Background: Food allergy is an important public health problem because it affects children and adults, can be severe and even life-threatening, and may be increasing in prevalence. Beginning in 2008, the National Institute of Allergy and Infectious Diseases, working with other organizations and advocacy groups, led the development of the first clinical guidelines for the diagnosis and management of food allergy. A recent landmark clinical trial and other emerging data suggest that peanut allergy can be prevented through introduction of peanut-containing foods beginning in infancy.

Pain in sickle cell disease (SCD) is associated with increased morbidity, mortality, and high health care costs. Although episodic acute pain is the hallmark of this disorder, there is an increasing awareness that chronic pain is part of the pain experience of many older adolescents and adults. A common set of criteria for classifying chronic pain associated with SCD would enhance SCD pain research efforts in epidemiology, pain mechanisms, and clinical trials of pain management interventions, and ultimately improve clinical assessment and management. BACKGROUND: Despite important progress in the field of innate immunity, our understanding of host immune responses to parasitic nematode infections lags behind that of responses to microbes. A limiting factor has been the obligate requirement for a vertebrate host which has hindered investigation of the parasitic nematode infective process. The nematode parasite Heterorhabditis bacteriophora offers great potential as a model to genetically dissect the process of infection. With its mutualistic Photorhabdus luminescens bacteria, H. bacteriophora invades multiple species of insects, which it kills and exploits as a food source for the development of several nematode generations. Recurrent Pseudomonas aeruginosa infections coupled with robust, damaging neutrophilic inflammation characterize the chronic lung disease cystic fibrosis (CF). The proresolving lipid mediator, 15-epi lipoxin A4 (15-epi LXA4), plays a critical role in limiting neutrophil activation and tissue inflammation, thus promoting the return to tissue homeostasis. Here, we show that a secreted P. aeruginosa epoxide hydrolase, cystic fibrosis transmembrane conductance regulator inhibitory factor (Cif), can disrupt 15-epi LXA4 transcellular biosynthesis and function. In the airway, 15-epi LXA4 production is stimulated by the epithelial-derived eicosanoid 14,15-epoxyeicosatrienoic acid (14,15-EET). Cif sabotages the production of 15-epi LXA4 by rapidly hydrolyzing 14,15-EET into its cognate …

OBJECTIVE: Uptake of meningococcal vaccine (MCV) and tetanus, diphtheria and pertussis (Tdap) vaccine among adolescents has approached Healthy People 2020 goals, but human papillomavirus (HPV) vaccination has not. This study evaluated an intervention using the 4 Pillars™ Practice Transformation Program to increase HPV, MCV and Tdap uptake among adolescents in primary care practices.

METHODS: Practices with at least 50 patients 11-17years old with estimated vaccination rates less than national goals, were assigned to intervention (n=11) and control (n=11) groups in a randomized controlled cluster trial; 9 intervention and 11 control sites completed the study. BACKGROUND: The primary objective was to determine whether topically administered Caphosol, rinsed orally four times daily at the initiation of conditioning, reduces the duration of severe oral mucositis (OM) compared with placebo among children and adolescents undergoing haematopoietic cell transplantation (HCT).

METHODS: This was a Children's Oncology Group multicentre randomised double-blinded placebo-controlled clinical trial. Patients between the ages of 4 and 21 years who were scheduled to undergo myeloablative HCT for any indication were randomised to Caphosol or placebo saline rinses four times daily from initiation of conditioning through day +20. Loss of epithelial cell polarity promotes cell invasion and cancer dissemination. Therefore, identification of factors that disrupt polarized acinar formation is crucial. Reactive oxygen species (ROS) drive cancer progression and promote inflammation. Here, we show that the non-polarized breast cancer cell line T4-2 generates significantly higher ROS levels than polarized S1 and T4R cells in three-dimensional (3D) culture, accompanied by induction of the nuclear factor κB (NF-κB) pathway and cytokine expression. Minimizing ROS in T4-2 cells with antioxidants reestablished basal polarity and inhibited cell proliferation. The United States continues to spend an unprecedented amount of annual money on healthcare. However, the costs of providing care may not appropriately reflect the quality of care patients are receiving. This is particularly concerning when examining the projected increase in total joint arthroplasty (TJA) procedures over the next few decades, which are expected to increase to nearly 4 million procedures annually. The Center for Medicare and Medicaid Services (CMS) has responded by increasingly shifting reimbursements from the less efficient fee-forservice repayment model to value-based repayment. The immense popularity and financial compensation associated with success in sports often results in a desire to win at all costs, which stimulates a never-ending appetite for use of ergogenic aids. Athletes have experimented with these substances (ranging from over-the-counter supplements to anabolic-androgenic steroids and beyond) if a possibility of advantage exists, whether scientifically or theoretically supported. Rigid intramedullary (IM) nailing is an effective procedure for treating fractures of the femoral shaft. Despite the existence of alternative devices and techniques (eg, unreamed nails, flexible nails, plates, external fixation, and traction), the use of rigid reamed nails has shown excellent healing rates, return of function, alignment, and low complications. This type of nail has been used in an antegrade approach to the entry site of the fracture, resulting in successful treatment. Background: The purpose of this study was to investigate the differences between the dorsal and lateral plate position for metacarpal and phalanx fractures.

Methods: We conducted a retrospective review of prospectively collected patient data for 186 fractures treated by a single surgeon between 2009 and 2011. Rates of plate removal, total arc of postoperative motion (TAM), and patient and injury demographics were tested for association with plating position.

Results: Increasing age, larger plates, and dorsal plating were found in univariate analysis to be associated with decreased TAM and increased plate removal in phalanx but not in metacarpal fractures.

Background: Insufficiency fractures at the sacrum represent a considerable cause of low back pain and disability within the geriatric population. However, there has never been a large-scale study examining the epidemiologic trends associated with the disease, and thus this condition has been largely under recognized.

Methods: This study analyzed a Nationwide Inpatient Sample (NIS) database to trend frequency and burden of sacral insufficiency fractures (SIF). We aimed to (1) understand the statistical methods of currently published orthopaedic studies using NIS and (2) examine the role of hierarchical modeling versus traditional regression analysis in a retrospective cohort of patients from NIS.

Methods: We conducted a systematic review to examine statistical methods of orthopaedic studies (published between 2005 and 2014) using NIS. Ultimately, 132 studies were identified. Background: In planning operative treatment of thumb carpometacarpal (CMC) joint osteoarthritis, surgeons should consider the presence of an arthritic scaphotrapeziotrapezoid (STT) joint. We aimed to determine the radiographic prevalence of concomitant (STT) osteoarthritis with thumb CMC osteoarthritis in patients who underwent surgical treatment at our institution. Background: Construct stability is a necessary characteristic of external fixators. Many commonly used fixators are constructed as symmetric one-plane frames. We postulate that asymmetric two-plane triangular constructs provide enhanced stability with simplicity and freedom in pin placement. We hypothesized that results of finite element analysis would determine optimal geometric configuration, and findings of mechanical testing would confirm the improved stability of two-plane triangular constructs.
