Bacteriophage 80α is a temperate double-stranded DNA phage that infects Staphylococcus aureus and is capable of mobilizing a variety of mobile genetic elements named S. aureus Pathogenicity Islands. The capsid of the mature virion is composed of subunits of the capsid protein, portal protein, and a minor capsid protein gp44 as well as packaged phage genomic DNA. The loss of the gp44 protein completely eliminates the phage titer of 80α but does not eliminate the transduction of the pathogenicity islands. Previously, abnormal nucleoli were noted as indicators of cancerous lesions andused as a pathologic readout of neoplasia. Nucleolar hypertrophy and increased nucleolar number have been correlated with increased breast cancer (BC) mortality rates. Although various functions of the nucleolus have been investigated, it remains widely understudied in the context of cancer. The nucleolus was initially thought to be solely the site of ribosome biogenesis. However, over half of the proteins found in the nucleolus are associated with other biological functions. The tumor suppressor Merlin is encoded by Neurofibromin 2 (NF2) gene. Merlin is predominantly located in the cell cortex where regulates cell proliferation by mediating cell contact-dependent growth inhibition. Metastatic breast cancer tissues presented with a remarkable loss of Merlin protein, revealing clinical relevance of Merlin. In order to examine the cellular effect of Merlin deficiency, breast cancer cell lines were silenced for NF2. Additionally, to assess the impact of Merlin loss at the organismal level, a mammary-specific NF2 knockout mouse mammary tumor model was engineered. Endogenous antioxidant responses defend the lung against oxygen toxicity. In premature neonates, oxygen toxicity and impaired antioxidant defenses contribute to the development of bronchopulmonary dysplasia (BPD), the leading cause of long-term morbidity among premature infants. In adults, oxygen toxicity and impaired antioxidant defense contributes to the pathogenesis of acute lung injury (ALI). Acute Respiratory Distress Syndrome (ARDS), a severe form of ALI, can cause permeant lung damage and mortality. Our lab has established that pharmacologic thioredoxin reductase-1 (TXNRD1) inhibition attenuates lung injury in murine BPD and ALI models. Immune dysregulation is a hallmark of various chronic lung diseases such as Chronic Obstructive Pulmonary Diseases (COPD) and Idiopathic Pulmonary Fibrosis (IPF). Previous studies in our lab have shown evidences of autoimmunity such as increases in autoantibodies, abnormal T cell profile, immune complex deposition and the presence of ectopic lymphoid structures in the lung of these patients. This thesis consolidates two separate projects about various aberrant immune responses in these diseases. The first project identified Glucose-Regulated-Protein 78 (GRP78) as the common autoantigen in COPD and characterized how GRP78 autoantibodies may increase COPD mortality via its atherogenic effects. C-reactive protein (CRP) is an evolutionarily conserved pattern recognition molecule that was first characterized for its participation in the acute phase response to bacterial infection. In the last 90 years, knowledge of CRP biology in innate immunity has expanded significantly. CRP is known to bind and activate the classical pathway of complement, to opsonize bacteria and inflamed tissue, and to modulate myeloid cell functions. CRP is normally present in the blood at low levels with its biosynthesis and serum concentration rapidly rises upon systemic inflammation. Glioblastoma (GBM) is the most common adult primary malignant brain tumor with a median survival of about 15 months, even after aggressive treatment. Treatment of GBM is difficult for multiple reasons including the location of the tumor, tumor invasiveness, and the high degree of both inter-and intra-tumoral heterogeneity. To combat this, new drug screening methodologies are required since the traditional drug screens are offering limited results or only improvements upon current treatments. The COVID-19 pandemic represents a major challenge to both technologically advanced and resource-poor countries. There are currently no effective treatments for severe disease other than supportive care and advanced life support measures, including the use of mechanical ventilators. Carbapenem-resistant Klebsiella pneumoniae (CRKP) has been increasing reported worldwide. The aims of this study were to investigate the mechanisms of carbapenem resistance in K. pneumoniae Thai isolates and study the synergistic activities of antibiotic combinations against CRKP isolates. Two-hundred and forty CRKP clinical isolates were obtained from patients at King Chulalongkorn Memorial Hospital between September 2016 and April 2020. The antibiotic susceptibility testing was determined by agar dilution to imipenem, meropenem, amikacin, fosfomycin, ciprofloxacin and by broth microdilution to colistin. Since current multiplex PCR cannot differentiate between serotypes 2 and 1/2, then serotype-specific antibodies (Abs) are required for serotype identification to confirm S. suis serotype 2 infection. This study aimed to generate Abs specific to S. suis serotype 2 by phage display from a human heavy chain variable domain (VH) antibody library. However, molecular mechanisms of stress response in Leptospira are not fully understood. Carbon Storage Regulator A (CsrA) is a well-characterized, conserved, post-transcriptional global regulator that plays a critical role in response to environmental changes in many bacteria. Colorectal cancer (CRC) is genetically and transcriptomically heterogeneous disease. Molecular subtyping of colorectal cancer using consensus molecular subtype (CMS) system demonstrated the potential predictive value for tumor progression and treatment response. However, the CMS system was developed from data of whole tissues containing both cancer and non-tumor transcripts components for classification which does not directly represent intrinsic heterogeneity of cancer cells. การศึกษาวิจัยเชิงทดลองแบบ non-concurrent controlled intervention study มีวัตถุประสงค์เพื่อศึกษาประสิทธิผลของโปรแกรมการฝึกสติบำบัด (MBTC : Mindfulness – Based Therapy and Counseling program : MBTC) ร่วมกับกระบวนการบำบัดฟื้นฟูด้วยโปรแกรม FAST model เปรียบเทียบกับกระบวนการบำบัดฟื้นฟูด้วยโปรแกรม FAST model ซึ่งเป็นโปรแกรมที่ใช้อยู่ในปัจจุบันต่อการกลับมาเสพสารเมทแอมเฟตามีนซ้ำ ผู้ติดสารเมทแอมเฟตามีนที่เข้ารับการบำบัดฟื้นฟูจำนวนทั้งสิ้น 70 คน ที่มีคุณสมบัติครบตามเกณฑ์คัดเข้าและคัดออกถูกแบ่งเป็นกลุ่มควบคุม (กลุ่มที่ได้รับการบำบัดตามกระบวนการบำบัดของโปรแกรม FAST Model) และกลุ่มทดลอง (กลุ่มที่ได้รับการบำบัดด้วยโปรแกรม MBTC ร่วมกับกระบวนการบำบัดของโปรแกรม FAST Model) กลุ่มละ 35 คน กลุ่มทดลองจะได้รับการฝึกสติเป็นเวลา 8 สัปดาห์ สัปดาห์ละประมาณ 1 ชั่วโมง และทั้งสองกลุ่มจะถูกติดตามหลังจำหน่ายกลับบ้านเป็นระยะเวลา 6 เดือน ผลการวิเคราะห์ข้อมูลด้วยสถิติ Generalized mixed model พบว่า เมื่อติดตามผลเป็นระยะเวลา 6 เดือนหลังจำหน่ายกลับบ้าน กลุ่มทดลองมีคะแนนเฉลี่ยความอยากเสพลดลงมากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (7.89 คะแนน, 95%CI = -15.47, -0.32) และพบว่ากลุ่มทดลองมีสัดส่วนของผู้ที่ตรวจพบสารเมทแอมเฟตามีนในปัสสาวะ (5/35 คน) น้อยกว่าสัดส่วนของกลุ่มควบคุม (16/35 คน) อย่างมีนัยสำคัญทางสถิติ (chi-square test; p-value = 0.004) และยังพบว่ากลุ่มทดลองมีความเครียด (7.44 คะแนน, 95%CI = -12.21, -2.67) ความซึมเศร้า (2.96 คะแนน, 95%CI = -5.31, -0.6) น้อยกว่ากลุ่มควบคุม ในขณะที่มีสติมากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (12.86 คะแนน, 95%CI = 9.37, 16.35) จากผลการศึกษาที่ได้นี้สามารถสรุปได้ว่า มีความเป็นไปได้ว่าการใช้โปรแกรม …

ที่มาของปัญหา: ประเทศไทยเป็นเมืองเกษตรกรรม มีการผลิตข้าวเพื่อบริโภคภายในและส่งออก ปัจจุบันมีการเร่งเพิ่มผลผลิต จึงมีการใช้สารเคมีและเกิดผลต่อสุขภาพมากขึ้น เพื่อบรรเทาภาวะนี้ จึงมีการใช้เกษตรอินทรีย์มาเป็นทางเลือกมากขึ้น วัตถุประสงค์: เพื่อศึกษาเปรียบเทียบความชุกของปัญหาสุขภาพและคุณภาพชีวิตทางสุขภาพระหว่างชาวนาที่ทำเกษตรอินทรีย์และชาวนาที่ทำเกษตรเคมีในจังหวัดสุรินทร์ วิธีการศึกษา: เป็นการศึกษาแบบ Cross-sectional analytic study ประกอบด้วยกลุ่มชาวนาที่ทำเกษตรอินทร์จำนวน 364 ราย และกลุ่มชาวนาที่ทำเกษตรเคมี 345 ราย ที่ทำการปลูกข้าวในพื้นที่จังหวัดสุรินทร์อย่างน้อย 1 ปี เก็บข้อมูลปัญหาสุขภาพช่วง 3 เดือน และ 1 ปีที่ผ่านมาและคุณภาพชีวิตทางสุขภาพด้วยแบบสอบถาม ทำการวิเคราะห์ทางสถิติด้วย Chi-square และ Binary logistic regression ผลการศึกษา: เมื่อเปรียบเทียบกับกลุ่มชาวนาที่ทำเกษตรอินทรีย์ พบว่า ความชุกของอาการท้องเสีย (adjusted OR=3.10, p-value 0.04) และปวดกล้ามเนื้อ (adjusted OR=2.19, p-value <0.01) ในกลุ่มชาวนาที่ทำเกษตรเคมีสูงกว่าอย่างมีนัยสำคัญทางสถิติ และคุณภาพชีวิตทางสุขภาพในกลุ่มชาวนาที่ทำเกษตรเคมีสูงกว่ากลุ่มชาวนาเกษตรอินทรีย์อย่างมีนัยสำคัญทางสถิติ สรุป: เพื่อบรรเทาผลกระทบทางสุขภาพจากการใช้สารเคมีในการปลูกข้าว ควรสนับสนุนการใช้เกษตรอินทรีย์อย่างเหมาะสม โดยคำนึงถึงความสมดุลระหว่างผลดีต่อสุขภาพและระดับคุณภาพชีวิตทางสุขภาพ

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาประสิทธิผลและประเมินต้นทุนประสิทธิผลและต้นทุนผลได้ของกิจกรรมการทาฟลูออไรด์วาร์นิชเพื่อป้องกันฟันผุ ในคลินิกเด็กดี 3 จังหวัด การศึกษาประสิทธิผลเป็นการศึกษาเชิงวิเคราะห์ชนิดย้อนหลังและไปข้างหน้าในเด็กอายุ 15 - 22 เดือน กลุ่มตัวอย่างจำนวน 460 คน ได้รับการสอบถามข้อมูลปัจจัยที่มีความสัมพันธ์กับจำนวนฟันผุ ถอน อุด และวัดระดับคราบจุลินทรีย์ ดูประวัติบันทึกการเข้าร่วมกิจกรรมการทาฟลูออไรด์วาร์นิชระหว่างอายุ 9 – 30 เดือน และได้รับการสำรวจสภาวะสุขภาพช่องปากและความจำเป็นต้องได้รับการรักษาเมื่อสิ้นสุดการศึกษา และวิเคราะห์ข้อมูลโดยใช้สมการ Quadratic regression ระหว่างจำนวนครั้งที่เข้าร่วมกิจกรรมและจำนวนฟันผุ ถอน อุด และควบคุมปัจจัยที่เกี่ยวข้อง การประมาณค่าต้นทุนและผลลัพธ์ทำโดยสอบถามข้อมูลจากทันตบุคลากร ใช้ข้อมูลการจัดซื้อของโรงพยาบาล และสอบถามผู้ปกครองที่พาเด็กมาร่วมกิจกรรมการทาฟลูออไรด์วาร์นิชและพาเด็กมารับการรักษา ผลการศึกษาพบว่า เมื่อควบคุมปัจจัยที่เกี่ยวข้อง การเข้าร่วมกิจกรรมการทาฟลูออไรด์วาร์นิชมีความสัมพันธ์กับจำนวนฟันผุ ถอน อุด (p=0.036) และการเข้าร่วมกิจกรรม 1, 2, 3 และ 4 ครั้งมีจำนวนฟันผุ ถอน อุด เฉลี่ยต่ำกว่ากลุ่มที่ไม่ได้เข้าร่วมกิจกรรมอย่างมีนัยสำคัญทางสถิติ [-1.31 (95% CI: -2.39, -0.23), -2.08 (95% CI: -3.73, -0.42), -2.31 (95% CI: -4.14, -0.48) และ -2.01 (95% CI: -3.93, -0.09) ซี่ตามลำดับ] และกิจกรรมมีความคุ้มค่าโดยการวิเคราะห์ต้นทุนประสิทธิผลส่วนเพิ่มและการวิเคราะห์ผลได้ส่วนเพิ่ม ในมุมมองผู้ให้และผู้รับบริการ (ICER = -427.81 และ -416.56 ผลได้ส่วนเพิ่ม 543.32 และ 529.03 บาท ตามลำดับ) ผู้วิจัยจึงแนะนำให้มีการดำเนินกิจกรรมการทาฟลูออไรด์วาร์นิชเพื่อป้องกันฟันผุในคลินิกเด็กดีให้เด็กและผู้เลี้ยงดูเข้าร่วมกิจกรรมอย่างน้อย 3 ครั้ง ในช่วงอายุ 9 – 30 เดือน ในวันเดียวกับการมารับวัคซีน

ความชุกและปัจจัยที่เกี่ยวข้องของอาการผิดปกติของระบบกล้ามเนื้อและกระดูกโครงร่างในพระสงฆ์ไทย, ธนู นพโสภณ

วัตถุประสงค์ เพื่อศึกษาความชุกของอาการผิดปกติของระบบกล้ามเนื้อและกระดูกโครงร่าง (Musculoskeletal Discomfort; MSD) ในพระสงฆ์ ปัจจัยที่มีความสัมพันธ์ และวิธีการดูแลสุขภาพเมื่อมีอาการ วิธีการ ทำการวิจัยเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่งโดยทำการศึกษาในพระสงฆ์จังหวัดฉะเชิงเทรา ระหว่างเดือนมิถุนายน -ตุลาคม 2563 เลือกวัดอำเภอละ 2 วัด และพระอารามหลวง 2 วัด เชิญพระสงฆ์ทุกรูปเข้าร่วมงานวิจัย ผลลัพธ์หลัก คือความชุกของอาการ MSD เก็บข้อมูลโดยใช้แบบสอบถามของ Nordic ผลลัพธ์รองคือปัจจัยที่เกี่ยวข้อง และวิธีการดูแลสุขภาพเมื่อมีอาการ ผลการศึกษา มีพระสงฆ์เข้าร่วมงานวิจัยทั้งสิ้น 400 รูป อายุเฉลี่ย 42.0±16.3 ปี มีความชุกของ MSD ในรอบ 12 เดือน เท่ากับร้อยละ 88.0 โดยพบบริเวณหลังส่วนล่างมากที่สุดร้อยละ 59.3 รองลงมาคือไหล่ อาการ MSD กระทบต่อกิจวัตรประจำวัน ร้อยละ 11.5 และทำให้ต้องหยุดปฏิบัติกิจวัตรประจำวัน ร้อยละ 6.5 ทำการวิเคราะห์มูลโดยใช้สถิติ unpaired t-test , Wilcoxon rank-sum สำหรับข้อมูลเชิงปริมาณ และ chi-square สำหรับข้อมูลเชิงคุณภาพปัจจัยที่พบว่ามีอาการ MSD มากกว่าได้แก่ อายุ (p-value < 0.001) พรรษาที่บวช (p-value < 0.014) ดัชนีมวลกาย ≥ 25กิโลกรัม/ตารางเมตร (OR 2.06, 95% CI 1.13 – 3.85 ) ส่วนมากพระสงฆ์ใช้วิธีรอให้หายเองเมื่อมีอาการ MSD ร้อยละ 59.5 สรุป พระสงฆ์มีความชุกของอาการ MSD ในภาพรวมค่อนข้างสูง เกิดขึ้นกับทุกส่วนของร่างกาย ปัจจัยที่มีความสัมพันธ์กับอาการ MSD ที่มากกว่าได้แก่ อายุ พรรษาที่บวช และดัชนีมวลกาย ≥25 กิโลกรัม/ตารางเมตร ส่วนใหญ่ใช้วิธีรอให้หายเอง ควรถวายควายรู้เกี่ยวกับ MSD ทั้งสาเหตุ และวิธีป้องกัน รักษาแก่พระสงฆ์ คำสำคัญ พระสงฆ์ สุขภาพ เจ็บป่วย อาการผิดปกติของระบบกล้ามเนื้อและกระดูกโครงร่าง
