The NOD-like receptor family pyrin domain containing 3 (NLRP3) inflammasome is a critical multiprotein complex of the innate immune system that functions as a molecular sensor for a wide array of danger signals. Background: Burn injuries remain a major therapeutic challenge, with conventional treatments often failing to achieve complete functional skin regeneration. Stem cell-based interventions, including mesenchymal stem cells (MSCs) and exosome derivatives, have demonstrated regenerative and immunomodulatory potential, yet their translation to clinical practice remains limited.

Background: The gold standard for diagnosing cerebral amyloid angiopathy (CAA), full brain post-mortem examination, is rarely performed. Background: Recent studies have highlighted the relationship between gut microbiome and Alzheimer’s disease (AD), revealing the ways of how gut microbial balance impacts conditions ranging from inflammatory bowel diseases to neurological disorders. Background: Vital capacity (VC) is a fundamental indicator of pulmonary health, but its routine assessment often depends on spirometry devices that may be unavailable (i.e. because of access, cost) in many clinical or community settings. Renal osseous metaplasia is a rare condition in a native kidney with end stage renal disease showing mature bone with marrow elements in a background of chronic pyelonephritis. Although the pathogenesis is unknow, chronic ischemia and inflammation seems to be a trigger factor for the same. Osseous metaplasia has been reported in renal allograft specimen, but in this case report we present a histologically rare finding of osseous metaplasia in a native kidney.

Background: In endemic countries, Crimean-Congo hemorrhagic fever (CCHF) is an important zoonotic disease with significant public health implications. Previous studies in Iran suggests that CCHF poses a significant health risk to slaughterhouse workers and butchers. The main objecive of this study was to investigate the knowledge, attitude, and practice of butchers regarding CCHF at their workplace in an endemic area in southeastern Iran.

Background: Glucose 6-phosphate dehydrogenase (G6PD) deficiency is a genetic disorder caused by impaired enzyme function or instability due to mutations in the G6PD gene, resulting in reduced enzyme activity. Guillain-Barré Syndrome (GBS), with a global incidence of 1–2 cases per 100,000 population annually, remains the most common cause of acute flaccid paralysis. Standard treatment includes plasma exchange within 4 weeks or intravenous immunoglobulin (IVIg) within 2 weeks of symptom onset. However, IVIg may still benefit selected late-presenting cases. The canary is a small passerine bird of the finch family. Canary spermatozoa are also morphologically similar to many other songbird species. In this study, the morphological characteristics of Gloster canary spermatozoa of different age ranges were examined, and the length values of the parts forming the spermatozoa were compared. Comparative Analysis Of Gut Microbiota In Post-Mortem Chronic Alcoholics Versus Minimal To No Alcohol Users In The Thai Population, Laiba Pervez

Chronic alcohol consumption induces long-term disruption of the gut microbiota, contributing to inflammation, metabolic imbalance, and impaired intestinal integrity. This study investigated 24 postmortem gut microbiota samples from the Thai population, including 12 chronic alcoholic and 12 non-alcoholic cadavers, focusing on cecal and fecal samples to identify site-specific microbial signatures of alcohol-associated dysbiosis. Following DNA extraction, the V3–V4 regions of the 16S rRNA gene were sequenced on the Illumina MiSeq platform. Data were processed in QIIME for OTU clustering and taxonomic profiling. Alpha (Shannon, Chao1, ACE) and beta (Bray–Curtis) diversity were analyzed with PERMANOVA (p< 0.05) while LEfSe, cladogram, and STAMP analyses identified differentially abundant taxa.. At the phylum level, alcoholic cecal samples (AC) showed a decrease in Firmicutes (59.07%) and increases in Proteobacteria (5.54%), Fusobacteria (8.48%), and Actinobacteria (2.78%) compared with controls (NAC). The rise in Actinobacteria was statistically significant. The class Bacilli was also elevated, reflecting a more oxygenated and inflammatory intestinal environment. At the genus level, Paeniclostridium was enriched in alcoholics, while the butyrate-producing Coprococcus was reduced, suggesting impaired SCFA metabolism and gut barrier function. In fecal samples (AF vs NAF), phylum-level differences were minimal, although Clostridium sensu stricto 1 was notably higher in alcoholics, consistent with inflammation-associated dysbiosis. When comparing gut regions, distinct spatial heterogeneity was observed. Within the alcoholic group, Firmicutes were more abundant in cecum, while Actinobacteria were lower compared to feces. Among controls, Actinobacteria and Bacteroidetes were enriched in feces, whereas Firmicutes predominated in the cecum. Alpha diversity analysis using ACE and Chao1 indices showed significant differences between cecal and fecal samples of the control group. Overall, postmortem gut microbiota retained distinct alcoholassociated signatures, with the cecum preserving clearer microbial patterns than feces. These findings highlight the cecum’s potential as a reliable site for detecting alcohol-related dysbiosis in forensics.

A previous study developed a messenger RNA (mRNA) vaccine encoding the carboxy-terminal domain 7-13 of LigA (LigAc) that was expressed with glycosylation in mammalian cells and elicited high humoral immune response in a hamster model but failed to provide protection against lethal challenge. การศึกษานี้เป็นการวิเคราะห์เชิงภาคตัดขวาง มีวัตถุประสงค์เพื่อประเมินความเหลื่อมล้ำและปัจจัยที่มีความสัมพันธ์กับการเข้าถึงการรักษาด้วยการฉายรังสีอัลตราไวโอเลตของผู้ป่วยโรคสะเก็ดเงินในประเทศไทย ปี พ.ศ. 2566 การศึกษาประกอบด้วย 2 ส่วน ได้แก่ (1) การวิเคราะห์ข้อมูลทุติยภูมิจากฐานข้อมูลการใช้บริการสุขภาพของสำนักงานหลักประกันสุขภาพแห่งชาติ ระบบสวัสดิการรักษาพยาบาลข้าราชการ และระบบประกันสังคม รวมทั้งสิ้น 202,776 ครั้ง จากผู้ป่วย 49,219 ราย และ (2) การสำรวจจำนวนแพทย์ผิวหนังและเครื่องฉายรังสีอัลตราไวโอเลตในสถานพยาบาลระดับตติยภูมิ ครอบคลุม 13 เขตสุขภาพทั่วประเทศ ผลการศึกษาพบว่า ผู้ป่วยโรคสะเก็ดเงินระดับปานกลางถึงรุนแรงได้รับการรักษาด้วยการฉายรังสีอัลตราไวโอเลตเพียงร้อยละ 3.28 ขณะที่ร้อยละ 96.11 ได้รับการรักษาด้วยยารับประทาน และร้อยละ 0.61 ได้รับยาชีวโมเลกุล นอกจากนี้ยังพบความเหลื่อมล้ำในการเข้าถึงบริการตามสถานะเศรษฐกิจและสังคม โดยกลุ่มที่มีระดับรายได้สูงมีโอกาสเข้าถึงการรักษาด้วยการฉายรังสีอัลตราไวโอเลตมากกว่ากลุ่มรายได้ต่ำ (Concentration Index = 0.215) ปัจจัยที่มีความสัมพันธ์กับการเข้าถึงบริการอย่างมีนัยสำคัญทางสถิติ ได้แก่ กลุ่มอายุ ชนิดของโรคสะเก็ดเงิน สิทธิการรักษาพยาบาล พื้นที่ที่พักอาศัย สถานะเศรษฐกิจและสังคม และการกระจายทรัพยากรด้านบริการสุขภาพ โดยเฉพาะพื้นที่ที่มีทั้งแพทย์ผิวหนังและเครื่องฉายรังสีอัลตราไวโอเลต เพิ่มโอกาสในการเข้าถึงการรักษามากขึ้นถึง 2.71 เท่า เมื่อเทียบกับพื้นที่ที่ไม่มีทรัพยากรดังกล่าว (OR = 2.71; 95% CI: 1.06-6.94) ข้อค้นพบจากการศึกษานี้สะท้อนถึงความจำเป็นในการกำหนดนโยบายด้านสุขภาพที่มุ่งลดความเหลื่อมล้ำในการเข้าถึงบริการ โดยเน้นการกระจายทรัพยากรบุคลากรทางการแพทย์และเครื่องมือเฉพาะทางอย่างเหมาะสมและทั่วถึง ควบคู่กับการพัฒนาแนวทางการส่งต่อผู้ป่วยที่มีความครอบคลุม เป็นธรรม และสอดคล้องกับบริบทของระบบหลักประกันสุขภาพของประเทศไทย

งานวิจัยนี้เป็นการวิจัยเชิงพรรณนาเชิงภาคตัดขวาง มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่เกี่ยวข้องกับความร่วมมือในการใช้ยาและระดับความร่วมมือในการใช้ยาในผู้ป่วยวัยทำงานที่เป็นโรคความดันโลหิตสูงที่ไม่มีภาวะแทรกซ้อน ณ โรงพยาบาลตติยภูมิแห่งหนึ่งในกรุงเทพมหานคร กลุ่มตัวอย่างทั้งหมดจำนวน 402 คน คัดเลือกโดยวิธีสุ่มตัวอย่างอย่างเป็นระบบ เก็บข้อมูลในช่วงเดือน พฤษภาคม ถึง มิถุนายน พ.ศ. 2568 แบบสอบถามประกอบด้วยข้อมูลปัจจัยส่วนบุคคล, ปัจจัยด้านการใช้ยา, ปัจจัยด้านการทำงาน และความร่วมมือในการใช้ยา ซึ่งได้จากแบบประเมิน Medication Adherence Questionnaire-20 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและสถิติเชิงอนุมาน ประกอบด้วยการวิเคราะห์ความสัมพันธ์แบบสองตัวแปร และการวิเคราะห์ความสัมพันธ์แบบหลายตัวแปร ด้วยสถิติการถดถอยโลจิสติกเชิงพหุ จากผลการศึกษาพบว่ากลุ่มตัวอย่างมีความร่วมมือในการใช้ยาในระดับเพียงพอร้อยละ 89.80 เมื่อควบคุมปัจจัยอื่นที่มีอิทธิพลแล้ว พบว่า ปัจจัยที่มีความสัมพันธ์ในทิศทางตรงข้ามกับความร่วมมือในการใช้ยามีดังนี้ ระยะเวลาในการป่วยเป็นโรคความดันโลหิตสูง (6-10 ปี) การสูบบุหรี่ วิธีการลดการบริโภคเกลือ โดยการเลือกซื้ออาหาร และเวลาในการรับประทานอาหารไม่สม่ำเสมอในวันทำงาน ผลการศึกษานี้ชี้ให้เห็นว่าการดูแลผู้ป่วยโรคความดันโลหิตสูงควรมีโปรแกรมหรือระบบที่ออกแบบเฉพาะสำหรับผู้ป่วยที่มีระยะเวลาป่วยนาน ซึ่งโปรแกรมเหล่านี้ควรแตกต่างจากโปรแกรมสำหรับผู้ป่วยใหม่ นอกจากนี้ ผู้ให้บริการสุขภาพควรให้ความสำคัญกับการสนับสนุนการเลิกบุหรี่ และเน้นย้ำถึงผลกระทบจากการสูบบุหรี่ต่อการควบคุมความดันโลหิต รวมถึงการให้ความรู้แก่ผู้ป่วยเกี่ยวกับปริมาณโซเดียมที่แฝงในอาหารแปรรูปและวิธีการอ่านฉลากโภชนาการอย่างถูกต้อง และควรจัดเตรียมแผนการใช้ยาที่ยืดหยุ่นและคำแนะนำในการจัดการกับกิจวัตรประจำวันเพื่อเพิ่มความร่วมมือในการใช้ยาและผลลัพธ์ด้านสุขภาพที่ดีขึ้น

ที่มา: ความรุนแรงต่อเด็กยังเป็นปัญหาสาธารณสุขสำคัญ หลักสูตร ParentChat ถูกพัฒนาขึ้นเพื่อป้องกันความรุนแรงต่อเด็กและส่งเสริมการเลี้ยงดูเด็กเชิงบวกโดยใช้รูปแบบผสมผสานผ่านออนไลน์และกระบวนการกลุ่ม การวิจัยมีวัตถุประสงค์: เพื่อประเมินประสิทธิผลของหลักสูตรต่อการลดการกระทำรุนแรงต่อเด็ก และผลลัพธ์ที่เกี่ยวข้องกับการเลี้ยงดูและสุขภาวะครอบครัว โดยวิธีการ: การทดลองแบบสุ่มระดับคลัสเตอร์ในโรงพยาบาลส่งเสริมสุขภาพตำบล 12 แห่ง ในจังหวัดอุดรธานี ผู้เข้าร่วมคือพ่อแม่หรือผู้ดูแลของเด็ก 2–17 ปี จำนวน 240 คน สุ่มเข้ากลุ่มทดลองและกลุ่มควบคุมในอัตรา 1:1 ประเมินผลโดยสัมภาษณ์แบบพบตัวในช่วงเริ่มต้นและหลังจบโครงการ 1 เดือน และวิเคราะห์ข้อมูลด้วย Generalized Estimating Equations (GEE) ที่ปรับตามชนิดของตัวแปร (Poisson และ Linear models) ผลการศึกษา: การวิเคราะห์เชิงพรรณนาพบแนวโน้มการลดลงของการกระทำรุนแรงต่อเด็กในทั้งสองกลุ่ม อย่างไรก็ตาม การวิเคราะห์เชิงอนุมานหลังสิ้นสุดโครงการ พบว่ากลุ่มทดลองมีอัตราการกระทำรุนแรงทางร่างกายลดลงร้อยละ 54 เมื่อเทียบกับกลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (Adjusted IRR = 0.46; 95% CI: 0.23, 0.88) ขณะที่มิติด้านการกระทำรุนแรงทางจิตใจพบการลดลงในกลุ่มควบคุมที่เด่นชัดกว่ากลุ่มทดลอง (Adjusted IRR = 2.41; 95% CI: 1.24, 4.67) และไม่พบความแตกต่างของการกระทำรุนแรงในภาพรวมอย่างมีนัยสำคัญ (Adjusted IRR = 1.15; 95% CI: 0.71, 1.86) สำหรับผลลัพธ์รอง ส่วนใหญ่ไม่พบความแตกต่างอย่างมีนัยสำคัญ ยกเว้นความรุนแรงของพฤติกรรมเด็กที่ผู้ดูแลระบุว่าเป็นปัญหามากที่สุดซึ่งลดลงอย่างมีนัยสำคัญทางสถิติ (coefficient = -0.51; 95% CI: -1.01, -0.01)สรุปผล: ในระยะสั้น หลักสูตรสามารถลดการกระทำรุนแรงทางร่างกายและความรุนแรงของพฤติกรรมเด็กที่เป็นปัญหาได้ แต่ยังไม่พบการเปลี่ยนแปลงในมิติด้านการกระทำรุนแรงทางจิตใจและภาพรวม ผลการศึกษาสะท้อนข้อจำกัดจากประชากรที่มีระดับความเสี่ยงตั้งต้นต่ำ ซึ่งอาจลดความไวในการตรวจจับการเปลี่ยนแปลงของเหตุการณ์ และข้อจำกัดเกี่ยวกับหลักสูตรที่ต้องการวิจัยเพิ่มเติม

Abstract:The human gut microbiome undergoes pronounced shifts following death; however, its early postmortem dynamics remain insufficiently understood. This study characterizes microbial diversity in cecal samples collected from Thai cadavers during the early postmortem interval (PMI) to establish foundational data for forensic applications. Samples obtained within two PMI ranges (1-6 hours and 6-12 hours) were subjected to 16S rRNA gene sequencing targeting the V3–V4 regions on the Illumina MiSeq platform. Background : Human immunodeficiency virus (HIV) causes acquired immunodeficiency syndrome (AIDS), which affects the immune system of individuals who are living with it. Syphilis, a sexually transmitted infection caused by the bacterium Treponema pallidum, causes a spectrum of clinical manifestations from asymptomatic to long-term sequelae. Both diseases can be vertically transmitted if left undetected and untreated. Routine testing for HIV and syphilis in all pregnant women during antenatal care consultation (ANC) is recommended. Major Depressive Disorder (MDD) is associated with immune dysregulation, yet the combined effects of T-cell activation, T regulatory (Treg) cell deficits, and the failure of immune checkpoint mechanisms on illness severity remain unclear. This research investigated these factors in patients with acute MDD compared with healthy participants. Using multi-parameter flow cytometry, we analyzed T-cell markers of activation (CD40L, CD71), regulation (CD152, GARP), apoptosis (CD95), and exhaustion (TIM3) under unstimulated and stimulated conditions; cytokine levels were also measured. Human head lice (Pediculus humanus capitis) are medically significant ectoparasites due to their capacity to harbor pathogenic bacteria and the increasing prevalence of insecticide resistance. A total of 112 head lice specimens were collected from children in Ang Thong, central Thailand. Phylogenetic analysis of the mitochondrial cytb gene identified two well-supported clades, with clade A (69.2%) predominating over clade C (30.8%) in the studied population. Molecular screening revealed a high incidence of bacterial associations, with Staphylococcus aureus detected in 96.4% of specimens and Acinetobacter spp. in 2.7%. The 2024 Doctor of Medicine curriculum at Chulalongkorn University introduced a new assessment system comprising outcome-based assessment system with milestones, a non-tiered grading model with recognition of excellence, a professional development dashboard (PDD), remediation with re-examination, and a faculty-level student profile (FLSP). This study employed a quantitative cross-sectional survey design, complemented by a sequential exploratory element, to compare student and faculty perspectives using a questionnaire across five themes, analyzed with descriptive statistics, Mann-Whitney U tests, and effect sizes (ES).
