Community health centers (CHCs) play a pivotal role in healthcare delivery to vulnerable populations, but have not yet benefited from a data warehouse that can support improvements in clinical and financial outcomes across the practice. We have developed a multidimensional clinic data warehouse (CDW) by working with 7 CHCs across the state of Indiana and integrating their operational, financial and electronic patient records to support ongoing delivery of care. We describe in detail the rationale for the project, the data architecture employed, the content of the data warehouse, along with a description of the challenges experienced and strategies used in …

The use of Electronic Health Records (EHRs) by healthcare professionals has been recommended as a way of improving healthcare quality, patient safety, and workflow efficiency, and lowering costs in the long run. While large hospital systems integrate EHRs into their services, individual and group practice-owning physicians, especially those of specialties and subspecialties, are often left wondering whether they should follow the trend and whether the transition from traditional, paper-chart systems or older EHRs to newer, commercial ones will truly benefit their patients and their practice as a whole. These physicians also often wonder whether benefits such as electronic-prescribing, clinical decision …

The era of "Personalized Medicine," guided by individual molecular variation in DNA, RNA, expressed proteins and other forms of high volume molecular data brings new requirements and challenges to the design and implementation of Electronic Health Records (EHRs). In this article we describe the characteristics of biomolecular data that differentiate it from other classes of data commonly found in EHRs, enumerate a set of technical desiderata for its management in healthcare settings, and offer a candidate technical approach to its compact and efficient representation in operational systems.
