Rubella Immunization Of Adult Females Using Hpv-77 Dk-12 Live Attenuated Rubella Virus, Donald A. Romig, E. L. Quinn, Frank Cox, Robert G. Brackett

This study demonstrates the serologic response as measured by the HAl test and the side reactions of the HPV-77 DK-12 live rubella vaccine in a small group of adult females. One hundred percent seroconversion was obtained using this vaccine. The mean titers obtained in two separate time periods post-vaccination are higher than those reported for several other rubella virus vaccines. A 66% occurrence of joint symptomatology was recorded post-vaccination with a mean duration of 11.6 days; 24% of women who received placebo reported joint complaints which had a mean duration of 2.0 days. Observations On Uric Acid Transport In Man, The Dalmatian And The Non-Dalmatian Dog, Howard Duncan, April S. Curtis

The urinary output of uric acid from the purebred Dalmatian is similar in amount to that in man. Also, this breed of dog has a higher plasma uric acid level than other dogs and, like man, this hyperuricemia is accompanied more frequently with renal and bladder lithiasis. Allopurinol is effective therapy in both man and dog. Study of the fate of uric acid in the Dalmatian shows that the liver does not oxidize the available uric acid completely although it is capable of doing so when liver homogenates are studied. Atrial flutter sounds are occasionally audible on bedside examination especially in patients with slow ventricular rates. A patient is described with audible flutter sounds and an illustrative phonocardiogram is presented. The value of careful auscultation in patients with atrial flutter is re-confirmed.

Gastrocolic And Duodenocolic Fistulas: An Experimental Study, Joseph L. Ponka, A. Karim Shaalan, P. C. Pesaros

In an experimental study of gastrocolic and duodenocolic fistulas, it was clearly seen in 10 animals that duodenocolic fistulas result in the most serious loss of nutrition. Death from starvation occurred in all 42 animals in 30 days. Gastrocolic fistulas similarly resulted in weight loss and death of the animals in an average of 42 days. Lengthy fistulas between the stomach and colon caused no obvious disturbances in the physiology of the gastrointestinal tract. The significant microscopic changes included destruction of the mucous membrane and a decrease in size of the villi. The other noteworthy observation was that fistulous aggregations …

Therapy Of Mycosis Fungoides, Harvey G. Shapero

Mycosis fungoides is best viewed as a cutaneous lymphoma. Over a period of years, it progresses through three stages—the first consisting of red scaly patches, the second of pruritic plaques, and the third of brownish-red tumors. The therapy of each stage is outlined, and several selected modalities are discussed in detail, including topical steroids, systemic steroids, localized superficial irradiation, topical nitrogen mustard, electron beam, and chemotherapy (alkylating agents and antimetabolites.

Diagnosis And Treatment Of Gonorrhea, Paul A. Kvale

The various methods for diagnosis of gonorrhea are reviewed. Gram-stained smear of urethral exudate in the mate is appropriate, but for the female emphasis is placed on the use of culture on Thayer-Martin medium. Currently effective modalities of treatment are detailed, pointing out that higher doses of antibiotics are generally required for females with pelvic inflammatory disease than for males with acute gonococcal urethritis. The necessity for follow-up is also discussed.

Cytological detail is well demonstrated in an improved method of staining aspirated bone marrow sections embedded in paraffin. The tinctorial patterns of the stain resemble some of its predecessors; thus, cell counting and identification involve little difficulty. The method has proven reliable, reproducible and a valuable addition in the diagnosis of bone marrow disorders. It is a useful tool in research problems in which objectives are the study of the bone marrow.

The problem with which this study was concerned was the development of criteria that could be used to determine when urinary catheter changes were necessary. By determining when encrustation occurred, it was felt that some complications could be eliminated by changing catheters before obstruction occurred due to encrustation.

It was the purpose of this study to: 1) explore some indications for catheter change, 2) determine if encrustation of urinary catheters necessitates catheter change, and 3) estimate potential criteria for a routine time for catheter changes.

Infiltration to an injury site or area of bacterial contamination by polymorphonuclear leucocytes (or neutrophils) is an early and essential phase of the early inflammatory reaction. Their action on foreign matter is the most critical step taken by body defense mechanisms in the first 24 hours. Anything which interferes with that action, phagocytosis, greatly hampers early inflammation. This study was designed to see the effect of elevated glucose levels on neutrophil phagocytosis in a unique strain of genetically diabetic mice.

In 98 patients with primary hyperparathyroidism, 15 had objective evidence of duodenal ulcer disease. Following operative correction of the hyperparathyroidism, improvement of duodenal ulcer disease occurred usually, but not uniformly, if unassociated with non-beta islet cell tumors. Correction of hyperparathyroidism in patients with the Zollinger-Ellison syndrome does not significantly influence the peptic ulcer which should be given primary aggressive surgical treatment.

Australia antigen and antibody were assayed in acute and convalescent (1, 4 and 6 months) phase sera from 80 children with infectious hepatitis in an institution for the mentally retarded. A higher incidence of Australia antigenemia was found in 18 of 40 (45%) mongoloid children as compared to 9 of 40 (23%) non-mongoloid children. Australia antigen persisted 7 years after the hepatitis epidemic in 11 mongoloid and 2 non-mongoloid children with hepatitis. None of the hepatitis group, with or without Australia antigenemia, demonstrated precipitable antibody to the Australia antigen. Because of theoretical as well as practical difficulty in the total hip endoprostheses available to orthopaedic surgeons at the present time, an anlysis of the present and potential difficulties reveals that substitution of a roller-bearing for the present slider-bearing prosthesis and the establishment of a larger end-bearing surface on the acetabular side would be of benefit. Thus, the Ring prosthesis was re-designed to satisfy these hopes. A modification of the Ring's approach gives complete acetabular exposure and control of the prosthetic insertion. A comparison is made between incidence of fever in 132 patients with various types of disseminated cancer and 57 patients with diseases other than cancer during a 2- month observation period. Fifty-three febrile episodes were observed in cancer patients. Of these, 15 were of undetermined origin and 6 were noninfectious; none of the episodes occurred immediately following surgery. In the control group of 57 patients, there were 12 febrile episodes; of these, 7 were due to proven bacterial infection, 2 were due lo possible bacterial infection, and 3 were of undetermined etiology. Hemagglutination-inhibition (HA-I) titres for rubella were studied in 200 nurses, 87 pregnant women, in plasma and serum pools, and in laboratory personnel. The percentage of individuals susceptible to rubella in the child-bearing ages was 11.5%> among the non-pregnant nurses and 23% among the pregnant women assayed at their first prenatal visit. Serodiagnosis of active rubella necessitates two or more serial serum studies during the first trimester of pregnancy.

The replica method aiding in rectal cytology is described and a modification of the slide applicator is recommended. Used in 75 patients, this replica method is a safe and useful tool for the diagnosis and followup of inflammatory bowel disease as well as for research purposes.

A patient is reported with Klinefelter's syndrome, systemic lupus erythematosus (SLE) and acquired cutaneous hepatic porphyria. Although patients with chromatin positive Klinefelter's syndrome may have elevated auto-antibodies, this patient had definite diagnostic criteria for SLE. Furthermore, evidence of disturbed porphyrin metabolism compatible with acquired cutaneous hepatic porphyria was found. It is speculated that the major disturbance in our patient's porphyrin biosynthesis may be distal to ^-ALA synthetase. Any association between the three diseases is conjectural: however, it is noteworthy that both SLE and porphyria are more common in the female and that our patient had two X chromosomes in addition …

Systemic immune status was studied in 86 idiopathic ulcerative colitis patients. Quantitation of the major serum immunoglobulins (IgG, IgA and IgM) and qualitation for specific enteropathogens (bacterial and viral) associated with the immunoglobulins are correlated with the severity and duration of the disease. The studies demonstrate the immunologic competence of these patients to respond to a variety of infections with normal activity and hyperactivity. In addition, systemic complications associated with ulcerative colitis, particularly liver involvement, are reflected in the immunologic studies.

Dose-Response Relationship Of Bile Flow To Cholecystokinin Stimulation As Measured By Duodenal Drainage, Robert J. Priest, Roberto F. Gluckmann, J. Edward Berk, Joseph A. Rinaldo

A study was done to evaluate the dose-response of cholecystokinin administered intravenously to normal patients and to patients with diseased gallbladders. The volumes of bile and bilirubin units were measured in duodenal aspirates. A dosage of cholecystokinin between 12.5 and 25 Ivy dog units differentiated the several categories studied. Lower doses of cholecystokinin resulted in no change in the volume of bile, but significantly reduced the output of bilirubin units.
