Aberrant landing biomechanics increase the risk of anterior cruciate ligament (ACL) injury and are a focus of rehabilitation after ACL reconstruction. The purpose of the present thesis was to develop and evaluate methods of assessing landing mechanics and investigate the effects of different rehabilitation strategies after ACL reconstruction. Three studies were conducted. The first study used a Delphi process to develop the content of a Clinician-Rated Drop Vertical Jump Scale to evaluate jump landing mechanics during rehabilitation after ACL reconstruction. Purpose
The overall purpose of this dissertation was to evaluate factors related to reducing length of stay after total hip and knee arthroplasty (THA and TKA).
Methods
Study 1 was a randomized controlled trial (RCT) to investigate the cost-effectiveness of outpatient THA compared to standard overnight stay. Study 2 was a prospective cohort study to assess patient- and caregiver-related factors and how they may relate to caregiver strain and level of assistance when caring for patients undergoing THA and TKA. Aim: This thesis explored the experiences of family medicine preceptors and family medicine residents regarding multimorbidity care and education.
Methods: Two qualitative descriptive studies were conducted using semi structured interviews with family medicine preceptors and focus groups with family medicine residents. Interviews and focus groups were audio-taped and transcribed verbatim. Data analysis was iterative and interpretive in nature and done both independently and as a team.
Findings: Multimorbidity care is predominantly provided by family physicians yet there is little focus on multimorbidity education in the medical education curriculum. Multimorbidity education is informal and experiential in nature. There are both facilitators …

Existing hospital- wide satisfaction tools don’t provide the specificity that various departments require to truly get a sense of where they stand in terms of patient satisfaction. Critically ill patients of all ages suffer from high burden of neurocognitive impairment during (i.e. delirium) and following (i.e. long term cognitive impairment) critical illness that is associated with worse patient and healthy system outcomes. Ischemia has emerged as a plausible mechanism given the high prevalence of hypotension and shock, ischemic injury on neuroimaging, and impairment of cerebral autoregulation in these patients. However, the burden of ischemic insults during critical illness and mechanisms responsible for these insults are poorly described. Impaired cellular tolerance of reactive oxygen species (ROS) has been suggested as a common mechanistic link associated with aging in both metabolic syndrome and nephrolithiasis. The mechanistic (mammalian) target of rapamycin (mTOR) activity is characteristic of metabolic syndrome. When nutrients are abundant, mTOR is active. Conversely, fasting inhibits mTOR. Metabolic syndrome is correlated with an increased risk of self-reported or imaging findings of nephrolithiasis. At the individual level, patients with a higher BMI have an increased prevalence of recurrent symptomatic nephrolithiasis, 24-hour urinary excretion of oxalate, sodium, uric acid, calcium, and phosphorous as well as lower pH. Facial skin cancer secondary to surgical treatment may be distressing due to the malignancy itself and from the consequences of its treatment. A visible postsurgical scar is an obvious reminder of the condition. This investigation sought to broaden our understanding of facial scarring and develop a novel tool for its objective evaluation. To this end, skin cancer as the most common etiology of facial scarring was reviewed. The scar scale literature was evaluated in the context of assessing scars through a biopsychosocial lens. Sepsis is a life-threatening dysregulation of host response to an infectious insult, which often leads to multi-organ failure. Sepsis survivors suffer from post-intensive care syndrome in part due to their prolonged bed rest and lack of mobility. Although early mobilization has been championed as a mechanism to counter the adverse effects associated with ICU immobility, due to sedation and mechanical ventilation, active early mobilization is not often administered in septic patients due to worries of hemodynamic instability and potential for inducing adverse events. The current gold standard surgical approach for Total Knee Arthroplasty (TKA) is a medial parapatellar approach (MPA). We aimed to study a novel lateral subvastus lateralis approach (SLA) for TKA and compare patient outcomes and joint kinematics to the MPA.
Patients with neutral or varus alignment undergoing primary TKA were recruited to undergo the SLA. At one year postoperative, patient outcomes (WOMAC, SF-12, KSS) and joint kinematics (using radio stereometric analysis (RSA)) were analyzed. 7 LPA and 7 MPA patients were compared.
The SLA resulted in improved medial femoral rollback early in flexion, but less “regular” (external) rotation of the …

Introduction: Major abdominal aortic surgery requires significant fluid resuscitation in the post-operative phase. Patients are at significant risk of perioperative morbidity and mortality which can be affected by the approach to post-operative fluid resuscitation. Point of care ultrasonography (POCUS) has evolved as a tool to perform whole-body assessments at the bedside to augment the physical exam and guide the resuscitation of the critically ill. The humeral head osteotomy influences shoulder arthroplasty component height, version and neck shaft angle and therefore outcomes. The purpose of this study is to develop and utilize navigation to execute the osteotomy.
3D printed models were used to develop a navigation technique, and then to execute planned osteotomies. Free hand, fixed angle guide, patient specific guides, and real time navigation osteotomies were completed. The height, neck shaft angle and version were recorded. Also collected, were the planes of the guides once they were placed.

Reactivation of the multi-subunit ribonucleoprotein telomerase is the primary telomere maintenance mechanism in cancer, but it is rate-limited by the enzymatic component, telomerase reverse transcriptase (TERT). While regulatory in nature, TERT alternative splice variant/isoform regulation and functions are not fully elucidated and are further complicated by their highly diverse expression. In this thesis, I characterized TERT expression across normal and neoplastic tissues using TCGA and GTEx RNA-sequencing data. In doing so, I demonstrated the global overexpression and splicing shift towards full-length TERT in neoplastic tissue. This exploratory qualitative study identified spiritual and psychosocial palliative care needs in one rural Eastern Ontario town, referred to as the pseudonym “Duffy’s Hill.” Six qualitative interviews with health care providers gathered insights, ideas, and stories related to the provision of spiritual and psychosocial palliative care in Duffy’s Hill. Participant responses were analyzed for shared values, beliefs, ideas, practices, and norms in providing this care to determine if the distinct needs observed could be attributed to cultural particularities in Duffy’s Hill. Colonoscopy is a technically challenging procedure to learn. The colonoscope is prone to forming loops in the colon, which can lead patient discomfort and even perforation. We hypothesized that expert endoscopists use techniques to avoid loop formation, and identify and straighten loops earlier, and thus exert less force.
Using a physical colon simulator model, electromagnetic tracking markers were used to follow the motion of the colon as the scope was advanced. Attending physicians exerted significantly lower mean colonic displacement than trainees.

Methods to measure early changes in joint structures and the effect of interventions are required. Introduction: Preterm birth (PTB) and socioeconomic status (SES) have both been associated previously with the onset of childhood asthma in high income countries. Yet, these factors have been studied minimally outside of an exposure or confounded relationship, and amongst the Canadian population.
Materials and Methods: Merging household interview data from Cycles 2 to 5 of the Canadian Health Measures Survey, this study analyzed data from weighted respondents in early (3-5 years, n = 1,096,609) and middle (6-11 years, n = 2,112,059) childhood. Bivariate, stepwise logistic regression, and logit decomposition were performed using STATA software. Intersectionality guided the study’s methodology.

The role and clinical significance of microorganisms in presumed aseptic revision total hip (THA) and knee arthroplasty (TKA) is unclear. The primary aim of this thesis was to determine the prevalence and infection-free survival of presumed aseptic revisions with unexpected positive intraoperative cultures (UPC) by analyzing the largest cohorts of UPC in the literature. Secondarily, a prospective pilot study using modern molecular techniques with an emphasis on stringent control of contamination was designed to determine how frequently microorganisms are present on implants of presumed aseptic revisions, as well as their location and association with reason for revision.

Scaphoidectomy and four corner fusion (4CF), and proximal row carpectomy (PRC) are salvage procedures that may be offered in advanced wrist arthritis. The optimal clinical treatment remains unclear. Biomechanical studies comparing outcomes of these procedures often only report uniplanar motion, rather than multiplanar motion which is more representative of functional wrist motion. Staphylococcus aureus is a major cause of prosthetic joint infection (PJI) in which it forms adherent biofilms, thick aggregates of extracellular polymeric substances (EPS) produced by the bacteria. The absence of quality assurance in training clinicians to perform pre-anesthesia consultations at a Canadian university is the Problem of Practice addressed in this Organizational Improvement Plan. This competency requires learners to apply their anesthesia knowledge to take medical histories; perform physical examinations; diagnose anesthetic risks; and generate anesthesia plans. Random chart audits of many learners identify deficiencies and suggest inconsistent training of this competency. Introduction: Frailty and sarcopenia are related concepts that can impact outcomes after kidney transplantation. Measures of these two entities and new/emerging metrics of sarcopenia remain to be validated.
Methods: In a prospective cohort study, kidney and kidney-pancreas transplant recipients were assessed at the time of transplant with the Physical Frailty Phenotype, bioimpedance analysis, quadriceps muscle layer thickness (QMLT), and CT. The impact on length of stay (LOS), prediction of frailty/sarcopenia, and relative concordance of metrics were analyzed.

Rapid prototyping refers to the manufacturing process in which a three-dimensional (3D) digital model can be transformed into a physical model by layering material in the shape of successive cross sections atop of previously layers. Otosclerosis is a relatively common hearing loss disorder characterized by abnormal bone growth in the otic capsule leading to stapes fixation. In approximately half of cases, otosclerosis is inherited as an autosomal dominant trait. Typically, gene discovery efforts rely on surgical confirmation, audiometry and occasionally acoustic reflexes to identify affected cases of otosclerosis within families, requiring that the otosclerosis was at an advanced stage to be detected. This makes it difficult to identify individuals with early otosclerosis. Post-operative readmission following colorectal surgery is a common and costly occurrence. Remote health monitoring via mobile applications has the potential to reduce post-operative readmissions by early identification of complications. This intervention depends on patient acceptance and compliance with available technology. The feasibility of home monitoring using automated daily surveys and wound photo uploads, delivered via a mobile health application, was tested in the immediate post-operative period after colorectal surgery. Patient compliance, the association between generated alerts and readmissions, and patient satisfaction were measured. Patient satisfaction was high; 80.5% of patients reported that they felt safer going home knowing that they …
