organizational culture; however, WPB cannot be predicted by organizational culture type. This study is the first known research to look at the relationship between WPB and organizational culture. While the study yielded several results, further research is needed to look at WPB and organizational cultural factors that may impact burnout among working parents.

This condition causes endometrial-type tissue to grow outside of the uterus (Kalaitzopoulos et al., 2021). Women diagnosed with endometriosis often have difficulty participating in their daily responsibilities and leisure activities. In a study completed by Lozano-Lozano et al., 2021, 22.6% of women reported limitations in at least one activity of daily living and instrumental activity of daily living.
