This work proposes novel selective feature fusion of structural and functional data for improved glaucoma detection. The structural data such as retinal nerve fiber layer (RNFL) thickness measurement acquired by scanning laser polarimetry (SLP) is fused with the functional visual field (VF) measurement recorded from the standard automated perimetry (SAP) test. The proposed selective feature fusion exploits correspondence between structural and functional data obtained over multiple sectors. The correlation coefficients for corresponding structural-function sector pairs are used as weights in subsequent feature selection. Lymphangioleiomyomatosis (LAM) is a low-grade neoplastic disease affecting primarily women. It is characterized by cystic lung disease as well as renal and retroperitoneal tumors called angiomyolipomas and lymphangiomas. Tumor cells have smooth muscle features as well as neuroendocrine cell surface markers, and the disease can be diagnosed by HMB45 staining of tumor cells. We questioned whether expression of melanocytic antigens, specifically gp100, creates an opportunity to treat LAM by adoptive T cell transfer. LAM lung lesions demonstrate poor immune surveillance, therefore adoptive T cell transfer could offer benefits in this disease.

Background: Emergence and spread of drug resistance to every anti-malarial used to date, creates an urgent need for development of sensitive, specifc and feld-deployable molecular tools for detection and surveillance of validated drug resistance markers. Background: In order to accurately interpret neonatal thyroid function tests (TFTs), it is necessary to have population specific reference intervals (RIs) as there is significant variation across different populations possibly due to genetic, environmental or analytical issues. Despite the importance of RIs, globally there are very few publications on RIs for neonatal TFTs primarily due to ethical and technical issues surrounding recruitment of neonates for a prospective study. To the best of our knowledge, this is the first report from Africa on neonatal RIs for TFTs.

Previously, urolithiasis was reported as being rare among the indigenous African population but recent data suggest otherwise. This study reviewed the demographic and clinical characteristics of patients with urolithiasis seen at the Aga Khan University hospital Nairobi (AKUHN) as well as the chemical composition of the stones and the modalities of therapy used.

β-thalassemia/HbE is an inherited hemolytic anemia caused by defect in β-globin synthesis resulting in accumulation of excess α-globin chains in red blood cells. A hypercogulable state leading to high risk of thromboembolic event is one of the most common complications observed in this disease, particularly in patients with splenectomy. The objective of this study was to investigate the differences of calculated MU between commissioning using corrected FOF from single detector and commissioning using average uncorrected FOF from three detectors. Measurements were carried out on 6 MV flattened photon beams from 1x1 cm2 to 10x10 cm2. Four detectors were utilized to measure depth doses and beam profiles: IBA CC01, IBA PFD, IBA EFD and Sun Nuclear EDGE. For FOF determination, Sun Nuclear EDGE was not used. งานวิจัยนี้ มีวัตถุประสงค์เพื่อศึกษาปัจจัยทางด้านประสบการณ์ส่วนบุคคลที่ส่งผลต่อการเข้าสู่อาชีพ พริตตี้ และกระบวนการรับและปรับใช้วัฒนธรรมของพริตตี้เพื่อให้ตนดำรงอยู่ในกลุ่มอาชีพและสังคมได้ โดยใช้แนวคิดเรื่องฮาบิทัส และทุน ของปิแอร์ บูร์ดิเยอร์ การศึกษานี้ ผู้วิจัยใช้วิธีวิจัยเชิงคุณภาพ โดยการสัมภาษณ์กลุ่มบุคคลที่ศึกษาแบบเจาะลึก (In-depth interview) และมีการเลือกกลุ่มตัวอย่างแบบเจาะจง (Purposive sampling) โดยมีพริตตี้ในเขตกรุงเทพมหานคร เป็นกรณีศึกษา จำนวน 11 คน ผลการศึกษาพบว่า ประสบการณ์ส่วนบุคคลที่ส่งผลต่อแนวจริตของพริตตี้ เกิดจากการเลี้ยงดูและสภาพแวดล้อมที่มีลักษณะการให้อิสระ และฝึกฝนการเอาตัวรอดให้อยู่ด้วยตนเองได้ รวมถึงการเติบโตท่ามกลางการแสดงออกในเวทีสาธารณะตั้งแต่อายุยังน้อย พริตตี้จึงมีลักษณะเด่นคือ มีความคุ้นชินกับพื้นที่สาธารณะ มีความสามารถในการแก้ปัญหาเฉพาะหน้า มีความช่างสังเกต และมีการวางท่วงท่าในการตอบสนองต่อความต้องการของกลุ่มเป้าหมายได้อย่างเหมาะสม เหล่านี้คือฮาบิทัส ที่ได้พัฒนากลายไปเป็นทักษะหรือทุนวัฒนธรรมประเภทที่มีอยู่ในตัวบุคคล (Embodied form) ซึ่งสามารถนำไปแลกเปลี่ยนเป็นทุนเศรษฐกิจ ทุนสังคม และทุนสัญลักษณ์ ต่อไป เมื่อเข้าสู่อาชีพพริตตี้แล้ว แนวจริตดังกล่าว จะเข้าไปมีผลตั้งแต่การเลือกรับงาน การเลือกลูกค้า โดยในระหว่างการทำงาน พริตตี้จะให้ความสำคัญกับการวางตัวให้เหมาะสมกับลูกค้ามากที่สุด เพื่อเป็นการป้องกันไม่ให้ลูกค้าสามารถเพิ่มระดับความเข้มข้นของรูปแบบงานที่ตนไม่ได้เลือกรับได้ โดยมีความสวยงามของรูปร่างหน้าตาและการพูด เป็นทักษะรองลงมา ความมั่นใจและความกล้าแสดงออก เป็นอันดับสุดท้าย นอกจากนี้ ฮาบิทัสยังส่งผลต่อการเลือกคู่รักของพริตตี้ ที่จะต้องให้อิสระและมีทักษะในการพึ่งตนเองได้เช่นเดียวกับตน รวมถึงการประกอบอาชีพในอนาคตหลังจากเลิกอาชีพพริตตี้แล้ว พริตตี้จะยังคงไว้ซึ่งกลุ่มอาชีพอิสระ เช่น เปิดร้านค้า หรือทำธุรกิจส่วนตัว ต่อไป

การทำงานของกล้ามเนื้อรอบข้อสะโพกและความสูงจากพื้นของเท้าขณะวิ่งเปลี่ยนทิศทางในคนที่มีและไม่มีภาวะความไม่มั่นคงของข้อเท้าเรื้อรัง, วรพงษ์ คงทอง

ภาวะความไม่มั่นคงของข้อเท้าเรื้อรัง (CAI) เป็นผลมาจากการเกิดข้อเท้าแพลงซ้ำ และคนที่มีภาวะ CAI ยิ่งมีความเสี่ยงสูงที่จะเกิดข้อเท้าแพลงซ้ำได้อีก การระบุถึงการเปลี่ยนแปลงที่เกิดขึ้นบริเวณรอบข้อสะโพกร่วมกับการระบุตำแหน่งของเท้าในคนที่มีภาวะ CAI ขณะมีการเคลื่อนไหว จะเป็นประโยชน์ในการฟื้นฟูคนที่มีภาวะ CAI ได้อย่างมีประสิทธิภาพมากขึ้น ดังนั้นในการศึกษานี้จึงมีวัตถุประสงค์ที่จะศึกษาการทำงานของกล้ามเนื้อรอบข้อสะโพก (hip muscle activities) และความสูงจากพื้นของเท้า (foot clearance) ในช่วง terminal swing phase ขณะวิ่งเปลี่ยนทิศทางในคนที่มีและไม่มีภาวะ CAI โดยมีผู้เข้าร่วมวิจัยเป็นนักกีฬาที่มีภาวะ CAI 22 คน และนักกีฬาที่ไม่มีภาวะ CAI 22 คน ทุกคนจะได้รับการติด surface electrode EMG ที่กล้ามเนื้อรอบข้อสะโพกข้างที่มีภาวะ CAI ในกลุ่ม CAI และข้างถนัดในกลุ่ม control เพื่อบันทึกข้อมูล muscle activity และติด reflective marker ที่ขาท่อนล่างและเท้าทั้งสองข้าง เพื่อบันทึกความสูงจากพื้นของเท้า จากนั้นผู้เข้าร่วมวิจัยวิ่งเปลี่ยนทิศทางด้วยความเร็ว 3.5 - 5 เมตรต่อวินาที ผลการศึกษาพบว่า percent change of muscle activation ของกล้ามเนื้อ Adductor Magnus กล้ามเนื้อ Gluteus Medius และกล้ามเนื้อ Tensor Fascia Latae มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p < 0.05) ระหว่างกลุ่ม CAI และกลุ่ม control และค่าความสูงจากพื้นของเท้าในกลุ่ม CAI มีค่าน้อยกว่ากลุ่ม control อย่างมีนัยสำคัญทางสถิติ (p < 0.05) ผลการศึกษาทำให้ทราบว่า คนที่มีภาวะ CAI มีความสูงจากพื้นของเท้าต่ำกว่าคนที่ไม่มีภาวะ CAI ขณะวิ่งเปลี่ยนทิศทาง ส่งผลให้กล้ามเนื้อรอบข้อสะโพกทำงานมากขึ้นเพื่อชดเชยภาวะดังกล่าว และเพื่อลดความเสี่ยงในการเกิดข้อเท้าแพลงซ้ำ ดังนั้นในโปรแกรมการฟื้นฟูผู้ป่วยที่มีภาวะ CAI จึงควรให้ความสำคัญกับการฝึกกล้ามเนื้อรอบข้อสะโพก รวมถึงการให้ความสำคัญกับตำแหน่งของเท้าในขณะเคลื่อนไหวด้วย

ผลของการออกกำลังกายต่อการไหลเวียนเลือดที่แขนในภาวะน้ำตาลในเลือดสูงหลังรับประทานอาหารในผู้ที่มีความดันโลหิตปกติซึ่งเป็นทายาทของผู้ที่มีความดันโลหิตสูง, วิไลวรรณ จุฑาภักดีกุล

วัตถุประสงค์ เพื่อศึกษาผลของการออกกำลังกายที่ระดับความหนักสูงสลับกับปานกลาง ต่อการไหลเวียนเลือดที่แขนในชายสุขภาพดีที่มีความดันโลหิตปกติและเป็นทายาทของผู้ที่มีความดันโลหิตปกติ (ONT) (n=13) เปรียบเทียบกับชายสุขภาพดีที่มีความดันโลหิตปกติและเป็นทายาทของผู้ที่มีความดันโลหิตสูง (OHT) (n=13) ซึ่งการวิจัยเป็นเชิงทดลองแบบสุ่มข้ามกลุ่ม โดยสุ่มแยกอาสาสมัครในแต่ละกลุ่มออกเป็น 2 แบบ โดยแบบที่ 1 ออกกำลังกายก่อนรับประทานน้ำตาล จากนั้นอย่างน้อย 1 สัปดาห์ ตามด้วยการรับประทานน้ำตาลเพียงอย่างเดียว ซึ่งการทดสอบแบบที่ 2 ทำสลับกับแบบที่ 1 โดยอาสาสมัครทั้งหมดได้รับการวัดระดับสูงสุดของการไหลเวียนเลือดที่แขน ก่อนและภายหลังการรับประทานน้ำตาลทุกๆ 30 นาทีจนครบ 4 ชั่วโมงพบว่า หลังการรับประทานน้ำตาลอย่างเดียว ระดับสูงสุดของการไหลเวียนเลือดที่แขนลดลงอย่างมีนัยสำคัญทางสถิติ ในกลุ่ม OHT (23.82±1.39 ml/100ml/min เป็น 20.61±1.75 ml/100ml/min, p<0.01) และในกลุ่ม ONT (26.84±2.47 ml/100ml/min เป็น 24.29±2.58 ml/100ml/min, p<0.01) และการออกกำลังกายก่อนรับประทานน้ำตาลพบว่า ระดับสูงสุดของการไหลเวียนเลือดที่แขนเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ ในกลุ่ม OHT (23.72±1.68 ml/100ml/min เป็น 27.25±1.91 ml/100ml/min, p<0.01) และในกลุ่ม ONT (26.74±2.86 ml/100ml/min เป็น 34.07±2.14 ml/100ml/min, p<0.01) ตามลำดับ และเมื่อเปรียบเทียบระหว่างกลุ่ม พบว่าเมื่อกลุ่ม OHT มีค่าระดับสูงสุดของการไหลเวียนเลือดที่แขนเพิ่มขึ้นน้อยกว่ากลุ่ม ONT อย่างมีนัยสำคัญทางสถิติ (p<0.05) จึงสรุปได้ว่าการออกกำลังกายที่ระดับความหนักสูงสลับกับปานกลาง สามารถเพิ่มระดับสูงสุดของการไหลเวียนเลือดที่แขนในภาวะน้ำตาลในเลือดสูงหลังรับประทานน้ำตาล ในคนที่มีความดันโลหิตปกติทั้งที่มีและไม่มีประวัติความดันโลหิตในครอบครัว

วัตถุประสงค์ เพื่อศึกษาผลของการฝึกออกกำลังกายโดยการปั่นจักรยานสองชนิดเป็นเวลา 12 สัปดาห์ต่อระดับความรู้สึกกดเจ็บขั้นต่ำ การทำงานของกล้ามเนื้อ และจลนศาสตร์บริเวณลำตัวส่วนบนในพนักงานออฟฟิศเพศหญิงที่มีอาการปวดกล้ามเนื้อและเนื้อเยื่อพังผืด อายุเฉลี่ย 29.5?4.09ปี สุ่มแบ่งเป็น 3 กลุ่ม กลุ่มละ 15 คน ได้แก่ กลุ่มควบคุม (ยืดกล้ามเนื้อ) กลุ่มปั่นจักรยาน A (นั่งหลังตรงบนจักรยานอยู่กับที่) และกลุ่มปั่นจักรยาน B (นั่งก้มตัวไปด้านหน้าบนจักรยานไฮบริดจ์) อาสาสมัครจะได้รับการวัดระดับความรู้สึกกดเจ็บขั้นต่ำและระดับความรุนแรงของอาการปวด หลังทดสอบพิมพ์งานหรือปั่นจักรยาน รวมถึงวัดการทำงานของกล้ามเนื้อและจลนศาสตร์การเคลื่อนไหวขณะพิมพ์งานหรือปั่นจักรยาน โดยทดสอบในวันแรกหลัง 6 สัปดาห์ 12 สัปดาห์ และติดตามผล 2 สัปดาห์ กลุ่มควบคุมจะได้รับโปรแกรมการยืดกล้ามเนื้อ ขณะที่กลุ่มปั่นจักรยานจะออกกำลังกายระดับปานกลางถึงหนัก 30นาที 3 ครั้งต่อสัปดาห์ เป็นเวลา 12 สัปดาห์ พบว่า ระดับความรู้สึกกดเจ็บขั้นต่ำทั้ง 3 กลุ่มเพิ่มขึ้น และระดับความรุนแรงของอาการปวดลดลงอย่างมีนัยสำคัญทางสถิติ (p<0.05) ส่วนการทำงานของกล้ามเนื้อ พบว่าขณะพิมพ์งานและปั่นจักรยาน กล้ามเนื้อ cervical erector spinae ทำงานมากกว่ากล้ามเนื้อ upper trapezius และ lower trapezius อย่างมีนัยสำคัญทางสถิติ (p<0.05) และพบว่าในกลุ่มปั่นจักรยาน A มีมุมศีรษะและลำตัวมากกว่ากลุ่มปั่นจักรยาน B แต่มีมุมช่วงไหล่น้อยกว่า อย่างมีนัยสำคัญทางสถิติ (p<0.05) ซึ่งสัมพันธ์กับท่าทางในการปั่นจักรยานแต่ละชนิด จึงสรุปได้ว่า การปั่นจักรยานทั้งสองชนิดที่ระดับความหนักปานกลางถึงหนัก (50-70%HRR) เป็นเวลา 12 สัปดาห์ สามารถเพิ่มระดับความรู้สึกกดเจ็บขั้นต่ำ และลดระดับความรุนแรงของอาการปวดได้เช่นเดียวกับการยืดกล้ามเนื้อซึ่งเป็นการรักษาพื้นฐานในพนักงานออฟฟิศเพศหญิงที่มีอาการปวดกล้ามเนื้อและเนื้อเยื่อพังผืด

Background: Ukraine has high rates of poor treatment outcomes among drug sensitive tuberculosis (DSTB) patients, while global treatment success rates for DSTB remain high.

We evaluated baseline patient factors as predictors of poor DSTB treatment outcomes.

Methods: We conducted a retrospective analysis of new drug sensitive pulmonary TB patients treated in Kyiv Oblast, Ukraine between November 2012 and October 2014. We defined good treatment outcomes as cure or completion and poor outcomes as death, default (lost to follow up) or treatment failure. Integrating behavioral health clinicians into primary care practices has been proposed as one of the most effective ways to increase access to necessary behavioral health services for many Americans. The C terminal binding proteins (CtBP) 1 and 2 are a family of transcriptional co-repressors overexpressed in a variety of cancers, and are frequently associated with poor prognosis and chemoresistance. CtBP has also been characterized in cell culture models as drivers of migration/invasion and epithelial-mesenchymal transition. CtBP mediates its transcriptional corepressor activity via its dehydrogenase domain, and inhibition of this domain interferes with CtBP oncogenic functions. The role of CtBP in APC mutant neoplasia remains obscure even though APC is responsible for degradation of both β-catenin and CtBP in suppressing colorectal tumorigenesis. Rationale: Tissue Factor (TF) is a transmembrane glycoprotein that canonically functions as the initiator of the coagulation cascade. Increased levels of TF have been associated with inflammatory airway diseases. Since lipopolysaccharide (LPS) is known to elicit and inflammatory response in airway epithelium, we hypothesized that airway epithelial cells release TF when exposed to LPS. Since TF aids in local wound healing, we also hypothesized that inhibition of TF would decrease NHBE growth. The regulation of microRNA (miRNA) biogenesis, function and degradation involves a range of mechanisms, including interactions with RNA-binding proteins. The potential contribution of regulatory miRNAs to the expression of these RNA interactor proteins that could control other miRNAs expression is still unclear. Here we demonstrate a regulatory circuit involving oncogenic and tumor-suppressor miRNAs and an RNA-binding protein in a chemotherapy-resistant ovarian cancer model. Falls and their consequences are significant concerns for older adults, caregivers, and health care providers. Identification of fall risk is crucial for appropriate referral to preventive interventions. Falls are multifactorial; no single measure is an accurate diagnostic tool. Major Depressive Disorder (MDD) is a complex multifactorial disorder, which would lead to disability. Environmental and genetic factors are involved in MDD etiology. The aim of this project was to identify loci modifying age at onset (AAO) of MDD using survival models after adjusting for Childhood Sexual Abuse (CSA). To achieve this aim, a dataset was made available by the China Oxford and VCU Experimental Research on Genetic Epidemiology (CONVERGE) consortium. DNA methylation is necessary for learning, memory consolidation and has been implicated in a number of neuropsychiatric disorders. Obtaining high quality and comprehensive data for the three common forms of methylation in brain is challenging for methylome-wide association studies (MWAS). To address this we optimized a panel of enrichment methods for screening the brain methylome. Results show that these enrichment techniques approach the coverage and fidelity of the current gold standard bisulfite based techniques. Purpose: The goal of this study was to describe the pathologic findings and early follow-up experience of patients who underwent a sentinel lymph node biopsy (SLNB) at Aga Khan University Hospital (AKUH) between 2008 and 2017.

Background

The optimal duration of dual antiplatelet therapy (DAPT) after percutaneous coronary intervention (PCI) remains in dispute, particularly after 12 months. The DAPT Score was developed from the DAPT trial to identify pts who may benefit the most from extended therapy. We sought to assess the performance of the DAPT risk score in the all-comers Assessment of Dual AntiPlatelet Therapy with Drug-Eluting Stents (ADAPT-DES) registry, and to test the utility of additional predictors of events on its performance.
