The development of graft atherosclerosis in heart transplant recipients has been associated with the development of post-transplant cytomegalovirus infection. The mechanism of this involvement, however, remains unclear. I undertook a study to determine the role of cytomegalovirus in the development of graft atherosclerosis. First, a rapid method of detecting cytomegalovirus based on capillary polymerase chain reaction and gel electrophoresis was employed to determine the presence of cytomegalovirus in a wide variety of clinical samples. Detection of cytomegalovirus is a lengthy procedure under normal conditions, and timely detection of cytomegalovirus in transplant recipients may allow intervention early enough to block the …

BACKGROUND: Coronary-stent placement is a new technique in which a balloon-expandable, stainless-steel, slotted tube is implanted at the site of a coronary stenosis. The purpose of this study was to compare the effects of stent placement and standard balloon angioplasty on angiographically detected restenosis and clinical outcomes.

METHODS: We randomly assigned 410 patients with symptomatic coronary disease to elective placement of a Palmaz-Schatz stent or to standard balloon angioplasty. Coronary angiography was performed at base line, immediately after the procedure, and six months later.

RESULTS: The patients who underwent stenting had a higher rate of procedural success than those who …

Retrospective analysis of 45 patients (33 females, 12 males) with cytologically-proven malignant ascites is presented. Abdominal pain was the most frequent symptom (69%). Fiftythree percent cases had low serum albumin. Ascitic fluid was haemorrhagic or serosanguinous in 48% cases, in the rest it was clear or straw-coloured. Peritoneal effusion was exudative in 84% cases. Mean glucose content of ascitic fluid was 95 mg/dl and the mean white cell count of 919 cells/cmm. Vast majority (82%) of the cases had metastatic adenocarcinomas. Primary malignancy was mostly ovarian (47%) followed by non-Hodgkin's lymphoma (11%) and gall bladder carcinoma (9%). In vitro studies suggest that neonates and adults may have different cardiac chronotropic responses to a-adrenergic stimulation. To investigate these differences in vivo, three groups of dogs were studied. Group I = 12 puppies, ages 3-7 days; group II = 12 puppies ages 8-15 days, and group III = seven adult dogs. Heart rate and blood pressure determinations were made in the control setting and then after combined β-adrenergic and parasympathetic blockade (propranolol 0.6 mg/kg and bilateral vagotomies). Alpha-stimulation was then achieved with phenylephrine given in doses of 0.5, 1.0, and 10.0 μg/kg/ min. A second, high dose of propranolol …

Previous studies have suggested that the atrioventricular nodal functional refractory period in the neonate is equal to or shorter than that of the ventricle, providing little or no protection to the ventricle against rapid atrial rates and allowing closely coupled atrial beats to fall within the ventricular vulnerable period. We evalu­ated atrioventricular node function in 21 mongrel neonatal puppies, 3-15 days old, and 15 adult dogs utilizing intracardiac His bundle recording and stimulation techniques. The mean atrioventricular nodal functional refractory period (173.1 ± 20.0 ms) exceeded both the ventricular effective refractory period (139.5 ± 14.3) and ventricular functional refractory period …

Ectopic origin of a coronary artery from the aorta is uncommon. In the last decade, its importance as a possible cause of sudden death has been documented. Initially, only the left coronary artery was implicated. Lately, the ectopically arising right coronary artery has also been shown to be involved in cases of sudden death. We reviewed the pathologic anatomy in 23 cases of ectopic origin of a coronary artery from the aorta. In three of these cases, death could be attributed to ectopic origin of a coronary artery. The effects of quinidine on steady state serum and tissue digoxin concentrations in the neonatal dog were studied. To determine the effects of quinidine on serum digoxin concentrations, two groups of neonates were evaluated: Group I (n = 11) was digitalized with 40 μg/kg body weight, intramuscularly, and placed on a 10 μg/kg per day maintenance dose; Group II (n = 7) was digitalized with 50 μg/kg per day, intraperitoneally, and placed on a 20 μg/kg per day maintenance dose. After 10 days of digoxin alone, quinidine was coadministered (30 mg/kg per day, intraperitoneally) for 7 days. Serum digoxin concentrations …

Maternal cardiovascular adaptations appear to be essential in order to supply extra circulation to both the developing fetus and maternal system during a successful pregnancy. Since inadequate cardiovascular adjustments may produce abortions or maternal morbidity, the characterization of maternal cardiovascular changes during pregnancy and the elaboration of the underlying mechanism for these changes are essential to the understanding of how the heart enlarges during pregnancy and what significance this enlargement might have. The effect of cardiac rehabilitation on blood lipids was studied in a sample of seventeen male participants of the Greenville Hospital Center cardiac rehabilitation program. These subjects all suffered myocardial infarctions, but none had surgery to by-pass the obstructed coronary circulation. Paired values were obtained for triglyceride, serum cholesterol and HDL cholesterol before, and after the exercise program. The time span between the before-exercise and after-exercise values ranged from five to thirty-one weeks. The differences in the paired values were analyzed by the t-test. No significant decrease occurred in triglyceride or serum cholesterol. A significant increase occurred in HDL cholesterol.

A computer model of the left ventricular (LV) epicardial surface was developed from Jang’s truncated ellipsoidal model, which has a minor to major axis ratio of 6:10. The model was applied in calculating the area of a segment of the myocardium, jeopardized by an impending occlusion of its associated artery. The endangered segment of the LV muscle was also visualized in different projections by rotations in three space and graphical displays simulating the three dimensions.

The purpose of this study was to find out if men with myocardial infarction followed exercise prescribed by the physician. The reasons the patients did or did not follow the exercise prescription were investigated.

An interview guide was used to gather the data for this exploratory study. The questions on the interview guide were designed to obtain information to compare the former exercise pattern of the patient to exercise after infarction. In the academic year 1959-1960 the National Science Foundation supported an undergraduate research program at Western: Michigan University for the spectrographic analysis of human hair. This preliminary work concerned itself with determining the quantities of magnesium, iron, and calcium in human hair. The hair samples were taken from a supposedly normal cross section of people. This group consisted of 41 women and 52 men. The ages varied from 2 to 72. The main method consisted of crushing living veins in the mesenteries of the small intestines of frogs and rats and, with horizontally aimed microscopes: 1. Surgery on the heart and paracardial structures or traction on the hilus of the lung may initiate serious arrhythmias which can result in death if not promptly controlled. Procedures involving the pericardium are particularly apt to be followed by severe derangements of rhythm. Thus anesthesiology is faced with another challenge.

It is now evident that the intravenous administration of procaine has become a valuable addition to the armamentarium of the anesthesiologist in his endeavor to preserve the integrity of the thread of life while the surgeon carries out his heroic assignment without restrictions.
