With the soaring adoption of in-ear wearables, the research community has started investigating suitable in-ear heart rate detection systems. Heart rate is a key physiological marker of cardiovascular health and physical fitness. Continuous and reliable heart rate monitoring with wearable devices has therefore gained increasing attention in recent years. Existing heart rate detection systems in wearables mainly rely on photoplethysmography (PPG) sensors, however, these are notorious for poor performance in the presence of human motion. OBJECTIVES: Evaluate a digital health intervention using Auricular Point Acupressure (APA) for chronic musculoskeletal pain in terms of participant retention, adherence, acceptability, and satisfaction. Chronic musculoskeletal pain is a global concern and there are persistent challenges in pain management. Despite the value of digital health interventions, these interventions need to be fully evaluated for feasibility.

This dissertation explores applications of representation learning and generative models to challenges in healthcare, astronautics, and aviation.

The first part investigates the use of Generative Adversarial Networks (GANs) to synthesize realistic electronic health record (EHR) data. An initial attempt at training a GAN on the MIMIC-IV dataset encountered stability and convergence issues, motivating a deeper study of 1-Lipschitz regularization techniques for Auxiliary Classifier GANs (AC-GANs). An extensive ablation study on the CIFAR-10 dataset found that Spectral Normalization is key for AC-GAN stability and performance, while Weight Clipping fails to converge without Spectral Normalization. Human papillomavirus (HPV) vaccinations are lower than expected. To protect the onset of head and neck cancers, innovative strategies to improve the rates are needed. Artificial intelligence may offer some solutions, specifically conversational agents to perform counseling methods. We present our efforts in developing a dialogue model for automating motivational interviewing (MI) to encourage HPV vaccination. We developed a formalized dialogue model for MI using an existing ontology-based framework to manifest a computable representation using OWL2. Electronic health (e-health) systems may outsource data such as patient e-health records to mobile cloud servers for efficiency gains (e.g., minimizing local storage and computation costs). However, such a move may result in privacy implications in the presence of semi-honest cloud servers. The coronavirus disease of 2019 (COVID-19) pandemic exacerbated barriers to care for people living with human immunodeficiency virus (HIV) (PLWH). The quick uptake of telemedicine in the outpatient setting provided promise for care continuity. In this study, we compared appointment and laboratory no-show rates in an urban outpatient HIV clinic during three time periods: (1) Pre-COVID-19: 9/15/2019-3/14/2020 (predominately in-person), (2) "Early" COVID-19: 3/15/2020-9/14/2020 (predominately telemedicine), and (3) "Later" COVID-19: 9/15/2020-3/14/2021 (mixed in-person/telemedicine). Previous research has connected phone use with mood and depressive symptoms but has typically relied on self-report measures of phone use. The current study examined the potential connection between objectively tracked phone use and self-reported depressive symptoms among 42 U.S. mothers with a young child (M age = 2.34 years). Their phone use was measured continuously for 5 days via an app installed on their smartphones, and mothers also completed a nightly survey each day. Mothers spent an average of 4.73 hours on their phone each day (SD = 2.87; Range = 0.70 - 13.59), and maternal phone use occurred …

BACKGROUND: Chronic lung disease affects nearly 37 million Americans and often results in significant quality of life impairment and healthcare burden. Despite guidelines calling for palliative care (PC) integration into pulmonary care as a vital part of chronic lung disease management, existing PC models have limited access and lack scalability. Use of telehealth to provide PC offers a potential solution to these barriers. This study explored perceptions of patients with chronic lung disease regarding a telehealth integrated palliative care (TIPC) model, with plans to use findings to inform development of an intervention protocol for future testing.

BACKGROUND: Family involvement and comfort are equally important in palliative care. Dignity undertook a new meaning and novel challenges as a result of restrictions on visits and companionship during the pandemic. Family-centered family dignity interventions have been shown to be effective in increasing patients' sense of dignity, increasing levels of hope, and reducing psychological distress; however, the effectiveness in enhancing family adaptability and intimacy in the survivor-caregiver binary and reducing expected grief have been inconclusive.

The use of Telecytology in conjunction with rapid on-site evaluation (ROSE) is currently at the forefront of discussion among cytologists and cytopathologists. In this meta-narrative review, the advancements of Telecytology and ROSE within the discipline of Cytology are discussed. Advancements in telecytology and ROSE enable off-site cytology slide evaluation and diagnosis through digital image transfer. ROSE allows for preliminary cytological assessment during image-guided biopsy procedures to determine specimen adequacy. The meta-narrative review concluded that the combination of these two technologies increases the efficacy of patient diagnosis without sacrificing the quality of patient care.

BACKGROUND: Digital neuropsychological tests reliably capture real-time, process-based behavior that traditional paper/pencil tests cannot detect, enabling earlier detection of neurodegenerative illness. We assessed relations between informant-based subtle and mild functional decline and process-based features extracted from the digital Trail Making Test-Part B (dTMT-B).

METHODS: A total of 321 community-dwelling participants (56.0% female) were assessed with the Functional Activities Questionnaire (FAQ) and the dTMT-B. Three FAQ groups were constructed: FAQ = 0 (unimpaired); FAQ = 1-4 (subtle impairment); FAQ = 5-8 (mild impairment).

Results: Compared to the FAQ-unimpaired group, other groups required longer pauses inside target circles (p < 0.050) and produced more total pen strokes to complete the test (p < 0.016). FAQ-subtle participants required more time to complete the entire test (p …
