INTRODUCTION: Shoulder arthroplasty systems are transitioning towards more modular prosthesis options with the goal of reducing complication profiles and increasing range of motion. Modularity may increase the potential for fretting, corrosion, and subsequent release of metal ions. These events have been associated with adverse reactions in hip and knee arthroplasty, but not in the setting of revision shoulder arthroplasty. The purpose of this study was to identify associations between implant designs, metallurgy, patient characteristics, and serum metal ion levels in patients undergoing revision shoulder arthroplasty. METHODS: 51 patients that underwent revision shoulder arthroplasty were retrospectively reviewed based on prospectively collected …

INTRODUCTION: A fully 3D printed prosthetic may offer numerous potential advantages including improved pore precision, overall porosity, topology optimization, spatial variability in stiffness, communication between pores, and strut size. However, there are numerous theoretical concerns surrounding inferior mechanical performance with fully 3D printed implants (e.g. potentially higher rates of fatigue, increased in vivo particle release, etc.) compared to conventionally fabricated ones. We are aware of no reports on the clinical performance of fully 3D printed, off-the-shelf, humeral prostheses. To validate fully 3D printed humeral designs, the purpose of this study was to provide minimum 2-year postoperative clinical outcomes and survivorship …

INTRODUCTION: Particle-induced osteolytic and chronic inflammatory effects of polyethylene (PE) wear are a source of failure after both anatomic total shoulder arthroplasty (ATSA) and reverse total shoulder arthroplasty (RTSA). Historically, PE wear has rarely been reported as a cause of revision in the setting of prior shoulder arthroplasty, unlike hip and knee counterparts. The goal of this study was to analyze and present the clinical, radiographic, and intra-operative findings in a consecutive series of patients who presented after shoulder arthroplasty with evidence of PE wear. METHODS: This was a retrospective review of prospectively collected data on a consecutive series of …

INTRODUCTION: Among surgical interventions for proximal humerus fractures, RSA has been shown to have superior or noninferior outcomes to hemiarthroplasty or open reduction internal fixation. Cemented humeral fixation in RSA has been the traditional treatment method for proximal humerus fractures, however, there is a growing interest in uncemented humeral fixation. Both cemented and cementless RSA are currently used to treat proximal humerus fractures with few studies comparing postoperative outcomes of these 2 techniques. The aim of this study was to compare postoperative outcomes between cemented and uncemented humeral fixation following primary reverse total shoulder arthroplasty (RSA) for proximal humerus fractures. …

Evaluating The Effectiveness Of Cryoneurolysis As A Preoperative Intervention In Total Knee Arthroplasty: A Literature Review, Paul Bonilla, Anesu K. Murambadoro, Juan Pablo Flanagan, Victoria Elizondo, Aamir Ahmad
Research Symposium
Background: Total knee arthroplasty (TKA) is a highly effective treatment for knee osteoarthritis, but significant postoperative pain often necessitates opioid use. Over 70% of patients use opioid drugs 3 weeks after TKA and up to 15% of TKA patients develop opioid dependence, increasing the risk of adverse effects, infections, and contributes to costly revision surgeries. Cryoneurolysis, a minimally invasive technique involving freezing peripheral nerves, has gained attention as a promising preoperative intervention. This study explores the potential of cryoneurolysis as a preoperative intervention to alleviate pain, reduce opioid dependence, and improve functionality.
Methods: A literature search was conducted using the …

Metallosis And An Adverse Reaction To Metal Debris Following Dislocation Of A Metal-On-Metal Hip Arthroplasty: A Case Report, John M. Gaddis, Amanda Wheeler, Bretton Laboret, Joel Wells
Research Symposium
Background: Primary total hip arthroplasties have a reported survivorship exceeding 95% at 10-year data. Metal-on-metal bearing surfaces were introduced to minimize wear rate for patients undergoing THA, however, long-term data has shown an unacceptably high failure rate requiring THA revision. The metal-on-metal implants can lead to failure with the generation of the metal ions, specifically cobalt and chromium. The rise in levels of these metal ions can result in adverse reaction to metal debris (ARMD) which can consist of macroscopic necrosis, corrosive osteolysis, sterile hip effusions, and pseudotumor. There are no specific clinical signs or symptoms indicating metallosis, rather, diagnosis …

Impact Of Diabetes On Fall Related Lower Limb Fractures, Bryce M. Bellon, Daniel Salinas, Patrick De La Torre Shutz, Hunter Butler, Sean Corcoran, Kelsey Baker
Research Symposium
Background: Diabetes is a disorder that affects many Americans, characterized primarily by hyperglycemia due to either insulin resistance or lack of insulin secretion. This condition not only impacts blood sugar levels but also has significant effects on bone health. In diabetes, bone density is often decreased due to reduced osteoblast activity, which is responsible for bone formation, and increased osteoclast activity, which leads to bone resorption. This imbalance accelerates the breakdown of bone. Additionally, diabetics frequently suffer from lower extremity arterial disease (LEAD), which results in poor vascularization of the lower limbs. These factors contribute to an increased risk of …

Introduction: Lumbar spinal fusion is among the most expensive procedures in the Medicare system, with costs exceeding $33 billion annually. Improving recovery outcomes for these procedures is critical, and emerging research suggests that microbiome-targeted therapies could play a key role. There is growing evidence linking spinal health with gut microbiota alterations, particularly through the gut-spine axis, which involves the gut-disc, gut-bone, and gut-immune interactions. This bidirectional relationship between gut microbiota and spinal health offers a novel approach for understanding recovery mechanisms. Additionally, magnesium deficiency, which is prevalent, may exacerbate both gut dysbiosis and poor bone health, contributing to chronic skeletal …

Introduction: Industry payments to physicians may risk introducing bias into patient care. The Open Payments (OP) database, established by Physician Payments Sunshine Act, reports these payments to promote transparency. Orthopedic surgeons receive the highest industry payments especially in the category of royalties/licensing, prompting questions about conflicts of interest and whether the database accurately reflects the status across the specialty. We hypothesize that the OP database underrepresents the relationships that orthopedic surgeons have with industry. We seek to assess the validity of reported industry payments, particularly in the categories of royalties and licensing payments to determine how to improve transparency and …
