Purpose

To perform a derivation study to determine in which subcompetencies marginal/unsatisfactory pediatric residents had the greatest deficits compared with their satisfactorily performing peers and which subcompetencies best discriminated between marginal/unsatisfactory and satisfactorily performing residents.

Method

Multi-institutional cohort study of all 21 milestones (rated on four or five levels) reported to the Accreditation Council for Graduate Medical Education, and global marginal/unsatisfactory versus satisfactory performance reported to the American Board of Pediatrics. Data were gathered in 2013–2014. The V3 loop of the human immunodeficiency virus type 1 (HIV-1) gp120 exterior envelope glycoprotein (Env) becomes exposed after CD4 binding and contacts the coreceptor to mediate viral entry. Prior to CD4 engagement, a hydrophobic patch located at the tip of the V3 loop stabilizes the non-covalent association of gp120 with the Env trimer of HIV-1 subtype B strains. Here, we show that this conserved hydrophobic patch (amino acid residues 307, 309 and 317) contributes to gp120-trimer association in HIV-1 subtype C, HIV-2 and SIV. Most known giant viruses, i.e., viruses producing giant virions, parasitize amoebae and other unicellular eukaryotes. Although they vary in the level of dependence on host nuclear functions, their virions self-assemble in the host cell's cytoplasm. Here we report the discovery of a new prototype of giant virus infecting epidermal cells of the marine arrow worm Adhesisagitta hispida. Its 1.25 μm-long virions self-assemble and accumulate in the host cell's nucleus. Conventional transmission electron microscopy reveals that the virions have a unique bipartite structure. การวิจัยนี้เป็นการวิจัยกึ่งทดลองมีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการอบอุ่นร่างกายฟีฟ่าอีเลเว่นพลัส (FIFA11+) ที่มีผลต่ออัตราส่วนความแข็งแรงของกล้ามเนื้องอเข่าต่อกล้ามเนื้อเหยียดเข่า และศึกษาอุบัติการณ์การบาดเจ็บที่เกิดขึ้นหลังจากได้รับการฝึกโปรแกรมในนักฟุตซอลหญิงไทย โดยผู้เข้าร่วมงานวิจัยเป็นนักฟุตซอลหญิงไทย อายุ 18-30 ปี แบ่งผู้เข้าร่วมวิจัยออกเป็น 2 กลุ่มแบบเฉพาะเจาะจง กลุ่มละ 14 คน โดยกลุ่มทดลองเป็นนักฟุตซอลหญิงทีมชาติไทยได้รับการฝึกโปรแกรม FIFA 11+ 3 วันต่อสัปดาห์ กลุ่มควบคุมทำการอบอุ่นร่างกายแบบอยู่กับที่ (Static stretching) เป็นระยะเวลา 8 สัปดาห์ มีการประเมินส่วนประกอบภายในร่างกาย การทดสอบความแข็งแรงของกล้ามเนื้อต้นขา (Isokinetic test) ความคล่องแคล่วว่องไว (Agility T-test) และการแกว่งของร่างกายขณะหยุดนิ่ง (The Footwork Pro balance test) พบว่ากลุ่มที่ได้รับการฝึก FIFA11+ มีการเปลี่ยนแปลงอย่างมีนัยสำคัญทางสถิติ (p<0.05) ดังนี้ มีการเพิ่มขึ้นของค่ามวลกล้ามเนื้อและการลดลงของค่าไขมันทั้งหมด อัตราส่วนความแข็งแรงของกล้ามเนื้องอเข่าต่อกล้ามเนื้อเหยียดเข่าขาข้างถนัดเพิ่มขึ้น การแกว่งของร่างกายที่ลดลงจากการทดสอบความสามารถในการทรงตัวของขาแต่ละข้างขณะหยุดนิ่งทั้งเปิดตาและปิดตา มีความแตกต่างระหว่างกลุ่มของค่าความคล่องแคล่วว่องไว อุบัติการณ์การบาดเจ็บที่เกิดขึ้นทั้งหมด 168.15 ครั้งต่อ 1,000 ชั่วโมงของการมีส่วนร่วมในการฝึกซ้อมและแข่งขัน ตำแหน่งของการบาดเจ็บที่พบมากที่สุด คือ ข้อเท้า 18.48% ข้อเข่า 18.48 % และกล้ามเนื้อต้นขา 17.7% เป็นการบาดเจ็บเอ็นข้อต่อที่เชื่อมระหว่างกระดูกอักเสบ การบาดเจ็บขาหนีบ และการฉีกขาดของกล้ามเนื้อต้นขา สรุปผลการฝึกโปรแกรม FIFA11+ สามารถพัฒนาสมรรถภาพทางกายของนักกีฬา ซึ่งช่วยลดการบาดเจ็บบริเวณรยางค์ส่วนล่างในนักกีฬาฟุตซอลหญิงไทย โดยเฉพาะอย่างยิ่งลดอุบัติการณ์การบาดเจ็บของข้อเท้าและข้อเข่าระดับรุนแรงได้

วัตถุประสงค์เพื่อศึกษาถึงความแตกต่างของเทคนิคการฟื้นฟูสมรรถภาพกายแบบมีกิจกรรมการเคลื่อนไหวระหว่างเทคนิคหะฐะโยคะกับเทคนิคฟีฟ่า 11+ และเทคนิคพื้นฐานภายหลังการออกกำลังกายส่วนขาด้วยระดับความหนักสูงสุดในนักกีฬาฟุตบอลชาย โดยทำการศึกษาในอาสาสมัครนักฟุตบอลเพศชาย อายุ 18-29 ปี ที่มีระดับการแข่งขันอยู่ในระดับมหาวิทยาลัยหรือสโมสรฟุตบอลต่างๆ จำนวน 59 คน แบ่งเป็น 3 กลุ่ม คือ กลุ่มเทคนิคพื้นฐาน (n=20) กลุ่มหะฐะโยคะ (n=20) และกลุ่มฟีฟ่า 11+ (n=19) กำหนดให้อาสาสมัครทุกคนได้รับโปรแกรมการออกกำลังกายกระตุ้นการล้าของขาด้วยการปั่นจักรยาน Wingate ที่ความหนัก 7.5% ของน้ำหนักตัว จำนวน 3 รอบ หลังจากนั้นจะแบ่งกลุ่มเข้าร่วมโปรแกรมฟื้นฟูสมรรถภาพเป็นระยะเวลา 20 นาที ซึ่งจะได้รับการทดสอบสมรรถภาพทุกๆ 5 นาที ได้แก่ อัตราการเต้นของชีพจร ระดับความล้าของกล้ามเนื้อ ค่าแลคเตทในเลือด ค่าการกระโดดสูงสุดและค่าความยืดหยุ่นของกล้ามเนื้อต้นขาด้านหลัง พบว่าผลการศึกษา เมื่อเปรียบเทียบกับก่อนออกกำลังกาย กลุ่มหะฐะโยคะ ค่าการกระโดดสูงสุดมีค่าเพิ่มมากขึ้น (p<0.01) และค่าความยืดหยุ่นของกล้ามเนื้อต้นขาด้านหลังมีค่าเพิ่มมากขึ้น (p<0.01) อีกทั้งไม่พบความแตกต่างของอัตราการเต้นของชีพจร ระดับความล้าของกล้ามเนื้อและค่าแลคเตทในเลือด จากการวิจัยสรุปได้ว่า การฟื้นฟูสมรรถภาพกายแบบมีกิจจกรรมการเคลื่อนไหวด้วยเทคนิคหะฐะโยคะภายหลังการออกกำลังกายส่วนขาด้วยระดับความหนักสูงสุด สามารถเพิ่มสมรรถภาพทางกายในด้านความแข็งแรงและความยืดหยุ่นของกล้ามเนื้อในนักกีฬา ในแง่ของการฟื้นฟูสมรรถภาพทางกาย จึงเป็นอีกหนึ่งทางเลือกที่สามารถใช้ระหว่างพักครึ่งการแข่งขันที่มีเวลาอย่างจำกัดเพื่อให้สมรรถภาพทางกายของนักกีฬาพร้อมต่อการใช้งานอย่างมีประสิทธิภาพต่อไป

วัตถุประสงค์ เพื่อศึกษาผลของการออกกำลังกายกล้ามเนื้อกลุ่มสะโพกต่อระยะเวลาที่ใช้ในการทำให้เกิดความมั่นคงขณะทำการกระโดดลงน้ำหนักขาเดียว และประเมินการทำงานของข้อเข่าก่อนและหลังจากการออกกำลังกายกล้ามเนื้อกลุ่มสะโพก โดยทำการศึกษาในอาสาสมัครที่มีประวัติการผ่าตัดซ่อมสร้างเอ็นไขว้หน้าและสามารถกลับไปเล่นกีฬาได้ แบ่งอาสาสมัครออกเป็น 2 กลุ่ม คือกลุ่มออกกำลังกาย (n=18) และกลุ่มควบคุม (n=18) กำหนดระยะเวลาของการออกกำลังกายเท่ากับ 6 สัปดาห์ ประกอบด้วยการออกกำลังกายทั้งหมด 5 ท่าทาง การทดสอบระยะเวลาที่ใช้ในการเกิดความมั่นคงของข้อเข่า คำนวณโดยใช้ค่าแรงกระทำจากพื้น (GRFs) ในขณะที่การทำงานของข้อเข่าจะใช้แบบประเมิน IKDC และประเมินความแข็งแรงของกล้ามเนื้อสะโพกและกล้ามเนื้อขาด้วยเครื่องทดสอบไอโซไคเนติก กำหนดความเร็วเชิงมุมเท่ากับ 60 องศาต่อวินาที ผลการศึกษาพบว่า อาสาสมัครกลุ่มที่ได้รับการออกกำลังกาย ใช้ระยะเวลาในการเกิดความมั่นคงหลังจากกระโดดลงน้ำหนักขาเดียวน้อยกว่า (กลุ่มควบคุม 1.67±0.5: กลุ่มออกกำลังกาย 1.22±0.49 วินาที, P=0.01) และมีค่าคะแนนการประเมินการทำงานของข้อเข่าที่เพิ่มขึ้น (กลุ่มควบคุม 77±14.14: กลุ่มออกกำลังกาย 88±8.69 คะแนน, P<0.01) อย่างมีนัยสำคัญทางสถิติ นอกจากนี้ความแข็งแรงของกล้ามเนื้อสะโพกและกล้ามเนื้อขา มีค่าเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติภายหลังจากการออกกำลังกาย จากการวิจัยจึงสรุปได้ว่าการออกกำลังกายกล้ามเนื้อกลุ่มสะโพกเป็นอีกหนึ่งทางเลือกสำหรับการวางแผนและออกแบบการฟื้นฟูการทำงานของข้อเข่า ในกลุ่มอาสาสมัครที่มีประวัติการผ่าตัดเปลี่ยนซ่อมเอ็นไขว้หน้าข้อเข่า เพื่อให้ผู้ป่วยสามารถกลับไปออกกำลังกายหรือเล่นกีฬาได้อย่างมีประสิทธิภาพ

Thymic epithelial cells (TEC) are essential for a proper adaptive immune response by regulating thymocyte development and establishing central tolerance. In the thymus, TECs differentially express Wnt proteins, which activate canonical and non-canonical Wnt signaling pathways. Wnt signaling is thought to regulate cell survival, proliferation, and development although the direct molecular mechanisms in TECs have yet to be elucidated. The inducible inhibition of canonical Wnt signaling with Dkk1 leads to a rapid loss of TEC progenitors as well as a decline in mature Aire-expressing mTECs. LncRNAs appear to play a considerable role in tumourigenesis through regulating key processes in cancer cells such as proliferative signalling, replicative immortality, invasion and metastasis, evasion of growth suppressors, induction of angiogenesis and resistance to apoptosis. LncRNAs have been reported to play a role in prostate cancer, particularly in regulating the androgen receptor signalling pathway. Background & Purpose: Distance education (DE) is a means to meet allied health workforce needs in rural locations where healthcare worker shortages are apparent. Five allied health programs were expanded to a rural campus teaching synchronously using distance education technology. Individuals born with primary immune deficiency diseases (PIDD) have a dysfunctional immune system, and many are treated by lifelong injections of immunoglobulin therapy. Studies have shown that these patients have low health-related quality of life (HRQOL) and well-being (WB) and that these outcomes might be improved by the availability of therapy innovated according to preferences for fewer needle sticks or a shorter infusion time. Recently, we and others have found that hyperfiltration-associated increase in biomechanical forces, namely, tensile stress and fluid flow shear stress (FFSS), can directly and distinctly alter podocyte structure and function. The ultrafiltrate flow over the major processes and cell body generates FFSS to podocytes. Our previous work suggests that the cyclooxygenase-2 (COX-2)-PGE2-PGE2 receptor 2 (EP2) axis plays an important role in mechanoperception of FFSS in podocytes. Background

Cadmium has been associated with increased risk of cardiovascular disease (CVD) in observational studies, however there has been a limited focus on this relationship in women.

Objectives

This study investigated the association of urinary cadmium (UCd) concentrations with CVD outcomes and all-cause mortality in elderly Western Australian (WA) women.

Methods

UCd excretion was measured at baseline in 1359 women, mean age 75.2 ± 2.7 years and 14.5 years of atherosclerotic vascular disease (ASVD) hospitalisations and deaths, including both the principle cause of death and all associated causes of death. Purpose: The purpose of this study was to examine the relationship between self-reported sedentary time (ST) and the cumulative risk of preserved ejection fraction heart failure (HFpEF) and reduced ejection fraction heart failure (HFrEF) using a diverse cohort of U.S. adults 45-84 years of age.

Methods: Using data from the Multi-Ethnic Study of Atherosclerosis (MESA), we identified 6,814 subjects (52.9% female). All were free of baseline cardiovascular disease. การวิจัยโดยการทดลองในครั้งนี้ มีวัตถุประสงค์ เพื่อศึกษาผลของโปรแกรมการฝึกเกร็งกล้ามเนื้อหน้าท้องต่ออัตราการตอบสนองของกล้ามเนื้อที่ใช้ในการเหยียดหลังขณะยืนก้มลำตัวในอาสาสมัครที่มีอาการปวดเอวที่มีอายุระหว่าง 20-40 ปี จำนวน 44 คน โดยถูกสุ่มออกเป็น 2 กลุ่ม คือกลุ่มทดลอง (N=22) และกลุ่มควบคุม (N=22) ซึ่งกลุ่มทดลองได้รับโปรแกรมการฝึกออกกำลังกายจำนวน 3 ครั้งต่อสัปดาห์ เป็นระยะเวลา 4 สัปดาห์ สำหรับกลุ่มควบคุมได้รับคู่มือเกี่ยวกับอาการปวดหลัง ผลการศึกษาพบว่าหลังจากการทดลองกลุ่มที่ได้รับโปรแกรมการออกกำลังกาย มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติระหว่างก่อนและหลังการทดลอง ของเปอร์เซ็นต์ของอัตราการตอบสนองของกล้ามเนื้อที่ใช้ในการเหยียดหลังเท่ากับ 42.52±25.32 และ 29.08±14.21 ตามลำดับ (P=0.015) มุมการเคลื่อนไหวของกระดูกสันหลังระดับเอวเท่ากับ 44.95±9.78 และ 37.28±8.66 องศา (P=0.017) มุมการเคลื่อนของข้อสะโพกเท่ากับ 42.17±17.62 และ 54.81±19.26 องศา (P=0.001) ค่าดัชนีชี้วัดระดับความสามารถในการทำกิจวัตรประจำวันเท่ากับ 16.89±2.16 และ 3.91±1.06 (P<0.05) และระดับความปวดระหว่างทำกิจกรรม 5.32±1.67 และ 2.26±1.76 (P<0.05) สำหรับกลุ่มควบคุมดังนั้นผลการศึกษานี้แสดงให้เห็นว่าโปรแกรมการฝึกเกร็งกล้ามเนื้อหน้าท้องส่งผลต่อการลดลงของอัตราการตอบสนองของกล้ามเนื้อที่ใช้ในการเหยียดหลังขณะก้มลำตัวในระยะสุดท้าย มีองศาการเคลื่อนไหวของกระดูกสันหลังระดับเอวลดลงและข้อสะโพกเพิ่มขึ้น การจำกัดการใช้ชีวิตประจำวันเนื่องจากอาการปวดน้อยลงและระดับปวดบริเวณเอวลดลง โดยโปรแกรมการออกกำลังกายนี้สามารถใช้เป็นทางเลือกในการรักษาและให้คำแนะนำสำหรับผู้ป่วยที่มีอาการปวดเอว

A diverse set of techniques have been developed to precisely measure the masses of single cells, with varying degrees of technical complexity and throughput. Interleukin-18 (IL-18) is a pro-inflammatory cytokine known to be involved in maintaining metabolic homeostasis; however, also capable of inducing cardiac dysfunction. Additionally, IL-18, has been shown to bind to a novel receptor, the Na-Cl Co-transporter (NCC). We hypothesized that NCC mediates IL-18 metabolic and cardiac signaling in mice. Using male C57BL/6J mice, we compared the metabolic and cardiac function changes after at least 8 weeks of high-saturated fat high sugar diet (Western Diet) in NCC knockout (NCCKO), IL-18 knockout (IL-18KO), and wild-type mice. Dual-energy computed tomography (DECT) has shown great potential in the reduction of uncertainties of proton ranges and low energy photon cross section estimation used in radiation therapy planning. The work presented herein investigated three contributions for advancing DECT applications. 1) A linear and separable two-parameter DECT, the basis vector model (BVM) was used to estimate proton stopping power. Compared to other nonlinear two-parameter models in the literature, the BVM model shows a comparable accuracy achieved for typical human tissues. Introduction: Data suggest a potential role for vitamin D in autism spectrum disorder (ASD) prevention and treatment. It is likely that the serum response to vitamin D supplementation contributes to its effectiveness. Multiple factors affect serum vitamin D 25(OH)D response to supplementation.

Methods: We conducted post-hoc analysis of two double-blind, randomized, placebo-controlled trials (RCT) of vitamin D3 supplementation, one RCT involving children with ASD and another involving children with asthma. Both trials were conducted in the same geographic location (Dublin, Ireland, 53°N), conducted over Winter season and utilized the same vitamin D3 dose (2000 IU/day).

Autism Spectrum Disorder (ASD) is a neurodevelopmental disorder of varying severity and phenotypic expression that is characterized by persistent deficits in social relatedness, communication skills, and interfering repetitive behaviors. Associated behavioral pathology can include inattention, aggression, irritability, hyperactivity, anxiety, and self-injury. Currently, FDAapproved treatments exist to treat secondary symptoms but not core symptomatology. The etiology of autism and other ASD is thought to be multifactorial but is not well understood. The study determined the prevalence ofMetS in patients with schizophrenia at the Psychiatric Unit of the Komfo Anokye Teaching Hospital (KATH), Kumasi, Ghana. This comparative cross-sectional study recruited 348 schizophrenic patients comprising 236 antipsychotic-treated and 112 newly diagnosed treatment-na¨ıve patients. The MetS prevalence was assessed based on World Health Organization (WHO), International Diabetes Federation (IDF), and the National Cholesterol Education Programme, Adult Treatment Panel III (NCEP ATP III) criteria. The aim of this study is to observe cell proliferation, cell viability, apoptosis, and autophagy on acute myeloid leukemia (AML) cell lines, NB4 and U937, with the drug repurposing of mefloquine (MQ). Methods such as the 3-(4,5-Dimethylthiazol-2-Yl)-2,5-Diphenyltetrazolium Bromide (MTT) assay and trypan blue staining have shown a decrease in live cells with high concentrations of mefloquine. Using their average perspective IC50 values of MQ concentration, Western blotting was applied by means of apoptosis and autophagy markers to determine if the induction of apoptosis and inhibition of autophagy was present in MQ-treated AML cells.
