We present a case of an anterior capsular tear of the right knee in a previously healthy, active individual. The patient was a 31-year-old male seen one month after the onset of throbbing anterior right knee pain, which progressed to sharp suprapatellar pain over the next three months. Both dynamic ultrasound (US) and MRI were obtained. Dynamic US revealed a tear of the anterior capsule of the right knee complicated by a suprapatellar effusion communicating into the vastus intermedius fibers of the quadriceps tendon. Brain extraction, or skull-stripping, is an essential pre-processing step in neuro-imaging that has a direct impact on the quality of all subsequent processing and analyses steps. It is also a key requirement in multi-institutional collaborations to comply with privacy-preserving regulations. Existing automated methods, including Deep Learning (DL) based methods that have obtained state-of-the-art results in recent years, have primarily targeted brain extraction without considering pathologically-affected brains. Accordingly, they perform sub-optimally when applied on magnetic resonance imaging (MRI) brain scans with apparent pathologies such as brain tumors. New treatment strategies are desperately needed for treating skeletal malignancy. Skeletal malignancies can be either primary cancer that originated in the bone, such as osteosarcoma, or metastatic cancer that spread from another organ to the skeleton, as in the case of breast or prostate cancer. In this thesis, I will detail two projects that focus on the discovery of new treatment strategies for both primary skeletal malignancy and metastatic skeletal malignancy.

Background

Many dermatologic diseases are implicated in the development of depression. Currently, there is no literature addressing the extent of dermatology residency training on depression screening.

Objective

Our study aimed to determine the extent of dermatology residency training on depression screening to potentially improve education in this area.

Methods

We designed a 12-question survey to assess the level of resident training and comfort in depression screening for patients with acne vulgaris, atopic dermatitis, psoriasis, hidradenitis suppurativa, and skin cancer. Fifty-six residents completed the survey, and data for each question was analysed in aggregate.

Introduction: Current patient-specific quality assurance measures to test deliverability of plans with dynamic intensity modulation involve delivering beams to static measurement device and comparing the planned dose to measurement. However, motion-induced dose errors are not detected with static measurement. Previous studies have investigated combining machine log data with respiratory tracking to determine moving-target dose. BACKGROUND: Bartter syndrome subtypes are a group of rare renal tubular diseases characterized by impaired salt reabsorption in the tubule, specifically the thick ascending limb of Henle's loop. Clinically, they are characterized by the association of hypokalemic metabolic alkalosis, hypercalciuria, nephrocalcinosis, increased levels of plasma renin and aldosterone, low blood pressure and vascular resistance to angiotensin II. Bartter syndrome type II is caused by mutations in the renal outer medullary potassium channel (ROMK) gene (KCNJ1), can present in the newborn period and typically requires lifelong therapy.

BACKGROUND: A family history of diabetes and family members' experiences with diabetes may influence individuals' beliefs and expectations about their own diabetes. No qualitative studies have explored the relationship between family history and experiences and individuals' diabetes illness representations.

METHODS: Secondary data analysis of 89 exploratory, semi-structured interviews with adults with type 1 or type 2 diabetes seeking care in an urban health system. Participants had a recent diabetes-related ED visit/hospitalization or hemoglobin A1c > 7.5%. Interviews were conducted until thematic saturation was achieved. Dysregulation of polyamine metabolism has been linked to the development of colorectal cancer (CRC), but the underlying mechanism is incompletely characterized. Here, we report that spermine synthase (SMS), a polyamine biosynthetic enzyme, is overexpressed in CRC. Targeted disruption of SMS in CRC cells results in spermidine accumulation, which inhibits FOXO3a acetylation and allows subsequent translocation to the nucleus to transcriptionally induce expression of the proapoptotic protein Bim. However, this induction is blunted by MYC-driven expression of miR-19a and miR-19b that repress Bim production. Heart failure (HF) is a leading cause of morbidity and mortality worldwide. Patients with HF exhibit a loss of junctophilin-2 (JPH2), a structural protein critical in forming junctional membrane complexes in which excitation-contraction takes place. Several mechanisms have been proposed to mediate the loss of JPH2, one being cleavage by the calcium-dependent protease calpain. The downstream mechanisms underlying HF progression after JPH2 cleavage are presently poorly understood. In this study, we used Labcas to bioinformatically predict putative calpain cleavage sites on JPH2. It results in adverse outcomes: elevated mortality rates, length of stay, readmissions, institutionalization, long-term cognitive changes, and diminished quality of life. The rate of iatrogenic delirium is astounding, ranging from 10%-89%. There are no curative treatments; thus, primary prevention is the key. Primary cutaneous melanoma frequently metastasizes to distant organs including the brain. Identification of cell-free microRNAs (cfmiRs) found in the blood can be used as potential body fluid biomarkers for detecting and monitoring patients with melanoma brain metastasis (MBM). In this pilot study, we initially aimed to identify cfmiRs in the blood of MBM patients. Normal donors plasma (healthy, n = 48) and pre-operative MBM patients' plasma samples (n = 36) were compared for differences in >2000 microRNAs (miRs) using a next generation sequencing (NGS) probe-based assay. Femoral shaft fractures are common injuries typically due to high-energy trauma. Injuries associated with femoral shaft fractures include ipsilateral femoral neck fractures, ipsilateral distal femur fractures, ipsilateral patella fractures, ipsilateral tibial shaft fractures, ipsilateral knee ligament injuries, acetabular and pelvis fractures, and small bone fractures. These injuries can go unnoticed, and it is important to recognize and treat these injuries accordingly.

Background: Hamstring (HS) graft size is an independent risk factor in determining outcomes related to anterior cruciate ligament reconstruction (ACLR). Although anthropometric correlations with HS tendon size have been studied, a predictable association with resultant graft size has not been established. This study directly compares the difference in diameter between a 4-strand (4S) and 5-strand (5S) HS autograft in the same patient. Older men and women have been found to be more vulnerable to negative outcomes should they contract Covid19, particularly if they also have comorbid conditions such as type 2 diabetes. Cultural, racial, ethnic, and social class differences exist in vulnerability to Covid19 and in the prevalence of type 2 diabetes. Background: There is a plethora of data regarding the negative health consequences of sitting, including increased cardiometabolic risk factors and all-cause mortality. Correctable actions should be identified. Methods: A literature review was performed to examine articles that identified sitting prevalence and health consequences. A survey querying average hours per day spent sitting was distributed to orthopaedic residents at a single institution. Statistical analysis of the data was performed using a 2-tailed t test. Flexor tenosynovitis and tendon rupture are rare. Yet these occurrences can be serious complications after treatment of a fractured distal radius with a volar plate. The highest incidents of rupture are associated with the plate on or distal to the watershed line of the radius. Hallux rigidus is a common cause of foot pain involving the hallux; however, the etiology is yet to be elucidated. There are many different treatments that have been described, including cheilectomy. Taking too much bone can cause joint instability and altered joint mechanics, whereas taking too little bone may not relieve pain. We present a reliable, safe, and reproducible technique for cheilectomy in the hallux rigidus.

Lateral knee injury is common among adolescent athletes in both contact and non-contact sports. Injuries in this population deserve special consideration regarding diagnosis and treatment due to the presence of open, closing, or recently closed physes. We describe two cases of isolated proximal fibula fractures with knee pain and laxity to varus stress in young athletes. Isolated proximal fibula fractures can mimic lateral collateral ligament injuries in the knee. Treatment for these injuries is conservative with bracing, protected weight bearing, and delayed or protected return to activity.

Femoral diaphysis fractures represent a small subset of sports-related orthopaedic trauma, accounting for less than 1% of all sports fractures. We describe a 22-year-old male ice hockey player who slid and crashed into the boards during practice. Radiographs revealed fracture of the femoral diaphysis, necessitating surgical intervention to insert an intermedullary nail. Rehabilitation was a hybrid of an anterior cruciate ligament reconstruction and hip labrum repair protocols. There was emphasis on rotator cuff strengthening and lower-extremity rehabilitation owing to a pre-existing shoulder dislocation that occurred during the same season. Primary bone and soft-tissue tumors may present by mimicking common sports injuries. Their diagnosis is contingent on maintaining a broad differential. We report a case of a 20-year-old male athlete who sustained a twisting injury to his planted right knee while playing college lacrosse. He was unable to bear weight or fully extend his knee. The team’s orthopaedic surgeon was concerned for a locked knee secondary to a possible flipped meniscal tear. Massive acetabular bone loss with pelvic discontinuity remains a challenging obstacle with few arthroplasty solutions. We present a case of a 61-year-old man with multiple myeloma who suffered a pathologic acetabular fracture resulting in acetabular protrusio and eventually extensive acetabular bone loss. His prognosis was initially poor; however, he ultimately responded to his radiation and chemotherapy and was limited in his mobility by his extensive acetabular bone loss. This is a case report of a 56-year-old man with 6.1 cm leg length discrepancy and end-stage arthritis in the hip. The patient had total hip arthroplasty prior to his lengthening procedure. A 24-year-old woman, with 5 years of left elbow pain, was given a preliminary diagnosis of focal pigmented villonodular synovitis based on magnetic resonance imaging. She underwent arthroscopic debridement and was found to have an intra-articular bony mass. Histopathology was consistent with an intra-articular osteoid osteoma. The mass was removed arthroscopically with excellent symptomatic improvement. There was no recurrence of symptoms at 1 year follow-up.

A 43-year-old man presented with persistent wrist pain and loss of range of motion after nonoperative therapy. Magnetic resonance imaging showed features consistent with pigmented villonodular synovitis (PVNS) of the scaphotrapeziotrapezoidal joint. He underwent arthroscopic excision and synovectomy. At 2 years postoperatively, he had no symptomatic recurrence. This case shows the importance of considering PVNS for chronic pain of small joints, and it highlights the viability for arthroscopic synovectomy as a treatment option.
